[Comparison between PET-CT and MRI in diagnosing nodal metastasis of nasopharyngeal carcinoma].

BACKGROUND & OBJECTIVE: Nodal metastasis pattern of nasopharyngeal carcinoma (NPC) influences treatment planning. This study was to compare the diagnostic value of integrated positron emission tomography-computed tomography (PET-CT) with that of magnetic resonance imaging (MRI) in detecting nodal metastasis of NPC, and explore the regulation of nodal metastasis of NPC. METHODS: Clinical data of 105 NPC patients, treated in Cancer Center of Sun Yat-sen University from Jun. 2003 to May 2004, were analyzed. All patients underwent PET-CT and MRI. Findings of PET-CT and MRI were evaluated with the results of follow-up. Distribution of metastatic nodes in different nodal groups was assessed. RESULTS: Among the 105 patients, nodal metastasis patterns shown on PET-CT and MRI were diverse in 35 patients. Thirty cervical nodes were positive on PET-CT, but negative on MRI; 25 of them were later confirmed positive by follow-up. Thirty-seven cervical nodes were negative on PET-CT, but positive on MRI; 21 of them were confirmed negative by follow-up. Based on the results of follow-up, 77 patients (73.3%) had nodal involvement. Level II nodes were the most frequently involved (87.0%). Along the jugular chain, the frequencies of nodal metastases in level III, level IV, supraclavicular fossa, and level VII were 42.9%, 14.3%, 7.8%, and 1.3%, respectively. The frequency of nodal metastases in retropharyngeal nodes was 70.1%. Cervical nodes involvement without retropharyngeal nodes involvement was seen in 23 patients (29.9%). CONCLUSIONS: The diagnostic accuracy of PET-CT in detecting nodal metastases of NPC is better than that of MRI. The nodal metastases of NPC reveal a decreasing frequency along the jugular chain. Both retropharyngeal and level II nodes are the first-echelon nodes of NPC.

[Experimental study of chronic renal tubular-interstitial injury induced by radix aristolochiae fangchi extract in rats].

OBJECTIVE: To observe the acute and chronic renal toxicity induced by Radix Aristolochiae Fangchi Extract (RAFE) in different doses in rats. METHOD: The conventional method of acute toxicity was used. RAFE at the dose of 25.0 mg x kg(-1) x d(-1), 120.0 mg x kg(-1) x d(-1) and 200.0 mg x kg(-1) x d(-1) and aristolochic acid (AA, 10.0 mg x kg(-1) x d(-1)) were interruptedly administrated to rats for 13 week by gastric tube, and the sample of blood, urine and kidney were collected at 4 week, 8 week and 13 week respectively. The indexes of renal function were measured and the morphology of kidney was observed. RESULT: LD50 of RAFE was 36.8 g x kg(-1) (the crude drug) and the 95% confidence limit was 38.8 - 28.9 g x kg(-1). The changes of renal functions were azotemia, massive proteinuria and the increase of urinary NAGase (beta-N-acetylglucosaminidase) in the earlier period of administration with RAFE in rats. Pathological changes of renal tissue were as follows: acute renal tubular necrosis mainly in the boundary of cortex and medulla was observed in the earlier period, and with the elongation of administration, the pathological process of renal interstitial fibrosis observed in the middle and high groups of RAFE and AA group. CONCLUSION: RAFE at middle and high doses administrated by interrupted gavage above 13 week can cause the injury of renal tubular functions in rats. NAGase can be used as one of observation targets in the earlier period of renal injury.

[P38 MAPK was involved in angiotensin-induced migratory potential of adventitial fibroblasts from spontaneously hypertensive rat].

OBJECTIVE: To test whether P38 MAPK is involved in angiotensin II (Ang II)-enhanced migration potential of adventitial fibroblasts (AFs) from spontaneously hypertensive rat (SHR). METHODS: Migratory potential was estimated by transwell chamber in vitro. Activation of P38 MAPK pathway was determined with phosphospecific antibodies by immunoblotting. RESULTS: Ang II induced migration of SHR-AFs was markedly increased in a dose-dependent manner when compared with WKY-AFs. Addition of the Ang II receptor type-1 (AT1-R) antagonist Losartan and P38 MAPK inhibitor SB202190 suppressed Ang II-induced migration of SHR-AFs. Ang II could induce P38 MAPK phosphorylation in SHR-AFs in a time-and dose-dependent manner. Phosphorylation of P38 MAPK was suppressed by Losartan and SB202190. CONCLUSION: This study indicated that Ang II-induced migration involves P38 MAPK pathway via AT1 receptor in aortic adventitial fibroblasts from SHR.

[Protective effect of tetramethylpyrazine on the respiratory inhibition induced by hypoxia in rats].

OBJECTIVE: To study the protective effect of tetramethylpyrazine (TMP) on the respiratory inhibition induced by hypoxia. METHODS: Forty-four SD rats were used. The hypoxic rat model was established. The effects of TMP on the responses of respiratory activity and life time to hypoxia were observed. The Nissl's staining and immunohistochemical technique were used to investigate the effect of hypoxia on the Nissl's bodies and FOS expression in the brainstem and the antagonistic action of TMP. RESULTS: In contrast to the group of hypoxia, the occurrence of respiratory inhibition was delayed and the life time was prolonged obviously in the rats of hypoxia plus TMP (P<0.01). Compared with the air contronl group, the intensity of the Nissl's staining of the brainstem neurons became weaker in the group of hypoxia, but in the group of hypoxia plus TMP, the Nissl's staining was stronger and the gray degree decreased obviously, compared with the group of hypoxia (P<0.05). FOS positive neurons were induced by hypoxia and mainly existed in the nucleus of solitary tract, area postrema, hypoglossal nucleus, lateral reticular nucleus, inferior olivary nucleus, nucleus raphe pallidus, facial nucleus, trapezoid nucleus, but in the group of hypoxia plus TMP, the level of FOS immunoreactivity decreased remarkably, compared with the group of hypoxia (P<0.05). CONCLUSION: TMP has protective effect on the respiratory inhibition induced by hypoxia and FOS may be involved in the protection.

[Synthesis of purpure-18-imide and study on its interaction with bovine serum albumins by fluorescence].

Purpure-18-imide was synthesized based on the porphyrin's special affinity for cancer cells and the antitumor activity of pyrrolidine compound. The binding reaction between purpure-18-imide and bovine serum albumins (BSA) in aqueous was studied by fluorescence and UV-Vis absorption spectra. The research results indicated that the combination reaction of them was a single static quenching process. In aqueous, purpure-18-imide strongly bound BSA with molar ratio of 1:1. The binding constant K0 was 5.386 x 10(5) L x mol(-1). The shortest binding distance (r = 3.54 nm) and energy transfer efficiencies (E = 0.26) between donor (BSA) and acceptor (purpure-18-imide) were obtained by Forster's nonradiative energy transfer mechanism.

[Effectiveness and safety of rhIL-11 in the treatment of chemotherapy-induced thrombocytopenia].

OBJECTIVE: To investigate the effectiveness, safety and possible mechanism of recombinate human interleukin 11 (rhIL-11) in the treatment of chemotherapy-induced thrombocytopenia. METHODS: Thirty-four patients (totally 76 cycles) with chemotherapy-induced thrombocytopenia received subcutaneous injection of rhIL-11 at the dose of 25 microg.kg(-1).d(-1) for 4 to 16 days. Serum IL-11 level was measured by ELISA, and IL-11 R alpha expression was detected by RT-PCR. RESULTS: The mean baseline platelet count before chemotherapy was (135.0 +/- 54.3) x 10(9)/L for the 1st cycle and (259.4 +/- 64.5) x 10(9)/L for the 2nd cycle. The time to administer rhIL-11 was 7 to 16 days (median 12 days) in the 1st cycle and 4 to 10 days (median 6 days) in the 2nd, respectively (P < 0.05). The duration of post-chemotherapy platelet count below 50 x 10(9)/L was 7 to 13 days (median 10 days) for the 1st cycle and 3 to 8 days (median 5 days) for the 2nd, respectively (P < 0.05). Platelet count reached 300 x 10(9)/L or above in 30 chemotherapy cycles. The maximum platelet count was found to appear at D10 to D 17 (median D14), and negatively correlated with the pre-chemotherapy serum IL-11 level after administration of rhIL-11. Major adverse reactions included edema, headache, muscle and joint pain. CONCLUSION: rhIL-11 is effective and safe for the treatment of chemotherapy-induced thrombocytopenia, with a relatively slow but sustained effect on the recovery of platelet count. Pre-chemotherpy serum IL-11 level might predict the efficacy of rhIL-11.

[Study with large slice technique and tissue microarray on lymph node involvement and micro-metastasis of rectal cancer].

OBJECTIVE: To study lymph node involvement and micro-metastasis of rectal cancer with large slice technique and tissue microarray. METHODS: Large slice technique, combined with tissue microarray, was used in pathologic study of 31 patients after total mesorectal excision (TME) for rectal cancer. RESULTS: Nine hundred and ninety- two lymph nodes were harvested and 148 were positive. More than 40% of positive lymph nodes were located in the outer layer of the mesorectum and in the same side of the mesorectum as the primary tumor was. Circumferential margin involvement was observed in 12 cases and correlated with the numbers of metastatic lymph nodes (Beta =1.166, P=0.041). Micrometastasis was found in 9 cases with negative pathological lymph nodes, but not correlated with tumor differentiation and stage (P> 0.05). CONCLUSION: Large slice technique combined with tissue microarray facilitates the detection of lymph node involvement and micrometastasis. There is a predominance of lymph node metastasis in the outer layer and the same side of the mesorectum. Micrometastasis can be discovered in different stages of rectal cancer.

[Nephrotoxicity of Aristolochia manshuriensis and aristolochic acids in mice].

OBJECTIVE: The acute toxic effects of Aristolochia manshuriensis (GMT) and the total aristolochic acids (TA) were compared in mice with aristolochic acid A (AA) as the dose standard. The dose relationship of the renal toxicity induced by Aristolochia manshuriensis was determined. METHOD: A single dose of GMT extract or TA was given intragastrically to mice at different doses. LD50 values, the blood levels of BUN, Cr and ALT were measured. A histomorphological study was also performed in livers and kidneys of mice. RESULT: LD50 value of GMT extract was 4.4 g x kg(-1) which was equivalent to 40 mg x kg(-1) as calculated by the content of AA in GMT extract, and this value was comparable with LD50 obtained from TA given intragastrically in mice (equivalent to 33 mg x kg(-1) of AA for male and 37 mg x kg(-1) for female). GMT extract caused a significant increase in blood BUN and Cr and an obvious morphological change in kidney in a dose-dependent manner at doses of AA 4.5 mg x kg(-1) and above. Liver damage, characterized by both an increase in blood level of AST and histomorphological change, was observed at doses of AA 25 mg x kg(-1) and above. All changes were in proportion to the doses of AA. CONCLUSION: GMT causes both renal and liver toxicity. The dose leading to nephrotoxicity is much lower than that inducing hepatatoxicity. Aristolochic acids existed in GMT are the main toxic components to cause renal toxicity which is a crucial cause to result in death. The lethality and nephrotoxicity of GMT is in proportion to the doses of AA.

[Changes of cell adhesion molecules and proinflammatory cytokines during cardiopulmonary bypass and the effect of intervention].

OBJECTIVE: To investigate the changes of the cell adhesion molecules and proinflammatory cytokines during cardiopulmonary bypass, and to observe the effect of intervention with ulinastatin. METHODS: Twenty-two ASA II-III patients (9 males, 13 females), aged 20-60 years, undergoing cardiac operation with CPB were randomly divided into 2 groups: the control group (Group C, n=11) and the ulinastatin group (Group W, n=11). In Group W, patient received ulinastatin 1.2 x 10(4) U/kg, and half of the dose was given intravenously after the induction of anesthesia, while the same amount of ulinastatin added into the primary solution. And in Group C, normal saline was given instead of ulinastatin. Blood samples were taken from radial artery before the operation (T1), 20 min after the initiation of CPB (T2), 1 h (T3), 6 h (T4 ), 24 h (T5) after the CPB for the determination of plasma TNF-alpha, IL-6, sICAM-1 and sP-Selectin concentrations. RESULTS: The concentrations of TNF-alpha, IL-6 increased significantly at T2-T4 in both groups compared with T1 (P < 0.05), and returned to the baseline level at Ts in Group W. The concentrations of TNF-alpha, IL-6 in Group C at T2-T5 were higher than that in Group W (P < 0.01). The concentrations of sICAM-1, sP-Selectin increased significantly at T3, T4 in both groups compared with that at T1 (P < 0.05). But at T5, the concentrations of sICAM-1, sP-Selectin decreased, especially in Group W the concentrations of sICAM-1, sP-Selectin returned to the baseline level. The sI-CAM-1, sP-Selectin concentrations in group C at T4, T5 were higher than that in group W (P < 0.05). CONCLUSION: Ulinastatin can reduce the increase of the cell adhesion molecules and proinflammatory cytokines during cardiopulmonary bypass and effectively weaken the inflammatory response to CPB.

[Effect of 2-methoxyestradiol on human cervical cancer cells HeLaS3 and xenografts].

OBJECTIVE: To evaluate the effects of 2-methoxyestradiol (2-ME(2)) on human cervical cancer HeLaS3 cells and cervical cancer xenografts. METHODS: Cell proliferation assay and cell cycle analysis were used to measure HeLaS3 cell growth and cell cycle progression after 2-ME(2) treatment. Fluorescent microscopy to observe the cell morphology and DNA electrophoresis to measure apoptosis. In addition, the effect of 2-ME(2) on the expression of inducible nitric oxide synthase (iNOS) was measured by Western blot. Moreover, human cervical cancer model was set up using HeLaS3 cells and 2-ME(2) (75 mg/kg) was orally given for 14 d. Tumor volume was determined and apoptosis was detected by in situ cell death. RESULTS: Newly-formed cell amount in treated group was 81% of that in control group after 1 micromol/L 2-ME(2) treatment for 48 h (P < 0.05), and was 19% of that in control group after 2-ME(2) treatment for 96 h (P < 0.01). G(2)/M phase cells were increased to 55% from 16% of the control (P < 0.01), and apoptotic cells were increased to 16% from 4% of the control, after 5 micromol/L 2-ME(2) treatment for 20 h. Nuclear condensation and abnormal metaphase cells were found by fluorescent microscopy. Typical DNA ladder was found by DNA electrophoresis. And the expression of iNOS was increased by 2-ME(2) in a time- and concentration-dependent manner, in parallel with apoptosis. Moreover, apoptosis was prevented by the iNOS inhibitor 1400W. In vivo, tumor volume was reduced 34% while compared with the control group. In situ cell death detection found more apoptotic and necrotic cells in 2-ME(2)-treated group. CONCLUSIONS: 2-ME(2) inhibits human cervical cancer HeLaS3 cells and tumor growth in cervical cancer xenografts. Thus 2-ME(2) has the therapeutic potential for cervical carcinoma.
