Successful treatment of angiosarcoma of the scalp by intralesional cytokine therapy and surface irradiation.

An 88-year-old woman presented to us with angiosarcoma of the scalp that had developed over a 6-month period following previous trauma. Despite explicit information concerning the extremely malignant potential of the tumour the patient refused any surgical intervention. However, she agreed to receive local, intralesional interferon alpha-2b and interleukin-2 therapy. After partial remission of the tumour, the intralesional cytokine injections were combined with surface radiotherapy. This combination therapy led to a 2-year remission of both the tumour and sonographically suspicious cervical lymph nodes. Apart from the typical, moderate side-effects of interferon alpha-2b and interleukin-2 the therapy was well tolerated. In conclusion, in our limited experience intralesional cytokine therapy--alone as well as in combination with surface irradiation--seems to be an alternative therapeutic option for patients who is not a candidate for surgery.

Identification of Triplet States in Carotenoids by Intracavity Absorption Spectroscopy and Measurements of Delayed Fluorescence.

Owing to the low quantum yield of phosphorescence, triplet states of carotenoids have been very difficult to identify. These states can be characterized by intracavity absorption spectroscopy, which allows the direct measurement of the spin-forbidden singlet-triplet transitions in low concentrated solutions, and by delayed fluorescence measurements.

Reliability of taurine concentrations measured in single urine samples obtained from dogs eight hours after eating.

OBJECTIVE: To evaluate the reliability of taurine concentrations measured in a single urine sample obtained from dogs 8 hours after eating, compared with taurine concentrations measured in 24-hour urine samples. ANIMALS: 18 healthy Beagles. PROCEDURE: After emptying the urinary bladder by transurethral catheterization, dogs were fed a canned maintenance diet. Approximately 8 hours later, urine, plasma, and serum samples were obtained for determination of fractional urinary excretion of taurine and urine taurine-to-creatinine concentration ratios (Utaur:Ucr). Results were compared with 24-hour urinary taurine excretion rate. RESULTS: Unbound and total fractional urinary taurine excretion correlated well with unbound and total 24-hour urinary taurine excretion. However, bound fractional urinary taurine excretion correlated poorly with bound 24-hour urinary taurine excretion. Unbound and total Utaur:Ucr correlated well with unbound and total 24-hour urinary taurine excretion. However, bound Utaur:Ucr correlated poorly with bound 24-hour urinary taurine excretion. CONCLUSION AND CLINICAL RELEVANCE: Fractional urinary excretion of unbound and total taurine, and unbound and total Utaur:Ucr are reliable indicators of 24-hour urinary unbound and total taurine excretion in healthy dogs. However, determination of 24-hour urinary taurine excretion is recommended for evaluating urinary bound taurine concentrations of dogs.

Evaluation of the morphologic characteristics and prevalence of canine urocystoliths from a regional urolith center.

OBJECTIVE: To determine whether morphology of single-mineral urocystoliths and age, sex, or breed data could be applied to facilitate radiographic and clinical urocystolith mineral type prediction, respectively, in dogs. SAMPLE POPULATION: Database of 2,041 dogs with pure mineral composition urocystoliths. PROCEDURE: All uroliths were characterized according to geologic descriptive terminology and by breed, sex, and age of dog at time of sample submission. Summary statistics were used to compare features with specific mineral types. Observed trends were analyzed for statistical relevance between observed and expected frequencies for age, sex, color, size, shape, and surface, using the null hypothesis that differences by urocystolith mineral type did not exist. On the basis of expected breed occurrence derived by equations, the null hypothesis that urocystolith occurrence paralleled canine breed popularity was tested. RESULTS: Urocystoliths > 10 mm in any dimension were > 92% likely to be magnesium ammonium phosphate hexahydrate (MAP). Smooth, blunt-edged or faceted, and pyramidal urocystoliths were usually MAP. Jackstone shapes were almost always silica. Botryoidal (grape-like clusters) urocystoliths were likely to be oxalates. Breeds with high relative likelihood of urocystoliths included: English Bulldog, Pekingese, Pug, Welsh Corgi, and West Highland White Terrier. Breeds with low relative likelihood of urocystolith production included: German Shepherd Dog, Shar-Pei, and German Shorthaired Pointer. About 94% of urocystoliths produced in females or spayed females were MAP, whereas males and neutered males produced a greater assortment. CONCLUSIONS AND CLINICAL RELEVANCE: For pure mineral composition urocystoliths, trends in mineral type among breeds and between sexes can be exploited clinically in the diagnosis and management of urolith-related disease. Size and shape, used in conjunction with age, breed, and sex, can facilitate pure urocystolith mineral type prediction.

17 beta-Estradiol attenuates acetylcholine-induced coronary arterial constriction in women but not men with coronary heart disease.

BACKGROUND: Women are protected from coronary artery disease until the menopause. Ovarian hormones are vasoactive substances that influence both hemodynamic parameters and atheroma formation. Intravenous ethinyl estradiol has been shown to reverse acetylcholine-induced vasoconstriction in cynomolgus monkeys and humans, and 17 beta-estradiol improves exercise-induced myocardial ischemia in female patients. We investigated the effect of the naturally occurring estrogen 17 beta-estradiol on the coronary circulation in postmenopausal women and men with coronary artery disease. METHODS AND RESULTS: We studied nine postmenopausal women 59 +/- 3 years old, mean +/- SEM, and seven men 52 +/- 4 years old with proven coronary artery disease. They underwent measurement of coronary artery diameter and coronary blood flow after intracoronary infusion of acetylcholine 1.6 and 16 micrograms/min before and 20 minutes after intracoronary administration of 2.5 micrograms of 17 beta-estradiol into atherosclerotic, nonstenotic coronary arteries. Changes in coronary artery diameter were measured by quantitative angiography, and changes in coronary blood flow were measured with an intracoronary Doppler catheter. In female patients, acetylcholine 1.6 and 16 micrograms/min caused constriction before the administration of 17 beta-estradiol (-6 +/- 2% and -8 +/- 5%, respectively, compared with baseline). This constrictor response was converted to dilatation after intracoronary administration of 17 beta-estradiol (+8 +/- 2% and +9 +/- 3%, respectively; P < .01 before versus after estrogen). Acetylcholine 1.6 and 16 micrograms/min increased coronary blood flow before and after the infusion of 17 beta-estradiol. However, the mean acetylcholine-induced increases in coronary flow were significantly greater (P < .009) after (126 +/- 37% and 248 +/- 89%, respectively) than before (94 +/- 31% and 143 +/- 49% mL/min, respectively) the administration of 17 beta-estradiol. 17 beta-Estradiol alone had no significant effect on coronary diameter or coronary blood flow (P > .05). Isosorbide dinitrate (1 mg) caused dilatation of the coronary arteries by 11 +/- 2% (P < .005). In men, acetylcholine 1.6 and 16 micrograms/min caused constriction both before and after the administration of 17 beta-estradiol and caused similar increases in coronary blood flow both before and after the intracoronary administration of 17 beta-estradiol. Infusion of intracoronary placebo in six female control patients 55 +/- 3 years old and six male control patients 56 +/- 3 years old did not change coronary diameter responses or coronary blood flow responses to acetylcholine. CONCLUSIONS: 17 beta-Estradiol modulates acetylcholine-induced coronary artery responses of female but not male atherosclerotic coronary arteries in vivo. These human data confirm reports from studies in cynomolgus monkeys that estrogen modulates the responses of atherosclerotic coronary arteries. An enhancement of endothelium-dependent relaxation by natural estrogen (as used in most hormone replacement therapy) may be important in postmenopausal women with established coronary heart disease and may contribute to the acute effect of 17 beta-estradiol on blood flow and its long-term protective effect on the development of coronary artery disease.

[Benzodiazepines in blood samples of alcohol intoxicated drivers].

1,000 blood samples from drivers tested for alcohol were screened specifically for active constituents and metabolites of medicines of the benzodiazepine class. 42 blood samples contained benzodiazepines and a total of eight different active constituents were detected. The active constituents of Valium, Vegesan or Tranxilium, Lexotanil and Seresta have to be dealt with primarily. In addition to benzodiazepine 35 samples showed an alcohol level of over 0.08%. This raised the question of whether the benzodiazepine level measured in the blood samples would intensify the effect of the alcohol. Four categories were formed in order to be able to compare the levels of the various benzodiazepines. According to this categorisation the benzodiazepine concentration was classed as very high in 4 cases, high in 7 cases, moderate in 26 cases and low in 10 cases.

[Several immunological characteristics of occupational exposure to asbestos and other mineral fibers].

The authors evaluated some immunologic factors in workers exposed to asbestos, basalt and glass fibres and compared the results with a control reference group. The findings indicate that there is a considerable difference between the immunologic reaction of the organism, mainly when the humoral component of the answer is taken into account, in the exposed and in the control group. The same observation was valid, when the asbestos exposed group was compared with the groups exposed to asbestos substitutes (basalt and glass fibres). The immunoglobulins of the classes A, G, M, complement components C3 and C4 were significantly increased, alpha-1-antitrypsin and phagocytic activity of leukocytes in peripheral blood was decreased. Most of the examined indicators in the basalt- and glass fibres exposed groups differed insignificantly from the control group. Transferrin level was not significantly different in any of the examined groups. The results indicate that there is higher tolerability of basalt and glass fibres in the organism compared with asbestos fibres.

Investigation of professional nickel exposure in nickel refinery workers.

For assessing the real exposure level of nickel in electrolytical nickel production workers we monitored the following indicators of Ni load. (a) Stationary air sampling of Ni, (b) personal air sampling of Ni, (c) urine Ni excretion determined on group basis, (d) the 4 hours specimen of Ni excretion determined on an individual basis. A tentative limit for Ni in urine, roughly corresponding to the MAC 0.05 mg/m3 was 12 micrograms/l Ni according to (c) and 30 micrograms/l Ni according to (d). The occupational exposure limit for Ni in urine accepted in Czechoslovakia is 30 micrograms/l Ni.

Structural requirements for anthracycline-induced cardiotoxicity and antitumor effects.

By employing rat cardiac myocytes in culture and mouse L-1210 leukemia cells, we have compared different anthracycline analogs with respect to their ability to kill cardiac myocytes and tumor cells. Anthracyclines induced a decrease in cellular ATP and glutathione from both cardiac myocytes and L-1210 cells in a time- and concentration-dependent fashion. Moreover, the decrease in ATP in cardiac myocytes was followed by release of the cytoplasmic enzyme lactic acid dehydrogenase and of adenine nucleotides after anthracycline treatment. At very low concentrations of anthracyclines, at which ATP and glutathione were not affected, the drugs induced complete cessation of the growth of L-1210 cells. Some structural alterations in the anthracycline molecule resulted in parallel changes in antitumor activity and in cardiotoxicity. But other structural alterations resulted in dissimilar changes in antitumor activity and cardiotoxicity. Although the results indicate that the structural requirements for inducing cardiotoxicity and antitumor activity may be different, they also indicate that the mechanisms by which anthracycline causes cell death in tumor cells and cardiac myocytes may be the same.

The blocking effect of disodium cromoglycate on carcinogenesis induced by benzo[a]pyrene.

Subcutaneous injection of 3 mg benzo[a]pyrene into the right hind leg of experimental rats resulted in local tumors in the area of application in all the animals. The same dose of disodium cromoglycate applied prior to benzo[a]pyrene prevented tumor formation or provoked a significant inhibition of the carcinogenic process, e.g. delay of tumor formation and of a reduced size, a number of mitoses and cytological abnormalities and a prolongation of the animal's life. In the in vitro Salmonella typhimurium mutation assay of Ames, disodium cromoglycate eliminated toxicity and mutagenicity of 5-nitro-2-furylacrylic acid. The results would suggest that the initial response of the cell to benzo[a]pyrene had been conducted through the calcium-dependent exocytic pathway, which can be efficiently blocked by disodium cromoglycate, probably by preventing the signals increase in free intracellular calcium concentration. The preserved calcium balance as a consequence of the effect of disodium cromoglycate on benzo[a]pyrene-induced cell response might be a factor responsible for the carcinogenesis inhibition phenomena. The results of the in vitro study in correlation with those obtained in vivo support the suggestion that disodium cromoglycate may influence parallel structures in various cell types.

[Role of drugs in traffic accidents. A pilot study of traffic accident offenders in Switzerland (Bern region)].

Toxicological analyses of blood and/or urine specimens collected between 1980 and 1981 revealed that 34.7% of all traffic accident victims (n = 144) had measurable concentrations of drugs. Analyses of blood samples taken from traffic offenders (n = 250) in 1979, on the other hand, showed 22.4% to have consumed drugs with or without alcohol. This percentage is comparable with values reported in other studies. Our results indicate that the widespread use of drugs represents a hazard for traffic safety in Switzerland. Road safety is additionally jeopardized by the often concomitant consumption of alcohol.

Pulmonary functions in children exposed to polluted air.

In a preliminary study of the effects of polluted air on the functional pulmonary profile in children, 101 children living in an area with heavy air-pollution (Group B) and 103 children from an area with relatively non-polluted air (Group A) were examined. In Group B we found out a significant impairment of the parameters of the flow-volume curve at low volume levels of air in lungs, i.e. FEF25, FEF25/FVC and FEF50/FVC. Tiffaneau's index FEV1% was impaired significantly in girls only. This impairment may be an expression of the peripheral airways lesions due to chronic exposure to polluted air.
