[Surgical treatment of chronic ischemic heart disease].

The authors suggest a new complex approach to the surgical treatment of ischemic heart disease. Operation of coronary shunting for the restoration of the coronary blood flow, operation of partial ileoshunting for correction of hyperlipoproteinemia, plexotomy for removal of the vasospastic component. The sequence of the operations may vary, each one can also be undertaken independently. The operations were conducted on 315 patients. The follow-up period is up to 8 years. Patency of the mammary-coronary shunts was 93.5%, of autologous veins shunt--65%. Disappearance of the signs of coronary spasm++ and stable normalization of the lipid spectrum are noted.

[Status of the emptying function of the stomach in patients with duodenal ulcer after vagotomy with draining operations].

Choice of draining operations for treatment of duodenal ulcer by vagotomy is greatly responsible for the evacuatory function of the stomach in the early and remote periods after operation. In patients with stenosis of the duodenum, low localization of ulcer and considerable scarring process in the ulcer area pyloroplasty after Finney and gastroduodenoanastomosis after Jaboulay are considered to be most advantageous draining operations of the stomach. If the operations are not possible technically, shortloop gastrojejunostomy is expedient. Pyloroplasty after Heinicke-Mikulicz is admissible in patients with ulcer of the duodenal bulb in patients with not disturbed evacuation from the stomach.

[Function of the myocardium after revascularization and the patency of coronary shunts].

The article analyzes the contractive function of the myocardium in 149 patients after operation of a single aortocoronary shunt (25 patients), mammaro-coronary shunts with mechanical anastomosis (109 patients) and with manual distal anastomosis (15 patients). Patency of the aorto-coronary shunts was 72%, mammaro-coronary shunts with direct distal anastomosis - 83.5%, with the end-to-side anastomosis - 88.8%. The myocardium function was improved in patients with patent shunts. In patients with incompetent shunts further deterioration of the contractive function of the left ventricle myocardium was noted. It was pointed out that the development of myocardium infarction in the zone of the coronary artery operated upon is not always accounted for by a shunt thrombosis.

[New approaches to the surgical treatment of coronary arteriosclerosis].

A new combined approach to surgical treatment of coronary heart disease is recommended: direct myocardial revascularization (mammary-coronary and aorto-coronary bypass) to recover coronary flow; partial ileo-shunting to control hyperlipoproteinemia, and Arnulf's plexectomy (heart denervation) to eliminate vasospastic component. The sequence of operations can be altered depending on the diagnostic findings. Each operation can be performed independently or in combination with others. Operations were performed in 315 patients. The follow-up covered up to 8 years. Long-term patency of mammary-coronary shunts was 93.5%, and that of autovenous shunts was 65%. Lipid composition showed a stable normalization and signs of coronary spasm disappeared.

[Vascular prosthesis made of polyester threads for microsurgery].

The results of experimental and medical application of worked out by the authors synthetic microvascular prosthesis with the internal diameter from 1 to 3 mm. The prostheses were implanted to the abdominal aorta and to the tail vein of rats and also to femoral and carotid arteries of cats for a period of 1 till 15 months. The angiographic and morphologic experiments of blood flow were carried out. The patency of the microprosthesis in the experiments was to 92.7%. The micro-implant adaptation was characterized by complete tissue isolation of a synthetic skeleton with formation of thin and even internal membrane covered with endothelium layer. In fact the possibility of arterial and venous prostheses having a 1-2 mm diameter with synthetic substitutional textile products creates new possibilities for the further development of the microvascular surgery. The microvascular prosthesis was applied to 10 patients in plastic surgery of small circumferential arteries of limbs achieving positive results.

[Effect of a drainage operation on gastric secretion in patients with duodenal ulcer after vagotomy].

The work analyzes alterations of gastric secretion in 687 patients subjected to various kinds of vagotomy in combination with or without draining operations on the stomach for ulcer of the duodenum. Draining operations were established to decrease activity of gastric secretion. The Jaboulay gastroduodenal anastomosis proved to be less beneficial since 50% of the patients had the positive insulin test and ulcer recurred almost in 11% of the cases. Results were most favorable after vagotomy in combination with pyloroplasty after Finney and Heineke-Mikulicz. The disease recurred after these interventions in 7 and 8% correspondingly.

[A Soviet-made blood vessel bioprosthesis and its clinical use].

The authors describe their experience with the using of Soviet made bioprosthesis of blood vessels made of skeleted conserved vein of the human umbilical vein showing its high effectiveness and positive properties such as: high thromboresistent properties of the bioprosthesis and its firmness, the absence of reaction of rejection and pronounced inflammatory reactions of the recipient organism to bioprosthesis, high possibility of the bioprosthesis in remote periods, its fitness for plasty of average diameter arteries.

[Use of microsurgical technic in abdominal surgery].

Results of the surgical treatment of 172 patients with tumors of the proximal part of the stomach and colon are analyzed. The patients were divided into 2 groups according to the method of forming the esophago-intestinal and colono-intestinal anastomoses. Suture incompetence and peritonitis were shown to appear much more rarely after using a microsurgical technique than after using surgical methods.

[Method of surgical treatment of patients with hyperlipoproteinemia].

In 200 operations of partial shunts of the small intestine in patients with hyperlipoproteinemia the authors have used their modification of the technique of partial ileoshunts in patients with normal weight and jejuno-ileoshunts in patients with morbid obesity. The modification has considerably reduced the amount of postoperative complications. The authors recommend using the procedure at the hospitals dealing with problems of metabolic surgery.
