[Breech presentation: cesarean section or vaginal delivery?].

The Authors studied breech presentation rate occurred in the Institute of Obstetrics and Gynecology of University of Parma during 1983. Perinatal morbidity and mortality occurred after vaginal delivery was also compared with those occurred after both emergency and elective cesarean section. Breech presentation delivery vaginally in 12,91% and by cesarean section in 87,09%. Only one case of perinatal death occurred in 1983 and it was not related to breech presentation. Breech delivery showed the 31,18% of neonatal morbidity with an higher rate in case of vaginal delivery (66,66%). In conclusion the Author's opinion is that vaginal breech delivery should be explained in a few cases according to all possible hazards.

[Pregnancy, labor and perinatal outcome: comparison between an obstetric-pediatric multi center study of the Perinatal Preventive Medicine subproject of the National Research Council and clinico-statistic study in the second semester 1981 at the Obstetrics and Gynecology Clinic of Parma].

The data collected from women who delivered in the Institute of Obstetrics and Gynecology of the University of Parma since 1/7/81 till 31/12/81 were compared to those of the Obstetric-Pediatric Policentric Study of the Subproject of Perinatal Medicine sponsored by N.R.C. (National Research Council) collected since 1973 to 1977. The maternal wish of pregnancy and the total number of obstetrical controls during pregnancy were higher in the data of Parma than in the data of N.R.C. No differences between the two groups were noted in the obstetrical pathological patterns. Spontaneous labor and elective cesarean section are more frequent in Parma meanwhile there is a minor incidence of low weight newborn and perinatal mortality. No difference was noted in the preterm labor incidence.

[Blood levels of maternal glycosylated hemoglobin (HbA1) in the 3d day of the puerperium and neonatal anthropometric parameters].

In the present study the Authors considered some neonatal anthropometric parameters such as head and chest circumference besides the neonatal weight and placental weight that may be more expressive of the fetal growth. All these parameters were correlated separately and jointly with maternal HbA1 levels of the third day of the post partum period. All mothers spent a pregnancy without any clinical and metabolic disorder. The data show a significant higher HbA1 mean value, than the controls, in mothers whose newborn showed one or more parameters like/superior the 90th centile. This finding may suggest that the evaluation of all the parameters together, the maternal HbA1 value can give more informations about the fetal growth.

Postpartum glycosylated hemoglobins (HbA1): an index of glucose control in pregnancy?

Postpartum HbA1 levels were studied in 114 mothers with a normal pregnancy. HbA1 levels were correlated with neonatal anthropometric parameters and with maternal metabolic parameters. A strong positive correlation was found between weight gain during pregnancy and postpartum HbA1 levels. There was also an increased frequency of infants with large thoracic circumference born to mothers with HbA1 levels above 8.6%. Thus, postpartum HbA1 levels in mothers in whom the serial HbA1 determination is not possible, may be a useful index of glucose control during the gestational period.

[Glycosylated hemoglobin (HbA1) in chemical gestational diabetic and diabetic pregnancy (author's transl)].

The authors have determined, by ionic resin exchange chromatographic method, HbA1 values in 16 chemical gestational diabetic pregnant, in 4 case with insulin dependent diabetes and 34 normal pregnant women. Statistical differences were found between normal's HbA1 mean values and clinical diabetics' but not with chemical gestational diabetics. HbA1 mean values in chemical gestational diabetics with diabetic familiarity were found statistically higher than normal subjects' values. Mothers with chemical gestational diabetic, who delivered newborn with less than 50th centile had HbA1 values significantly elevated.

[The pregnancy in women above 40 years (author's transl)].

The Authors have studied 272 pregnancies in women above 40 years. This record was on 27.215 deliveries in ten years (1970-1979) at the Department of Obstetrics and Gynecology of the University of Parma. From 1970 to 1979 this phenomenon has decreased by 50%. The incidence of obstetric pathology has been elevated (38,87%): this percentage has been more in premature rupture of membranes, pre-eclampsia and uterine inertia. Of these 272 pregnancies, the 37,35% had pathologic cases before pregnancy: these cases were hypertension, heart troubles and diabetes. Cesarean section was elevated (38,98%), so also premature delivery (12,86%) and low birthweight (12,68%). The incidence of malformations was 10,50%. The perinatal mortality was elevated (49,05%0): the majority of this mortality was recorded before delivery.

[Maternal hemoglobin level and fetal and neonatal prognosis].

Maternal and feto-neonatal pathology in pregnant women with Hb values less than 10 gr./100 ml. in the last ten weeks of pregnancy was estimated. WHO considers HB values (gr./100 ml.) a valid and sufficient parameter for a clinical classification of the different degrees of anaemia in pregnancy: the authors agree with this suggestion.

[Cystine aminopeptidase (CAP) in induced labour and puerperium (author's transl)].

Serum cystine-animo-peptidase (CAP) is a valid test for placental function. We found it interesting following up the variations of CAP during induced labour and early puerperium; with this aim in view we determined the values of the enzyme in 33 pregnant nullipares. Six blood samples were taken from each woman. (174 determinations in all): 1) before labour; 2) at 5 cm. dilatation of the uterine neck; 3) immediately after the expulsion of fetus; 4) two hours after delivery; 5) first day of puerperium; 6) in the fourth day of puerperium. Statistical analysis -- Wilcoxon's test and rank bivalent variants analysis of Friedman (X2r) -- show that the variations of values in the first four groups are not statistically significant; values from 1th and 4th day of puerperium show a mean reduction in level of CAP which is remarkable and statistically significant.

[Cystine aminopeptidase (CAP) and intrauterine death of the fetus].

13 pathologic pregnancies with intrauterine fetal death, serially monitored by C.A.P. determination, were retrospectively examined. In 81,81% of the cases the last C.A.P. value determinated before fetal death is situated below the--1 of standard deviation. Furthermore 50 cases with the last C.A.P. value situated below the--1 of standard deviation were considered: in 26% of these cases subsequentely happened intrauterine fetal death.

The metabolic fate of the anti-parkinsonian drug budipine in rats.

The metabolic fate of the anti-Parkinsonian drug budipine was studied in rats after oral administration. The presence of an aromatic hydroxylation product, metabolite M1, and its O-sulphate conjugate was confirmed. Three new minor metabolites, budipine N-oxide, metabolite M1 N-oxide and a secondary metabolite derived from M1 via hydroxylation of a methyl of the tert-butyl group, were isolated and identified in rat urine. The presence of a metabolite M1-glucuronic acid conjugate, was also established through different enzymatic treatments of the rat urine.

In vivo metabolism of the anti-inflammatory agent 2-(5-ethylpyridin-2-yl)benzimidazole.

The metabolic fate of anti-inflammatory agent 2-(5-ethylpyridin-2-yl)benzimidazole (KB-1043) was studied in rats after oral administration. An average of 12.2 +/- 1.5% of the dose was excreted in the urine in the course of 0-48 h; 56.7 +/- 2.6% with feces. Two metabolites were also detected in the urine and isolated by reverse phase HPLC. Structures have been given after identification by comparison with authentic samples. The more abundant metabolite proved to be 2-(5-(1-hydroxyethyl)pyridin-2-yl)benzimidazole, a benzylic oxidation product, which was also excreted as glucuronic acid conjugate; the other metabolite was confirmed to be 2-(5-acetylpyridin-2-yl)benzimidazole. Carrageenin edema and gastric ulcerogenic activity were also tested for the two identified metabolites.

Drug design based on biosynthetic studies: synthesis, biological activity, and kinetics of new inhibitors of 2,3-oxidosqualene cyclase and squalene epoxidase.

Various classes of inhibitor of 2,3-oxido squalene cyclase have been synthesized and tested on rat liver and Saccharomyces cerevisiae microsomes, 3T3 fibroblast cultures, and various bacteria, fungi, and yeasts. The compounds include azasqualenes, azasqualanes, bis-azasqualenes, bis-azasqualanes, and N-oxide and ammonium derivatives of squalene. In order to better mimic the transition state involved in the SN2-like opening of 2,3-oxidosqualene, we synthesized squalene N-methyloxaziridine. Other derivatives tested were N-methylimine, aminalic hydroperoxide, and N-methylamide. We also attempted to produce new "suicide" inhibitors of SO cyclase, such as a squalenoid epoxide vinyl ether. Many of the products described inhibited the various cyclases, the best having an IC50 of 0.3 microM on plants and 1.5 microM on rat liver microsomes, and good antibacterial and antifungal activity. In a search for inhibitors of squalene epoxidase, a series of mono- and bifunctional squalenoid acetylenes and allenes were synthesized. Some of them proved to be inhibitors of squalene epoxidase.

Docetaxel in combination with epirubicin in metastatic breast cancer: pharmacokinetic interactions.

Epirubicin (75 mg/m2) and docetaxel (75 mg/m2) were administered to 16 patients affected by metastatic breast cancer following two different schedules: (1) docetaxel as infusion administered 1 h after epirubicin administration (schedule A); and (2) docetaxel as infusion immediately (10 min) after the end of epirubicin i.v. bolus administration (schedule B). Experimental non-compartmental analyses such as AUC and Css, were affected very little by the drug combination, irrespective of whether the administration of docetaxel was immediately after the epirubicin bolus (10 min) or delayed (1 h). However, serum levels showed evidence of transient drug interaction: in schedule A, docetaxel infusion was associated with a transient increase of plasma epirubicin in correspondence with Cssmax of docetaxel. Bi-compartmental analysis showed a significant difference in epirubicin clearance between protocols A and B. It is suggested that polysorbate 80, used in minimal amounts to formulate docetaxel, may interfere with epirubicin plasma level.

Preliminary screening of some squalenoid derivatives for toxicity towards dermatophytes.

We report a preliminary study of the in vitro anti-dermatophyte activity of six squalenoid derivatives that inhibit 2,3-oxidosqualene cyclase and squalene epoxidase: 2-aza-2,3-dihydrosqualene, 22,23-epoxy-2-aza-2,3-dihydrosqualene, azasqualene alcohol, 19-aza-18,19,22,23-tetrahydrosqualene, 2,3-epoxy-19-aza-18,19,22,23-tetrahydrosqualene and hexafluorosqualene epoxide. The tests were done by inoculating 10 microliters of Trichophyton mentagrophytes (Robin) Blanchard or Microsporum canis Bodin homogenate into 1 ml of Sabouraud glucose liquid medium containing serial dilutions from 100 to 0.25 micrograms ml-1 of the substance. For each compound the minimum inhibitory concentration (MIC) and the minimum fungicidal concentration (MFC) were determined. The most effective compounds were 22,23-epoxy-2-aza-2,3-dihydrosqualene and azasqualene alcohol, with MICs respectively of 3 and 6.25 micrograms ml-1 for each of the two species of dermatophyte. The first of these compounds was the only one to show fungicidal activity over the range of concentrations tested.

In vitro susceptibility of fungi to acyclic inhibitors of 2,3-oxidosqualene cyclases.

In the present study we determine the antifungal properties of two acyclic inhibitors of 2,3-oxidosqualene cyclases: 22,23-epoxy-2-aza-2,3-dihydrosqualene (EAS) and azasqualene alcohol (ASA). Fungistatic and fungicidal activity towards dermatophytes and other fungi involved in cutaneous and systemic infections was tested (48 isolates from 10 species). The tests were carried out by inoculating 10 microliters of mycelial homogenate in 1 ml of Sabouraud glucose liquid medium containing serial dilutions of 100 to 0.25 micrograms ml-1 of the substance. For each isolate, the minimum inhibitory concentration (MIC) and the minimum fungicidal concentration (MFC) of both compounds were determined. EAS was more active (MIC range 1.5-25 micrograms ml-1) than ASA (MIC range 3-50 micrograms ml-1). At the highest concentration tested, EAS also showed fungicidal action towards some isolates of Trichophyton mentagrophytes, T. terrestre, Epidermophyton floccosum, Microsporum canis and Scopulariopsis brumptii. The most sensitive species was T. mentagrophytes, the most resistant T. rubrum.

[Cystine-amino-peptidase (C.A.P.) and D.B.P. in "at risk" pregnancy with "poor intrauterine fetale growth" (P.I.F.G.) (author's transl)].

The normal range of serum cystine-amino-peptidase (C.A.P.) in 54 cases of uncomplicated pregnancies was established. 21 patients whose pregnancies were though to be "at risk" were serially monitored by sonar biparietal cephalometry and serum cystine-amino-peptidase determination. The results show that exists a correlation between C.A.P. and biparietal cephalometry values. Furthermore the "predictive power" towards neonatal conditions (by birth weight and Apgar score determination) was of 77,55% for the biparietal cephalometry and 76,10% for C.A.P.
