[Changes in clinical characteristics and interventional modalities of renal artery stenosis in recent years].

OBJECTIVE: To observe the changes of clinical characteristics and interventional methods of patients with renal artery stenosis in recent 17 years. METHODS: Comparing the clinical characteristics and interventional treatment of patients diagnosed renal artery stenosis by the means of renal angiography in recent 17 years. RESULTS: The numbers of renal artery stenosis were increasing year by year. the average ages before the year of 2000 was younger than that after 2000 (P < 0.05) and the main cause of renal artery stenosis was aorto-arteritis at that time, however, it turned to atherosclerosis after the year of 2000. Same patients indicated renal angiography due to plash pulmonary edema, unstable angina on abnormal serum creatinine. The interventional methods to renal artery stenosis shifted from balloon angioplasty only before the year of 2000 to stent implantation post-balloon dilatation after the year of 2000. The distal protective device was more used during the procedure. CONCLUSIONS: The numbers of patients indicated renal angiography had been increasing in the two decades. The main causes of renal artery stenosis had been considered arthro-sclerosis instead of aorta-arteritis before. The methods of treatment to renal artery stenosis had been changed to renal stenting post-balloon angioplasty after the year of 2000.

[Experimental study of chronic renal tubular-interstitial injury induced by Radix Aristolochiae Fangchi Extract in rats].

OBJECTIVE: Following the former report, we continue to observe the chronic renal tubular-interstitial injury induced by Radix Aristolochiae Fangchi Extract(RAFE) in rats in order to understand whether RAFE in different doses causes the renal tubular-interstitial injury or not. METHOD: RAFE at the dose of 25.0 mg x kg(-1) x d(-1), 120.0 mg kg(-1) x d(-1) and 200.0 mg x kg(-1) x d(-1) and aristolochic acid (AA, 10.0 mg x kg(-1) d(-1)) was interruptedly administrated by gastric tube for 22 w and 4 w durg withdrawal. Blood, urine and kidney were taken out respectively in 17 w, 22 w and 26 w to measure the indexes of renal function. The morphology of kidney was observed, and Masson staining of kidney were made respectively to compare RAFE groups with AA group. RESULT: Pathological changes of renal tissue forms were as follows: All RAFE groups and AA group could develop the pathological process of renal tubular injury-chronic renal interstitial fibrosis. The pathologic changes of RAFE were similar with AA. CONCLUSION: RAFE at all doses administrated interruptedly by gastric tube above 13 w caused chronic renal tubulo-interstitium fibrosis. The renal injury in functions and tissue forms in rats were similar with AA closely. The results showed that AA was the main toxic composition of RAFE.

[Comparison of treatment efficacy of the second and the third generations of chemoradiotherapy regimens containing platinum on advanced non-small cell lung cancer].

BACKGROUND & OBJECTIVE: Platinum-based chemotherapy regimens are better than other regimens when treating patients with advanced non-small cell lung cancer (NSCLC). The third generation of platinum-based regimens {NP [Navelbine (NVB), cisplatin (DDP)], TP (Taxol, DDP), GP (Gemzar, DDP)} is better than the second generation {MVP [mitomycin (MMC), desacetylvinblastin amide (VDS), DDP], MIP [MMC, ifosfamide (IFO), DDP], EP [etopside (VP-16), DDP], VDS+DDP}. This study was to compare the efficacy between the second and the third generations of platinum-based regimens combined with radiotherapy on advanced non-small cell lung cancer (NSCLC). METHODS: From Jul. 1999 to Dec. 2001, 47 pathologically confirmed advanced NSCLC patients received chemoradiotherapy: 24 received the second generation of platinum-based regimens, 23 received the third generation; all patients received routine external irradiation of (60)Co (65-76 Gy). Characteristics of the patients were comparable. Kaplan-Meier analysis was used to evaluate survival rates, and log-rank test to study differences between the 2 groups. RESULTS: The objective response rates were 41.7% in the second generation group, and 56.5% in the third generation group (Chi (2)=0.53, P=0.47). The median time to progression and median survival time were significantly longer in the third generation group than in the second generation group (12.6 months vs. 6.0 months, Chi(2)=6.93, P=0.01; 14.0 months vs. 9.0 months, Z=-2.17, P=0.03). The 1-, and 2-year survival rates were significantly higher in the third generation group than in the second generation group (56.3% vs. 30.7%, 15.6% vs. 0%, Chi(2)=6.59, P=0.01). The major adverse events were leukocytopenia, nausea and vomiting, radiation-induced esophagitis and pneumonia, which were tolerable. CONCLUSIONS: The third generation of platinum-based regimens in combination with radiotherapy for advanced non-small lung cancer may be more advantageous over the second generation of platinum-based regimens in prolonging survival of advanced NSCLC patients, which would be worth being studied further.

[The expression and role of VEGF-C and its receptor FLT-4 in laryngeal and hypopharyngeal squamous cell carcinoma metastasis].

OBJECTIVE: To investigate the expression of VEGF-C and its receptor FLT-4 in laryngeal and hypopharyngeal squamous cell carcinoma, and to analyse its role in lymphatic metastasis and micrometastasis. METHOD: Forty nine cases postoperative specimens verified by histopathology were studied. The expression of VEGF-C, FLT-4 were detected with immunohistochemical SP method, image analysis method. The clinicopathological parameters of tumor were compared with the immunohistochemical results of VEGF-C and FLT-4, followed by the further analysis on the relationship between VEGF-C/FLT-4 expression and lymph nodal metastasis or micrometastasis. RESULT: There was a statistical correlation between the expression of VEGF-C and grades of pathological differentiation in laryngeal and hypopharyngeal squamous cell carcinoma. Moreover there was also a statistical correlation between the expression of VEGF-C and lymph node metastases. But no correlation was observed between the expression of VEGF-C and clinical T grades. The statistical correlation also existed between VEGF-C and FLT-4 expression in laryngeal or hypopharyngeal tumors. There was a significant difference of expression of FLT-4 in laryngeal and hypopharyngeal tumours with positive and negative lymph node metastases. No correlation was observed between FLT-4 and grades of pathological differentiation or clinical T grades. There was no statistical correlation between the expression of VEGF-C/FLT-4 and nodal micrometastasis (P > 0.05). CONCLUSION: VEGF-C and its receptor FLT-4 play an important role in the lymphatic metastasis of laryngeal and hypopharyngeal squamous cell carcinoma. VEGF-C/FLT-4 may be a useful biological marker to foresee the cervical lymph node metastasis and prognosis.

Substantial reaction between histamine and malondialdehyde: a new observation of carbonyl stress.

OBJECTIVES: To explore the mechanism of carbonyl stress-related toxification on neurotransmitter histamine and the potential de-carbonylation function of histamine. METHODS: The reaction mixture of malondialdehyde (MDA) and histamine (HA) at pH7.4, 37 degrees C was assayed by high performance liquid chromatography (HPLC), spectrophotometry and spectrofluorometry. The reaction products were assayed by LC/MS. RESULTS: In physiological condition, the reaction of MDA and HA yielded two products: a nonfluorescent enaminal derivatives and a fluorescent 1,4-dihydropyridine adducts. The fluorescence maxima of the fluorescent products (Ex. 393 nm/Em. 464 nm) were similar to those of lipofuscin pigment. The fluorescence intensity of reaction mixture was in direct proportion to the MDA concentration. CONCLUSIONS: HA can react with MDA to form stable products, a non-fluorescent enamine (product 1) and a fluorescent 1,4-dihydropyridine (product 2) which are ceroid/lipofuscin-related adducts. The reaction of HA with MDA may reveal toxic effect of unsaturated carbonyls in the brain and may reflect a novel de-carbonylation function of histamine under various pathological conditions.

[Clinical characteristics and ultrastructural features of livers in children with Wilson disease manifested mainly as hepatic injuries].

OBJECTIVES: To study the feasibility and possibility to diagnose Wilson disease with electronmicroscopical examination of liver biopsies. METHODS: Clinical analysis, histological observation and ultrastructural examination were performed on 15 children with Wilson disease. RESULTS: All 15 subjects had symptoms of hepatic disorders, such as jaundice. Morphological signs of hepatocyte injury in three phase, namely steatosis, mitochondrion changes and cholestasis in bile canaliculi of the early phase, nucleus injury, dilation of endoplasmic reticulum, increase of lysosomes and appearance of residual bodies of the second phase, and massive autophagy and cirrhosis of the late phase were shown. A few inflammatory cells in the liver specimens were observed. Accumulation of copper in lysosomes and autophagosomes was found by energy-dispersion X-ray. CONCLUSION: The diagnostic signs for Wilson disease are autophagosomes in hepatocytes, cirrhosis accompanied with a few of inflammatory cells. A certain diagnosis of the disease depends on the finding of copper accumulation in hepatocytes.

[Preliminary study of VEGF and its receptor expression on childhood acute lymphoblastic leukemia and its relativity to clinical manifestations].

OBJECTIVE: To investigate the relationship between differential expression of VEGF and its receptors and clinical characteristics of childhood acute lymphoblastic leukemia (ALL). METHODS: Expressions of VEGF and its receptors (Flt-1, KDR) were assayed by ELISA and RT-PCR in healthy donors(20 cases), ALL patients in remission (20 cases), with low risk (29 cases) and with high risk (10 cases). The clinical data of all the patients and volunteers enrolled in this study were collected and analyzed according to the expression of VEGF and its receptors. RESULTS: The expressions of VEGF were (574.37 +/- 208.45) ng/L, (387.93 +/- 175.86) ng/L, (135.80 +/- 111.28) ng/L and (91.16 +/- 41.34) ng/L in patients with high risk, standard risk, in remission and healthy donors, respectively. Expression levels of VEGF receptors were downwards with risk grades. The clinical manifestations were also in accord with the expression levels of VEGF and its receptors. CONCLUSION: ALL patients with highly expressed VEGF and its receptors are usually with higher tumor burden, and refractory treatment. Detection of VEGF and its receptors might be one of prognostic marker for ALL treatment.

[Effects of RhG-CSF on apoptosis of HL-60 cell induced by cytarabine].

To investigate the effects of rhG-CSF on apoptosis of HL-60 cell induced by cytarabine (Ara-C), HL-60 cell were cultured with Ara-C, with or without rhG-CSF, and the changes of HL-60 cells were detected by morphology, including Wright's and acridine orange (AO) and ethidium bromide (EB) staining, and MTT assay. The results showed that comparing with Ara-C group, the apoptosis level of rhG-CSF + Ara-C group was enhanced (P < 0.01), and the OD value of MTT assay declined (P < 0.01). It is concluded that rhG-CSF can induce the apoptosis of HL-60 cells and enhance the apoptosis induced by Ara-C.

[Effective control of excitable waves in 2D cardiac excitable media].

We propose a method for effective control of patter on dissipative system by use of little perturbation analysis, and apply this nonuniform feed back method to control the polarization wave of heart represented by FHN equation. In the numerical experiment, we successfully alter the propagating direction of planar waves and drift the spiral waves to boundary without resonant repulsion. The effective control of the excited system will be used to study the mechanism of defibrillation and that is our interesting work.

[Effects of ginsenoside Rh2(GS-Rh2) on cell cycle of Eca-109 esophageal carcinoma cell line].

OBJECTIVE: To investigate the effects of ginsenoside Rh2 (GS-Rh2) on growth inhibition and cell cycle of Eca-109 esophageal carcinoma cell line in culture. METHOD: The effects of GS-Rh2 on cell growth inhibition was detected by MTT assay. Cell cycle was analyzed by flow cytometry (FCM). Cell morphology was observed by a light microscope after HE staining. The protein expression of cell cycle components (cyclinE, CDK2, p21WAF1) were examined by immunocytochemistry and Western blot. The mRNA expression were examined by semiquantitative RT-PCR. RESULT: GS-Rh2 inhibited the proliferation of Eca-109 cells in dose and time-dependent manners. The inhibition rate was about 50% after 1-day treatment with 20 microg x mL(-1) GS-Rh2 x 20 microg x mL(-1) GS-Rh2 induced the mature differentiation and morphological reversion. With increasing dose of GS-Rh2 treatment, the cell number of G0/G1 phase was increased, whereas it decreased at S and G2/M phase. There was significant difference between 10, 20 microg x mL(-1) GS-Rh2 groups and the corresponding group without GS-Rh2 treatement. After treating cells by 20 microg x mL(-1) GS-Rh2 for 1, 2, 3 days individually, the protein and mRNA expression of both cyclinE and CDK2 reduced, while the expression of p21WAF1 enhanced gradually. CONCLUSION: GS-Rh2 could arrest Eca-109 cells at G0/G1 phase and induce cell differentiation tending to normal. Furthermore, GS-Rh2 had an effect on expression of cell cycle components (cyclinE, CDK2 and p21WAF1) to inhibit Eca-109 cell proliferation.

[Changes of neutrophil myeloperoxidase in coronary circulation among patients with acute coronary syndrome].

OBJECTIVE: To investigate the changes of neutrophil myeloperoxidase (MPO) blood concentration gradient between the systemic circulation and the coronary circulation among patients with acute coronary syndrome and its clinical value. METHODS: Fifty patients underwent coronary angiography, which including 10 patients in AMI group, 20 patients in UA group, 10 patients in SA group and 10 subjects served as control. The levels of MPO and hs-CRP were measured in the serum of blood collected from femoral vein, aortic artery root and coronary sinus. RESULTS: Compared with the control, concentrations of LDL in the AMI, UA and SA groups were significantly increased, while the latter three groups did not differ from each other. In the UA patients, the in-gate percentage of MPO decreased in the coronary sinus compared with that in the root of aortic artery (P < 0.01); the in-gate percentage of MPO decreased through coronary circulation more than through systemic circulation (P < 0.001); the average fluorescent intensity of MPO and the concentrations of hs-CRP showed no difference between samples from the coronary sinus and that from the root of aortic artery. In the AMI patients, the average fluorescent intensity of MPO in the coronary sinus was weakened compared with that in the root of aortic artery (P < 0.05); it decreased through coronary circulation more than through systemic circulation (P < 0.001); neither the in-gate percentage of MPO nor the concentrations of hs-CRP showed significant difference between samples from the coronary sinus and that from the root of aortic artery. In the control and SA groups, samples from the femoral vein, the root of aortic artery, and the coronary sinus did not show differences at the serum level of MPO and hs-CRP. In the UA group, the in-gate percentage of MPO correlated positively with the concentration of hs-CRP (r = 0.78, P < 0.01), and with the level of LDL as well (r = 0.52, P < 0.05); In the AMI group, the average fluorescent intensity of MPO correlated negatively with the concentration of hs-CRP (r = -0.80, P < 0.01), and showed no correlation with the level of LDL (r = 0.22, P > 0.05). CONCLUSIONS: MPO is a better marker for inflammation of the local plaques. It may be one of the mechanisms that MPO induces the transforming from LDL to ox-LDL in plaques vulnerability.

Dendritic cells generated from acute myeloid leukemia (AML) blasts maintain the expression of immunogenic leukemia associated antigens.

Recently, the focus is on new specific immunotherapies for AML such as cellular therapies employing dendritic cells (DCs) generated from AML blasts. AML-DCs express constitutionally leukemia-associated antigens (LAAs) present in AML blasts they are generated from. Here we investigated whether the generation of AML-DCs would alter the expression level of LAAs. Moreover, we evaluated the presence of HLA and costimulatory molecules on AML blasts versus AML-DCs. Quantitative real-time polymerase chain reaction (PCR) was performed for the following LAAs: preferentially expressed antigen in melanoma (PRAME), the receptor for hyaluronic acid mediated motility (RHAMM/CD168), Wilms' tumor gene 1 (WT-1) and proteinase 3. The expression of HLA-ABC, HLA-DR, CD40, CD80, CD83 and CD86 was evaluated by flow cytometry. RHAMM protein expression was evaluated by immunocytochemistry, recognition of AML-DCs by PRAME epitope-specific T cells was evaluated in a chromium-release assay. Quantitative real-time PCR for AML-DCs versus AML blasts showed an alteration in mRNA expression of LAAs. An elevated PCR signal for PRAME was detected in 7/12 AML-DC preparations. 6/12 AML-DC preparations showed a significant upregulation of the PCR signal for RHAMM. A stronger WT-1 and proteinase-3 signal was observed in PCR for only 2/12 and 1/12 AML-DCs , respectively. All preparations showed a strong expression of at least one of the LAAs examined. As demonstrated by flow cytometry, AML-DCs strongly upregulated costimulatory molecules like CD40 and CD80 in comparison with AML blasts. AML-DCs tested positive for RHAMM protein. PRAME positive AML-DCs were recognized by specific T cells. AML-DCs might constitute a powerful tool in immunotherapy for AML. Real-time PCR allows a quick and quantitative assessment of immunologically relevant LAA expression with only 10(5) DCs and might be helpful for the decision whether the AML-DC vaccination strategy is favourable or not.

Effect of contrast on the active control of a moving line.

In many passive visual tasks, human perceptual judgments are contrast dependent. To explore whether these contrast dependencies of visual perception also affect closed-loop manual control tasks, we examined visuomotor performance as humans actively controlled a moving luminance-defined line over a range of contrasts. Four subjects were asked to use a joystick to keep a horizontal line centered on a display as its vertical position was perturbed by a sum of sinusoids under two control regimes. The total root mean square (RMS) position error decreased quasi-linearly with increasing log contrast across the tested range (mean slope across subjects: -8.0 and -7.7% per log2 contrast, for the two control regimes, respectively). Frequency-response (Bode) plots showed a systematic increase in open-loop gain (mean slope: 1.44 and 1.30 dB per log2 contrast, respectively), and decrease in phase lag with increasing contrast, which can be accounted for by a decrease in response time delay (mean slope: 32 and 40 ms per log2 contrast, respectively). The performance data are well fit by a Crossover Model proposed by McRuer and Krendel, which allowed us to identify both visual position and motion cues driving performance. This analysis revealed that the position and motion cues used to support manual control under both control regimes appear equally sensitive to changes in stimulus contrast. In conclusion, our data show that active control of a moving visual stimulus is as dependent on contrast as passive perception and suggest that this effect is attributed to a shared contrast sensitivity early in the visual pathway, before any specialization for motion processing.

Anti-IL-13 monoclonal antibody inhibits airway hyperresponsiveness, inflammation and airway remodeling.

Asthma is a chronic inflammatory disease characterized by reversible bronchial constriction, pulmonary inflammation and airway remodeling. Current standard therapies for asthma provide symptomatic control but fail to target the underlying disease pathology. Furthermore, no therapeutic agent is effective in preventing airway remodeling. Interleukin 13 (IL-13) is a pleiotropic cytokine produced mainly by T cells. A substantial amount of evidence suggests that IL-13 plays a critical role in the pathogenesis of asthma. Therefore, a neutralizing anti-IL-13 monoclonal antibody could provide therapeutic benefits to asthmatic patients. To test the concept we have generated a neutralizing rat anti-mouse IL-13 monoclonal antibody, and evaluated its effects in a chronic mouse model of asthma. Chronic asthma-like response was induced in ovalbumin (OVA) sensitized mice by repeated intranasal OVA challenges. After weeks of challenge, mice developed airway hyperresponsiveness (AHR) to methacholine stimulation, severe airway inflammation, hyper mucus production, and subepithelial fibrosis. When given at the time of each intranasal OVA challenge, anti-IL-13 antibody significantly suppressed AHR, eosinophil infiltration, proinflammatory cytokine/chemokine production, serum IgE, and most interestingly, airway remodeling. Taken together, these results strongly suggest that a neutralizing anti-human IL-13 monoclonal antibody could be an effective therapeutic agent for asthma.

Intercalation, delivery, and expression of the gene encoding green fluorescence protein utilizing nanobiohybrids.

Non-viral gene vectors have attracted much attention recently due to setbacks with viral delivery systems. Here a new non-viral delivery system based on nanobiohybrids synthesized by the intercalation of a full gene and promoter encoding Green Fluorescent Protein (GFP) between the layers of an inorganic host is reported. The nanobiohybrids were delivered to 9L glioma cells, JEG3 choriocarcinoma placental cells, and cardiac myocytes. All cells were able to internalize and tolerate the nanobiohybrids. In addition, all cells expressed the gene with some cell lines having up to 90% transfection efficiency. This new, bio-mimetic delivery system shows promise for use in non-viral gene therapy.
