[Juxtapapillary capillary hemangioma and central serous retinopathy: description of a case with spontaneous resolution].

CASE REPORT: A patient presented with a unilateral decrease of visual acuity and was found to have a juxtapapillary capillary hemangioma by ophthalmoscopy (confirmed by fluorescein angiography) and atrophy in the retinal pigmentary epithelium with leakage points suggesting bilateral central serous retinopathy. Spontaneous resolution of the hemangioma, as well as the central serous retinopathy, subsequently occurred. DISCUSSION: The decrease of visual acuity in a patient with a juxtapapillary capillary hemangioma has been associated with complications from the lesion itself, but an association of central serous retinopathy, as a possible predisposing factor, has not been previously described.

Relation between axial length of the eye and hypotensive effect of latanoprost in primary open angle glaucoma.

AIMS: To study the effect of axial length on the hypotensive effect of latanoprost in primary open angle glaucoma (POAG) in a prospective, observational study. METHODS: The authors measured axial length and baseline intraocular pressure (IOP) of 109 eyes with POAG, and then repeated the IOP measurements at 1, 3, and 6 months after starting treatment with latanoprost. RESULTS: The mean IOP level was significantly lower in eyes with a shorter axial length compared with the eyes with a longer axial length both at 3 and 6 months of treatment (p = 0.03 and p = 0.04, respectively, ANOVA). CONCLUSION: The hypotensive effect resulting from treatment with latanoprost could be related to ocular axial length.

Incidence of iris colour change in latanoprost treated eyes.

AIMS: To determine the incidence of increased iris pigmentation in eyes treated with chronic latanoprost. This is an observational, cohort study. METHODS: The anterior segments of both eyes of 43 patients with glaucoma were photographed after unilateral latanoprost therapy. Two independent, masked observers analysed the slides of both eyes of the study participants, and compared the iris pigmentation of both eyes of each patient using a stereo viewer. RESULTS: 30 patients (69.7%) had a definite acquired iridial anisochromia; 15 patients (50%) had increased superficial iris pigmentation with a "granular" appearance (type 1 change), and 15 patients (50%) had an increased "stromal pigmentation"-that is, the affected iris appeared darker than the fellow eye, but without a "granular" appearance (type 2 change). CONCLUSION: The incidence of increased iris pigmentation induced by latanoprost is much higher than previously reported. The high prevalence of mixed iris colour found in this population and the fact that the patients studied were undergoing unilateral therapy may explain these results.

[Retinal hemorrhages following phacotrabeculectomy. Differential diagnosis].

PURPOSE: We report a case of a patient who underwent uneventful facotrabeculectomy. Right after surgery patient she complained of very poor vision, showing hemorrhage spots scattered throughout the retina. Differential diagnosis is studied. CONCLUSION: An ocular decompression retinopathy should be the first diagnosis; however clinical details and diagnosis tests suggest the ocular ischemic syndrome as the reason of the problem.

Intraocular pressure as a risk factor for visual field loss in pseudoexfoliative and in primary open-angle glaucoma.

OBJECTIVE: To analyze the relationship between intraocular pressure (IOP) and visual field loss in patients with primary open-angle glaucoma (POAG) and in those with pseudoexfoliative glaucoma (PEXG). DESIGN: A cross-sectional, observational study. PARTICIPANTS: Thirty-one patients with PEXG and 31 patients with POAG that was newly diagnosed were included in this study. MAIN OUTCOME MEASURES: The authors recorded the untreated IOP and the amount of the visual field loss, at presentation, in both study groups. RESULTS: The authors found a significant relationship between IOP and visual field mean deviation (MD) index (P = 0.0001, r = 0.68) in PEXG but not in POAG eyes (P = 0.7). CONCLUSION: The authors found that untreated IOP levels can explain the amount of visual field loss, as measured by the MD index, much better in patients with PEXG than in comparable patients with POAG. Thus, vulnerability of the optic nerve head to increased IOP appears to be different in these two diagnostic categories.

Viscoelastic-injecting cystotome.

This continuous curvilinear capsulorhexis (CCC) technique is for use in complicated surgical cases such as when the anterior chamber is shallow, the red reflex is not good, or eye movements are present. This technique is easier and safer in such cases because it uses a cystotome connected to a viscoelastic syringe. First, the anterior chamber is filled with viscoelastic material using a conventional cannula. The cannula is replaced with a bent needle (or cystotome), and the CCC is performed in the usual way. This instrument allows the surgeon to inject small amounts of viscoelastic material exactly where and when it is needed. The anterior chamber remains deep while the CCC is performed, and the anterior capsule tear is done in a more controlled fashion.

Ocular hypotensive effect of pilocarpine before and after argon laser trabeculoplasty.

PURPOSE: The purpose of this study was to compare the hypotensive response of human glaucomatous eyes to 1% pilocarpine solution before and after argon laser trabeculoplasty. METHODS: We designed a prospective, 'repeated measures' study comparing the ocular hypotensive effect of acutely administered 1% pilocarpine solution in 30 medically uncontrolled glaucomatous eyes, before and after argon laser trabeculoplasty. RESULTS: The pilocarpine-induced IOP decrease was 3.6 +/- 0.59 mmHg before argon laser trabeculoplasty, and 1.8 +/- 0.53 mmHg after laser therapy. In both instances, the pilocarpine hypotensive effect was significantly correlated with pretreatment IOP (p = 0.02, r = 0.41 before argon laser trabeculoplasty, and p = 0.003, r = 0.52 after argon laser trabeculoplasty). We built a 'repeated measurements ANCOVA' model, and found no statistically significant difference between the pilocarpine IOP lowering effect before and after argon laser trabeculoplasty (p = 0.4). CONCLUSION: Argon laser trabeculoplasty does not modify the IOP lowering effect of 1% pilocarpine solution.

Presentation and long-term follow-up of exfoliation glaucoma in Greece, Spain, Russia, and Hungary.

PURPOSE: To evaluate clinical presentation and long-term follow-up of exfoliation glaucoma in separate European population groups. METHODS: A four-center, retrospective, case series analysis in which 200 charts of patients with exfoliation glaucoma or patients with elevated intraocular pressure (IOP) associated with exfoliation syndrome in at least one eye with at least 5 years of follow-up were consecutively reviewed. RESULTS: This study found an average follow-up time of 6.0+/-2.1 years. Patients in Hungary and Spain statistically presented at an older age (79 years) than Greek patients (67 years). Patients with exfoliation glaucoma in Greece and Hungary had more glaucomatous damage, had more severe glaucoma, had a higher untreated IOP (31.8 to 32.1 mmHg), and were more difficult to control, showing a greater number of changes in medicines during the follow-up period, a greater number of medicines at the end of the follow-up period, and more progression. On long-term follow-up, Greek, Russian, and Hungarian patients also had the highest mean IOP (18.8 to 20.8 mmHg) and the greatest incidence of progression (approximately 50%). Spanish patients demonstrated the lowest mean IOP (17.6+/-3.6 mmHg) and the lowest rate of progression (28%) during the follow-up period and the fewest number of medications per patient (0.7) to control the IOP at the end of the follow-up period. CONCLUSIONS: The severity of exfoliation glaucoma presentation and its course may differ within distinct geographic populations in Europe.

Toxic keratolysis from combined use of nonsteroid anti-inflammatory drugs and topical steroids following vitreoretinal surgery.

PURPOSE: To evaluate the corneal complications associated with the combined use of non-steroid anti-inflammatory drugs (NSAIDs) and topical steroids following vitreoretinal surgery. METHOD: Description of corneal lesions in three patients after vitrectomy with use of topical ketorolac and prednisolone acetate. RESULTS: Three eyes of three patients developed an atrophic central corneal ulcer with stromal thinning following a pars plana vitrectomy under local anesthesia. Lesions were asymptomatic and were found during a routine examination 2, 3, and 8 weeks after surgery, respectively. Surgical indications were as follows: a preretinal membrane, choroidal neovascularization, and massive uveal effusion following cataract surgery. Topical postoperative treatment was as follows: ketorolac 4 times a day, a combination of prednisolone acetate, polymyxin B, and neomycin 6 times a day, and 1% cyclopentolate 3 times a day. Suspension of ketorolac and ocular occlusion led to the resolution of corneal lesions between 2.5 and 3 months later, yielding a central superficial scarring, which showed no changes after a follow-up of 3 years. CONCLUSIONS: Toxic keratolysis may appear as a secondary effect of the combined use of topical NSAIDs and steroids following vitreo retinal surgery and must be taken into account in the differential diagnosis of postoperative corneal lesions.

Iris melanocytoma mimicking the Cogan-Reese syndrome with monocular pigment dissemination.

PURPOSE: To report a case of iris melanocytoma mimicking the Cogan-Reese syndrome. METHODS: A 37-year-old woman presented with pigmentary glaucoma in her left eye. There was diffuse pigmentary dispersion in the anterior segment, pedunculated pigmented nodules on the anterior iris surface, mild iris atrophy, and ectropion iridis. Neither intrinsic vasculature nor a sector cataract was found. The angle was open with marked trabecular pigmentation and no anterior synechiae. The intraocular pressure was 30 mmHg with maximum medical treatment and there was glaucomatous optic atrophy. The differential diagnosis included iris pigmented tumor and iridocorneal endothelial syndrome (Cogan-Reese syndrome). An iris biopsy was performed for diagnostic purposes. RESULTS: Histologic diagnosis after evaluation of the specimen was iris melanocytoma. CONCLUSIONS: This case presents signs considered quasi-pathognomonic of iridocorneal endothelial syndrome (Cogan-Reese syndrome): glaucoma, mild iris atrophy associated with pedunculated iris nodules, and ectropion iridis. Therefore, iris melanocytoma can present with features that mimic the Cogan-Reese syndrome.
