Serotonin 5-HT(2) receptor stimulation of dopamine release in the posterior but not anterior nucleus accumbens of the rat.

The objective of the present study was to examine the involvement of serotonin 5-HT(2) receptors within the rat nucleus accumbens (Acc) in the regulation of dopamine (DA) release using in vivo microdialysis. Perfusion with the 5-HT(2) agonist (+)-1-(2, 5-dimethoxy-4-iodophenyl)-2-aminopropane (DOI), at concentrations of 25-250 microM, through microdialysis probes located in the posterior Acc increased extracellular DA levels to a maximum of 200% of baseline. DOI-induced increases in the extracellular levels of DA were Ca(2+) dependent and were inhibited by co-perfusion with the 5-HT(2) antagonist LY-53,857. DOI enhancement of the extracellular concentrations of DA was observed when probes were implanted in the Acc core and shell regions posterior to anteroposterior +1.2 mm from bregma, whereas a small reduction in the extracellular levels of DA was observed in the anterior Acc. There were no differences between core and shell subdivisions within either the anterior or the posterior Acc. These results suggest that activation of 5-HT(2) receptors within the posterior, but not anterior, Acc stimulates DA release, indicating rostral-caudal differences in the interactions of 5-HT with DA systems in the Acc.

Unrecognized traumatic atlanto-occipital dislocation.

This report describes the case of a 67-year-old male victim of an auto-pedestrian accident who was alert and neurologically intact on initial presentation. The patient denied head or neck pain and was without evidence of craniocervical injury. A lateral c-spine radiograph did not show any abnormal findings. The patient deteriorated during his trauma evaluation and was refractory to resuscitation. At autopsy, he was found to have an atlanto-occipital dislocation (AOD). The case is discussed and the anatomic, clinical, and radiographic features of atlanto-occipital dislocation are reviewed.

Diffuse well differentiated lymphocytic lymphoma: a clinical study of 22 patients.

An analysis is presented of the clinical and prognostic features of 22 patients diagnosed with diffuse well differentiated lymphocytic lymphoma at Cleveland Metropolitan General Hospital between 1974 and 1987. At presentation, the disease was usually advanced with 86% of patients having stage IV disease and 73% of patients exhibiting bone marrow involvement. Ten second malignancies were documented in 7 (32%) of our patients. The goal of treatment in our patients was palliation except in the 2 stage 1 patients. The response rate to initial treatment was 55% (15% complete), although 75% of patients ultimately responded to some form of therapy; and the mean survival was 58 months, a figure not different from historical controls. There were no differences in survival between patients given simple versus complex therapy at diagnosis or at any time during the course of their disease; and no differences in survival between complete, partial and nonresponders.

The benefits of using humor in health care include stress reduction and improved psychological well-being for patients and the nursing staff. The authors discuss model to incorporate humor into a specific health-care setting, and include a detailed description of two humor programs: the Chuckle Wagon and Nursing Humor Resource Center.

Encore performance. Discovering new directions in gaming.

The use of games as a teaching method for mandatory education programs has increased in popularity in recent years. Games provide a fresh, enjoyable learning experience for content often viewed as relatively dry, boring material. The challenge remains for educators to develop new approaches for learners to maintain interest, enthusiasm, and knowledge of essential content. This article offers a variety of creative approaches that can be utilized to "keep the drive alive" and to continue this effective, efficient, and highly accepted strategy of game use.
