[Effectiveness of diet therapy in patients with ischemic heart disease and a history of myocardial infarction at young age].

To elucidate the effect of dietetics on the onset and course of coronary heart disease (CHD) patients who survived myocardial infarction at young age were investigated. The analysis of clinical and metabolic changes, psycho-vegetative disorders permitted the authors to distinguish two groups of patients: group 1--patients with normal body mass, and group 2--those with excessive body mass who received, respectively, A1 (basic) and Ar (reduced) variants of theanti- arteriosclerotic diet. It was established that the Ar diet produced positive effects on the dynamics of body mass reduction, on the cardiovascular system, on parameters of lipid metabolism, hemostasis and psycho-physiologic parameters of group 2 patients. Diet therapy combined with tranquilizers is recommended for group 1 patients.

[Effects of anti-atherosclerosis diet containing omega 3 fatty acids on the lipid spectrum of blood and cell membranes in patients with ischemic heart disease and essential hypertension].

A total of 27 patients received an anti-atherosclerotic diet containing fatty fish Greenland halibut, 100 g/day. Significant reduction of the total cholesterol level, cholesterol levels of very low and low density proteins, triglycerides, atherogenic index, elevation of blood fibrinolytic activity in the presence of lowered levels of arachidonic acid and an increased relative content of omega 3 polyunsaturated fatty acids (PUFA) were recorded in membranes of red blood cells and platelets. The results of the investigations conducted have shown that even rather low doses of omega 3 PUFA introduced in the presence of the hypolipidemic diet are conductive to the alimentary correction of the blood serum lipid spectrum of the fatty-acid content in cellular membrane structural lipids.

[The effect of dietotherapy and supplementary calcium intake on phosphorus-calcium homeostasis in hypertensive patients].

A total of 25 patients with essential hypertension received a hyponatrium, hypocaloric antisclerotic diet providing 800 mg Ca and 1100 mg P/day, during 18-20 days. As a result of the dietotherapy, Ca concentration in the patients' blood serum rose from 9.9 +/- 0.2 to 10.4 +/- 0.1 mg/100 ml (p = 0.02), total cholesterol level was lowered from 6.58 +/- 0.43 to 5.42 +/- 0.3 mmol/l (p less than 0.05), Na+ and K+ distribution between the blood plasma and red blood cells was improved, and the arterial blood pressure was normalized in all the patients investigated. It dropped from 162 +/- 3.8/102 +/- 1.8 mm Hg before the treatment to 129 +/- 2.1/83.4 +/- 2.2 mm Hg after the treatment. Additional intake of 850 mg Ca/day, as CaCO3, decreased parathormone level in the blood serum from 0.40 +/- 0.03 to 0.23 ng/ml (p less than 0.01), intensified the hypolipidemic effect of the ration, and did not influence the degree of arterial blood pressure reduction under the action of this ration. Optimization of Ca consumption plays an important role in the combined dietotherapy of essential hypertension.

[The role of emotional-volitional traits in patients with ischemic heart disease in carrying out dietetic recommendations during long-term clinical observation].

The influence of subjective (individual) prerequisites on the possibility of long-term following anti-atherosclerotic diets by coronary patients was investigated. Basing on the examination of the patients they were divided into three groups: those who strictly kept the diet (I), patients who moderately violated the diet (II), and those who did not keep the diet at home (III). Specific psychological methods used in the study allowed differentiation of these groups of patients. A conclusion has been made on the relation between the capacity for keeping the diet and some psychological personality traits of coronary patients. The analysis of the results enabled the authors to reveal a complex of problems for prognosing the coronary patients' readiness to keep the diet at home. The data obtained may be conductive to a rise in the medicosocial adaptation of coronary patients to long-term diet therapy by means of directed psychotherapy.

[Anti-atherosclerotic properties of some components of food (a clinico-experimental study)].

Platelet lysosomal proteinase and phospholipase activity, cholesterol content in the blood serum and circulating immune complexes, as well as blood serum atherogenic potential were studied with the use of human aortal intima atherosclerotic cell culture in patients with postinfarction cardiosclerosis (males aged from 45 to 62 years), 1-3 h before and 2-6 h after a single intake of one of the following products (50 g): sunflower oil, butter, cod fat, glucose, fructose, starch, dried milk protein, or 22 g of soybean protein isolate 500 U. The total data obtained have evidenced that the soybean protein isolate produces hypolipidemic and hypocholesterolemic effects, as well as possesses pronounced anti-atherosclerotic properties.

[The effect of the alimentary factor on erythrocyte ATPase activity in ischemic heart disease patients].

Functional disorders in the ion-transport ATPase system were detected in young and middle-aged males suffering from coronary heart disease (CHD). The degree of the ATPase activity impairment (general ATPase, Mg-ATPase and Na, K-ATPase) correlated with hyperlipoproteinemia (HLP) which was revealed in CHD patients. Different direction of changes was observed in the concentrations of cations in plasma and erythrocytes, that, in some cases, could not be correlated with the degree of HLP and suppression of ion-transport ATPase activity. Positive correlation was observed with respect to increased concentration of Mg2+ in serum and Na+ in erythrocytes, and hypercholesterolemia, as well as with respect to decreased concentration of Mg2+, suppressed Mg-ATPase activity, and hypo-alpha-cholesterolemia. A significant tendency to normalization of parameters of lipid metabolism and lipid-dependent ion-transport ATPases was recorded in CHD patients after a course of the anti-atherosclerotic dietotherapy.

[Effect of wheat bran included in an anti-atherosclerotic diet on the lipid metabolic indices of ischemic heart disease patients].

A study was made of the effect of wheat bran contained by the antisclerotic diet on lipid metabolism in patients with chronic coronary heart disease. Eighty-two patients (males) aged 36-59 years with a history of myocardial infarction of 1- to 15 year-standing were entered into the study. It was ascertained that inclusion into the antisclerotic diet of wheat bran in an amount of 70-80 g does not exert any hypolipidemic action. No elevation of the serum alpha-cholesterol content was noticed either. The augmentation of the cholic acid concentration together with a reduction in the cholesterol content in the hepatic bile attests to its reduced lithogenicity.

[Ways of improving the diet therapy of industrial workers].

A conceptual model of the organizational system of the diet therapy of workers at industrial enterprises has been developed. The system analysis permitted the authors to reveal its ineffective components and to outline the ways for their correction. Approbation of the system at industrial enterprises contributed to the improvement of the social-medical and social-economic effectiveness of the diet therapy under industrial conditions.

[Effect of pectin on indices of lipid and carbohydrate metabolism in patients with ischemic heart disease].

The influence of the diet containing 10 g of citrus pectin, made by "Copenhagen-Pectin-Fabric", on some parameters of lipid and carbohydrate metabolism was studied in 33 coronary heart disease male patients, aged from 29 to 40 years, who had survived documented myocardial infarction. The results of the investigation have evidenced that increased pectin content in the diet promotes the reduction of the total cholesterol and triglyceride levels in the blood serum, the rise of tolerance to carbohydrates and produces no appreciable effect on the lithogenic potential of bile in the coronary patients.

[Thrombocyte lysosomal hydrolase activity in patients with ischemic heart disease, hyperlipidemia and obesity against a background of different diets].

Investigation of lysosomal hydrolase activity in platelets of patients has revealed drastic activation of cathepsins B, C and phospholipase A1, the degree of which rose in the following range: coronary heart disease; coronary heart disease aggravated by obesity: obesity and hyperlipidemia (type II). Administration of the adequate dietotherapy resulted in normalization of enzymologic parameters, whereas the results of the clinico-biochemical analysis of the blood were less informative in all cases. The data obtained could be used in the evaluation of the dietotherapy effectiveness, as well as for the early diagnosis of the corresponding diseases.

[Comparative evaluation of the hypolipidemic action of sunflower oil and "Zdorov'e" margarine incorporated into the anti-atherosclerotic diet of ischemic heart disease patients].

Patients with ischemic heart disease attended by type II hyperlipidemia received diets containing sunflower oil (group I) or margarine "Zdorovie" (group II) as a source of poly-unsaturated fatty acids. Pronounced reduction of the blood cholesterol level was recorded in both groups of patients studied. Margarine "Zdorovie" is recommended for subjects intolerant to vegetable oils.

[Effect of diets with a varying protein content on fibrinolysis in ischemic heart disease].

A comparative influence of an antiatherosclerotic diet with a reduced proportion of protein (65--70 g) and of a diet with an amount of protein in line with the physiological standard (100--110 g) on the fibrinolysis and lipids spectrum of the blood was studied in 60 patients with ischemic heart disease. The antiatherosclerotic diet is shown to have most favourable effect both on the fibrinolytic system and on the lipids metabolism with the protein proportion of 100--110 g, i.e. in keeping with the physiological standards. A reduction of the protein content in the diet of patients with ischemic heart disease lowers the effect of treatment and does not have any positive influence on the function of the fibrinolytic system.

[Changes in the fatty acid composition of erythrocyte and platelet membranes after diet therapy in patients with hyperlipoproteinemia type II].

Fatty acid spectrum of blood plasma, erythrocyte, and thrombocyte membranes as well as its alterations after diet therapy, containing polyunsaturated/saturated fatty acids at the ratio of I and less than 350 mg of cholesterol per a day, was studied in patients with hereditary hypercholesterolemia using gas chromatography. Content of linoleic acid was increased and that of oleic and arachidonic acids--decreased in lipids of the patients blood plasma as compared with healthy persons; an increase of linoleic acid was also detected in thrombocyte lipids. Alterations in fatty acid spectrum of erythrocyte membranes were less pronounced. After the course of diet therapy within I month a further increase in content of linoleic acid was observed in blood plasma. Under these conditions efficiency of essential fatty acids incorporation into lipid structures of thrombocyte membranes was higher and of erythrocytes--lower in the patients as compared with healthy persons. Possible alimentary correction of fatty acid spectrum in blood lipids is discussed.

[A computer-assisted method of evaluating the actual nutrition of patients with ischemic heart disease during their clinical observation].

The questionnaire method was used for the study of the actual nutrition of coronary patients after their discharge from the Clinic for Dietotherapy, during dispensary follow-up. The chemical composition of their diets was evaluated with computer, taking into consideration the losses during food preparing, and compared with the diets recommended. Certain variations in the chemical formula of the patients' home diets were revealed using this method. It is stressed that the dietetic recommendations given to coronary patients should be followed, while their physicians should thoroughly control their adherence to the diet.
