The use of monoclonal antibodies to estrogen receptors (ER) for immunoperoxidase detection of ER in paraffin sections of human breast cancer tissue.

Two monoclonal antibodies against MCF-7 human estrogen receptors were used for immunoperoxidase staining of paraffin sections of human breast cancer tissue. The staining was predominantly located in the nucleus of epithelial cells. Variation in the staining intensity was observed among individual cells. A significant positive correlation between the number of positively stained cells and cytosol estrogen receptor content (fmol of bound estrogen/mg of protein) was observed. The potential and the limitations of the present techniques are discussed.

The management of patients with carcinomas in fibroadenomatous tumors of the breast.

This study focuses upon 38 patients found to have unsuspected carcinomas or lobular neoplasia in fibroadenomatous tumors. Combining the pathologic and clinical features of these patients with those of 56 comparable instances reported upon in the literature, it becomes apparent that the biologic and clinical behavior of lobular neoplasia, intraductal carcinomas and invasive carcinomas is essentially the same regardless of whether the lesions arise in or outside a fibroadenoma. In addition, they are often a part of a multifocal malignant condition in the ipsilateral or contralateral breast. These patients, therefore, should be treated as if the carcinomas or lobular neoplasia arose in the usual setting.

Multiple primary tumors in association with prostatic cancer.

One hundred forty-six patients with clinically nonmetastatic carcinoma of the prostate were treated at Columbia-Presbyterian Medical Center from 1964 through 1976. Of these 146 patients, 24 (16.4%) had developed at least one additional primary cancer. This review contrasts the 122 patients who had a single primary prostate cancer only with the 24 patients who had additional primary carcinomas with respect to age, racial distribution, clinical stage, and prostate cancer histology including Gleason's score, patterns of failure of prostate cancer, and survival data. Local failure and distant failure were less in the multiple primary group. Patients with high Gleason's scores appear to be at greater risk for second primaries. Five-year observed survival (by actuarial life table method) for the single prostate primary group was 76.5%, and 5-year observed survival of the prostate multiple primary group where prostate cancer appeared first was 71%.

Oestrogen receptors in human breast cancer. Problems of correlation with histopathological features.

Histopathological factors which might explain inconsistency in published data attempting to correlate oestrogen receptor content (ER) and pathological features in primary breast tumours have been investigated in 194 cases. It was found, that unequal assessment of tumour type and of histological grading between observers is one important factor. In terms of grading, however, heterogeneity of growth pattern within the same tumour seems to be of greater significance. No significant correlation was found between histological type of tumour and ER content. However, a trend towards a correlation between the extent of tubule formation (as an indication of differentiation) and ER content was observed.

Lymphomas and pseudolymphomas of the alimentary tract. An immunohistochemical study with clinicopathologic correlations.

One hundred one lymphoproliferative lesions of the gastrointestinal tract (66 malignant lymphomas, 20 pseudolymphomas, and 5 borderline lesions) were reviewed. The pathologic features were compared to the clinical findings, with reference to differential diagnosis and prognosis. Special attention was paid to immunohistochemical features. The gross appearance was diagnostic in only a limited number of cases. Endoscopic biopsy alone was also of limited value because only a diagnosis of probability could be made in several cases. In most of them the definitive diagnosis had to be based on histologic examination of the resected specimen. Immunohistochemical examination was found to be very useful as an ancillary diagnostic technique. Malignant lymphomas displayed either cytoplasmic immunoglobulins with a monoclonal pattern (47.1 per cent) or a negative reaction, whereas the pseudolymphomas were generally characterized by polyclonal immunoglobulins. Ninety per cent of malignant lymphomas with cytoplasmic immunoglobulins contained lambda chains. The survival probability was found to be related to the size of the lesion, the depth of infiltration, and the immunohistochemical characteristics of the tumors. The histologic type of the lymphomas was of limited value as a predictor of prognosis in the present series.

[Histologic diagnosis of lymphoproliferative diseases of the digestive tract].

The pathologic diagnosis of lymphoproliferative diseases of the gastrointestinal tract is sometimes very difficult or even impossible. In the majority of cases the gross appearance of the lesion is not helpful. Three histological criteria, when present, justify a definite diagnosis: cellular anaplasia and nodal involvement in malignant lymphomas, and the presence of germinal centers in pseudolymphomas. All other histological criteria, including transmural extension of the lesions, are of only relative diagnostic value. The definite diagnosis of these diseases is based on histological examination of the entire lesion. Only a diagnosis of probability is generally possible on endoscopic biopsies. The demonstration of different chains of immunoglobulins by means of immunohistochemistry may help in revealing malignant lymphomas with secretory activity and a polyclonal pattern in reactive lesions.

Morphological and functional recovery of the canine gallbladder mucosa following two hours' ischemia.

Temporary interruption of the circulation to the canine gallbladder causes hemorrhagic necrosis of the mucosa and an abolition of active transport of glycine in vitro. The first signs of epithelial recovery following two hours' ischemia are seen in animals examined 7 days after the injury and are characterized by the appearance of a layer of cuboidal epithelial cells in which some mitotic figures are observed, accompanied by a decrease in the amount of necrosis and the presence of granulation tissue. At the same time, there is some slight resumption in glycine uptake. Two weeks after injury, the new epithelium covers a larger surface of the gallbladder wall but recovery is still incomplete. Four weeks after operation, structural and functional recovery has occurred in almost all the animals studied: the epithelium is columnar and does not differ greatly from that seen in the normal gallbladder. Mucosal folds are present although their morphological appearance is slightly altered in comparison with those of control specimens. The gallbladder wall is thickened by fibrosis. Measurements of active glycine transport in vitro confirm that the new epithelium functions normally.

Neoplastic involvement of nipple and skin flap in carcinoma of the breast.

This study was carried out to determine the frequency of neoplastic involvement of the nipple and of the skin flap in 1,000 mastectomy specimens with primary breast carcinomas. Nipple involvement (Paget's, and carcinoma in ducts or stroma or in lymphatics) was encountered in 23.4% of the cases and appeared to be more frequently associated with tumors of large size or located in the central sector of the breast, and with the intraductal and small cell carcinomas. The skin flap was invaded by the underlying tumor in 101 cases, particularly when the tumors were over 2 cm in diameter and when the nipples were involved. In 12 additional cases, tumor emboli in dermal lymphatics were discovered microscopically in random sections of the skin flap. The neoplastic involvement could not be recognized grossly in 58% of the involved nipples, in 28.7% of the skin flaps directly invaded by an underlying tumor, and in nine of the 10 cases with microscopic emboli dermal lymphatics and in which the gross appearance of the skin was recorded. A high incidence of axillary metastases was found in association with nipple and skin involvement.

Hamartoma of the breast: diagnostic observation of 16 cases.

Of 10,000 mammographies done over a 9-year period, 16 cases of hamartoma of the breast were diagnosed. The entity is a well delimited mass composed of dysplastic-appearing mammary tissue admixed to fat. It can be readily recognized and should not be confused with fibroadenoma or mammary dysplasia. The accuracy with which it can be diagnosed by mammography makes possible the avoidance of surgical excision in selected patients.

The histiocytic nature of dermatofibrosarcoma protuberans. Tissue culture and electron microscopic study.

Specimens from four cases of dermatofibrosarcoma protuberans when explanted in vitro exhibited a typical ameboid outgrowth, confirming the histiocytic nature of these tumors. The cultures displayed the same evolution from macrophages to fibroblast-like cells observed previously in cultures of other types of histiocytomas. Electron microscopic study of the cultures confirmed the light microscopic findings and suggested that the fibroblast-like cells were histiocytes with some secondarily acquired fibroblastic features. The possibility that neoplastic histiocytes may transform into true fibroblasts was deemed unlikely, but could not be excluded.
