Immunological status of patients in carcinoma head and neck.

The cell mediated immune response in 45 patients of cancer, head and neck was evaluated by various parameters and compared with 20 control cases. (DNCB) Dinitrochlorobenzene cutaneous reactivity in vivo was compared with T lymphocytes, estimated as active rosette forming cells (ARFC) and Total rosette forming cells (TRFC) in peripheral blood. The ratio of TRFC/ARFC was studied in various histological subtypes of cancer head and neck. Lymphocytic infiltration around tumor mass was correlated with peripheral blood lymphocyte (PBL), ARFC and TRFC levels in blood. Significant difference was noted in the level of peripheral blood lymphocytes (PBL). TRFC and ARFC between control cases and cancer patients. There was a marked increase in all three parameters i.e. PBL, TRFC, ARFC (P less than 0.001) and a positive DNCB reaction in Adenocarcinoma of nasopharynx, thyroid and salivary gland and a significant decrease in Squamous cell Carcinoma of oral cavity and larynx (p less than 0.001) associated with impaired DNCB sensitivity. These parameters serve as an assessment of the degree of immune reactivity of the host to the malignant tumours.

Significance of serum leucine aminopeptidase in tumours of female genital tract.

Serum leucine aminopeptidase (LAP) was estimated in 57 female patients with tumours of the genital tract and compared with 50 age-matched controls. Serum LAP was increased in leiomyomas (p less than 0.01) but significant rise was seen in leiomyosarcomas and endometrial carcinoma of the uterus (p less than 0.001). No increase in serum LAP level, above mean control value was observed in carcinoma of cervix. LAP levels were also increased in cystadenomas of the ovary (p less than 0.01) and highly significant increase, noted in cystadenocarcinomas and dysgerminoma stage III (p less than 0.001). However, no increase of enzyme activity was observed in poorly differentiated papillary carcinoma of the ovary. The serum enzyme level was higher in cystadenocarcinomas as compared to solid ovarian carcinomas in the same clinical stage.

Aetiology of recurrent aphthous ulcers (RAU).

A significant decrease in the pH of saliva has been detected in patients with recurrent aphthous ulcers (RAU). The decrease in pH was more prominent at night than between meals, and some persistent decrease was noticed even after recovery. The total proteins of saliva in RAU were also significantly decreased. These findings were similar to those observed in patients complaining of peptic ulcers, so it is recommended that patients with RAU should be treated with antacids as well as anticholinergic drugs, and that the possibility of concomitant peptic ulceration should also be investigated.

Intravitreal ceftazidime in a rabbit model: dose- and time-dependent toxicity and pharmacokinetic analysis.

We delineated the dose-and time-dependent retinal toxicity of intravitreal ceftazidime using electroretinography (ERG) in phakic rabbit eyes. Toxicity was evaluated following intravitreal doses ranging from 0.5 to 50 mg/0.1 ml. Eyes were examined prior to injection, at one day, and at seven days after injection. ERG patterns at one and seven days indicated a toxic response to doses of 20 to 50 mg. Doses of 10 mg or less gave a transient depression of the B-wave at one day, but a return to normal was observed at the seven-day evaluation. Pharmacokinetic analysis following intravitreal injection of 2 mg of ceftazidime into uninfected phakic rabbit eyes disclosed vitreous levels of: 1711 micrograms/ml at 0 hr; 1340 micrograms/ml at 8 hr; 451 micrograms/ml at 24 hr; 270 micrograms/ml at 48 hr and 67 micrograms/ml at 72 hr. Peak aqueous humor level was 139 micrograms/ml 8 hr post-injection. Potentially therapeutic drug levels were obtained in the aqueous and vitreous humors following a nontoxic intravitreal dose of ceftazidime in a rabbit model.

Subconjunctival administration of ceftazidime in pigmented rabbit eyes.

The ocular kinetics of ceftazidime, a third-generation cephalosporin, were examined in phakic and aphakic pigmented eyes of rabbits following subconjunctival injection (100 mg). Peak ceftazidime concentrations (mean +/- SE, n = three to five rabbits per determination) were as follows: phakic eyes, 40.2 +/- 7.3 mg/L in aqueous humor and 11.2 +/- 0.6 mg/L in vitreous humor at one hour; aphakic eyes, 30.5 +/- 4.8 mg/L in aqueous humor and 15.8 +/- 2.4 mg/L in vitreous humor at one hour. The ability of ceftazidime to eliminate an incipient bacterial infection was also studied. Ten aphakic rabbits received intravitreal injections of 50 colony-forming units (cfu) of Pseudomonas aeruginosa. Six of the ten immediately received a subconjunctival injection of ceftazidime (100 mg). At 48 hours following injections, four of four control eyes yielded bacterial counts greater than 6.2 X 10(6) cfu/mL. Of the six that received ceftazidime, five were sterile and one yielded 10 cfu/mL.

The safety profile of imipenem/cilastatin: worldwide clinical experience based on 3470 patients.

The relative safety of imipenem/cilastatin for 3470 patients was reviewed to see if the safety profile was similar to that seen for the first 1723 patients treated. The most common clinical adverse experiences were local ones related to the site of intravenous infusion. Gastrointestinal adverse experiences included nausea, vomiting, or diarrhoea. The frequency of pseudomembranous colitis was low (0 X 1%). The most common central nervous system abnormality was seizure. The most common background factor was central nervous system abnormality including prior history of seizure. Dermatological adverse experiences included rash, pruritus and urticaria. Bleeding and decreased renal function were uncommon. The most common laboratory changes included transient increased liver function values, eosinophilia, positive Coombs' test (not associated with haemolysis) and increased platelets. The current clinical and laboratory safety data are similar to those obtained in the early part of the clinical trials.

Double-blind study of ivermectin and diethylcarbamazine in African onchocerciasis patients with ocular involvement.

In a randomised double-blind study, ivermectin was compared with diethylcarbamazine (DEC) and placebo in the treatment of onchocerciasis in 30 male patients from Mali with moderate to heavy Onchocerca volvulus infections and ocular involvement. 10 patients received a single oral dose of ivermectin, 12 mg, 10 received DEC daily for eight days (total dose 1.3 g), and 10 received matching placebo. Patients were examined periodically for twelve months. Punctate keratitis disappeared in 6 of 7 ivermectin patients but increased in DEC patients. Numbers of O volvulus microfilariae (mf) in the anterior chamber decreased slowly and eventually disappeared in most ivermectin patients during the six months following treatment; anterior chamber mf disappeared more rapidly in some patients after DEC, but reappeared within six months of stopping treatment. Both ivermectin and DEC caused a prompt decrease in mean skin mf density; density then increased in both groups over the twelve month observation period, reaching 9% of pretreatment values in ivermectin patients and 45% in the DEC group. Analysis of adult O volvulus from nodules excised at three and twelve months post treatment showed no effect of either drug on viability; however, there was evidence of degeneration of intra-uterine developing mf in the ivermectin group. Side-effects were less frequent and less severe in ivermectin patients than in DEC patients. Ivermectin as a single oral dose appears to be a more effective microfilaricidal drug than DEC in onchocerciasis.

Ultrastructural localization of immunoreactive calcitonin in the two cell types of the ultimobranchial gland of the common toad (Bufo bufo L.).

The ultimobranchial (UB) glands of the common toad Bufo bufo consist of several cellular masses containing two quite different cell types which line a central lumen filled with amorphous material. The morphologically defined Type I cell is akin to a typical calcitonin secretory cell as observed for all vertebrates, with small dense-core secretory granules. On the contrary the Type II cell displays large apical dense bodies which may be related to the secretion and/or absorption of the amorphous material. Cells morphologically related to Type II cells have been described in the UB glands of Sauropsidea and in the UB follicles of mammalian thyroid gland. An immunocytochemical stain using an antiserum raised against synthetic salmon calcitonin demonstrated the specific localization of an immunoreactive product in both Type I and Type II cell granules, suggesting that both cell types could be involved in calcitonin metabolism. Moreover, the presence of immunoreactive calcitonin in the Type II cells of the Bufo UB gland raises the question of the function of the morphologically equivalent cells of other species.

Levamisole therapy: clinical and immunological evaluation in herpetic keratitis.

Thirty patients with herpetic keratitis, recurrent in 17, were examined and submitted to an immunological evaluation for cell-mediated immunity (E rosette forming cells, absolute number of T lymphocytes, skin tests). A group of 15 patients was treated with Levamisole (2.5 mg/kg body weight, 3 days a week), while the control group received no Levamisole. The observation period varied from 6 to 24 months. The clinical follow-up showed a reduction in the severity and duration of herpetic attacks and in the frequency of relapses compared with the control group. The immunological findings indicated a normalization of E rosettes in all the patients (treated and controls).

Phonocephalography extra-cephalic.

Phonocephalography is the amplification and recording of sounds from the surface and cavities of the head aiming for the introduction of a non-invasive pre-angiographic technique. Nasopharyngeal Phonocephalography was found successful for recording of normal tracing. The effect of the extra-cephalic sounds and factors on the tracing were discussed. Phonomyelography is the application of the idea on the vertebral column. The tracing recorded was demonstrated. Modern technology has undergone considerable progress for the introduction of non-invasive techniques in medical diagnosis. Since Laennec introduced his stethoscope auscultation, amplification and recording of sounds in cardiology had reached a respectable diagnostic procedure. This article represents a trial for the introduction of a simple non-invasive pre-angiographic diagnostic technique by amplification and recording of sounds from the surface and cavities of the body.

[Management in Senegal of the 1st efficacy and tolerability studies of ivermectin (MK 933) in human onchocerciasis].

In 1981, 32 adult Senegalese males infected with Onchocerca volvulus but without ocular involvement were treated by groups of 8 with single oral dosages of 5, 10, 30 or 50 micrograms/kg of ivermectin . As soon as the second day after treatment, we observed a marked decrease of the dermal microfilaremic charge after a posology of 30 micrograms/kg. However the decrease is most important in the group subjected to 50 micrograms/kg and 75% of them had no more microfilariae in the dermis 28 days following treatment.
