[Type A Wolff-Parkinson-White syndrome associated with left bundle-branch block. Electrocardiographic, vectorcardiographic, and endocavitary electrophysiological study].

The rate association of a "type A" W.P.W. syndrome with a left bundle branch block gives a characteristic electrocardiographic picture:--the left bundle branch block is partially masked, the delay in the basial region of the left ventricle being in part cancelled by pre-excitation--the features of the W.P.W. syndrome are also modified, since the ventricular axis is corrected by the left bundle branch block. The electrocardiographic tracings taken during the various tachycardias and during treatment for arrhythmia, together with the intra-cavitary recordings allow a precise diagnosis to be made. In this case, vectocardiography was particularly useful as it gave a clear demonstration of the median delay of the ventricular loop, the only pathognomic feature of left bundle branch block, during a period when the left bundle of Kent was functioning.

[Bi-directional tachycardia of infrabundle of his origin. Arguments for reciprocal rhythm].

A new case of bi-directional tachycardia is reported. Intracavitary recordings show that the site of the disorder lies below the main trunk of the bundle if His. Analysis of the surface electrocardiogram and of electrical stimulation has led to a number of arguments for the hypothesis of a reentry circuit which uses the right branch in the reverse direction, and then the anterior and posterior sub-branches alternately in the normal direction. The recurrent attacks of tachycardia can be prevented by permanent ventricular stimulation at a rate above that which occurs between the attacks. However, the relief which this affords has not altered the prognosis of this arrhythmia, which remains very poor.

[Cardiac stimulation: intermittent capture and loss of control, immediate lowering of the threshold].

The threshold for pacemaking by electrical stimulation is always above the threshold of excitation; it is possible to observe a progression, with intermittent capture and then loss of control, if the amplitude of the stimulus is progressively increased and then decreased. It has also been shown that, after a few minutes of stimulation, it is possible to reduce the amplitude of the impulse slightly without losing control, which is evidence of an immediate reduction in the threshold for electrical stimulation. Permanent loss of all response has been shown to occur at a lower level than that at which the original intermittent capture occurred. The threshold of excitation itself is therefore also lowered. These phenomena occur within the electrical levels, for which the titles "gradients of resistance" and "gradients of facilitation" have already been proposed. The present study, using bipolar intracavity stimulation of the right side, shows how easily the investigation was carried out in 20 patients, and their individual variations. These gradients are the expression of a constant inherent quality of the heart, and form new parameters of myocardial excitability; they open up new horizons for electrophysiological research, for cardiac stimulation, and for study of disorders of rhythm.

[Comparative study of endocavitary recording and the 24-hour electrocardiogram in the study of sinus function. Apropos of 174 cases].

We compared the results of electrophysiological investigations of sino-atrial node function in a series of 174 patients (including the sino-atrial conduction time (SACT) according to the method of Narula and Strauss, the corrected sino-atrial recovery time (CSART) and, in, 22 patients, the atropine test) with the results of 24 hour Holter monitoring. 73 patients presented a pathological Strauss curve, 47 had a prolonged SACT (Strauss' method) and 19 had a pathological CSART. Of the 22 bradycardic patients, the injection of atropine relieved the bradycardia in 45.5 per cent of cases. 19 patients showed signs of sino-atrial node dysfunction on the 24 hour Holter recording. Of the 35 patients with a surface electrocardiogram (ECG) suspicious of sino-atrial node dysfunction, 21 (60%) had a prolonged SACT and 13 (37.1%) had a pathological CSART: 15 of them (42.8%) showed major electrophysiological abnormalities and 19 (54.3%) had a positive Holter. In two-thirds of the cases of pure sino-atrial bradycardia, the results of electrophysiological and Holter investigations were normal; in 50 per cent of cases of sino-atrial block and/or pause on the surface ECG, major abnormalities were found on both electrophysiology and the Holter monitor. The SACT, estimated according to the method of Strauss, is discorsdant with the SACT estimated according to Narula's method in 46 p. cent of cases. It seems that Strauss' SACT was more sensitive in our study. In 102 cases (59.3%), the SACT and the CSART were concordant. In 10 cases, the SACT was normal while the CSART was pathological and in 42 cases, the SACT was pathological while the CSART was normal.(ABSTRACT TRUNCATED AT 250 WORDS)

[Which tests should be performed after phlebitis and/or pulmonary embolism?].

The lack of knowledge of the origin of venous thrombosis (VT) causes frustration and distress to the clinician. Some results published in the literature suggest a lot of potential aetiologies. Anyhow, we are far from getting a proof that a systematic and exhaustive search for a potential cause, for instance impairment of blood coagulation or infraclinical cancer, could be of benefit for the patient and could present a good cost/efficacy ratio. Consensus recommendations limit the extent of blood coagulation studies only to some precise clinical situations. New biological data evidencing a resistance to activated C protein in 20 to 50 per cent of VT cases, will likely bring new rules. As far as infraclinical cancers are concerned and whether their relationship to idiopathic VT is well-established, it is still doubtful that such systematic heavy and expensive checking up could be of any benefit.
