Electron microscopy of large cell undifferentiated and giant cell tumors.

Electron microscopy is a valuable morphologic method for the diagnostic evaluation of undifferentiated tumors composed of polygonal or oval large cells and mononuclear or multinucleated giant cells. Although few ultrastructural details are pathognomonic, electron microscopic findings may add significantly to the formulation of the final diagnosis if used in the context of other pathomorphologic and clinical data. Contributions of electron microscopy to tumor diagnosis are summarized and illustrated with appropriate examples from personal experience in a routine university hospital laboratory.

Detection of cells with multidrug-resistant phenotype in myeloproliferative disorders before therapy.

The expression of the multidrug resistance (mdr) phenotype is connected with the overexpression of the P-glycoprotein. By applying the immunocytochemical assay, we have demonstrated that in myeloproliferative diseases (AML, ALL, MDS, CGL) in single cases in smear preparations from the peripheral blood as well as from the bone marrow P-glycoprotein-positive cells, respectively, cells with mdr-positive phenotype can be detected in the material obtained from patients before therapy and without clinically and anamnestically known exposure to cytotoxic or immunosuppressive drugs. In the control group of probands without hematologic disorders and also without clinically or anamnestically confirmed contact with cytotoxic or immunosuppressive drugs, we have found P-glycoprotein-positive subpopulations of cells with positive mdr phenotype in a few cases as well. The uniqueness of our results lies in the fact that this finding demonstrates the presence of subpopulations of mdr-positive cells in leukemias and myelodysplastic syndromes before therapy, and furthermore makes evident that a positive mdr phenotype is not necessarily associated with a malignant phenotype or a malignant cell transformation.

Endoscopic studies of nabumetone in patients with rheumatoid arthritis. A comparative endoscopic and histologic evaluation.

Gastric tolerance to 1 g of nabumetone administered in a single nightly dose was assessed in two trials in patients with rheumatoid arthritis. Gastroscopy and histology of mucosal biopsy specimens were performed before and after the end of treatment in both trials. Trial 1 was an open study that compared the effects of 1 g of nabumetone at night with those of naproxen (dose, 500 mg twice daily) and indomethacin (dose, 50 mg three times daily) in 41 hospitalized patients. After three weeks of treatment, nabumetone was significantly better tolerated than naproxen or indomethacin. Trial 2 was a randomized trial with 24 patients per group that compared 1 g of nabumetone given at night with 250 mg of naproxen given in the morning and 500 mg given at night for a period of three months. This single-blind study revealed that the number of patients with microscopic or macroscopic mucosal lesions was significantly smaller following intake of nabumetone. Concerning efficacy, as judged clinically by a rheumatologist, treatment with nabumetone was superior as well.

Similar concentrations of thyroidal iodothyronines and T3/T4 ratio in normal thyroids from two Yugoslav districts with different iodine intake.

In spite of different daily iodine intake in two yugoslav districts (littoral-Split 297 +/- 76 (Mean +/- SD) and continental-Zagreb 111 +/- 36 micrograms/g creatinine, p less than 0.001), thyroidal iodothyronine concentrations and T4/T3 ratio in normal post mortem thyroids were comparable. T4 was 260.0 +/- 42.6 micrograms/g w.w. (Mean +/- SE) in Split group vrs. 279.9 +/- 59.4 in Zagreb. T3 was 12.8-2.0 vrs. 12.8 +/- 2.2 and T4/T3 ratio 21.6 +/- 1.9 vrs. 21.1 +/- 2.9 in Split and Zagreb, respectively. It has been concluded that relatively small difference in iodine intake has no effect on thyroidal concentrations of iodothyronines and T4/T3 ratio.

Clinical relevance of peripheral blood lymphocyte morphology and lymph node histology in chronic lymphocytic leukemia.

Peripheral blood lymphocyte (PBL) morphology and lymph node (LN) histology were evaluated in a prognostic study conducted in 247 patients with low-grade malignancies of B lymphocytes. The patients were classified according to total tumor mass (TTM) distribution pattern. A simple classification based on quantification of lymphoid cells that were morphologically different from small lymphocytes of peripheral blood was evaluated in the 166 leukemic patients. The lymph node histology was studied in 211 patients who had lymph node enlargement. The pattern of growth (pseudofollicular, diffuse, and tumorous), the proportion of distinct lymphocytes, and the degree of capsule infiltration were evaluated in 130 patients who showed leukemic involvement as well as enlargement of lymph nodes and/or spleen. Both PBL and LN morphologies correlate with the TTM distribution pattern and demonstrate a prognostic value that is independent of clinical characteristics. From the PBL morphology the LN morphology can be predicted. The prognostic power of the PBL morphology is lost after adjustment for the pattern of growth in LN and vice versa. A significant overlap between the PBL morphology and LN histology suggests some independence between the two variables and points to the possibility of biased classification if only one of the compartments is evaluated. Capsular infiltration is, however, a prognostic factor independent of both clinical and morphological characteristics. Morphological analysis in CLL is clinically relevant. Precise classification of patients requires analysis of both PBL and LN morphology to ensure comparability of patients admitted to clinical trials.

Thyroidal thyroxine and triiodothyronine in autonomously functioning thyroid nodule and paranodular tissue.

Thyroidal thyroxine (T4), triiodothyronine (T3), thyroglobulin (Tg) and T4/T3 ratio were investigated in nodular and paranodular tissue from 16 patients with autonomously functioning thyroid adenomas. The concentration of T4 and T3 in the nodule were 97.7 +/- 20.5 (Mean +/- SE) and 10.2 +/- 2.4 micrograms/g wet weight (w.w.). Both iodothyronines were significantly lower in paranodular tissue (22.6 +/- 4.8 and 1.45 +/- 0.32 micrograms/g w.w., respectively), but with disproportionately decreased T3 which resulted in T4/T3 ratio (25.4 +/- 6.4) higher than in adenoma tissue (11.2 +/- 1.6). In patients with high normal or supranormal serum T3 concentration, thyroidal concentration of T3 in adenoma tissue was higher (15.6 +/- 5.0 vs. 6.3 +/- 1.2 micrograms/g w.w.) and T4/T3 ratio lower (8.05 +/- 2.1 vs. 13.2 +/- 1.9) than in patients with normal serum T3. The results suggest that thyroid release of T3 from adenoma is relatively higher than T4 in patients with autonomously functioning thyroid nodules and increased peripheral T3.

Low thyroidal T3 in nodular goitrous tissue.

Six nodular tissues of non-treated and four of treated patients (suppressive treatment with thyroid hormones from three months to two years until the operation) with nodular non-toxic goitre contained low T3 (less than 1 ug/g w.w.). The results of iodothyronines and thyroglobulin (Tg) were compared with respective tissues containing T3 greater than 1 ug/g w.w. In non-treated patients, nodular tissues with low T3 and very high T4/T3 ratio showed T4 and Tg concentrations not different from the tissues with T3 greater than 1 ug/g w.w. In the goitres with low T3 of treated patients, T4 was also reduced but disproportionately to T3. Microscopically, nodular goitres with low T3 were characterized with gross fibrous infiltration and diffuse haemorrhage which was substantially different from histological findings in nodular goitres with T3 greater than 1 ug/g w.w. High T4/T3 ratio in the tissues with low T3 is similar to increased T4/T3 ratio in paranodular tissues of autonomously functioning adenomas. The results suggest that low T3 and high T4/T3 ratio in nodular goitrous tissue could be due to grossly impaired thyroid function or due to suppressed secretion of TSH.

Diabetogenic action of alloxan-like compounds: cytotoxic effects of 5-hydroxy-pseudouric acid and dehydrouramil hydrate hydrochloride on rat pancreatic beta cells.

The effects on islet morphology and blood glucose concentration of intravenous administration of alloxan to rats have been compared with those of two new diabetogenic agents, 5-hydroxy-pseudouric acid (5-HPUA) and dehydrouramil hydrate hydrochloride (DHU). Administration of alloxan (0.35 mmol/kg) caused a classical triphasic change in blood glucose characterised by initial hyperglycaemia, subsequent hypoglycaemia and a delayed persistent hyperglycaemia. In contrast, 5-HPUA and DHU elicited persistent hyperglycaemia as early as 30 min after administration. Morphological evidence for degranulation, pyknosis, necrosis and widening of pericapillary spaces was obtained with all three agents. However, both 5-HPUA and DHU elicit considerably more rapid and extensive changes than alloxan, with evidence for extensive pyknosis occurring as early as 15 min after administration of DHU and 5-HPUA compared with 24 h for alloxan. The more marked potency of DHU and 5-HPUA may be at least partially attributable to the greater stability of these agents compared with alloxan, since solutions of DHU or 5-HPUA kept for 15 min prior to administration retained full diabetogenic activity, whereas similar treatment of alloxan solution completely abolished its diabetogenic activity. Since both 5-HPUA and DHU are potential metabolites of uric acid, their marked diabetogenic potency raises the possibility of a role for uric acid metabolites in the pathogenesis of diabetes mellitus.

Embryo-derived teratocarcinoma. IV. The role of immune factors in the regulation of teratocarcinogenesis.

Seven-day-old mouse embryos were transplanted under the kidney capsule of adult syngeneic recipients which were either X-irradiated, treated with cyclophosphamide, splenectomized or immunized with teratocarcinoma cell lines prior to embryonic transplantation. The yield of malignant tumors was decreased in preirradiated, as well as in cyclophosphamide-pretreated animals, as compared with untreated hosts. The effect of the pretreatment could be fully abrogated by allowing animals to spontaneously recover for 4 weeks or by adoptive transfer of thymus cells from untreated animals. Partial recovery was obtained by the adoptive transfer of spleen cells. Splenectomy at the time of embryonic transplantation did not affect the embryo-derived teratocarcinogenesis in C3H mice but reduced the yield of malignancy and the weight of tumors produced in C57BL/6 mice. Immunization of hosts with teratocarcinoma cells did not influence the subsequent embryo-derived teratocarcinogenesis. These data indicate that embryo-derived teratocarcinogenesis is regulated by X-ray and cyclophosphamide-sensitive cells residing in the thymus and the spleen. The permissiveness to teratocarcinogenesis seems to be a function of the total number of these regulatory cells in the host.

Embryo-derived teratocarcinoma. III. Development of tumors from teratocarcinoma-permissive and non-permissive strain embryos transplanted to F1 hybrids.

Seven-day-old mouse embryos of two teratocarcinoma-permissive (C3H and BALB/c) and two teratocarcinoma-non-permissive (C57BL/6 and AKR) strains were transplanted to their F1 hybrids to determine the role of the genetic background of the recipient animals in controlling embryo-derived teratocarcinogenesis. The yield of teratocarcinomas in F1 hybrid recipients of embryonic grafts was either identical with the yield in syngeneic recipients or increased or decreased, depending on the strain of the embryo and the F1 hybrid combination. In certain hybrids, the yield of malignant tumors remained the same as in the syngeneic recipients but the F1 hybrids exerted a stimulatory effect on tumor growth and the tumors weighed more than those in syngeneic recipients. A matroclinous effect was also seen in certain hybrids. These data indicate that embryo-derived teratocarcinogenesis in histocompatible F1 mice depends on the genetic background of adult recipients. The teratocarcinogenic potential of non-permissive and permissive strain mouse embryos can be modified by choosing appropriate F1 hybrids for embryonic transplantations.

[Diagnosis and treatment of carcinoma of the prostate].

Cancer of the prostate is one of the most common malignancies that appear in men, and immediately after the neoplasms in the bronchial tubes and stomach, it is the third most common cause of death in the Socialist Republic of Croatia. According to the anatomic division of the prostate gland (micro-anatomic structure of the gland), in most cases cancer develops in the peripheral zone which is constructed of very small and simple branching canals and small round sinuses whose cells are more regular and lower than those in the central zone. We insist on a classification according to the TNM system, while histologists emphasize the findings of a differentiation of cell "grading. 2" Although a rectal probe is quite effective in discovering malignancies of the prostate gland, a sure diagnosis can only be made by cytological examination, by which the type of carcinogen can be ascertained. From 1971 to 1979, the authors performed aspiration transrectal cytological punctures on 526 patients. Cancer was suspected in 142 patients. 95 of these patients (66.9%) were found to have malignant cells. With a needle specially designed by the authors, the puncture treatment was performed on 57 patients. The authors recommend that every male over 50 years of age should undergo an annual digito-rectal examination, as well as all those in advanced years who have unusual rheumatic or pelvic problems. Of course, every suspicious knot on the prostate gland receive the puncture treatment and by cytologically examined.
