Epithelial morphology of duodenal bulb and Campylobacter-like organisms.

We have carried out ultramicroscopical investigations on 30 patients with endoscopical aspects of congestive or erosive duodenitis associated or not with bulbar ulcer in relation to the presence of Campylobacter-like organisms (CLO), considered as a possible etiologic agent of peptic disease. Bacteria were found in all of the 14 patients with duodenal ulcer and in only five patients without ulcer. The ultrastructural studies provided elements for the evaluation of epithelial colonization of the duodenal bulb by CLO and which may lead to a new interpretation of gastric metaplasia.

[The hospitalized drug addict. Social assistance and therapeutic care].

Ninety drug addicts observed in 1973-83 were examined in a study of the main anamnestic and behavioural parameters, the reasons for hospitalisation, the presence of withdrawal symptoms during treatment and any prospects of long-term cure. In conclusion, it is emphasised that the low frequency of real withdrawal pathology often renders "scaled" treatment with methadone superfluous. Finally it is pointed out that in itself, the hospitalisation period can do almost nothing to break the addiction spiral unless it forms part of a wider prospect of global treatment.

[Physical health and drug dependence].

90 hospitalized drug addicts observed in 1979-83 were examined. The investigation of the main anamnestic, clinical and instrumental parameters made it possible to construct a profile of the main physical handicaps affecting drug addicts and the main pathologies they encounter. In conclusion it is emphasised that it is probably not the drug in itself but rather their unhealthy life style that is the primary cause of drug addicts' poor health.

Electrical hazards in endoscopic services.

Despite the extensive use of endoscopic diagnostic procedures, the proper standards aimed at preventing electrical hazards are generally little known and inadequately observed. Cases of electrical injury ensuing from endoscopic manoeuvres may have gone unreported either to avoid legal consequences, or owing to incorrect identification of the true cause of the injury. The authors describe the most important safety standards dealing with the risks connected with the use of both endoscopic devices and electrosurgical units, and recommend that in the future more attention be paid to identifying and preventing these risks.

Pain measurements in right-left cerebral lesions.

Left- and right-brain damaged (BD) subjects were examined to ascertain whether psychophysical parameters of pain-pain threshold (P), tolerance threshold (T), and pain endurance (E)-were modified by brain damage and whether differences exist between LBD and RBD. Noxious stimulus was provided by electrical stimulation. Results showed that P and T scores for the paralysed arms were consistently higher than those of the contralateral side. This was not simply due to modified sensitivity alone; in the RBD group hemi-inattention and aphasia in the LBD group also played a role. A significantly hightened value of pain endurance was found only in the healthy arm in the RBD group.

[Variations in the ratio of total calcium to plasma calcium ions in chronic liver diseases].

Total Ca and Ca++ were evaluated with a Perkin Elmer 305 atomic absorption spectrophotometer and a specific Orion 93-20 electrode respectively in 29 cirrhotics and 76 non-cirrhotics. Stress is laid on the reliability of the Orion electrode. Mean C++ was 52.02 +/- 4.46 mg% in the non-cirrhotics, in cirrhotics was 49.90 mg% +/- 6.14 mg%. An inverted ratio vis-à-vis total Ca was also noted: 54.73 +/- 3.65% in the cirrhotics and 56.51 +/- 3.73% in the non-cirrhotics. Although lacking in significance, these data underscore the importance of both partial and total evaluation of plasma Ca in the prevention of mistaken therapeutic approaches when liver patients.

Pain endurance in unilateral cerebral lesions.

The purpose of the present experiment was to study whether a unilateral cerebral lesion modifies pain parameters with respect to a control group. Electric stimuli were applied to brain damaged patients on the forearm ipsilateral to the lesion, and to both forearms of a control group. No significant difference was found between experimental groups for either pain or tolerance thresholds, whereas pain endurance--the difference between pain threshold and pain tolerance threshold--was significantly greater in right brain damaged patients, compared to both the control group and the left brain damaged group. In the latter group, pain endurance was not significantly different from controls, although there was a tendency towards its decrease. Result are discussed in relation to the problem of the hemispheric specialization for emotional processes.

Concomitant syndactyly and polydactyly in a pediatric foot.

Syndactyly and polydactyly in a child may warrant surgical treatment to avert resultant emotional and psychologic problems. The authors present a unique case report of a bilateral polydactyly of the hallux and bilateral syndactyly of the second and third toes in a normal 6-year-old Hispanic female. The article will discuss the surgical management of these problems, using the skin harvested from resection of the extra hallux for the adjacent side of the lesser toes after desyndactyly.
