Different types of proteinuria in diabetes mellitus.

Urinary protein concentration and urinary protein pattern as determined by sodium dodecyl sulphate polyacrylamide gel electrophoresis, were investigated in 53 type 1 diabetic children and in 56 (41 type 1 and 15 type 2) diabetic adults. Reversible tubular proteinuria was found in 26 diabetic children with hyperglycaemic ketoacidosis. Among the patients, 43 exhibited physiological, 31 fixed tubular and nine glomerular type proteinuria. The prevalence of retinopathy and neuropathy in the different patient groups has also been evaluated. Urinary protein concentration appeared to be above the usual reference values during hyperglycaemic ketoacidosis, and was normalized with the successful control of metabolism. In the other three groups, urinary protein concentration was increased only in the case of glomerular type proteinuria. It is suggested that the above classification characterizes the development of diabetic nephropathy, progressing from reversible tubulointerstitial dysfunction through fixed tubulointerstitial dysfunction to glomerulopathy.

Isolation of inhibin from ovine follicular fluid using reversed-phase liquid chromatography.

This report describes the application of high performance liquid chromatography (HPLC) and associated techniques to the isolation of inhibin from ovine follicular fluid (oFF). Two modes of HPLC have been assessed. High speed gel permeation chromatography on a Waters I-125 support showed that at pH 7.0 inhibin bioactivity was associated with an approx. 80000 mol. wt protein fraction which co-eluted with the bulk of the protein in oFF and gave a yield of 30-44% of the applied activity. Reversed-phase HPLC (RP-HPLC) of a peptide-rich fraction extracted from oFF was also investigated. Using analytical liquid chromatography on octadecylsilylsilica (ODS-silica), two distinct inhibin bioactive fractions were obtained, with a resultant purification of 23-fold and 3-fold respectively and an overall recovery of 8.5%. These results demonstrate that RP-HPLC may be employed as a direct and rapid technique for the isolation of inhibin from oFF.

Reversible tubular proteinuria associated with hyperglycemic ketoacidosis in type 1 diabetes mellitus.

Urinary protein excretion has been investigated in 14 children with hyperglycemic ketoacidosis secondary to diabetes mellitus. In 9 patients hyperglycemic ketoacidosis was the first manifestation of their disease and 5 patients were known diabetics and accidentally developed hyperglycemic ketoacidosis. A control group consisted of 15 diabetic children aged 7-16 years with good metabolic control. Duration of the disease ranged from 1 month to 9 years. A second group of controls consisted of 24 healty children aged 10-16 years. Urinary protein fractions were separated by sodium dodecyl sulphate polyacrylamide gel electrophoresis. Tubular proteinuria was found in all diabetic patients during hyperglycemic ketoacidosis and disappeared in all cases within 4 weeks of establishing satisfactory diabetic control. The pathological basis and effect of transient tubulointerstitial insufficiency as indicated by transient tubular proteinuria is discussed.

Immunological and virological investigations in Down's syndrome.

Lymphocyte responsiveness to phytohaemagglutinin and viral antigens was studied in children with Down's syndrome and in controls. Mitogen-responsiveness in the patients was significantly reduced as compared to the control values. Using the lymphocyte transformation test, trisomic patients showed more than a twofold increase in sensitivity to herpes simplex virus as compared to controls. The same test did not show any essential difference between the two groups when adeno- and influenza viruses were used. Immunofluorescence technique, with specifically conjugated antiviral sera, permitted the detection of specific fluorescence in 30% of the patients with Down's syndrome indicating the presence of oncogenic adenovirus type 12 antigen in the circulating lymphocytes. No antibodies--or only very low titres--against adeno- and herpes simplex viruses were demonstrated in the sera of trisomic patients. Mononuclear leukocytes from these patients often showed structural alterations. The incidence of infectious herpes simplex virus and Candida albicans in the saliva of patients was higher than in the control group. It seems that Down's syndrome involves partial disturbance of both the cellular and humoral immune functions--at least with respect to certain viral antigens.

The specificity of inhibin bioassay using cultured pituitary cells.

It has been established that the inhibition of the cellular content of FSH in cultured pituitary cells can be used as a sensitive and precise bioassay for inhibin. During studies on the inhibin content of human plasma, FSH suppression was noted to occur together with LH suppression. Further studies revealed that where combined FSH and LH suppression occurred, cytological changes in the pituitary cells suggestive of toxicity were found, indicating non-specific effects of these substances. Charcoal treatment or gel filtration of seminal plasma removed or separated the toxic substances from the inhibin activity, the latter characterized by FSH suppression parallel to the standard preparation, no LH suppression and no light-microscopic changes in pituitary cells. It is recommended that careful evaluation of all inhibin bioassay systems should be undertaken to detect substances producing non-specific effects and additional guidelines for the assessment of specificity are suggested.

Chromosome mutations and chromosome stability in children treated with different regimes of immunosuppressive drugs.

The chromosome mutations and the number of sister chromatid exchanges induced by different kinds of immunosuppressive treatment were investigated in children and adults with certain types of renal diseases. The aim of the study was to find among the treatment schedules those promising good therapeutic results with the least mutagenic effects. A slightly decreased chromosome stability was found in the patients treated by cyclophosphamide therapy.

[Ultrastructural changes in the liver and leukocytes of a child with Niemann-Pick disease].

Morphological findings in Niemann-Pick disease of an infant are described. By electron microscopy in the cytoplasma of leukocytes and hepatocytes mostly moderaty electrodense unmature lipid-containing cytosomes were seen. Morphological alterations found in the leukocytes and hepatocytes in contrast to the earlier findings appeared to be similar. Authors stress the importance of electron microscopic investigation of peripheric leukocytes.
