Post-pubertal female psychosexual orientation in incomplete male pseudohermaphroditism type 2 (5 alpha-reductase deficiency).

An 18 year-old 46,XY female-reared patient with incomplete male pseudohermaphroditism type 2 (5 alpha-reductase deficiency) was studied. She had a male habitus, Wolffian ducts derivatives, normal testes and small phallus; there were no Mullerian duct derivatives nor gynaecomastia. Clinical and genetic data were typical of the diagnosis which was corroborated by endocrinological studies. Normal LH, FSH, testosterone (T) and oestradiol and decreased dihydrotestosterone (DHT) plasma levels before and after hCG administration were found; the T:DHT ratio was highly increased. The histopathological studies of a testis biopsy showed a normal adult male pattern, and the meiotic chromosomes were interpreted as normal. After assessment of her psychosexual orientation, successful surgical and medical therapy to maintain and improve her femaleness was effectuated. The post-pubertal gender role switch commonly observed in these female-reared patients is discussed.

[Effect of ACTH administration on liberation of testosterone by the Leydig cell (author's transl)].

The possible rôle of the Leydig cells ithe changes of testosterone levels induced by ACTH administration is studied. 14 male Wistar rats weighing about 300 g were separated in two groups. One was treated with I mU.I./100 g/day of ACTH i.m. for 6 days and the other with saline solution as control Rats, testes and adrenal glands were weighed and plasma testosterone levels were measured. The Leydig cells dispersed collagenase was inbubated for 120 min at 37 degrees C in the presence and in the absence of 10 mU.I. of HCG. The weight of adrenal glands in the treated rats was greater than in the control group. Treated rats had lower plasma values than the controls. Testosterone secretion by the Leydig cells, in basal situation and after stimulation with HCG, was lower in the treated group. Leydig cells of untreated rats were incubated with 12.5, 6.2, 3.1, 1.5 mU.I. of ACTH and supplemented with 2.5 pU.I. of HCG. No difference in testosterone secretion in either groups was observed.

Loss of immunological and biological activity of insulin with a human muscle extract.

The isolation of a human muscle extract that degrades insulin to products incapable of any immunological or biological activity is reported. The extract Km value for human insulin is 0.79, and 1.43 for porcine proinsulin. It does not degrade Human Growth Hormone at the concentrations tested, it is inactivated by N-ethyl-maleimide, Trasylol, and heat, and it does not require glutathione for its action. Human muscle, therefore can contain an enzyme similar to "insulin-specific-protease" present in rat muscle.

[Modifications of insulin and growth hormone after medium chain triglycerides ingestion (author's transl)].

Authors determined the variation of glucose, ketone bodies (KB), free fatty acids (FFA), insulin and growth hormone (HGH) in blood produced by the ingestion of 1.5 g./kg. of medium chain triglycerides (MCT) in 10 healthy children between 5 and 11 years. Blood values were determined starting 30 minutes before the MCT ingestion and at 30 minutes intervals until 120 minutes post ingestion and a final determination at 180 minutes. The glucose did not change. The KB were increased (p less than 0.01) from 30 to 120 minutes and the FFA from 90 (p less than 0.01) to 120 minutes (p less than 0.001) after ingestion. Insulin secretion was elevated between 30--90 minutes with a peak value at 60 minutes (p less than 0.001). HGH began to increase at 60 minutes, remaining elevated at the last determination at 180 minutes. The HGH basal value was 0.5 +/- 0.2 ng./ml.; began to increase at 60 minutes and reached the value of 3.9 +/- 1.06 ng./ml. and 4.8 +/- 2.04 ng./ml. at 90 and 120 minutes respectively (p less than 0.001). We do not know the origin of the HGH increase. The changes may explain FFA elevation and other metabolic actions of MCT. The glucose-insulin ratio showed that the hyperinsulinemia was not caused by an increased glucose level.

[Early detection of colonic and rectal cancer; extensive study of occult blood].

Due to the high frequency and usually late diagnosis of cancer of colon and rectum the "haemoccult test" has been recommended as an easy, quick, and unexpensive method for stool determination of occult blood that will allow us to suspect the existence of these tumors in their early asymptomatic stages. The test was performed in 1437 patients attending the gastroenterology clinic which had no clinical evidence of bleeding, of these, 1150 returned the samples. In 52 (4,6%) the test was positive for occult blood. The studies performed revelated that 4 were upper gastrointestinal ilions, one ileal diverticulum and 40 (3,47%) patients had colonic disease of the latter 19 were diverticular disease, 1 crohnis disease of rectum, 2 adenomatous and 2 villous polips, 1 Hodkgin disease and 7 carcinomas. There were 6 false positive results. The high frequency of positive "haemoccult test" in colonic disease, specially benign and malignant tumors, make this test and appropiate method of this time in the detection for colo-rectal carcinoma.

Double pylorus and esophageal achalasia.

29 cases of double pylorus have been reported so far. It is thought that in 85% of the cases it has been due to a fistulous gastro-duodenal ulcer and the remaining 15% would be of a congenital origin. In the case we have reported the double pylorus is believed to be a congenital anomaly associated with a mega-esophagus of similar etiology.

[Lumboscopic drainage of retroperitoneal abscess. Case report].

Existing reports evaluate the management of infected pancreatic necrosis. Most of the authors choose between percutaneous and retroperitoneoscopic drainage. The high morbidity and mortality of this pathology distorts the objective evaluation of the treatment employed. With present a case of infected pancreatic collection favorably resolved with retroperitoneoscopic drainage evidencing the low morbidity of this technique.

Non-pigmenting cutaneous-mucosal fixed drug eruption due to piroxicam.

BACKGROUND: Piroxicam is a widely used anti-inflammatory drug. Most adverse reactions affect gastrointestinal system, liver and skin. Fixed drug eruption although very unusual, has also been described, but with cutaneous involvement exclusively. We present the case of a 49-year-old man who suffered three episodes of fixed drug eruption with cutaneous-mucosal involvement, even simulating an autoimmune disease, whenever he was treated with oral piroxicam. METHODS AND RESULTS: He was patch tested on normal skin with the GEIDC standard series and an NSAIDs series. He was patch tested on normal skin and on fixed eruption with piroxicam, meloxicam and tenoxicam (all of them 1 % pet). Oral challenge test was not performed due to the severity and reproducibility in previous reactions. Results showed a positive patch test to piroxicam (1 % pet) on fixed eruption, with negative results to the rest. CONCLUSIONS: Adverse drug reactions may present a wide variability of clinical symptoms. In these situations an accurate clinical history is necessary. To our knowledge this is the 1st report of non-pigmenting fixed drug eruption with cutaneous-mucosal involvement due to piroxicam. Cross-reactivity between oxicams could not be demonstrated by patch test on fixed eruption.
