Epidural extension of a lumbar vertebral haemangioma.

Vertebral haemangiomas are relatively common, but those extending into the epidural space are rare. A 59-year-old man with severe lower back and right leg pain that did not resolve with conservative treatment was seen in an outpatient clinic. Magnetic resonance imaging of the lumbar spine identified an L3 vertebral corpus lesion with epidural extension. The diagnosis was unclear, so the patient underwent surgery. The pathologic diagnosis was capillary haemangioma, so angiography-guided embolization was performed postoperatively. Vertebral haemangioma must be considered when there is evidence of a vertebral corpus lesion with epidural extension on magnetic resonance imaging.

Can sarcoidosis cause autoamputation of a finger phalanx?

Sarcoidosis is a multisystemic granulomatous disease. In the case presented, autoamputation of the distal phalanx of a little finger of the left hand was observed. The possibility of autodigital amputation as a result of sarcoidosis is discussed.

The preparation of ciprofloxacin hydrochloride-loaded chitosan and pectin microspheres: their evaluation in an animal osteomyelitis model.

Ciprofloxacin hydrochloride-loaded microspheres were prepared by a spray-drying method using pectin and chitosan. The effects of different polymers and drug ratios were investigated. The most appropriate carriers were selected by in vitro testing. A rat methicillin-resistant Staphylococcus aureus osteomyelitis model was used to evaluate the effects of the loaded microspheres. The drug was released rapidly from the pectin carrier but this was more sustained in the chitosan formulation.Chitosan microspheres loaded with ciprofloxacin hydrochloride were more effective for the treatment of osteomyelitis than equivalent intramuscular antibiotics.

Breast carcinoma with three coexistant type of metaplasia: sarcomatoid, giant cell and squamous differantiation.

We report on a women with metaplastic carcinoma of the right breast. After diagnosis she had a simple mastectomy operation. Pathological investigations were carried out on mastectomy material. Immunohistochemical analyses were performed. Bizarre, malignant giant cells and diffuse inflammatory reactions were predominantly noted. Sarcomatous metaplasia and rare squamous metaplasia were also seen. There was not any area of classic intraductal, invasive ductal, lobular or papillary carcinoma. The patient is still alive and well for more than five years. This uncommon case is described and discussed, and the related literature is reviewed.

Unusual case of laryngeal tumor (capillary hemangioma) obturating the larynx.

OBJECTIVES: Capillary hemangioma isolated to the free edge of the vocal fold is a rare clinical finding. We see many young patients, mostly infants under a year, who are having surgical intervention for laryngeal hemangiomas. A rapid proliferative phase occurs during the first 9 to 12 months of life; this followed by an involution phase that may be completed by 3 to 5 years of age, but can last up to 12 years. MATERIAL AND METHODS: We report the case of a 15-year-old boy who had hoarseness for a two-year related to a laryngeal mass which growth pattern was unknown. RESULTS: On endoscopic examination a polypoid mass, which was diagnosed as a capillary hemangioma later, appeared on the left vocal fold. The lesion was excised with cold-knife instrumentation by using mini-flap technique. CONCLUSION: The clinical and histopathological features and therapeutic options are discussed.

Persistent serpentine supravenous hyperpigmented eruption associated with docetaxel.

Various mucocutaneous reactions have been reported with the use of systemic docetaxel. We describe a 47-year-old man who developed a persistent serpentine supravenous hyperpigmented eruption (PSSHE), beginning at the site of docetaxel injection and spreading along the superficial venous network in the anterior aspect of the right forearm and distal arm. The eruption occurred after the first infusion of docetaxel following insufficient venous washing. A second infusion was administered through a vein in the other forearm, but this time, abundant venous washing was performed and a similar eruption did not occur. To our knowledge, this is the second report of docetaxel-induced supravenous discoloration and we discussed the terminology and mechanism of this unique reaction.

Non-Hodgkin's lymphoma and auricular hypoplasia: associated with juvenile colloid milium or ligneous conjunctivitis?

The exact nature of amyloid-like hyaline material deposits in the skin is not well understood in some disorders. Three of those--ligneous conjunctivitis, ligneous periodontitis and colloid milium--have been rarely reported in a same patient. We report a case of mucosal and skin deposits of an amyloid-like homogeneous material associated with non-Hodgkin's lymphoma and congenital auricular hypoplasia. We discussed and reviewed the literature on these unique associations to determine whether these are the same pathological process. We also noted whether this case represents a new syndrome or a coincidental association.

Morgagni cyst within tubal lipoma.

Both benign and malignant tumors of the fallopian tube are uncommon. Benign tumors are most often of mesodermal origin and usually are small enough to be incidental findings at laparotomy. We describe a fallopian tube lipoma that inserted a Morgagni hydatid cyst in a 48 year old woman.

Penetrating type intraosseous ganglion cyst of the lunate bone.

Intraosseous ganglion cysts of the carpal bones located in the lunate are one of the rarely seen pathologic conditions. Here we present a case of the penetrating type of intraosseous ganglion cyst located in the lunate as an uncommon cause of wrist pain. The patient was successfully treated by surgical intervention. Intraosseous ganglion cyst should be considered in the differential diagnosis of chronic dull wrist pain.

Bilateral multiple piloleiomyomas on the breast.

Piloleiomyomas are benign smooth muscle tumors arising from the erector pilorum muscles in the skin. They vary in size and number, and are often spontaneously painful or sensitive to touch and cold. In this paper, we present two young female patients who have multiple breast piloleiomyoma and discuss the related findings.

Tuberculous orchitis in chronic renal failure.

Tuberculosis confined to the testes with no epididymal involvement is uncommon. Chronic renal failure patients requiring hemodialysis have increased risk for developing tuberculosis. We report a 47-year old chronic renal failure man presenting with right testicular tuberculous orchitis. A high index of suspicion is required to recognize the unusual presentation of tuberculosis in this group of patients, and routine screening for tuberculosis may be recommended in patients undergoing hemodialysis.

Blue nevus with an unusual presentation: two patients with endocervical location.

OBJECTIVE: To report two cases of endocervically located blue nevus. SUBJECT: Case report. SETTING: University of Abant Izzet Baysal, Duzce, Turkey. SUBJECT AND METHOD: Blue nevus is a benign lesion, which is usually found on the skin, rarely on mucous membranes. In two patients the endocervical location of blue nevus was described. CONCLUSION: Blue nevus when located on the cervix may be confused with other benign or malignant lesions.

Measurement of epidermal thickness in a patient with psoriasis by computer-supported image analysis.

The aim of the present study was to measure full epidermal thickness, stratum corneum thickness, rete length, dermal papilla widening and suprapapillary epidermal thickness in psoriasis patients using a light microscope and computer-supported image analysis. The data obtained were analyzed in terms of patient age, type of psoriasis, total body surface area involvement, scalp and nail involvement, duration of psoriasis, and family history of the disease. The study was conducted on 64 patients and 57 controls whose skin biopsies were examined by light microscopy. The acquired microscopic images were transferred to a computer and measurements were made using image analysis. The skin biopsies, taken from different body areas, were examined for different parameters such as epidermal, corneal and suprapapillary epidermal thickness. The most prominent increase in thickness was detected in the palmar region. Corneal thickness was more pronounced in patients with scalp involvement than in patients without scalp involvement (t=-2.651, P=0.008). The most prominent increase in rete length was observed in the knees (median: 491 microm, t=10.117, P=0.000). The difference in rete length between patients with a positive and a negative family history was significant (t=-3.334, P=0.03), being 27% greater in psoriasis patients without a family history. The differences in dermal papilla distances among patients were very small. We conclude that microscope-supported thickness measurements provide objective results.

Subepidermal calcified nodule.

Subepidermal calcified nodules are rarely seen and are classified under idiopathic subepidermal calcifications. They commonly affect children. It is most frequently encountered in the face, and it is less common in the extremities. Here, we present a case where the lesion was localized on the knees of a 23-year-old female patient.

An experimental study on the application of extracorporeal shock waves in the treatment of tendon injuries: preliminary report.

The use of low-energy extracorporeal shock wave therapy (ESWT) has recently been introduced in pseudoarthrosis, fracture, and wound healing for both clinical and experimental purposes. In this study, we created an experimental rat model to investigate the histopathological and biochemical effects of ESWT in the healing of tendon injury that may accompany fractures. Twenty-eight rats were used for the histopathological investigation. The right achilles tendon of the rat was cut and then sutured, using Modified Kessler stitches method. Five hundred shock waves, set at 14 kV, were applied to the achilles tendon under fluoroscopic guidance. Fourteen rats that underwent a sham operation served as the control group. Biochemical studies were done on 20 rats, divided into four groups of 5 rats in each. The first and second groups received 500 shock waves, at 14 kV, after the cutting and repairing of the Achilles tendon. To measure hydroxyproline levels, the rats in the first and second groups were killed on postoperative days 3 and 9, respectively. The third and fourth groups served as controls. Hydroxyproline levels were found to be high in the study groups. The histopathological findings and the increased hydroxyproline levels revealed that ESWT may facilitate tendon healing after trauma.
