Pathophysiological significance of the global and regional left-ventricular ejection fraction (LV-EF) on ischaemic heart disease patients at rest and during ergometric exercise load (from radiocardio-cyclography (RKG-RCG) to parametric amplitude and phase scan).

As early as in the graphical RKG-RCG period a close inverse correlation could be established between the LV-EF and the serum myoglobin level during the acute course of myocardial infarction, in 10 patients in repeated follow up examinations. Corr. coeff. -0.91, p less than 0.01. In the mid seventies it could be shown by RKG-RCG, in 15 IHD patients with angina pectoris that the decrease of the basal LV-EF during ergometric load reflected the severity of IHD, compared with the increasing LV-EF tendency of 15 normal subjects. This fact could be verified on 19 middle age males (mean age, 41 years) by 99mTc RBC gamma camera ventriculography, i.e. that under modest load (100 W ergometry) a more than 10% decrease was a non-specific sign of main branch or three-vessel coronary heart disease. So in this extreme case our nuclear stethoscope-like RKG-RCG method alone may be satisfactory for staging and screening of coronary ischaemic heart disease (IHD) patients. All the 11 normal subjects belonged to the load-reaction group with more than 5% LV-EF increase, while the extensive anterior and inferior scar patients reacted without exception with more than 10% deficit (their basal LV-EF value was already under 45%). Supported by data in the literature in the comparison of load ECG and coronarography and two-step load, we could gain more refined data, but in accordance with the one-step load on the same patients. As regards the reproducibility of our global LV-EF investigations with gamma camera computer program Supersegams, it was within 5%.(ABSTRACT TRUNCATED AT 250 WORDS)

Frequency, sensitivity and specificity of roentgenographic features of slight and moderate asbestos-related respiratory diseases.

The possibility of early detection of asbestos-related respiratory diseases was examined on the basis of four x-ray abnormalities, namely, pulmonary fibrosis, pleural plaque, diffuse pleural thickening and diaphragmatic calcification, in a group of workers exposed to chrysotile asbestos. The frequency of these phenomena was compared to the unexposed control group of similar distribution of number, sex and age. Besides the pleural plaques, which had a high specificity, the combination of minor x-ray abnormalities proved to be most characteristic of exposure to asbestos. The more frequent one of the abnormalities, the less specificity it had to asbestos exposure.

Asbestosis↗

Changes in clinical status and lung functions of patients with chronic respiratory diseases over 10 years.

After a period of 10 years the authors reexamined the respiratory health status of 381 dust-exposed males. Of the lung function values examined Raw, RV/TLC%, VC, TGV and PaO2 were found to be of significant prognostic importance. Among the radiological changes characteristic of silicosis the most serious B-C category, among the respiratory complaints dyspnoea accompanied by regular cough and expectoration, and among the physical changes extended rhonchi and rales accompanied by an emphysematous thorax are to be considered as most essential with respect to prognosis. Smoking habits significantly affected the total death rate, but did not prove to be significant with regard to respiratory death. In the development of bronchial obstruction recurrent febrile respiratory diseases proved to be more important than smoking habits and mild bronchitic complaints.

Asbestos bodies in a bile duct cancer after occupational exposure.

A case of asbestosis after 5 years of occupational asbestos exposure is presented. At the postmortem examination, cholelithiasis and a cancer developing from the cystic duct were found and identified histologically as a squamous cell carcinoma. After digesting the tumor tissue, short asbestos bodies and fragments could be identified having a similar shape as those observed in the lung. The possible role of asbestos in the carcinogenesis of the observed bile duct cancer is proposed. The digestive examination of all malignant tumors of any origin after an asbestos exposure is suggested for the identification of the possible presence of ingested asbestos fibers, and to obtain further comprehensive human pathological data.

Effect of haemo- and peritoneal dialysis on the cell-mediated immune response in chronic uraemia.

Lymphocytopenia, decreased spontaneous rosette formation, and a decreased T lymphocyte count have been found in patients with non-uraemic glomerulonephritis (71 cases) and in different stages of uraemia (68 cases). In chronic glomerulonephritis and in the early stage of uraemia, cell-mediated hypersensitivity (lymphocyte migration inhibition) to glomerular basement membrane (GBM) characteristic of glomerulonephritis could be demonstrated. Hypersensitivity disappeared in the terminal stage of uraemia indicating endogenous immunosuppression.

[Histological reactions caused by various types of suturing threads].

Wounds caused in the dorsal musculature of guinea pigs were sutured with various types of threads and the histological reactions caused by the latter were studied. Light and electron microscopic methods revealed that the best resorption can be achieved with Dexon threads. This synthetic thread is resorbed in 14 to 16 weeks causing only a minimal histological reaction and without cicatrization in its vicinity. Around threads which are not absorbed a connective tissue capsule is formed; this one is thin in the case of Ethilon and Chrom cat gut and somewhat more marked when Rixo or Supramid threads are used. The histological reaction is the most intense around the traditional linen thread which leads to cicatrization. In the resorption of Dexon a decisive role is attributed to the giant cells of the foreign body while in the case of non-absorbable synthetics and linen threads the fibre forming roles of the histiocytes, mastocytes and fibroblasts dominate.
