[Treatment of chronic infantile hepatitis with D-penicillamine (author's transl)].

Detailed discussion of action, indication and side effects of D-Penicillamine which was used for the treatment of chronic hepatitis of infancy. Of 18 patients, 7 had chronic-active hepatitis, 6 chronic persisting hepatitis, 2 subacute hepatitis and 3 fibrosis of the liver. Control of results was based on numerous clinical chemical investigations and repeated liver biopsies. The transaminases and histology of the biopsies were the essential parameters. Doses between 15 and 35 mg/kg of body weight gave very favorable results in these 18 patients, treated over 5 to 24 months. Australia antigen-negative, chronic active hepatitis appeared to be particularly suited for this type of treatment.

[The congenital lobar emphysema (author's transl)].

The congenital lobar emphysema has no uniform etiology. We can however suppose that malformations especially of the cartilage are of importance. The whole lobe is mostly involved, with predilection of the left upper-lobe. The symptomatology of the congenital lobar emphysema is a manifold one. Striking are respiratory embarrassement with dyspnea and cyanosis. For the diagnosis an X-ray is necessary. Remarkable is a transparent lung in which you can still see an unimportant pattern. For the differential diagnosis et first congenital cysts come in question. By intercurrent infections a sudden severe deterioration can result, also if the cases show a favourable course. The method of choice is a timely operation, necessary is the resection of the affected lobe. This is a report of 17 own observations. In 11 cases we found a congenital lobar emphysema, in 6 cases congenital lung-cysts. We could examine 10 patients till to eleven years from the beginning of the disease. All the children had a normal development, except of insignificant residues.

Experience with amoxycillin in neonates and premature babies.

Amoxycillin, a semi-synthetic penicillin, resembles ampicillin in many respects. There is cross-resistance between the two antibiotics. Numerous Gram-positive and Gram-negative organisms are sensitive to amoxycillin at relatively low dosage. Investigations using oral amoxycillin, 50 mg/kg body weight were carried out in neonates and premature babies in the first days of life. Peak values of 38 microng/ml and 59 microng/ml at 4 1/4 hours were obtained for the neonates and premature babies respectively. The 10-hour value for the first group was 13 microng/ml and for the second 19 microng/ml. With a single dose of amoxycillin, serum levels of 0.9 microng/ml were still obtained after 24 hours. In children who received the same dose twice daily, the serum values were 6.5 microng/ml after 24 hours. The urinary excretion of amoxycillin was, on an average, 41% in 24 hours. Amoxycillin was also found in the meconoium, but the quantity in the different portions showed a large dispersion. Amoxycillin can, therefore, be recommended as an effective and well tolerated antibiotic for use in the neonatal and premature period. In general the drug should be administered 12-hourly at a dose of 50 mg/kg body weight. Only in severe illnesses is 8-hourly administration required.

[Amoxicillin and its excretion into bronchial secretion (author's transl)].

Various dates on amoxicillin (resorption, distribution, elimination, tissue passage, urinary excretion) are presented. The bacterial spectrum is more fully discussed. Amoxicillin is compared with ampicillin. Dosage and therapeutical results are presented. Within repeated bronchoscopic examinations necessary on account of bronchopulmonary affections determination of germs were done in 88 children. At the same time examination of ampicillin levels in bronchial secretions were performed after amoxicillin therapy with different high doses (3 x 125 mg to 3 x 750 mg) for 7 days. It could be shown that good antibiotic levels could be found in secretion specimens above all obtained 2 to 4 hours after the last amoxicillin administration. They could be found too, if there did not exist any purulent secretion or stronger inflammation. There are relations between the amount of dosage, the level in secretion as well as the influence on germs.

[Clotrimazol in the treatment of candida infections in premature and newborn children (author's transl)].

The early infancy is especially affected by moniliasis in pediatrics. The causes are: broad spread of Candida albicans in the environment of these children, a growing immunity, the immaturity of the skin and an insufficient candida-static activity of the serum. The prematures and the newborns with an irregular birth are especially endangered. This danger is increased by simultaneous antibiotic therapy. The arising mycosis can reach from the relative harmless mouth- and diaper-thrush to serious septic diseases. An effective therapy therefore is very much desired. As new efficacious medicaments the derivates of imidazol, Clotrimazol, have proved satisfactory for the treatment of mycoses. It is as Canesten marketed. The drug possesses a strong activity against numerous fungi, some protozoa, and bacteria. In this paper it is reported on own pharmacokinetic and therapeutic investigations with Clotrimazol. The substance has been very useful for the treatment of mouth- and diaper-thrush. Clotrimazol was administered as BAYb 5097 peroral and local as Canesten -solution 1% and -cream 1%. There were 171 treatments carried out. Essential side-effects were not observed.

[Clinical and pharmacokinetic researches of doxycyclin vibravenös in childhood (author's transl)].

Doxycyclin is one of the first efficacious depot antibiotics. It is used in two applications as Vibramycin (R) (peroral and Vibravenös (R) (parenteral). Doxycyclin has a favourable effect not only on different gram-positive and gram-negative bacteria but also on a lot of other germs as mycoplasms, rickettsias, protozoons and large viruses. The tissue invasions is a good one, because there is only a low fraction of free Doxycyclin and further a high lipid activity. Pharmacokinetic investigations of different age-groups show that good serum-levels can be reached because of the sensitivity of numerous germs. The levels are higher, as our experiences demonstrate, if the dose of 4 mg/kilo/weight is injected in two single doses. Our investigations are reported on 49 patients with diseases of the respiratory tract and on 25 patients with diseases of the urogenital tract. Doxycyclin was very useful especially in different forms of pneumonias. The results in infects of the urogenital tract are not so good ones. The compatibility of Doxycyclin can be characterised as very good for children.

[On the epidemiology of recurrent and chronic bronchopulmonary diseases-an examination of about 18000 children. 1. Planning and organization of the investigation (author's transl)].

The planning and organization of an epidemiological investigation is reported to find out the frequency of chronic nonspecific lung diseases (CNSLD) in childhood as a whole and in special groups of age. By investigating regions with different air pollution we tried to study the influence of air pollution on the frequency of childhood lung diseases. Therefore, we examined nearly 5000 children in the city of Halle and in the districts of Quedlinburg, Merseburg and Bitterfeld. The children belonged to the age-groups 5.-6., 8.-9., 11.-12. and 14.-15. years of life in nearly the same number. The investigations were performed by different groups of investigators. However, by equal terms of reference and uniform evaluation of the results by two of us the results of the four districts are comparable with each other.

[Intravenous administration of doxycycline vibravenös in childhood--clinical and pharmacokinetic problems].

The paper describes special advantages of doxycycline, especially also in comparison with other tetracyclines, the possbile side-effects of the preparation, and the various ways of application-under special consideration of the pathogenetic spectrum. Pharmacokinetic examination were carried out in eight permature babies, nine new-born, eight normal babies, and in two groups of school children, comprising 7 and 8 test persons each. During these examinations, the serum levels after 1, 6, 10, and 24 hours were evaluated. The values found showed levels in the range of bacteriologic efficiency as far as premature babies and new-born - after 24 hours and with 0.6 gamma/ml - were concerned. In older babies and school children, the values had decreased, after a single application, within 24 hours to 0.2 to 0.15 gamma/ml. However, it has been possible to achieve final values of about 1.5 gamma by applying the preparation twice, i. e. 2 x 2 mg/kg body-weight, within 24 hours. The complete course of elimination of Vibravenös was determined in 8 school children by blood sample taking after 5, 10, 15, and 20 minutes as well as after 1, 3, 6 hours. The values found herein were calculated by means of computer analyses. Half-life of doxycyclin in infantile individuals is subject to rather strong variations. Therefore, a higher dose than usual so far is recommended, viz 6 MG/KG B.W. as initial dose and 3 mg/kg b.w. as maintenance dose.

[The significance of immunologic phenomenons in pulmonary diseases of children (author's transl)].

Under special conditions the lung develops reactions of hypersensitivity, e.g. by the contact with antigens consisting of foreign proteins, organic dusts or different pathogen organisms. It must be distinguished between uncomplicated allergic and autoimmune diseases. The exact separation is difficult, because transitions from one state to the other are possible. The autoimmunisation is a very complicated phenomenon, where endogensou cells are effective as auto-antigens and induce the development of auto-antibodies. From the pathological-anatomical point of view thate are no typical structural changes in the lung, which define a disease as a special autoimmune disease. But the prevalence of round cells in inflamed tissue, the appearance of granuloma with epithelial cells point to immunological phenomenons. Examples for allergic diseases are: "Asthma bronchiale, pigeon-breeder disease, mite-allergy, farmer's lung, drug-induced allergy", and for autoimmune diseases: "Lupus erythematosus, rheumatoid arthritis, polyarteritis nodosa, idiopathic pulmonary hemosiderosis, idiopathic pulmonary fibrosis." The drug-therapy consists of corticoids and immune-suppressive drugs.
