99-m technetium (Dupont Cardiolite) investigations in postinfarction patients with Holter-checked silent ischaemia.

Sixteen middle-aged, normotensive, slightly overweight male patients with previous myocardial infarction were studied during Holter-checked silent myocardial ischaemia. As reference, stress and late 201-T1 scintigraphy served for comparison with Cardiolite-MIBI silent ischaemic perfusion scan, both carried out in planar mode. The circumferential profiles differed in 9 cases, on region of interest basis the segment number difference was 10, but the late distribution segment number was near to both ischaemic numbers. The quantitative scores were distinctive (ratio 133-128/103) indicating the silent ischaemia appeared in the peri-infarct area. The silent ischaemic MIBI and stress 201-T1 ischaemic score difference was reduced by means of repeated SPECT investigation. With gated radionuclide ventriculography there was -4.3% difference between the left ventricular ejection fractions, measured with first pass MIBI technique during silent ischaemia and afterwards in basal state. The impairment of the left ventricular function was reflected on the stroke pattern of our Holter-based radiocyclogram, as well. Taking the 43.7-48.0 = -4.3% "ischaemic shift" into consideration it was a close correlation (r = 0.90) between the two kinds of ejection fraction determination. The major rhythm failures (occurring during the 24 h Holter monitoring) decreased to a higher degree the left ventricular ejection fraction than silent ischaemia or silent ischaemia and minor rhythm failure together (38-42-50%).

Study of the antitumoral activity of S-carbamoyl-L-cysteine derivatives in animal experiments.

The antitumoral activity of three amino acid derivatives, S-carbamoyl-L-cysteine, S-ethyl-carbamoyl-L-cysteine, and S-chloroethyl-carbamoyl-L-cysteine, was studied. The ethyl and chloroethyl derivatives had a pronounced curative effect on certain animal tumors. It is interesting in the case of S-carbamoyl-L-cysteine that N-ethylization of the substance resulted in increased chemotherapeutic effectivity with no simultaneous change of the toxicity. According to various studies this effect is different in many respects from that of the known cytostatics, and a possibility of a specific selective or surface activity is raised.

[Etiology of pancreatitis in 10 years of case material].

The aetiology of the 166 pancreatitis-casts observed in the authors' department between January 1, 1966, and December 31, 1975 is analysed. In 123 (74%) cases the aetiology could be clarified, while in 3 cases even the autopsy failed to throw light on the pathogenesis of the fatal pancreatitis. The distribution of complications and mortality according to aetiology is presented. The conclusions drawn from the material indicate that the frequency of pancreatitis is increasing and a shift occurs towards the young age groups. The less frequently occurring aetiological factors are pointed out and the importance of a careful history is stressed. Early operation of patients with gallstones is an important factor in the prevention of pancreatitis.

Adenocarcinoma

The effect of dibromodulcitol (Elobromolr, Mitolactolr, nsc 104 800) on the postoperative recurrences of the Guerin carcinoma of the rat.

In all cases of Guerin carcinoma of the rat the recurrence of the tumor in 7-10 days can be observed following surgical removal. Local treatment with Dibromodulcitol combined with intraperitoneal administration of the same drug results in delayed occurrence of tumor recurrences, the growth of which is slower, and in a significant increase of life span of the animals. In exceptional cases no recurrence appeared. The local treatment combined with intraperitoneal administration was the most effective. Peroral treatment was also effective although in a lesser degree.

[Electron microscopic changes of the intestinal mucosa following truncal vagotomy].

Ultrastructural changes on the mucosa of the jejunum and ileum after truncal vagotomy were studied in the dog. Tests performed 2, 3, 7 and 10 days, 2, 4, 5, 6, 7, 8 and 9 weeks as well as 4, 5, 6, 9 and 12 months after vagotomy showed no significant changes in the epithelial cells. The microvilli, smooth and rough surface endoplasmic reticulum, the free iebosomes and the terminal web area were completely intact. In some cells a swelling of the mitochondria was observed. It is concluded that vagotomy causes no such morphological changes of the intestinal mucosa which could explain postvagotomic diarrhoea.

[Explorative laparotomy].

In the 10 year surgical material of the authors the incidence of explorative laparotomy was 9.3%. In the 850 exposures the findings of 263 patients were negative. Of the 263 cases, 207 are analyzed in detail, including the history, laboratory results and findings during operation. At follow-up, in 41% of the patient the pain had ceased, while in 30% it improved. A mortality of 0.51% could be ascribed directly to the exploration. It is concluded that explorative laparotomy is an indispensable and useful procedure.

[Damage and regeneration of the skeletal muscles in the presence of foreign bodies].

Studies of experimental muscle regeneration revealed that certain constituents of talc are actively damaging the regeneration of muscle tissue and produce characteristic reaction of the tuberculoid type. Bio-SorbR which contains starch and has been marketed for a similar use affects regeneration only as a foreign body. The elimination of talc from surgical practice and its substitution by some neutral material is recommended.
