Synthetic peptides from Plasmodium falciparum apical membrane antigen 1 (AMA-1) specifically interacting with human hepatocytes.

Plasmodium falciparum apical membrane antigen 1 (AMA-1) is expressed during both the sporozoite and merozoite stage of the parasite's life cycle. The role placed by AMA-1 during sporozoite invasion of hepatocytes has not been made sufficiently clear to date. Identifying the sequences involved in binding to hepatocytes is an important step towards understanding the structural basis for sporozoite-hepatocyte interaction. Binding assays between P. falciparum AMA-1 peptides and HepG2 cell were performed in this study to identify possible AMA-1 functional regions. Four AMA-1 high activity binding peptides (HABPs) bound specifically to hepatocytes: 4310 ((74)QHAYPIDHEGAEPAPQEQNL(93)), 4316 ((194)TLDEMRHFYKDNKYVKNLDE(213)), 4321 ((294)VVDNWEKVCPRKNLQNAKFGY(313)) and 4332 ((514)AEVTSNNEVVVKEEYKDEYA(533)). Their binding to these cells became saturable and resistant to treatment with neuraminidase. Most of these peptides were located in AMA-1 domains I and III, these being target regions for protective antibody responses. These peptides interacted with 36 and 58 kDa proteins on the erythrocyte surface. Some of the peptides were found in exposed regions of the AMA-1 protein, thereby facilitating their interaction with host cells. It is thus probable that AMA-1 regions defined by the four peptides mentioned above are involved in sporozoite-hepatocyte interaction.

Small supernumerary marker chromosomes--progress towards a genotype-phenotype correlation.

Small supernumerary marker chromosomes (sSMC) are still a major problem in clinical cytogenetics as they are too small to be characterized for their chromosomal origin by traditional banding techniques, but require molecular cytogenetic techniques for their identification. Apart from the correlation of about one third of the sSMC cases with a specific clinical picture, i.e. the i(18p), der(22), i(12p) (Pallister Killian syndrome) and inv dup(22) (cat-eye) syndromes, most of the remaining sSMC have not yet been correlated with clinical syndromes. Recently, we reviewed the available >1600 sSMC cases (Liehr T, sSMC homepage: http://mti-n.mti.uni-jena.de/~huwww/MOL_ZYTO/sSMC.htm). A total of 387 cases (including the 45 new cases reported here) have been molecularly cytogenetically characterized with regard to their chromosomal origin, the presence of euchromatin, heterochromatin and satellite material. Based on analysis of these cases we present the first draft of a basic genotype-phenotype correlation for sSMC for all human chromosomes apart from the chromosomes Y, 10, 11 and 13.

Growth rate and associated factors in small abdominal aortic aneurysms.

OBJECTIVE: To study the growth rate and factors influencing progression of small infrarenal abdominal aortic aneurysms (AAA). DESIGN: Observational, longitudinal, prospective study. PATIENTS AND METHODS: We followed patients with AAA <5 cm in diameter in two groups. Group I (AAA 3-3.9 cm, n = 246) underwent annual ultrasound scans. Group II (AAA 4-4.9 cm, n = 106) underwent 6-monthly CT scans. RESULTS: We included 352 patients (333 men and 19 women) followed for a mean of 55.2+/-37.4 months (6.3-199.8). The mean growth rate was significantly greater in group II (4.72+/-5.93 vs. 2.07+/-3.23 mm/year; p<0.0001). Group II had a greater percentage of patients with rapid aneurysm expansion (>4 mm/year) (36.8 vs. 13.8%; p<0.0001). The classical cardiovascular risk factors did not influence the AAA growth rate in group I. Chronic limb ischemia was associated with slower expansion (< or = 4 mm/year) (OR 0.47; CI 95% 0.22-0.99; p = 0.045). Diabetic patients in group II had a significantly smaller mean AAA growth rate than non-diabetics (1.69+/-3.51 vs. 5.22+/-6.11 mm/year; p = 0.032). CONCLUSIONS: The expansion rate of small AAA increases with the AAA size. AAA with a diameter of 3-3.9 cm expand slowly, and they are very unlikely to require surgical repair in 5 years. Many 4-4.9 cm AAA can be expected to reach a surgical size in the first 2 years of follow-up. Chronic limb ischemia and diabetes are associated with reduced aneurysm growth rates.

[Usefulness of relaxation techniques for patients with ischaemic cardiopathy: intervention in a health district].

OBJECTIVE: To determine the effectiveness of a programme of relaxation used in primary care for patients with ischaemic cardiopathy (IC) through evaluation of the anxiety levels and the quality of life of these patients. DESIGN: Controlled intervention study without randomisation. SETTING: Two primary care teams in the city of Sabadell. PARTICIPANTS: Patients from 15 to 69 years old with IC: intervention group (IG), 57 patients; control group (CG), 47. Interventions. In the IG, conventional treatment and relaxation techniques (RT). In the CG, conventional treatment. MAIN MEASUREMENTS: Degree of anxiety was measured by means of the STAI scale-questionnaire. Quality of life was measured by means of the SF-36 health questionnaire. Blood pressure and smoking were recorded. RESULTS: At 12 months, 70% of patients practised relaxation at least for a short time. In the IG, anxiety dropped significantly at 3, 6, and 12 months, especially the anxiety characteristics scale, which was still significant at 12 months (P=.002). Quality of life at 3 and 6 months in the IG showed better scores in most dimensions except body pain. At 12 months, scores continued to be better than at the start (social function P=.012), except in the mental health dimension. No significant changes were found in blood pressure or tobacco consumption. CONCLUSIONS: Our results suggest that an RT workshop in primary care is beneficial if correctly applied and leads to improved quality of life.

Detection of abdominal aortic aneurysm in patients with peripheral artery disease.

OBJECTIVE: To describe the prevalence of abdominal aortic aneurysms (AAA) in patients with peripheral artery disease (PAD). DESIGN: Observational, descriptive, transverse study. PATIENTS AND METHODS: We performed an abdominal ultrasound in 1190 consecutive patients with lower limb chronic ischemia (1/99-12/04). We registered cardiovascular risk factors and clinical data for analysis. RESULTS: The ultrasound was inconclusive in 24 (2%) patients; 1166 patients completed the study. They were mostly male (93.7%), with an age mean of 67+/-9.9 years (37.7-93.4). The main cardiovascular risk factors were: smoking (80.9%), hypertension (41.7%) and hypercholesterolemia (31.4%). The prevalence of AAA was 13% (n = 151). Only 1.5% (n = 17) of the patients had a large AAA (>5 cm). The AAA was clearly more prevalent in men (n = 148; 13.6%) than in women (n = 3; 4.1%) (RR 3.47; 95% CI 1.11-10.89; p = 0.02). The prevalence significantly increased with age, with a maximum of 17.1% in over 75-year-old men (p = 0.006). Patients with tibial disease had a significantly higher prevalence of AAA than aortoiliac or femoro-popliteal disease (p = 0.02). CONCLUSIONS: The prevalence of AAA in patients with PAD is much higher than that reported in the general population. We recommend that an abdominal ultrasound be routinely included in the study of these patients. Over 75-year-old men are at particularly high-risk.

Development of a rat model of spinal cord injury and cellular transplantation.

BACKGROUND AND PURPOSE: Intravenously or intraspinally delivered human umbilical cord blood (UCB) cells and mesenchymal stem cells have been previously shown to improve the functional recovery of spinal cord-injured rats. Obtaining an animal model in the laboratory setting is critical for the development of experimental therapies. We have established a rat model of spinal cord injury (SCI) with basic histological and functional evaluations, ready to use for cell transplantation experiments. METHODS: In the first phase 10 Sprague-Dawley (SD) rats were used to standardize the laminectomy at D9-D10 without secondary lesions. In a second phase, 28 SD rats were laminectomized and injured at D9 by spinal cord compression for 3 to 5 seconds with an aneurysmal clip. Open-field behavior was assessed at days 2 and 7 postoperatively, and weekly until their sacrifice, using the Basso, Beattie, and Bresnahan locomotor rating scale. Two weeks postinjury, 14 immunosuppressed rats received a double intraspinal cell transplant of previously frozen UCB mononuclear cells (MNCs). Using a Hamilton syringe, 2.5 x 10(5) unlabelled MNCs in 10 microL medium were transplanted, rostrally and caudally to the lesion site. Rats were sacrificed at 4 weeks posttransplant by transcardial perfusion with 4% paraformaldehyde, and spinal cords were dissected and further fixated for histological analysis. RESULTS: No wound infections were observed. Thirteen rats developed urinary tract infections and two animals showed autophagia grade 3. We observed a common spontaneous mobility improvement until a certain limit, depending on the degree of lesion and intrinsic characteristics of the animal. CONCLUSIONS: An animal model of SCI has been established. Critical parameters in the survival and correct functional analysis are continuous animal care postinjury, urinary tract infections, autophagia, and weight loss. In addition, electrophysiological measures might be necessary to properly assess functional modifications.

Denitrification potential of industrial wastewaters.

Nitrate utilisation batch experiments have been carried out with different agro-food industrial wastewaters. The results obtained were modelled using a modification of the ASM2d to determine the main kinetic and stoichiometric parameters: denitrification rate with fermentation products (q(N-SA)) and with fermentable substrates (q(N-SF)); fermentation products and fermentable substrates consumption rates, (qSA) and (qSF), respectively; yield coefficient with fermentation products (Y(H-SA)) and with fermentable substrates (Y(H-SF)) and saturation coefficient for growth on fermentation products (K(H-SA)). It was found that fermentation products (SA) and fermentable substrates (SF) consumption rates can be assumed to be both constant and independent of the agro-food industrial wastewater used, with values of 14.6 and 5.4 (mg S(COD) g COD-X(-1)OHO h(-1)), respectively. The mean denitrification rates obtained with each agro-food industrial wastewater ranged from 1.5 to 2.5 (mg S(N) g COD-X(-1)OHO h(-1)), values slightly lower to the obtained with domestic wastewater (2.7 mg S(N) g COD-X(-1)OHO h(-1)). Given the substrate consumption rate and the denitrification rate, the yields with fermentation products and fermentable substrates for each wastewater were determined. The values obtained for the yields ranged from 0.50 to 0.69 on using SA and from 0.50 to 0.70 on using SF. Having determined these parameters, the denitrification potential (DNP) was evaluated for each agro-food wastewater. Wastewater from slaughterhouses gave the lowest DNP (22.6 mg S(N) g S(-1)COD), whereas wastewater from potato and tomato processing plants gave the highest values (126.5 and 128.2 mg S(N) g S(-1)COD), respectively), which are very similar to that obtained with domestic wastewater (130.3 and 128.2 mg S(N) g S(-1)COD). Taking into account the low N/COD ratio presented in the agro-food wastewaters, the last wastewaters are the most indicated to enhance the nitrogen removal in the WWTP. Finally, a relationship between the denitrification rates of several agro-food industrial wastewaters and their respective S(A)/(S(A) + S(F)) ratio was obtained. This relationship can be used to estimate the mean denitrification rate of an agro-food industrial wastewater.

Effect of losartan and spironolactone on left ventricular mass and heart sympathetic activity in prehypertensive obese subjects: a 16-week randomized trial.

Obesity has been shown to be associated with increased left ventricular mass (LVM) and heart sympathetic activity even in nonhypertensive subjects. These factors are predictors of cardiovascular morbidity and mortality independent of other traditional risk factors. We evaluated the effect of losartan and spironolactone on LVM and heart sympathetic activity in prehypertensive obese subjects. A 16-week blinded randomized trial was performed in middle-aged men selected from a Health Public program. Anthropometric and clinical variables were measured at baseline and after losartan (50 mg/day; n=25) or spironolactone (25 mg/day; n=25) treatment. Heart sympathetic activity was evaluated with 60-min electrocardiograph monitoring, and spectral analysis was carried out. LVM was measured by echocardiography according to Devereux and Reicheck's formula indexed for body height (m2.7) to account for obesity. Anthropometric variables, systolic, and diastolic blood pressure levels decreased in both groups of treatment without statisticall difference between them. Losartan increased heart rate variability (from 70.0 to 82.3 ms; P=0.01), and decreased low frequency-high frequency index (from 6.6 to 4.9; P=0.001), and LVM (from 49.2 to 45.2 g; P=0.004). In the multiple regression analysis for factors associated with reduction in LVM; treatment with losartan, and decrease in SBP were the only factors included in the model (R2=0.60; P=0.003). To conclude losartan, but not spironolactone, decreased LVM and heart sympathetic overactivity in prehypertensive obese subjects after 16 weeks of treatment. Regression on LVM was associated with reduction on SBP levels.

Cloning, characterisation, and expression of three oestrogen receptors (ERalpha, ERbeta1 and ERbeta2) in the European sea bass, Dicentrarchus labrax.

Three oestrogen receptor [ER] subtypes have been described in teleost fish, namely ERalpha, and two ERbeta subtypes, called ERbeta1 and ERbeta2 (or ERbeta and ERgamma in Atlantic croaker). Their expression during embryonic development and gonadal growth has evoked interest in their potential role in sexual differentiation and gonadal development in fish. We cloned three oestrogen receptors from adult liver (sb-ERalpha cDNA) and ovary (partial sb-ERbeta1 and sb-ERbeta2 cDNAs) of the European sea bass, and according to their phylogenetic relatedness to other ERs in teleosts, named them sea bass [sb-] ERalpha, ERbeta1 and ERbeta2. Deduced amino acid numbers for sb-ERalpha, sb-ERbeta1 and sb-ERbeta2 were 639, 517 and 608, respectively, representing in the case of sb-ERbeta1 and sb-ERbeta2 about 90% of the open reading frame. Highest amino acid identities were found for sb-ERalpha with eelpout ERalpha (88.7%), for sb-ERbeta1 with Atlantic croaker ERgamma (85.8%), and for sb-ERbeta2 with Atlantic croaker ERbeta (90.1%). Southern analysis confirmed that all three sea bass oestrogen receptors (sb-ERs) are the products of three distinct genes. In adult sea bass, ERalpha was predominantly expressed in liver and pituitary, while sb-ERbeta1 and sb-ERbeta2 were more ubiquitously expressed, with highest expression levels in pituitary. In a mixed-sex population of juvenile sea bass, sb-ERalpha expression was significantly elevated in gonads at 200 days posthatch (dph), while for sb-ERbeta1 and sb-ERbeta2 highest expression levels were observed in gonads at 250 dph. For sb-ERbeta2, expression was also significantly higher in the brain at 250 dph. The cloning of these three ER subtypes in the European sea bass together with the results obtained on expression levels in adult and juvenile animals has given us the foundation to investigate their possible role in sexual differentiation and development in this species in future studies.

Effect of photoperiod manipulation on the daily rhythms of melatonin and reproductive hormones in caged European sea bass (Dicentrarchus labrax).

Reproduction in fish is cyclical and timed to guarantee the survival of the offspring. Seasonal variations in reproductive hormones of fish have been deeply investigated in fish over the last years. However, there are few studies regarding the daily changes in reproductive hormone profiles in teleosts. The aim of the present research was to investigate the effects of photoperiod manipulation on melatonin and reproductive hormones (pituitary sbGnRH, pituitary LH and plasma LH, testosterone [T], and 11-ketotestosterone [11KT]) daily rhythms in male sea bass, kept in net cages under farming conditions in winter (9L:15D). Fish were distributed in two groups, one under constant long photoperiod (18L:6D) and the other under natural photoperiod. The photoperiod strongly influenced the daily melatonin profile, so that the duration of the nocturnal melatonin rise was longer in the control group than in the group exposed to the artificial photoperiod (18L:6D). A daily rhythm was observed in the pituitary sbGnRH profile in both groups, showing the lowest levels during the dark period. A daily rhythm of pituitary LH was detected in the control group, which was suppressed in the group under long photoperiod. Daily variations in plasma LH were observed, the highest levels being found in the dark phase in both groups, although this profile was significantly altered by artificial light, maintaining a fixed relationship between the first nocturnal rise of melatonin and the nocturnal peaks of plasma LH in both groups. Plasma T levels showed significant fluctuations in their daily cycle following a sinusoidal pattern with an acrophase around sunrise in both groups, without any influence of light regime. No significant daily variations in plasma levels of 11-KT were observed in none of the groups. Our results provide the first evidence of the presence of daily variations in pituitary sbGnRH content, pituitary and plasma LH, and plasma T in sea bass. Artificial lights suppressed the circulating melatonin and significantly affected the daily rhythm of LH storage and release.

Washing of cord blood grafts after thawing: high cell recovery using an automated and closed system.

BACKGROUND AND OBJECTIVES: Cord blood (CB) progenitor cells are an alternative source of haematopoietic stem cells for bone marrow reconstitution. The critical importance of cell dose in the clinical outcome has motivated the need to develop techniques aimed at reducing cell losses and increasing reproducibility. This aim of this study was to evaluate an automated CB washing protocol of thawed cord blood units using the Sepax device. MATERIALS AND METHODS: After an initial 1:1 dilution using a dextran/albumin-containing buffer, the cells were washed in order to obtain a final product ready for transplantation. The automatic method was compared with the conventional manual washing procedure. Blood samples were taken after thawing and after washing. The processing time, viability and mean recovery of nucleated cells (TNC) and progenitors were determined. RESULTS: The automatic procedure resulted in a median recovery of 93% CD34+ cells and 89% TNC; no significant differences were observed between methods. In addition, median viability, as assessed by annexin V and 7-aminoactinomycin D (7-AAD), was 98% and 94%, respectively, within CD34+ cells. CONCLUSIONS: The automatic washing method described is as effective as the manual method in terms of viability and progenitor cells recovery, but faster and easier for the operators to perform. Overall, our data suggest that the automatic method is safe and suitable for the routine washing of thawed CB grafts in the clinic.

[Parotid tuberculosis following intravesical BCG instillation: a case report].

First case of parotid gland tuberculosis publicated to date is presented after intravesical instillation of BCG for a superficial bladder cancer. The diagnosis of the parotid gland tuberculosis is difficult because it is usually indistinguishable from a neoplasm. Combined fine-needle aspiration cytology with PCR or LCR amplification of mycobacterial DNA could be a good diagnostic tool avoiding an open biopsy.

A retrospective, observational study comparing the four available immunomodulatory treatments for relapsing-remitting multiple sclerosis.

We performed an observational, retrospective analysis of outcome in a sequential cohort of patients with relapsing-remitting multiple sclerosis (RRMS) in Argentina. Patients treated for 16 months with interferon beta-1a (Avonex; 30 micrograms intramuscularly, once a week), interferon beta-1a (Rebif); 44 micrograms subcutaneously, thrice weekly), interferon beta-1b (Betaferon; 250 micrograms subcutaneously, every other day) or glatiramer acetate (Copaxone; 20 mg subcutaneously daily) were compared with a non-treated group of patients. The different treatment groups were similar in baseline demographic and clinical variables. A significant fall in the annual relapse rate was observed for all four treatments, with the largest effect observed with glatiramer acetate (81% reduction in relapse rate, compared with pre-treatment values). The proportion of patients remaining relapse-free for the entire 16-month treatment period varied from 37% in untreated patients to 83% in the glatiramer acetate treated group. No statistically significant changes in disability scores were observed over the treatment period. This first such comparative study in Latin America shows that treatment of multiple sclerosis patients with immunomodulatory therapies in the context of current standards of care in Argentina provides clinically important benefit, and suggest that some of these therapies may be better than others.

Rickettsia typhi infection in wild rodents from central Spain.

When sera from 218 rodents captured in two provinces of central Spain were tested, by indirect immunofluorescence, for antibodies to Rickettsia typhi, 46 (21.1%) were found to be positive, at titres between 1:20 and 1:320. The seropositives included rodents of each of the five species collected: Mus musculus, Mus spretus, Apodemus sylvaticus, Rattus rattus and Rattus norvegicus. Rickettsia typhi infection appears to be common and widespread among the rodents of central Spain.

Differential thermal lensing in extended samples.

We present a new scheme for a pump-probe mode-mismatched thermal lensing experiment aimed at extracting the value of low-absorbing components in the presence of a high-absorbing background. The method allows for the use of large path-length samples. Changes in the background absorption of a column of ethanol of 20 cm of 1% and a pollutant concentration detection limit of 0.04 ppm are obtained when using a pump light power of 34 mW.

[Testicular and epididymal appendages. Contribution about it's embryology and pathogenesis].

OBJECTIVE: The testicular appendages are structures with biologic activity and with the possibility to become ischaemic or to degenerate before determined stimulus. Laterly, we have seen an apparent increase in our cases of torsion of testicular appendages. The said, together with the questions about embriology and pathogenesis of this structures, encourage us to do this work. PATIENTS AND METHODS: 1- Retrospective clinical study of patients operated of torsion of testicular appendages, from January 1984 to December 2001. We asses age, year of operation and clinical features. 2- Prospective clinical study of patients operated of torsion of testicular appendages, between March 1999 and March 2000. We asses age, time of evolution, degree of ischaemia, nutritional status, sexual maturity stage and testicular volume. 3- Inmunocytochemical study of testicular and epididymal appendages. We identified the oestrogen receptors. RESULTS: There is a meaningful increase in the yearly incidence of ischaemia of testicular appendages. The mean age of the patients was 9.3 +/- 2.5 years, similar alone of the study. In the patients of group 2, the mean time of evolution was 49.2 +/- 46.6 hours. The appendages showed oedema and several degrees of ischaemia in the absence of torsion (14/21). The body mass index was 19.5 +/- 3.7, and 90% of patients had stages I and II of Tanner. The mean of testicular volume was 2.1 +/- 1.6 ml. By means of inmunocytochemical study we identified oestrogen receptors in 7 of all testicular appendages studied and absence of oestrogen receptors in all epididymal appendages. CONCLUSIONS: The clinical syndrome of the acute scrotum in the majority of cases is due to oedema and ischemia of the appendages, in absence of torsion, probably related to its enlargement in response to hormonal stimulation, at least in some grade, in prepubertal boys. The difference found, related to oestrogen receptors, suggest a different origin for testicular and epididymal appendages.
