[1st cured case of bovine herpes mamillitis in Switzerland].

The first outbreak of bovine herpes mamillitis in Switzerland has been observed and diagnosed in autumn 1983. Bovid herpesvirus type 2 (BHV-2) could be isolated from lesions of the cows' teats and was characterized by its morphology and by serological methods. Serological examinations of all the animals of the affected herd did not reveal conclusive data relating to time and source of the primary infection. Questions concerning epidemiology, especially in view of latency, therapeutic and prophylactic possibilities are discussed.

[Ewing sarcoma. Diagnostic and therapeutic problems in children].

A pathological X-ray of the skeleton with the clinical correlation "painful swelling of soft tissue" is always an alarming sign in childhood. Radiology is our most important device to decide if the disease is of traumatic, inflammatory, incretory or neoplasmatic as well as of degenerative origin. Considering the group of "neoplasmatic alterations" there are more than 50 different entities. The differential diagnosis of a "bone tumor" requires large experience from the radiologist. Supporting the oncologist an experienced pathologist must be capable to distinguish a reticulosarcoma, a neuroblastoma metastasis, a leukemic bone-infiltrate or an embryonal rhabdo-myosarcoma from an Ewing's sarcoma. An experienced surgeon is of the same importance capable of not only collecting detritus out of necrotic tumor centres or altered tissues, but of gathering several samples from vital tumor tissue. This experienced team is especially required when children are in concern, as this age represents the peak of manifestation of malignant bone-sarcoma as Ewing's sarcoma or osteosarcoma. The quoted case-descriptions point our how difficult it may be to find the proper biopsy location and how initial false diagnosis lets valuable time pass by and prevents early diagnosis.

[Functional diagnosis after anorectal injuries].

Anorectum - rectum and rectosigmoid functions can be separately investigated by employing water-filled probes which have been connected to a pressure transducer. Sphincter ani externus - internus and the puborectal muscle functions particularly well. In cases of injuries to the continent organ preoperative electromanometry clarify the extent of damage. Postoperative examination then enables the evaluation of the success or failure of the reconstruction procedure. Four select cases are presented graphically and in detail illustrating the complexity of the process.

Simultaneous determination of amiodarone and its major metabolite desethylamiodarone in plasma, urine and tissues by high-performance liquid chromatography.

A simple and sensitive high-performance liquid chromatographic method for the simultaneous assay of amiodarone and desethylamiodarone in plasma, urine and tissues has been developed. The method for plasma samples and tissue samples after homogenizing with 50% ethanol, involves deproteinization with acetonitrile containing the internal standard followed by centrifugation and direct injection of the supernatant into the liquid chromatograph. The method for urine specimens includes extraction with a diisopropyl ether-acetonitrile (95:5, v/v) mixture at pH 7.0 using disposable Clin-Elut 1003 columns, followed by evaporation of the eluate, reconstitution of the residue in methanol-acetonitrile (1:2, v/v) mixture and injection into the chromatograph. Separation was obtained using a Radial-Pak C18 column operating in combination with a radial compression separation unit and a methanol-25% ammonia (99.3:0.7, v/v) mobile phase. A wavelength of 242 nm was used to monitor amiodarone, desethylamiodarone and the internal standard. The influence of the ammonia concentration in the mobile phase on the capacity factors of amiodarone, desethylamiodarone and two other potential metabolites, monoiodoamiodarone (L6355) and desiodoamiodarone (L3937) were investigated. Endogenous substances or a variety of drugs concomitantly used in amiodarone therapy did not interfere with the assay. The limit of sensitivity of the assay was 0.025 micrograms/ml with a precision of +/- 17%. The inter- and intra-day coefficient of variation for replicate analyses of spiked plasma samples was less than 6%. This method has been demonstrated to be suitable for pharmacokinetic and metabolism studies of amiodarone in man.

[Clinical, manometric and electron microscope follow-up studies of the transposed gracilis muscle in anorectal incontinence].

The article reports on six children with complete incontinence. Two patients had lumbar myelomeningocele, whereas four had a supralevatory anorectal atresia. They were given a modified sphincter substitute graft by means of the m. gracilis. Proof of full functional ability of the "neosphincter" is supplied via manometrical and electron microscope studies.

Goat herpesviruses: biological and physicochemical properties.

Two herpesvirus isolates from goats are known which cause afflictions of the digestive tract in kids and, in some cases, abortion. An antigenic relationship of these goat herpesviruses with infectious bovine rhinotracheitis/infectious pustular vulvo-vaginitis virus (bovid herpesvirus 1, BHV-1) was reported and because of the species-specific pathogenicity, the goat isolates were named caprine herpesvirus 1. In this report the two isolates are further characterized and compared with BHV-1. Although the caprine herpesviruses share many biological and physicochemical properties with BHV-1, they can be differentiated from the bovine viruses with respect to growth cycle, one-way cross-neutralization and, most importantly, the restriction endonuclease fragments of their DNAs. The molecular weight of the caprine herpesvirus DNA, based on electron microscopic length measurement is 90 X 10(6), similar to that of BHV-1 (95 X 10(6]. On the basis of these genomic differences, we propose that DNA restriction endonuclease patterns of the caprine herpesviruses should be designated as prototypic of bovid herpesvirus 6 (BHV-6).

[Cystic hygroma of the neck].

Cystic hygroma of the neck is usually localised to the postero-lateral triangle. Acute intervention is necessary if there is compression of the respiratory passages. Other indications for surgical intervention are related to cosmetic appearances, the results of infection, rapid growth or rupture. Cystic lymphangioma should be removed early. If vital structures are involved in the lymphangioma, it is better to leave a residue of lymphangioma behind than to attempt radical excision.

[Renal venous thrombosis in the newborn (author's transl)].

The renal venous thrombosis is mostly produced by severe dehydration. The following, however, are also predisposing factors: infections, birth traumata, paranephritic processes, lack of oxygen, diabetes of the mother and cyanotic heart defects. The diagnosis is correctly made by means of the palpable flank tumor, the macrohematuria, together with the urogram and sonography. The most urgent therapeutic measure is the balancing of the water and electrolyte deficit to eliminate the dehydration, i.e. the existing oliguria or anuria. The therapy is primarily always conservative. In the event of a consumption coagulopathy, a therapeutical attempt can be undertaken with heparin but if this is unsuccessful, an immediate nephrectomy must be performed. A further nephrectomy will be necessary if hypertony persistent infection and renal atrophy occur. Two infants with renal venous thrombosis were used for this study on the causes and diagnosis of, and the therapy for the illness.

[Acute renal vein thrombosis in infancy (author's transl)].

Two cases of acute renal vein thrombosis are presented. Instead of angiography, sonography was used to perform the diagnosis. After short discription of the etiology, clinic and roentgensymptomatology, the sonographic semiology is emphasized. In the discussion the sonographic differentialdiagnosis is reviewed and the possible therapeutic proceedings are mentioned. The value of sonography as non-invasive method, especially in newborn is therefore established.

Characteristics and experiences of women with and without full dentures in a city of the Netherlands.

A combined dental and sociological study was completed in Leeuwarden in 1976. Approximately 600 children filled in a questionnaire and were dentally examined. Their mothers were also interviewed. 536 mothers took part in the study; of these, 45% had full dentures and only 33% still had a normal dentition. These two groups of mothers have been compared in this article. There was very little difference between the mean ages of the two groups, but a considerable difference in education, family income and the husband's occupation. Women with full dentures used to visit the dentist less frequently when young. There also was a slightly more sceptica- attitude as to possible preservation of teeth among those who did not succeed in keeping their natural teeth. A description of some of the characteristics and the experiences of the two groups, those with and those without dentures, has also been included. The group of women with full dentures were rather satisfied about their dental state although even a part of this group still have problems with their dentures. The group of women still having natural teeth were fairly optimistic about the state of their teeth and their expectancy of the need for dental care in the future.
