[Surgical correction of hyperlipoproteinemia in patients with ischemic heart disease].

The experimental results obtained in 102 animals showing a beneficial effect of partial ileac bypass on hyperlipoproteinemia and experimentally-induced atherosclerosis prompted the authors to perform this surgical intervention in 34 patients with coronary heart disease (CHD) and attendant hyperlipoproteinemia resistant to therapeutic agents. The follow up period was up to five years. The patients exhibited a persistent and substantial decrease in blood lipids and atherogenic lipoproteins, a reduced incidence of angina attacks and an improved CHD course. Furthermore, the surgery resulted in a considerable and stable elevation of the cholesterol levels of antiatherogenic high density lipoproteins (by an average of 85,3%) and in a 45.8% decrease of the cholesterol coefficient of atherogenicity which contributes to atherosclerosis regression.

[Long-term results of emergency microsurgery operations in severe injuries and amputations of large segments of the upper limbs].

Under study were blood circulation, innervation and functions of distal parts of upper arms in 114 patients operated upon for traumatic abjunction and severe injuries of large segments. The state of blood circulation was found to be satisfactory within the terms of from 4 to 27 months. The degree of the extremity reinnervation was dependent on the level of the nerve injury and when observed longer than a year it was found to repair up to the level of hypesthesia without hyperpathia. The functional results of such operations were noted to depend on the character of trauma, the completeness and quality of repair of damaged nerves and tendons, on adequacy of rehabilitation therapy in the postoperative period.

[Use of the microsurgical technics in the treatment of occlusive diseases of the main arteries of the lower limbs].

Microsurgical techniques permit high-quality intervascular anastomosis in small and mid-caliber arterial surgery of lower extremities. This improves immediate outcomes in surgical patients with obliterating atherosclerosis of lower extremities. Microsurgical femoropopliteal autovenous shunting in patients with ischemia of the third or fourth degree (by Fontana) reduced the rate of shunt thrombosis from 37 to 27%. Microsurgical autovenoplasty of the deep femoral artery showed actually no thromboses at follow-up.

[Early postoperative period in patients subjected to surgery for noncorrectable obesity].

The early postoperative period after jejunoileoshunting for high degree of the alimentary-constitutional obesity was found to have a number of specific features and to be an important period greatly responsible for the outcome of the operation. The very early active management of such patients was shown to improve the postoperative course and to reduce the number of pulmonary complications. In 85 patients an original technique of jejunoileoshunting was used.

[Reconstructive operations on the aorta and iliac arteries without the use of clamps].

A method of reconstructive operation on the vessels of the aorto-iliac zone not requiring forceps is proposed. A specially made balloon probe is used instead. It is introduced in the vessel lumen and stops the blood flow. The surgical interventions on the aorta and iliac arteries became much easier and less traumatic due to this innovation. The method was successfully used in 28 patients aged 39 to 72. Two thirds of them had different associated diseases. In the postoperative period the patients had stable hemodynamics and no pareses of the gastro-intestinal tract. The proposed method is especially promising in reconstructive operations on the vessels of the aorto-iliac zone with the retroperitoneal access.

[Surgical treatment in a severe degree of obesity by ileo-jejunal bypass].

The article presents the material on the surgical treatment of 35 patients with non-corrected obesity by the operation of jejuno-shunting. The results were followed up for three years. In the nearest postoperative period 2 patients died, the other 33 patients had good results. The body mass diminished, the working capacity of the patients recovered. They believe the special importance of careful selection of patients for the operation. They believe the surgery to be possible but for strict indications and specialized hospitals. After the operations the patients need a continuous follow-up observation.

[Emergency microsurgery in traumatic amputation of the fingers and hand].

The article is concerned with the results of the first experience of the work of a new Leningrad Center of microsurgery for the replantation of fingers and hand after the traumatic amputation. The operations were performed urgently within the first hours after traumas. The replantation included a microsurgical step recovering the integrity of the vessels and nerves by means of an operation microscope, special instruments and suture material. In eleven patients 22 operations were fulfilled with good near and late results. The results of surgery were found to depend on the damage character and time of admission of the patients to the hospital.

[Intestinal ischemia after aortoiliac reconstructions].

The analysis of lethal outcomes and complications following the reconstruction of the abdominal aorta and iliac arteries has shown that in the small and large intestines of 6 patients (12,6%) among 225 operated upon there were deep morphological alterations of the ulcero-necrotic type. The main cause of intestinal ischemia and severe enteropareses following the reconstructive operations on the aorto-iliac segment was found to be a continuous hypotonia and reduced capacitance blood flow in the system of celio-mesenterial arteries. Prophylactic measures against hypotensive reactions during the operations and in the postoperative period in combination with peridural blockade, decompression of the intestine, administration of sorbitol and other drugs are of great significance in the prevention of intestinal ischemia, severe postoperative gastro-enteropareses following reconstructive operations on the abdominal aorta and iliac arteries.

[Blood circulation in the lower extremities after reconstructive operations on the vessels in arteriosclerosis obliterans of the aorta and major arteries].

Circulation in the lower limbs of 22 patients with obliterating arteriosclerosis of the aorta and main arteries was studied during operation by means of occlusion plethysmography and from the results of manometry and flow measurement. All patients underwent reconstructive operations on the vessels. Ejection of blood into the limb with a large volume of the contributing vascular bed was encountered immediately and in the first few days after the operation; the specific blood flow (ml/min/100 cm3 tissue) sharply increased in the limb most affected by ischemia. Circulation in the lower limbs was gradually normalized by the 5 th--7 th day. Some indices obtained during the study are important for understanding the course of the postoperative period in patients with the pathological condition discussed.

[Preliminary clinical experience with antibacterial surgical sutures].

Surgical sutures made of Letilan-Lavsan and polyvinyl alcohol fibers with morphocycline and neomycin and filamentous Letilan were applied in 802 patients in the past 4 years. These sutures display considerable antibacterial properties. The antibacterial effect of Letilan-Lavsan sutures is reduced during the healing because of formation of fibrous capsules, the fibers themselves not being deprived of their antibacterial properties for a long period. The Letilan fibers undergo gradual fragmentation and degradation. The antibacterial properties of polyvinyl alcohol fibers added with morphocycline and neomycin are retained for 2 months in the body tissues. The antibacterial Letilan-Lavsan sutures and filamentous Letilan sutures can be sterilized by autoclaving, applying hydrogen peroxide--formic acid mixture, gamma irradiation and ethylene oxide. The unsterilized antibacterial sutures can be used as cutaneous sutures in surgical procedures.
