[The contribution of umbilical and cerebral Doppler velocimetry in the diagnosis and monitoring of intrauterine growth retardation].

The authors studied the arterial circulation in the umbilicus and brain in 140 cases of intrauterine delayed growth using a pulsed Döppler system. Results were expressed in terms of a circulatory index. The Pourcelot resistance index is defined as S-D/S, where S represents the maximum systolic rate and D the maximum telediastolic rate and is used both for the umbilicus (Rp) and the brain (Rc). In order to evaluate the diagnostic and prognostic performance of the Döppler method, the values of the various indices were analyzed as a function of the foetal cardiac rhythm during labour, the method of delivery and the state of the infant at birth. The authors concluded that umbilical blood flow measurement was of moderate diagnostic values, while the unitary placental resistance index was of undeniable prognostic value (76 p. cent of foetal distress at birth, perinatal mortality of 30.30 p. cent). A pathological brain resistance index preceded the occurrence of abnormalities in foetal cardiac rhythm by 24 hours to 5 days in 64.28 p. cent of the cases and was always associated with foetal distress at birth. The authors considered it was important to take the respective short-term changes in values of the various circulatory indices into consideration rather than the isolated values themselves. The brain-placenta ratio (BPR = Rc/Rp) may perhaps constitute the best indication in the early screening of foetal hypoxia.

Chlamydial antibodies and tubal infertility.

Prevalence of Chlamydia trachomatis antibodies was studied in infertile women with and without tubal obstruction, and in a control group of pregnant women in a Central-African country with a high infertility rate. In comparison with the control group, tubal infertility patients were significantly more likely to have serum antibodies of 1:64 or greater with an age-adjusted odds ratio of 7.8 (95% confidence interval (CI) 3.2-19.1). Infertile women without tubal obstruction had antibody titres similar to the control group with an odds ratio of 1.1 (95% CI: 0.6-1.9). In the tubal infertility group there was a kind of dose-response relationship between chlamydial antibodies and the severity of tubal damage and pelvic adhesions on laparoscopy, with odds ratios of 3.2 (95% CI: 0.7-14.8), 6.2 (95% CI: 1.9-21.6) and 18.1 (95% CI: 6.0-68.5) in the group with mild, moderate and severe pelvic adhesions respectively. This survey adds more evidence to the hypothesis that C. trachomatis is a major agent responsible for tubal infertility in Central Africa.

Interpretation of antibodies reacting solely with human retroviral core proteins in western equatorial Africa.

Out of 4176 sera from asymptomatic adults originating from Chad, equatorial Guinea and Gabon tested for HIV-1 antibodies, 146 (3.5%) were positive by an enzyme immunoassay (EIA). By Western blot (WB), 20 (0.5%) were positive, i.e. with antibodies to the core and the envelope proteins, 96 (2.3%) were indeterminate, i.e. with antibodies to the viral core proteins only and 30 (0.7%) were negative. On testing for HIV-2 by WB, two of the 96 indeterminate sera had antibodies to the HIV-2 envelope glycoproteins. Two complementary tests were used: a radioimmunoprecipitation assay (RIPA) and a HIV EIA recombinant assay (ENVACOR) to check 53 of these indeterminate sera. Forty-one were positive for the p25 protein in RIPA, of which 34 were negative in ENVACOR; six were positive for core proteins only and one was positive for envelope and core proteins using this assay. Twelve of the 53 indeterminate sera were negative in RIPA, of which 11 were negative and one positive for core proteins in ENVACOR. Thus, 42 of these sera remained indeterminate even after the two additional tests which did not allow a distinction between retroviral infection or non-specific reactions. We were able to isolate an unusual HIV-1 virus from lymphocyte cultures of two subjects presenting antibodies directed only against the core proteins.

Infertility in Central Africa: infection is the cause.

Determinants of infertility were studied in 340 women in Eastern Gabon, an area situated in the "infertility belt" of Central Africa. Fallopian tube occlusion was diagnosed in 82.8% of cases, showing the importance of infection-related causes. Women with tubal occlusion did not differ significantly from women with normal tubes in obstetrical history or prevalence of Neisseria gonorrhoeae or Chlamydia trachomatis on endocervical culture. Antecedents of pelvic inflammatory disease or a pelvic mass were significantly more common in the group with tubal occlusion. This group also had a significantly higher prevalence of serum chlamydial antibodies at a titer of 1/64 or higher. Hormonal factors were found in 31.7% of women, a cervical factor in 29.0% and mechanical factors in 5.6%. No diagnosis could be made in 12.2% of cases. During the investigation, 4.4% of women became pregnant. The predominance of infectious related causes of infertility makes it imperative to focus resources on prevention programs of upper genital tract infections in women.

Urogenital Chlamydia trachomatis in Gabon: an unrecognised epidemic.

Samples from 218 men with urethritis, 517 women with pelvic pain or pelvic pain and vaginal discharge, 218 women consulting for infertility, and 598 postpartal women were screened for Chlamydia trachomatis by culture and direct immunofluorescence. Chlamydiae were detected in 18% (39/218) of the men, 18% (45/252) of women with vaginal discharge, 14% (38/265) of those with pelvic pain, 10% (21/218) of infertile, and 10% (59/598) of postpartal women. A chlamydial prevalence of 18% (41) was observed in 229 postpartal women aged under 21, whereas only 5% (10) of 360 postpartal women over 21 had C trachomatis. In the other clinical groups, an age related decrease in prevalence was noted in women over 25. The direct immunofluorescence test correlated well with culture. The small difference in isolation between symptomatic and postpartal women indicates that women in this population do not seek medical attention for chlamydial infections and expose themselves to chlamydial salpingitis and infertility.

Changing antibiotic susceptibility of Neisseria gonorrhoeae in Franceville, Gabon.

Susceptibilities to penicillin, cefotaxime, kanamycin, tetracycline, and spectinomycin were measured for 5 reference strains and 302 Neisseria gonorrhoeae isolates collected between 1980 and 1985. After an initial rise, the number of penicillinase-producing strains decreased. A gradual decrease in susceptibility to spectinomycin and the appearance of tetracycline-resistant strains were also documented.

Importance of chlamydial antibodies in acute salpingitis in central Africa.

Of 35 women with acute salpingitis on laparoscopy, 86% had chlamydial antibodies at a titre of 1/16 or higher and 49% at a titre of 1/512 or higher. Geometric mean titres of chlamydial antibodies correlated significantly with a laparoscopic observation of chronic salpingitis (p less than 0.001), with a history of infertility (p less than 0.05), and with severe inflammation (p less than 0.10), but not with a history of salpingitis or a positive cervical culture for Chlamydia trachomatis. In the subgroup of infertile women, the geometric mean titre of antibodies to chlamydiae correlated significantly with the presence of chronic salpingitis (p less than 0.005). These data indicate that chlamydial infections play a major part in salpingitis and infertility in central Africa. In this area, which is known as the "infertility belt", programmes to control chlamydial infections should be implemented.

[Toxoplasmosis in pregnant women in Haut Ogooué Province (Gabon)].

Anti-toxoplasma antibodies have been studied in 268 pregnant women. Two serological tests have been used: direct agglutination, before and after treatment with 2-mercaptoethanol, and indirect immunofluorescence. Agreement between the tests is over 98%. 60.5% of women have anti-toxoplasma antibodies at first examination. This percentage remains unchanged during pregnancy. Two cases of evolving toxoplasmosis have been observed. The mode of infection in Gabon is discussed.

Molecular cloning, full-length sequence and preliminary characterization of a 56-kDa protein induced by human interferons.

Among the various proteins which are induced when human cells are treated with interferon, a predominant protein of unknown function, with molecular mass 56 kDa, has been observed. With the aim of exploring the molecular basis of the regulation of this protein and of its mRNA, in order to understand its biological function and its possible contribution to the various antiviral and non-antiviral actions exerted by interferons, we cloned a full-length cDNA copy of the 1.8-1.9 X 10(3)-base 56-kDa-protein mRNA and determined its sequence. The cDNA contains 1662 nucleotides derived from the 56-kDa-protein mRNA, including a poly(A) tail of 20 residues. Primer extension experiments indicate that the 5' end of this cDNA clone is probably located only 13 nucleotides downstream of the actual cap site of the 56-kDa-protein mRNA. It consists of a 64-nucleotide-long 5'-non-coding segment, a coding segment of 1434 nucleotides terminated by a TAG triplet and a 141-nucleotide 3'-non-coding segment. The encoded protein of 478 amino acid residues has a molecular mass of 55335 Da and a single potential site for N-glycosylation. The protein contains an excess of basic amino acids and most of them are localized in the carboxy-terminal half of the molecule. A single [35S]methionine-labeled 56-kDa protein was obtained using an SP64 construction to allow the cell-free transcription and translation of the cloned cDNA. Microinjection of this labeled protein in Xenopus oocytes indicates that the 56-kDa protein is cytoplasmic.

[Study of Salmonellae in sewage of Chambery town: epidemiological relations and efficiency of water treatment plant (author's transl)].

An investigation on salmonellae of Chambery sewage has been conducted during a whole year (from July 1977 to June 1978); 563 strains belonging to 36 serotypes have been isolated. Salmonellosis being seldom, one has to admit an important carriage by healthy animals and human beings. The inefficiency of water-treatment plant for these germs has been confirmed.
