[HELLP syndrome - a serious complication in pregnancy].

Report on a severe Hellp-Syndrome. The Hellp-Syndrome fully develops in the postpartal period accompanied by a breakdown of the kidney and liver functions. Therapeutical success has been achieved only by plasmapheresis.

Platelet function in familial hypercholesterolaemia in South Africa and the effects of probucol.

Patients with Familial Hyperlipoproteinaemia Type II showed evidence of increased platelet aggregability when compared to normal controls. This abnormality was not influenced by oral therapy with probucol whether or not the serum cholesterol levels were lowered during the study period. Probucol assays indicated adequate compliance during the study period but did not demonstrate any uptake of the drug by platelets. In vitro studies also failed to demonstrate any inhibition of platelet function at therapeutic drug levels although higher levels were inhibitory.

Role of calcium in the regulation of acetylcholine receptor synthese in cultured muscle cells*.

Embroyonic muscles differentiated in vitro were used to study the effects of intracellular Ca2+ ([Ca2+1]i) variations on the amount of acetylcholine receptors ([AChR]) in the cell membrane. 2. Increased Ca2+ concentration in the growth medium ([Ca2+]o) caused a marked elevation of AChR levels, apparently through de novo synthesis. 3. Agents known to increase [Ca2+]i and its accumulation in the sarcoplasmic reticulum (SR), such as ionophore A23187, sodium dantrolene (DaNa), or high [Mg2+]o all enhanced alpha-bungarotoxin (alpha-BGT) binding after 48 h of treatment. 4. Electrical stimulation or caffeine, both affectors of SR calcium release, brought about a decrease in [AChR] probably by suppressing its synthesis. 5. The effects of simultaneous treatment with two AChR-inducing agents, namely, high [Ca2+]o in the presence of tetrodotoxin (TTX) or high [Mg2+]o were not additive, thus suggesting action via a common saturable mediator. 6. Intermediate AChR levels obtained following simultaneous treatments with opposing effects, e.g., electrical stimulation in the presence of high [Ca2+]o or DaNa, suggest contradictory actions on a common mediator. 7. All these observations indicate a strong correlation between SR calcium levels and [AChR] on myotubes; while calcium accumulation in the Sr was followed by increased AChR synthesis, calcium release was accompanied by suppression of receptor synthesis.

Salmonella typhi facial infection. Report of case.

The following case report describes the evaluation, assessment, and treatment of a facial infection due to Salmonella typhi, caused by bites from the family's pet dog. The causative agent and the mode of transmission are unique in that there are no reported cases of this nature in the literature at this time.

Intraoral localized sarcoid lesion.

A case of an intraoral sarcoid lesion is presented. It is histologically similar to a lesion of generalized sarcoidosis, and the various clinical, radiographic, and laboratory modalities employed for a differential diagnosis are discussed.

[Pros and cons in the therapy of hypertensive gestoses. II. Chronic hypertension].

The authors discuss some controversial aspects of the therapy of chronic hypertension in pregnancy. In cases in whom the blood pressure has been treated before conception the therapy should be continued. But there is no agreement about beginning of the hypotensive therapy during pregnancy. Some authors recommend an early treatment, others give hypotensive drugs only for those women who show no fall of the blood pressure in the second trimester or when the blood pressure exceeds 170/110 mm. Hg. These controversial opinions result from our defective notice of the uteroplacental blood flow during a long-continued application of antihypertensive drugs and their dose-response. As hypotensive agents methyldopa, hydralazine and beta adrenergic substances are recommended, on part of internists also medicaments which are used in nonpregnant patients. In severe chronic hypertension premature induction of labor is necessary. In hypertension III degree and IV degree the pregnancy should be interrupted.

Fat emboli syndrome.

A review of the medical records of 30 patients with major trauma was undertaken. One third had fat emboli syndrome characterized by hypoxemia, confusion, and petechiae. Fluid balance was similar for all patients in the study. Hematologic indices had no prognostic value in identifying patients who were to become symptomatic from fat embolization. The serum albumin concentrations were significantly lower in the symptomatic group. The free fatty acid binding capacity of albumin is discussed.

Fiberoptic bronchoscopy following thermal injury.

In a series of 25 burned patients, 15 had inhalation injury as evidenced by fiberbronchoscopic examination. The criteria for post-thermal pulmonary injury were airway edema and inflammation, mucosal necrosis and presence of soot and charring in the airways. This technique of direct visualization of the pulmonary tree can be performed easily, without complications, and provides an objective means for the early diagnosis of inhalation injury in burned patients and prompt institution of treatment.
