Quality of life in young adults with type 1 diabetes in relation to demographic and disease variables.

Sixty-nine young adults (mean age 21 (range 15-25) years) with Type 1 diabetes completed measures of diabetes knowledge and quality of life. Factor analysis of the quality of life scale resulted in the identification of three subscales (social relationships, diabetes concerns, and impact). There was no relation between any of the quality of life subscales with knowledge, or with multiple versus twice-daily insulin injection regimens. Higher self-rated diabetes satisfaction was related to lower fructosamine levels and better clinic attendance (p less than 0.05). Females reported a more negative impact of diabetes on their lives compared with males (p less than 0.05).

Campylobacter jejuni in pregnancy.

Case notes of pregnancies with proven Campylobacter jejuni infections were collected from 2 Queensland teaching hospitals and reviewed. Of these cases, 2 pregnancies (3 fetuses) resulted in stillbirths, 2 neonates required treatment with antibiotic therapy within 2 days of birth, while the remaining 2 pregnancies were treated at the time of the infection and were not associated with adverse outcome. Maternal Campylobacter infection should be actively sought for in the patient with suspected infectious diarrhoea.

Geophysical variables and behavior: LXVIII. Distal and lunar variables and traffic accidents in Saskatchewan 1984-1989.

The daily total number of traffic accidents and traffic accident severity for the years 1984 to 1989 in Saskatchewan were analyzed to estimate relationships with day of the week, season of the year, and various lunar variables (lunar phase, distance of the moon from the earth, and moon-sun tidal index). A regression model with both a linear term and a trigonometric term was fitted to the data. No relationship between lunar cycles and total accidents or severity of accident was found. A larger number of total accidents was found during the winter months and weekends; however, more severe accidents tended to occur during the summer months. A slight linear trend was uncovered for total accidents over the time period under consideration.

General practice estimates of the prevalence of common chronic conditions.

This study measured the prevalence of chronic medical conditions in 4,549 middle aged persons attending three large general practices in Dublin over the course of a calender year. The prevalence of the following conditions were measured: coronary heart disease, hypertension, stroke, diabetes, asthma, chronic bronchitis, rheumatic disorders, dyspepsia, depression, anxiety disorders, psychoses, and cancer. In order to obtain a valid denominator for the study a second community based study was carried out in the same areas to determine what proportion of persons visit their general practitioner over the course of a year. Overall 40.5% of males and 44% of females suffered from a least one of the twelve conditions, with rheumatic disorders having the highest prevalence (14.5%) and psychotic disorders the lowest (0.75%).

An explanation for the difference in general practitioner consultation rates between GMS and non-GMS patients.

This study compares annual consultation rates between 3,111 General Medical Services and private patients, and their family doctors in three large general practices in Dublin. Visiting rates were substantially different between practices and between GMS and private patients. However, GMS patients were older and had 50% more chronic illnesses. When the effects of age and illness were controlled in a multiple regression model, GMS status accounted for less than 5% of the variation in visiting rates. GMS patients consult their practitioners in proportion to their medical need.

Critical comment on Hicks-Caskey and Potter, "Effect of the full moon on a sample of developmentally delayed, institutionalized women".

Hicks-Caskey and Potter (1991) claim to have found a "full moon effect" on women in a developmental center. Further, they suggest the discrepancies in findings on lunar effects can be accounted for by (i) a lack of equivalent operational definitions and (ii) a person selection factor. It is argued that the Hicks-Caskey and Potter findings are undermined by weekday, holiday, season, weather, particular staff-subject interactions, and expectancy effects. In addition, the proposed explanations for differing outcomes in lunar studies do not explain both the negative findings and conflicting positive findings.

Peanut agglutinin shows specificity for bone marrow plasma cells.

A panel of lectins was used to study surface carbohydrate expression on myeloma cells with the aim of finding a possible agent for in vitro bone marrow purging. Peanut agglutinin (PNA, galactose beta 1,3 N-acetylgalactosamine-binding) bound to all plasma cells in 33/34 bone marrow samples from myeloma patients and to all plasma cells in 11 bone marrow from patients with monoclonal gammopathy of undetermined significance and 10 normal bone marrow samples. Bone marrow and peripheral blood monocytes reacted weakly with PNA except in one case of acute monoblastic leukaemia and two of chronic myelomonocytic leukaemia in which monocytes were strongly positive. The only case of plasma cell leukaemia studied was PNA negative. All other bone marrow mononuclear cells were negative for PNA but became positive after sialidase treatment. Peanut agglutinin may have potential as an agent to be used in myeloma for in vitro marrow purging prior to autologous transplantation in combination with high dose chemotherapy.

Hepatic drug metabolizing enzyme activity in the channel catfish, Ictalurus punctatus.

1. Several pathways of drug metabolizing enzymic activity were measured in hepatic fractions of the channel catfish and rat using model substrates. The pathways examined included the O-demethylation of p-nitroanisole, microsomal ester hydrolysis of procaine and glucuronidation of p-nitrophenol, and the cytosolic acetylation of sulfamethazine and sulfation of 2-naphthol. Catfish liver preparations were incubated at both 25 degrees C and 37 degrees C. 2. The oxidative metabolism of p-nitrophenol was only one-eighth that of the rat at 37 degrees C and one-twelfth that of the rat at 25 degrees C. 3. Procaine ester hydrolysis was negligible in catfish microsomal preparations. 4. At 37 degrees C, p-nitrophenol glucuronidation was equivalent in catfish and rat microsomes. 5. Catfish cytosolic preparations exhibited N-acetyltransferase and arylsulfotransferase nearly comparable to those of the rat. 6. Rates of glucuronidation and sulfation were higher at 37 degrees C than at 25 degrees C in hepatic fractions of catfish.

Reduced oxidative function in gingival crevicular neutrophils in periodontal disease.

Measurable amounts of viable and functional polymorphonuclear neutrophils (PMNs) are recovered from pooled washings of the gingival crevice of healthy individuals. In the present study, we have assessed the ability of the PMNs removed from single healthy or diseased pocket sites to mount an oxidative burst when challenged with phorbol myristate acetate (PMA) and compared these activities with each other and with those obtained with autologous peripheral-blood PMNs. The oxidative burst after PMA stimulation was evaluated by using methods developed for the flow cytometer. The results showed that the PMNs collected from untreated disease sites were minimally responsive to PMA when compared with peripheral-blood PMNs collected at the same time from the same individual. Thus, whereas the peripheral-blood PMNs exhibited significantly lower resting oxidative product formation and a 500% increase when stimulated with PMA, all gingival-crevicular PMNs exhibited significantly higher resting formation of oxidized products but only a 150% increase after PMA stimulation. PMNs obtained from a consistently healthy site had significantly higher resting production of oxidized products and were able to mount the greatest absolute increase in oxidized products after PMA stimulation when compared with PMNs collected from diseases sites. Mechanical debridement of these diseased sites, which both reduced the bacterial numbers and restored clinical health, resulted in the recovery of gingival-crevicular PMNs that exhibited an oxidative burst more typical of that observed in PMNs obtained from healthy gingival sites and from the peripheral blood. This suggested that the PMNs collected from the diseased sites either had been exhausted by the large numbers of bacteria present in these sites or had been specifically inhibited by these bacteria.

Muscle fiber composition and enzyme activities in elite female distance runners.

This study has attempted to describe the fiber composition and respiratory potential in the leg (gastrocnemius) muscle of highly trained, elite female distance runners. As noted, many of these women were competing successfully in middle-distance (1500-3000 m) rather than long distance (10 km and marathon) events. This fact appears to explain many of the differences observed between these "elite female runners" and the "elite male runners" studied in the 1975 Dallas study (6). With the exception of differences in muscle fiber areas, the female runners appear to show the same enzyme adaptations to endurance training that have been reported for male runners. The most notable finding of this study was the remarkable similarity of muscle fiber composition and enzyme activity for male and female runners when matched according to their preferred competitive distance.

Fractures of the femur treated by femoral plating using the anterolateral approach.

A review of 56 fractures of the femoral shaft plated by the anterolateral approach was undertaken. The final results were graded as excellent or good in 37 cases (66 per cent). Loss of flexion of the knee of more than 10 degrees occurred in 17 cases (30 per cent); these were mostly patients whose plating was delayed by more than 14 days, or was done as a secondary procedure. Patients with open fractures and with other fractures in the same femur tended to have poorer final results. The late complication rate was 18 per cent, which was similar to that for the posterolateral approach for plating the femoral shaft. It is our view that the anterolateral approach is satisfactory for plating the femoral shaft.

A hole in (B) one.

Radiolucencies are frequently observed in the carpus, often in asymptomatic patients. Evaluation of the clinical significance of these radiolucent areas is aided by use of bone scanning. In this case, correlation of the physical exam, bone scan and tomograms allowed effective direct surgical treatment of a symptomatic intraosseous ganglion.
