[Protective effect and mechanism of morphine on acute myocardial ischemia/reperfusion injury in rats].

OBJECTIVE: To explore the protective effect of morphine and its mechanism on acute myocardial ischemia/reperfusion (AMIR) injury in rats, by the method of detecting calcitonin gene-related peptide (CGRP) and endothelin-1 (ET-1) levels, as well as myocardial infarct size. METHODS: Forty SD rats were randomly divided into four groups: ischemia/reperfusion group (n=10), morphine preconditioning group (n=10), morphine and naloxone hydrochloride group (n=10), and normal controls (n=10). The animal model of AMIR was established in rats. The left anterior descending branch (LAD) of rat coronary was tied and un-tied. Animals were then sacrificed and hearts were harvested to determine myocardial infarct size by 2, 3, 5-triphenyl tetrazolium chloride (TTC). Radioimmunoassay was used to detect CGRP and ET-1 levels in plasma, and routine method was used to measure creatine kinase isoenzyme (CK-MB) in serum. RESULTS: Plasma ET-1 and CGRP levels were increased significantly than that in normal controls in acute myocardial infarction (AMI) at 10 minutes of LAD tied (all P<0.01). Plasma ET-1 and CK-MB levels in morphine preconditioning group in AMIR at 45 hours of reperfusion were decreased significantly as compared with that in the same group in AMI at 10 minutes, and myocardial infarct size decreased significantly (all P<0.01), while, plasma CGRP levels were markedly increased. Significant differences in those parameters were found between morphine preconditioning group and morphine combined with naloxone hydrochloride group (all P<0.01). CONCLUSION: Intravenous morphine has protective effects on AMI by increased plasma CGRP level, decreased plasma ET-1 level, and reduced myocardial infarct size.

Gain functionalization of silica microresonators.

Erbium-doped solgel films are applied to the surface of silica microspheres to create low-threshold microactivity lasers. This gain functionalization can be applied by use of a number of different dopants, thereby extending the wavelength range of this class of device. Also, by varying the doping concentration and thickness of the applied solgel layer, one can vary the laser dynamics so that both continuous-wave and pulsating modes of operation are possible.

[Influence of large amount of shengmai injection on blood coagulation in patients with chronic heart failure].

OBJECTIVE: To investigate the effect of Shengmai Injection (SMI) on blood coagulation in patients with chronic heart failure (CHF). METHODS: Sixty patients with CHF were randomly divided into two groups, the 30 patients in the treated group were treated with SMI plus conventional treatment of western medicine, and the 30 in the control group treated with conventional treatment alone. The changes of cardiac function were observed and levels of plasma P-selectin, von Willebrand's factor (vWF) and D-dimer were determined. RESULTS: The total effective rate and the markedly effective rate in the treated group were higher than those in the control group respectively. The levels of P-selectin, vWF and D-dimer lowered in both groups significantly after treatment, but the effect of lowering was better in the treated group than that in the control group. CONCLUSION: Hypercoagulative state exist in patients with chronic heart failure. SMI could improve the state in patients, which may reduce the occurrence and developing of emboic events to certain extent.

[Prospective study of systemic inflammatory response syndrome of patients with nosocomial G- bacteria infection].

OBJECTIVE: To study the characteristics of systemic inflammatory response syndrome (SIRS) of inpatients with nosocomial G- bacteria infection in order to find on effective treatment. METHODS: Eighty-two inpatients of SIRS with lower respiratory tract infection with G- bacteria were studied prospectively until discharge or death. They were divided into two groups: observation group (42 cases) and control group (40 cases). Bacteria culture of sputum and drug sensitivity was performed. Routine treatment was carried out in the control group, and rhubarb and antibiotics with lower endotoxin releasing property were given to the observation group. The course of SIRS, the incidence of MODS, and the mortality were compared. RESULTS: The duration with SIRS in observation group and control group was respectively (6.2+/-1.3) days and (7.4+/-1.2) days, u=3.91, P<0.05; the incidence of MODS was 11.4 percent and 32.3 percent, respectively, chi(2)=4.27, P<0.05. The mortality rates of the patients with SIRS in two groups were 8.6 percent and 29.0 percent, respectively. CONCLUSION: The results indicated that the treatment with rhubarb could obviously reduce the duration of SIRS compared with routine method. The same is true in the incidence of MODS and mortality rate.

[Phase II clinical trial on gemcitabine in the treatment of stage IIIB-IV non-small cell lung cancer].

OBJECTIVE: To evaluate the efficacy and safety of domestic Gemcitabine in the treatment of patients with stage IIIB approximately IV non-small cell lung cancer (NSCLC). METHODS: 124 NSCLC patients were randomized into three groups: Group A: single drug group, 40 cases, gemcitabine 1 000 mg/m(2) + NS 100 ml or 200 ml was infused within 30 approximately 60 minutes on D1, 8 and 15, with 28 days taken as one cycle. Group B: combined treatment group, 36 cases, in addition to the above protocol, cisplatin 30 mg/m(2) was infused within 60 approximately 120 min, on D1, 2 and 3. Group C: combined control group: 39 cases, the protocol applied was the same as group B except domestic gemcitabine being replaced by imported gemzar. The efficacy and side effects of treatment were evaluated after 8 weeks of treatment. RESULTS: 115 patients could be evaluated for response rate. PR was observed in 9/40 (22.5%) of group A, 15/36 (41.6%) in group B and 15/39 (38.36%) in group C. There was no significant difference of PR rates between group B and group C (P = 0.552). 117 patients who received the second cycle of treatment were evaluated for toxicity. The incidence of grade III approximately IV nausea, vomiting and loss of appetite was much higher in group B. Hematological toxicity of groups B and C was higher than that of group A. There was no significant difference between groups B and C. CONCLUSION: The efficacy and incidence of side effects between domestic gemcitabine and the imported gemzar are similar.

Randomized comparisons among health informatics students identify hypertutorial features as improving web-based instruction.

Hypertutorials optimize five features - presentation, learner control, practice, feedback, and elaborative learning resources. Previous research showed graduate students significantly and overwhelmingly preferred Web-based hypertutorials to conventional "Book-on-the-Web" statistics or research design lessons. The current report shows that the source of hypertutorials' superiority in student evaluations of instruction lies in their hypertutorial features. Randomized comparisons between the two methodologies were conducted in two successive iterations of a graduate level health informatics research design and evaluation course. The two versions contained the same text and graphics, but differed in the presence or absence of hypertutorial features: Elaborative learning resources, practice, feedback, and amount of learner control. Students gave high evaluations to both Web-based methodologies, but consistently rated the hypertutorial lessons as superior. Significant differences localized in the hypertutorial subscale that measured student responses to hypertutorial features.
