Papillary Hürthle cell tumor of thyroid: report of a case with a cytomorphologic approach to diagnosis.

BACKGROUND: Hürthle cells are founded often in fine needle aspirates of the thyroid from nonneoplastic and neoplastic lesions. Papillary Hürthle cell tumors of the thyroid are composed of oxyphilic cells with a prominent papillary component. Papillary Hürthle cell carcinoma is a relatively uncommon type of thyroid carcinoma. CASE: A 42-year-old man had two asymptomatic nodules (2.5 and 3.5 cm) in the left lobe and one (2.5 cm) in the right lobe of the thyroid. Fine needle aspiration biopsy of the tumor in the left lobe showed large cells in monolayers and papillary clusters with moderate to abundant eosinophilic, granular cytoplasm with sharp borders, a granular nuclear enlargement with pleomorphism and single nucleoli. Histologic examination showed capsular invasion and infiltration of vessels. CONCLUSION: The cytologic appearance of papillary Hürthle cell carcinoma is not specific but can be suspected on fine needle aspiration.

Pathogenesis of primary internal ophthalmoplegia after head injury.

Primary internal ophthalmoplegia is a rare ocular syndrome after head injury. Its pathogenesis remains unclear and it is only a hypothesis that the pupillomotor fibres on the ventromedial aspect of the oculomotor nerve are slightly contused against the petroclinoid ligament at the moment of impact. We observed a patient after head injury who presented with a fixed, dilated pupil and died within 15 hours of the accident. The postmortem examination revealed an isolated group of fibres on the medial aspect of her oculomotor nerve, which had been torn out of the midbrain. This suggests an alternative explanation in terms of partial injury to the oculomotor nerve to account for primary internal ophthalmoplegia.

[Proliferative activity of glial neoplasms of the brain].

The aim of the study including 89 brain gliomas was to determine their proliferative activity assayed with immunohistochemical methods (PCNA and Ki-67) and with the method of AgNORs, as well as to evaluate the correlation between the proliferative activity and features of histological malignancy. The study reveals that the estimation of PCNA, Ki-67 and AgNORs are effective methods for the determination of the proliferative activity of brain gliomas. Statistically significant differences were noted in the proliferative PCNA, Ki-67 and AgNORs between groups of gliomas with lower and higher malignancy, which indicated a distinct correlation between histological malignancy of the tumours and their proliferative activity. High values of PCNA and Ki-67 (> 40%) and AgNORs (> 15) were found to considerably deteriorate prognosis in brain gliomas.

[A sporadic case of Creutzfeldt-Jakob disease with peculiar neuropathological lesions].

The authors report a case of Creutzfeldt-Jakob disease in a man aged 60 years from a family without a history of similar disease. The disease extended over 11 months. In the clinical picture initially equilibrium disturbances and dementia with psychotic symptoms predominated, EEG pattern was not typical of CJD. Neuropathological examination revealed extensive spongiform lesions in the cortex of all cerebral lobes, in striatum and substantia nigra, moreover a considerable number of kuru plaques was found in cerebellar cortex. The authors consider that the case meets the criteria accepted for the sporadic form of CJD but believe that the final differentiation from the Gerstmann-Streussler-Scheinker syndrome should be based on genetic studies.

[A case of tuberculosis of the central nervous system].

A case of meningoencephalic tuberculosis in a 38-year old man is presented. The diagnosis was confirmed by postmortem and histological examinations. The clinical picture was dominated by transverse lesion of the spinal cord at C3-C7 level that was due to specific inflammation of blood-vessel walls and massive thromboses found in their lumina.

[20-year cooperation between the I Department of Internal medicine of the District Hospital and the Department of Pathological Anatomy, Medical Academy, in Białystok (analysis of diagnostic consistency)].

Statistical analysis of 1421 deaths (98% of all who had died at the Ward) of patients during 1965-1984 was performed. The material was divided into 3 groups according to the age: up to 59 years, from 60 to 69 and over 70 years. Among those who had died men prevailed (55.1%), but both among women and men the age was over 70 years. In the 2nd decade of the analysed period the authors found gradual increase of death percentage among elderly people, that can be ascribed to "geriatrization" of the Ward and higher accessibility to the hospital for this group of patients. Statistical analysis which included a 10-year period (1975-1984) dealt with the causes of deaths according to the clinical and pathological recognition. There dominated deaths of cardiovascular diseases (59.4%), twice increased the percent of deaths due to neoplasia and diabetes complications. On the other hand, the proportion of decreased of heart infarction fell (from 33% to 16.9%) as did of cardiac defects (from 9.5% to 5.9%). This decrease was the effect of hospital structure changes, i.e.a creation of the Ward of Cardiology and Intensive Cardiological Care in 1981. During 1981-1984 the proportion of deaths of complications of arterial hypertension, mainly in the persons at very old age, increased threefold. In this group of age there dominated deaths of cardiac infarction and circulatory insufficiency. The authors made an analysis of the conformability of clinical diagnosis and the results of autopsy. Out of 1421 deaths in 1149 cases (80.8%) the results of autopsy fully agreed with clinical diagnosis. As partially conformable the clinical diagnosis was in 15.5%, and divergent in 3.7% of cases. The latter, according to the authors, resulted from an incomplete or unproper interpretation of diseases symptoms influenced by several objective factors. As could be expected, the highest proportion of divergent diagnosis was found in the group of the eldest patients afflicted with cardiovascular and malignant diseases.
