[Cardiomyoplasty--a surgical method of treatment in heart failure].

Until recently heart transplantation has been the only method of surgical treatment of severe myocardial heart failure. In the recent years operations of cardiomyoplasty have been performed more and more often for treatment of patients with this disease. The article presents the literature and personal data on the operation technique, indications and contraindications for it. First clinical results of the operative interventions fulfilled since 1994 are discussed.

[Ways to enhance the efficacy of treating patients with the terminal stage of arterial ischemia of the lower extremities].

Experience with treatment of 410 patients with obliterating diseases of lower extremities arteries with the terminal stage of ischemia and occlusions of the femoro-popliteal segment have shown that successful results of small amputations can be obtained but against the background of complex treatment based on a thorough and comprehensive examination of the patient, and aimed at the recovery of adequate blood supply of distal parts of the extremity by operations of direct and indirect revascularization, normalization of microcirculation and struggle against infection. It allows performing amputation distal to the knee joint in 79.5% of cases, and to reduce the amount of postoperative complications in 38.8% of cases.

[Jejunoileal bypass in the prevention and treatment of clinical manifestations of atherosclerosis in patients with morbid obesity].

Jejunoileostomy effects on blood lipids and cardiovascular system were investigated in obese patients. They were found to lose 30% of their body mass within 6 postoperative months with parallel reduction in concentrations of total cholesterol, triglycerides, LDL and VLDL cholesterol and apoprotein B. These changes persisted for 4.5 years of the follow-up. The surgery in males has a more pronounced effect on blood lipids than in females. Significant changes were not registered in post- and preoperative levels of HDL cholesterol and apolipoprotein A-1. Jejunoileostomy promoted a fall in blood pressure, a rise in exercise tolerance, alleviation of atherosclerosis symptoms.

[Effects of jejunoileal shunting operation on the blood circulation system in patients with morbid obesity].

With the use of radiocardiography, the indices of central hemodynamics were studied in 22 patients with III-IV degree morbid obesity who underwent the operation of jejunoileal shunting. A special attention should be paid to the state of the patients within the first 18 months after the operation, because at this period, reformation of the cardiovascular system is most pronounced. The formulas for prediction of the state of the central hemodynamics at the late postoperative period are suggested.

Long-term follow-up of hand function after digit replantation.

A long-term follow-up study of 26 patients with 28 replanted digits was performed with respect to circulatory, sensory, and motor function. Peripheral circulation in the replanted digits was comparable with corresponding uninjured digits on the opposite hand. Excellent sensory recovery was observed in the territories supplied by 30 of the 36 repaired digital nerves. Best functional results were noted with the thumb and with fingers amputated through the middle phalanx. By objective criteria, 21 of the 28 replanted digits demonstrated satisfactory function. Subjectively, however, only 1 patient was dissatisfied with the outcome.

[Surgical treatment of ischemic heart disease using internal thoracic artery and partial ileal bypass].

The authors recommend complex two-stage treatment of ischemic heart disease, including a reconstructive operation on the coronary arteries and an operation for correcting the disturbed plasma lipid spectrum (partial ileal bypass). The results of operative treatment of 150 patients with the use of the internal thoracic artery were studied. The authors emphasize the advantages of this operation over one using an autovein graft. Complex operative treatment was conducted in the clinic in 60 patients with a good clinical effect. The authors also recommend operation for partial ileal bypass as an independent intervention in dyslipoproteinemia and diffuse affection of the coronary arteries.

[Kinetics of cellular elements in the neointima of synthetic prostheses implanted into the rabbit aorta during development of experimental atherosclerosis].

Experimental atherosclerosis in 30 rabbits was combined with the implantation into the abdominal aorta of the synthetic prosthesis MPL-79. Formation of neointima and transformation of mesenchymal cells into the endothelial ones are studied using scanning and translucent electron microscopy and autoradiography. Neointimal hyperplasia is shown to result in significant narrowing of the prosthesis lumen up to its complete occlusion. It is important that neointima fibroplasia is particularly intense against the background of progressing atherosclerosis.

[Total vessel obliteration in arteriosclerosis].

Chronic occlusion of atherosclerotic genesis in the femoro-popliteal zone is one of the most widespread vascular diseases. Surgical correction of this pathology still remains far from being solved. The authors propose a new kind of operation-a total disobliteration of the occluded artery which is a variant of semiclosed endarterectomy. An analysis of 116 operations of total disobliteration with an average 30-month period of postoperative observations has shown high percentage (81%) of patency of the disobliterated artery. The authors consider that operation of the total disobliteration type is indicated for the 3-4 stage ischemia of lower extremities, for high risk of the development of infectious complications and the absence of adequate autovein as a plastic material.

[Internal thoracic artery in coronary surgery].

The authors emphasize advantages of using the internal thoracic artery for mammary-coronary shunts in patients with ischemic heart disease. 257 operations have been performed in the clinic since 1964. Among them there were operations of mammary-coronary anastomosis (after V. I. Kolesov) and mammary-coronary shunts. Long-term results were followed-up which evidence high effectiveness of operative treatment. In 109 patients control angiographic examinations were fulfilled at different terms (up to 12 years). Patency of mammary-coronary shunts was 83.5% as compared with ACSh where it was about 63%.

[Comparative evaluation of the effectiveness of epidural anesthesia with spontaneous respiration and general anesthesia in aorto-femoral bifurcation shunt].

Results of aorto-femoral bifurcation shunts (AFBSh) were compared with special reference to the methods of anesthesia (general anesthesia--GA and epidural anesthesia--EA) with spontaneous respiration and minimum sedation. It was found that use of GA gave lethality 4 times as high as after EA. In addition, GA was followed by considerably greater incidence of pneumonias and atelectases, myocardial infarctions, enteropareses, acute renal insufficiency. The results obtained allow to think EA with spontaneous respiration to be the method of choice in operations on the abdominal aorta in patients with a severe concomitant pathology.
