[Neuroblastoma sympathicum problem: our own experiences with 10 neuroblastoma patients].

Special problems of neuroblastoma sympathicum are being demonstrated by means of 10 cases. Although in the past 20 years therapy ahs been improved considerably, the 2-year survival rate in 35--40% was hardly increased. Three children are specially presented: case I with myoclonus encephalopathy, case II with spontaneous regression and case III, where therapy was carried through by radical surgery. Various possible causes of myoclonus encephalopathy are being discussed and, regarding prognosis, assessed favourably. Several samples of excision should be required, since the tumor substance varies histologically. It is further reported about immunological phenomena. We point out especially that, if a tumor is manifested in a child under one year of age, the prospect is very good. The tumor should be excised by radical surgery, and, if vital structures are included, a resection en bloc might be necessary, with re-implantation of important blood vessels. Favourable factors with regard to prognosis are being counted up. Therapy should be carried out individually, depending on a low risk or high risk case.

[Diagnosis and therapy of Wilms' tumor based on a survey of 16 children with nephroblastoma].

On the second place in malignant diseases of children, after the leukemias, we could find the Wilms-Tumor. Of the 16 children we treated in the years from 1965 to 1977, 8 children died, 2 patients couldn't be any longer observed and 6 patients are still alive. 5 children of these have obtained now a relapse-free survival from 1 year and 2 months to 2 years and 7 months. We could see, that the prognosis depends only on the stage of the tumor and not on the age of patients. On 3 cases we will try to show, how difficult the diagnosis of the Wilms-Tumor is and how important it is, to treat resolutely pulmonary and liver metastases. As diagnostic methods were used i. v. P., the sonography and the selective renalangiography. Since 1976 our treatment consists of preoperative irradiation of tumors, nephrectomy, postoperative irradiation of tumor-bed and adjuvant chemotherapy (actinomycin D, vincristine and adriamycin). Pulmonary and liver metastases are a domain for chemotherapy, combined with irradiation (danger of pneumonitis and toxic hepatitis) and eventually a resection is necessary. Only the good cooperation of an oncologic team could achieve an improvement of the cure rate of this tumor, as we know from the American countries (survival 75%).

[Additional comments on the differential diagnosis of rare bone diseases (author's transl)].

We report about two seldom types of diseases of bones. Both came to our hospital with the diagnosis Osteomyelitis. The first case, the Mafucci-Syndrome, is presented--a systemic Chondrodysplasia with Angiomatosis. This disease is not hereditary. There are found multiple Enchondromas mostly in the longer bones. In 20% the cases of the Mafucci-Syndrome get malignant. The second case shows the Conradi-Hünermann-Syndrome a Chondrodysplasia calcificans congenita, which seems to be hereditary. The characteristic sign is the minor growth of the bones, caused by early deposition of lime in the regions of ossification. Malignity is not described.

Immunologic evaluation of rheumatoid arthritis and therapy with levamisole.

The haemolytic complement activity and the complement components C3, C4 and C1q were significantly increased in the sera of 43 patients with classic or definite rheumatoid arthritis. The increased complement activity was fairly well correlated with an increased sedimentation rate, increased IgG levels and rheumatoid factor positivity. The E-rosette formation of T-lymphocytes measured in 16 rheumatoid arthritis patients was significantly depressed. These 16 patients were treated with levamisole 50 mg t.i.d. Four patients discontinued treatment because of side-effects. The remaining 12 patients were re-examined clinically and immunologically after a mean treatment period of 3.9 (range : 2.5-8) months. A significant reduction in haemolytic complement activity, C3, IgG and sedimentation rate and a significant increase in E-rosette forming cells were observed, and these were accompanied by a significant clinical improvement. These findings are discussed in view of the new concept that the basic defect in rheumatoid arthritis could be a disturbance of the interaction of T-and B-cells and faulty suppressor T-cells. According to this hypothesis, levamisole restores cell-mediated immune reactivity, thereby affecting the natural course of rheumatoid arthritis.

Computational studies of the early intermediates of the bacteriorhodopsin photocycle.

Starting from a refined model of bacteriorhodopsin's ground state, alternative models of the K and L intermediates with retinal in either 13-cis or 13-14-dicis configuration have been generated by molecular dynamics simulations. All models have been submitted to electrostatic calculations in order to determine the pK1/2 values of particular residues of interest in the active site. Our pK1/2 calculations for the refined ground state can reestablish our former results, this time without adjusting the intrinsic pK of the Schiff base. For the K intermediate the electrostatic calculations show no significant change in the pK1/2 values compared to the ground state for most of the titrating groups in the active site. For the L intermediate where retinal possesses a 13-cis configuration, we found that electrostatic factors decrease the pK1/2 value of the Schiff base by 4-5 pK-units compared to the ground state. The calculations suggest that changes of the electrostatic environment via a pure 13-cis model are sufficient to produce a pK reduction of the Schiff base that will promote subsequent proton transfer steps.

[Factors influencing the postpartal course of children with gastroschisis].

Based on twelve cases of gastroschisis in newborn babys it has been possible to demonstrate that both, the time elapsed between birth and surgery, and adequate protection against heat loss, have a profound influence on postoperative recovery. To ensure a planned birth the cesarean section is to be preferred to vaginal delivery. The importance of a primarily correct post natal care is discussed.

[Bilateral hydrothorax of the newborn infant].

Bilateral hydrothorax of the newborn is not compatible with life. It must, therefore, be diagnosed sonographically before birth and drained immediately following birth. The therapy which can be applied is discussed with reference to a case.

[Intestinal lymphagiectasis. Use of immunoregulators].

The authors present a patient with intestinal lymphangiectasis discussing the pathophysiology, diagnosis and immunologic treatment instituted which was with mebendazole, sulfazalosopyridine and oral calcium. Immunoglobulins and T and B leukocytes showed increase together with clinical improvement in the diarrheal picture.

[Electron microscopy of the intestinal mucosa in a case of lymphangiectasis].

The morphology of the intestinal mucosa was reviewed using light microscopy and transmission electron microscopy from a two year-old male patient with protein losing primary intestinal lyphangiectasis. Diagnostic studies ruled out recognized forms of secondary intestinal lymphangiectasia (e.g cardiac, hepatic, renal diseases or lymphoreticular cancer). Furthermore, certain features distinguish this patient from the secondary type: decreased immunoglobulin and albumin levels, lymphocytopenia, chylous ascitis and delated lymphatics on small-bowel biopsy.
