[Analysis of the etiological structure and dynamics of antibiotic resistance in the causative agents of suppurative inflammatory diseases in the patients at a large general hospital].

The structure of the causative agents isolated from patients with pyoinflammatory infections in 1980-1983 was analysed. It was shown that the surgical and urological infections were mainly caused by gram-negative bacteria. The other pyoinflammatory infections were mainly due to gram-positive cocci. A relatively high frequency of the strains of gram-negative bacteria, especially among Pseudomonas spp. and Enterobacter spp., resistant to aminoglycoside antibiotics, such as gentamicin, sisomycin and tobramycin with preserved sensitivity to amikacin and netilmicin in the majority of the strains was shown. Among the beta-lactam antibiotics cephotaxim and cephalotin were most active against gram-negative bacteria and staphylococci, respectively. The majority of the antibiotic resistant strains of gram-negative bacteria had analogous structures and levels of resistance to 7-12 antibiotics which might indicate the occurrence of 1-2 resistance plasmids among the clinical strains.

[Characteristics of the composition of higher fatty acids in methicillin-resistant and methicillin-sensitive Staphylococci].

The composition of fatty acids of methicillin-resistant (MR) and methicillin-sensitive (MS) strains of Staph. aureus and Staph. epidermidis was determined with the method of reactive gas liquid chromatography. The MS staphylococci of the above species differed by the content of acids with branched chains of iso- and anteisostructures and straight chains. Anteisoacids in the cells of Staph. epidermidis amounted almost to 80 per cent of the total number of the acids, while in the cells of Staph. aureus, their total number amounted only to a half of the fatty acid pool. Comparison of the composition of the fatty acids of the MS and MR strains of Staph. aureus revealed differences in the proportions of the anteiso- and isoacids. The total number of the long-chain C20.0 acid in the cells of Staph. epidermidis resistant to methicillin was lower as compared to that in the sensitive cells.

[Plasmids of antibiotic-resistant clinical strains of E. coli].

Analysis of 53 antibiotic resistant clinical strains of E. coli isolated from patients with various purulent inflammatory diseases is presented. According to the data of the electrophoretic study 83 per cent of them carried 2 to 6 plasmids. Thirteen of them carried the conjugation R-factor. The antibiotic resistance in the other strains was due to the non-conjugation plasmids.

[Antibacterial activity of rifampicin against the causative agents of suppurative, septic diseases].

The antibacterial activity of rifampicin was studied in comparison with other antibiotics with respect to clinical strains isolated from cases with various purulent inflammatory processes caused by Staphylococcus, E. coli, Ps. aeruginose, Proteus. The aim of the study was to define the role of rifampicin in the treatment of the above infections. No rifampicin resistant strains were found among staphylococci belonging to the phenotype carrying the determinants of resistance to 2-8 antibiotics. Rifampicin was less active against gramnegative organisms. High heterogeneity of the microbial population of rifampicin was shown with respect to all microbial strains tested. The rate of the spontaneous mutants was high. The average rate of the mutants was 1-7.7-10-8. The studies on the dynamics of the rifampicin resistance increase in the strains of Staphylococci, E. Coli, Ps. aeruginosa and Proteus showed that the resistance increased after 1-2 passages, which means that one-stage mutation was characteristic rifampicin.

[Chemotherapeutic effectiveness of rifampicin in experimental infections].

Studies on the chemotherapeutic action of rifampicin in treatment of staphylococcal sepsis and sepsis caused by gramnegative organisms showed its high efficacy only in treatment of the staphylococcal infection. By the level of its efficacy rifampicin was much superior to benzylpenicillin and especially tetracycline. No difference in the activity level of the antibiotic in treatment of staphylococcal infections caused by sensitive and multiple resistant staphylococcal strains was found. In treatment of the infections caused by gramnegative organisms the drug activity was moderate.

[Complications from antibiotic therapy].

Analysis of 302 side effects of antibiotic theraphy is presented. The side effects were studied comparatively as dependent on the antibiotic group. Dependence of the toxic and toxicoallergic reactions to the antibiotics on the antibiotic blood levels were noted. Previous sensitization resulted in more frequent and earlier side effects. The analysis and clinical observations showed that antibiotic therapy should take into account the results of the laboratory tests, i.e. examination of the kidney functional state, antibiotic levels in the blood and urine, tolerance of the drug by the patient.
