[Ovarian cancer--diagnostic dilemmas and unusual therapeutic response].

The aims of this paper are both to highlight some dilemmas concerning the diagnosis of primary epithelial ovarian carcinoma mainly in its early stages and to underline the capricious responses of this type of malignancy to an otherwise well coded modern management. This study is based on the analysis of the records of 78 patients with ovarian carcinoma admitted for diagnosis and surgical treatment to the Department of General Surgery of Craiova C.F.R. Clinic from 1993 through 2003. The results of this analysis are difficult to interpret due to loss to follow up (in terms of response rates) of some of our 78 operated on ovarian carcinoma patients who went on with their platinum-based chemotherapy (following surgical cytoreduction) under the supervision of different Oncology Departments nationwide. Nevertheless, it is worth mentioning that most of this study patients (71.9%) presented with advanced-stage (III and IV) ovarian carcinoma which sometimes seemed quite confusing by its clinical polymorphism but its prognosis was very much related to both the degree of surgical cytoreduction accomplished and tumor sensitivity to chemotherapy. Finally, although this study does not allow us to draw firm conclusions it is an attempt to share out our current perception on the primary epithelial ovarian cancer management.

The study of the diaphoresis and leucinaminopeptidasis activity in experimental enteroviroses.

This paper studies, through histochemistry methods, the answer of some enzymes (Diaphoresis and Leuciaminopeptidasis) from intestinal wall structure to the viral aggression experimentally obtained. The most characteristic aspects occur at the intestinal mucous membrane level. The microbial aggression stimulates both macrophagous and immune processes, particularly those of humoral type. Thus, a cellular defence phagocyte and immune barrier with release of Ig and especially IgA is achieved subepithelial.

Histoenzymatic changes at splenic level in experimental enterovirosis.

An experimental model was established to observe the lymphoplasmocytic and macrophage reaction at distance, in spleen, following the intraperitoneal administration of Coxsackie B3 virus. In the aftermath of the experimental viral aggression the thymo-dependent immune system did not seem altered, while the thymo-independent one was severely challenged. The immune reaction of the humoral type was accompanied by a stimulation of the macrophagous type-cells.

The coexistence of atypical intraductal hyperplasias with breast carcinoma.

We present a study made during 4 years (1992-1996), which pursued the underlining of the atypical intraductal hiperplasias (A.I.D.H.) lesions, met isolated or in the association with mamar carcinoma. Our study included a 188 number of the breast tumors, among: in the 23 cases we established the existence of the modification by the A.I.D.H, type at the fibrocystics disease associated or not with the other benign diseases of the breast (fibroadenosis, intraductal papiloma) and in the 63 cases there were the modification by the AIDH associated with in situ or invasiv carcinoma. Epithelial hyperplasia is frequently associated with the fibrocystic changes, being included in the category of fibrocystic or proliferating modifications. The synonymous terms used for the epithelial hyperplasia are the hiperplazia ductala, or the epitelioza, or the papilomatosis The last two are suggested by the proliferation possibility (papillary or linear) of the epithelial or the mio-epithelial cells. Regardless of the microscopic aspect of the lesion, that should be acknowledged and treated as it is, due to the increased risk of the development of a carcinoma later on, and also due to the ratio of association between the modification and the mammary carcinoma. The risk of occurrence of subsequent carcinoma is augmented in the presence of the epithelial atipii and also increases in the presence of a mammary carcinoma at the relatives of the first rank (1.3). In this context, the importance of the differential diagnosis between the simple intraductal hyperplasia and the atypical one, the difficulty of differentiation from intraductal carcinoma in some cases, and finally the association with an increased risk of subsequent occurrence of carcinoma, constitute into sufficient arguments to consider this topic separately.

Diagnosis problems in a case of carcinoma with clear cells at the uterine cervix level.

We present the case of a patient of 50 years old who developed a tumoral formation at the level of the uterine cervix. The histopathologic examination beside the clinic examination constituted an important element in diagnosis elaboration. The histopathologic study performed on serial sections from the tumoral fragment of the cervix allowed establishment of the positive diagnosis through the exclusion of some lesions with microscopic relatively similar images but which have a complete different prognosis and therapy.

Positive and differential diagnosis in a case of mucinous ovarian cystadenocarcinofibroma.

We present the case of a 50-year old patient with left tumoral pelviabdominal formation of the size of a foetus head. The histological study performed on sections from different areas of the tumour enabled the establishment of the diagnosis of mucinous cystadenocarcinofibroma of enteroid type with undifferentiated mucipar areas with ring in seal. The contralateral ovary was unaffected.

The immunohistochemical profile of luminal epithelial neoplastic component from pleomorphic adenomas of salivary glands.

The immunohistochemistry study made on 30 cases of salivary glands pleomorphic adenomas aims to establish the antigen profile of luminal epithelial neoplastic component proliferates into these tumors. We ascertain that luminal epithelial neoplastic cell were almost exclusively positive to antibody peculiar to epithelial differentiation (AE1/AE3, MNF 116, CK 8, CK 19, CK20, EMA) and occasional to S-100 protein and negative to vimentin, alpha-smooth muscle actin, calponin, Glial Fibrillary Acidic Protein (GFAP). This kind of immunoreactivity was similar to ductal cells from striated channels of salivary glands, suggesting the origin of such pleomorphic adenomas from this place.

Value of immunohistochemistry in confirming undifferentiated ovarian carcinomas.

The present immunohistochemical study was carried out on ten cases of ovarian tumours diagnosed with usual staining as undifferentiated ovarian carcinomas. A panel of antibodies was chosen in order to confirm the epithelial origin of these tumours and to eliminate some possible ovarian metastasis. The cytokeratin AE1/AE3, EMA and BerEP4 positivity supported their epithelial origin even if thirty per cent of the cases co-expressed cytokeratin and vimentin. Even if there is not a specific marker to confirm the ovarian origin of the tumours, the CA125 and CEA positivity suggested that these carcinomas might represent a serous or a mucinous dedifferentiation. Associated with these antibodies, the calretinin, CK7 and CK20 stainings allowed the separation of these tumours from peritoneal mesotheliomas with ovarian extension and from ovarian metastasis originating in the gastrointestinal tract. The evaluation of the PCNA labelling index confirms the high degree of cell proliferation and the aggressiveness of these tumours.

Histopathologic and immunohistochemical study in one case of cystadenoma of parotid gland becoming malignant.

Cystadenoma and cystadenocarcinoma of the salivary glands are rare tumors, approximately half of cases being diagnosed at the level of parotid. The studied tumor associated both benign and malign zones, with deep infiltrating character in the adjacent structures. In both areas the growing pattern was predominantly cystic and papillary, and the proliferated neoplastic cells were majority cubical or columnar, with low grade of nuclear pleomorphism in the becoming malign areas, discordant with the infiltrative character of the neoplasm. Immunohistochemically, we investigated the tumor with AE1-AE3, CEA, SMA, S-100 and PCNA.

Histopathologic aspects in microglandular hyperplasia of endocervix.

In this study we analysed 373 cases of microglandular hyperplasia of endocervix belonging to patients from the III-VII decades of life. Most of the lesions had the typic aspect of proliferation of small glandular lumen lined by endocervix type epithelium, with or without areas of basal proliferation or immature squamous metaplasia. In 8 cases the growing patterns were peculiar, with mucinous or florid aspects, three of the cases rising differential diagnosis problems with cervical carcinoma.

[The mirage of the first lesion (gallstones) and laparoscopic cholecystectomy are able to defer the diagnosis of colon cancer].

The study's aim was to analyze a series of colon cancer cases in which the mirage of the first (clinically most obvious) lesion (gallstones) along with its minimally invasive approach - that explored only the biliary disease - had contributed to the delay of large bowel malignancy' diagnosis and treatment. 1327 patients aged between 17 and 83 years and diagnosed with cholecystolithiasis were operated upon laparoscopically in the Department of General Surgery of Craiova CFR University Hospital from 2000 through 2004. Four out of these 1327 patients (0,3%) were readmitted with the diagnosis of colon carcinoma between 1 and 16 months after the laparoscopic cholecystectomy. Our retrospective study gives a full report on these 4 cases insisting upon the links between their clinical - laboratory evaluations and final diagnosis. Despite the low laparoscopic cholecystectomy overlooked colon cancer' incidence it seems reasonable to both improve the technique of peritoneal cavity exploration during this type of surgery and extend the preoperative evaluation whenever the slightest suspicion of associated pathology is raised especially in patients over 50 years of age.

[The experience at the 4th Surgery Department of the University Hospital C.F. Craiova in the surgical treatment of inguinal hernia].

This paper aim is to present the experience of Surgery Department IV of University Hospital C. R. Craiova in groin hernias treatment using prosthetic meshes, also describing an original technical procedure of mesh-plasty that we have been practicing successfully in our clinic. The study is based on a number of 1757 groin hernias operated in Surgery Department IV of University Hospital C. F. Craiova during a period of 11 years (1993-2003). There have been used prosthetic meshes in a number of 230 hernias operated for the most part in the last years since mesh repair has become habitually. We have been using so far only prosthetic mesh made in Romania (polyester mesh). We had a single recurrent hernia and the immediate complications were minimal. The study refers us to practice prosthetic mesh repair in an extensive way and offers a technical alternative in using of prosthetic meshes by a simple and efficient procedure with good postoperative results.

[Invasion of the optic nerve in undifferentiated retinoblastoma].

The case of a 2- and a half-year-old child is presented. The diagnosis was left-eye retinoblastoma and the left-eye enucleation was done. The anatomic-pathological exam emphasizes a microscopic structure of undifferentiated retinoblastoma, with the invasion of the optic nerve. After about 11 months, the child came again, presenting a tumoral mass which occupies almost entirely the orbital cavity, and also an altered general status.
