Anatomical configuration of the spinal column in the supine position. I. A study using magnetic resonance imaging.

In order to clarify the anatomical configuration of the spinal column in the supine position, we have examined T1-weighted sagittal midline magnetic resonance images of the spinal column in 20 healthy volunteers (11 men, nine women) in the supine position. The mean maximum angles of decline of the lumbar spinal canal in men and women were 12.6 (SD 3.9) degrees and 13.4 (3.3) degrees in the cephalad direction, respectively. The maximum angles of incline of the upper thoracic spinal canal in men and women were 20.3 (4.0) degrees and 18.5 (2.5) degrees, respectively. The median highest points of the lumbar spinal canal in men and women were located at L4 (range L3-4 to L4) and L4 (L4), respectively. The lowest point of the thoracic spinal canal was located at T8 (T7-T9) in both men and women. We have demonstrated that both lumbar lordosis and thoracic kyphosis differ between individuals, particularly with respect to the lowest point of the thoracic spinal canal, which is located between T7 and T9.

Anatomical configuration of the spinal column in the supine position. II. Comparison of pregnant and non-pregnant women.

To assess the changes in the curvature of the spinal column in the supine position during pregnancy, we studied seven pregnant (32-37 weeks of pregnancy) and seven non-pregnant women using magnetic resonance imaging. T1-weighted sagittal midline magnetic resonance images of the spinal column were obtained with subjects in the supine position with left tilt. There was no significant difference in the maximum angle of decline of the lumbar spinal canal between the pregnant (mean 12.4 (SD 3.3) degrees) and non-pregnant (13.4 (3.9) degrees) groups. The maximum angle of incline of the upper thoracic spinal canal was smaller in the pregnant (15.8 (2.9) degrees) than in the non-pregnant (22.7 (6.0) degrees) group. The highest point of the lumbar spinal canal was located at a lower lumbar region in the pregnant (median L4-5 (range L4 to L4-5)) than in the non-pregnant (L4 (L3-4 to L4)) group. The lowest point of the thoracic spinal canal was located at a higher thoracic region in the pregnant (T6-7 (T6 to T7-8)) than in the non-pregnant (T8 (T6-7 to T9)) group. This study revealed that the apex of lumbar lordosis was caudad and thoracic kyphosis was reduced in the supine position in the later stages of pregnancy. These changes in the curvature of the spinal column may explain, in part, the enhanced cephalad spread of subarachnoid hyperbaric anaesthetic solutions in the later stages of pregnancy.

Computerized tomography in the assessment of brain shifts in acute subdural hematoma.

Computed tomography (CT) was used to characterize brain shifts in 30 adult patients (mean age 51.9 years) with fatal supratentorial acute subdural hematomes (ASH) who were admitted within 3 hours of the head trauma. The Glasgow Coma Scale (GCS) score, hematoma width, and midline shift were tabulated. The width of the cistern surrounding the brainstem (CSBS) and the distance from the floor of the fourth ventricle to the posterior clinoid process (IVv-PC) were measured. Opening (+) or disappearance (-) of the suprasellar cistern (SC) was determined using axial CT. The data were compared with those in agematched controls (n = 26) using the Wilcoxon and Kruskal-Wallis tests. The hematomas in the SC(-) group (n = 22, mean GCS score: 3.8 +/- 1.0) were wider (p = 0.02) than those in the SC(+) group (n = 8, mean GCS score: 4.7 +/- 1.5). The CSBS in the SC(+) group was significantly smaller than that in the controls (p < 0.01). In the SC(-) group the CSBS was smaller and the IVv-PC was significantly larger than those in the controls (IVv-PC; p < 0.01) and the SC(+) group (IVv-PC; p < 0.05). In the evolution of brainstem shifts in ASH, when the SC is open the predominant vector is in the direction of the skull base, while when the SC is not observed, an axial vector shifts the brainstem.

Nuclear localization and transforming activity of human papillomavirus type 16 E7-beta-galactosidase fusion protein: characterization of the nuclear localization sequence.

A MAb 9F6 was capable of staining HPV16 E7 in a human cervical carcinoma line, CaSki, and rat 3Y1 cells stably expressing HPV16 E7 gene. Contrary to the current understanding of E7 as a nuclear protein, the site of staining was clearly cytoplasmic. The subcellular localization of E7 was further studied by using the beta-galactosidase (beta-gal) receptor method. A fusion protein composed of E7 and beta-gal was stably expressed in rat 3Y1 cells. The beta-gal activity in these cells was detected mostly in the nucleus, even though 9F6 still stained the cytoplasm of these cells. The fusion protein was also found to be oncogenic since transfected 3Y1 cells acquired transformed phenotypes such as increased saturation density and anchorage-independent growth. These results indicate that biologically active E7 exists mostly in the nucleus, but nuclear E7 is masked from 9F6. A series of deletion mutants of E7 further demonstrated that the amino acid sequence from 16 to 41 was enough to transport beta-gal into the nucleus. A mutation either at amino acid 24 or 26 which is known to disrupt the binding of E7 to RB, the retinoblastoma gene product, did not strongly affect the nuclear localization of the fusion protein, suggesting that the nuclear transportation of E7 is mostly independent of RB binding.

Cholecystokinin is not a major regulator in the digestive system in the chicken.

To find out whether physiological concentrations of cholecystokinin (CCK), a gastrointestinal hormone in mammals, are also active in chickens, the pancreatic amylase secretory response to CCK-8 was investigated in vitro. Rat pancreatic acini responded to the physiological concentration of CCK-8, but in chickens amylase release was induced at a concentration of CCK-8 1000 times higher than that observed in rats. In another experiment, biliary flow was tested with several doses of CCK-8. The bile flow was stimulated in a dose-dependent fashion, but a significant enhancement was not obtained at a concentration of 0.5 micrograms CCK-8/kg body weight, which was considerably higher than physiological ones. It is concluded that endogenous CCK does not have an important role in the digestive system in the chicken.

Nitric oxide controls feeding behavior in the chicken.

To investigate the effect of nitric oxide (NO) on food intake in the chicken, L-NG-nitro-arginine methyl ester HCl (L-NNA), an inhibitor of NO synthase, was applied. When i.p. administered, L-NNA significantly inhibited the food intake of broiler chickens in a dose response manner. Food ingestion was also depressed by the i.c.v. injection of L-NNA in a dose response fashion. The effect of L-NNA was attenuated by i.c.v. administration of L-arginine. These results suggest that central NO may control feeding behavior in the chicken.

Endogenous cholecystokinin is not a major regulator of food intake in the chicken.

This study investigated whether or not endogenous cholecystokinin exerts satiety effects in chickens. After several doses (0, 1, 2 and 4 micrograms.kg body weight-1) of intravenous injection of caerulein, the bile flow was increased in a dose-dependent fashion. However, the pharmacological level of caerulein failed to suppress the food intake of chickens. Two potent stimulators of endogenous cholecystokinin, i.e., soybean trypsin inhibitor and phenylalanine were administered to chickens before feeding and food intake was determined over 2 h. The soybean trypsin inhibitor and phenylalanine did not alter food intake. Devazepide, a cholecystokinin-A receptor antagonist, significantly decreased amylase release from the dispersed chicken pancreatic acini stimulated by caerulein. However, devazepide did not improve food intake of the chicken. The results obtained suggest that endogenous cholecystokinin may not act as a satiety signal in chickens.

Interactions between medium-chain and long-chain triacylglycerols in lipid and energy metabolism in growing chicks.

The combined effects of dietary medium-chain triacylglycerols (MCT) and long-chain triacylglycerols (LCT) on lipid and energy utilization in chicks were investigated. Corn oil was used as the LCT source, and trioctanoin (8:0) was used as the MCT source. The efficiency of dietary energy utilization (such as metabolizable energy values and fat and energy retention) decreased linearly as the level of MCT increased, but the efficiency of dietary protein utilization (protein retained per protein consumed) was not affected in a consistent manner. Fecal saturated and monounsaturated fatty acid composition was dependent on the dietary fatty acid composition, whereas fecal linoleic acid content was not. It is concluded that dietary MCT and LCT influence each other to some degree, with respect to protein and lipid metabolism in chicks. Moreover, in most cases, the nutritional characteristics of each triacylglycerol, including food efficiency and fat and energy retention, are independent of each other in growing chicks.

Aspirin toxicity in chicks given diets deficient in linoleic acid.

The toxicity of dietary aspirin on growth rate and lipid metabolism was investigated under linoleic acid (LA; 18: 2n-6) deficient conditions. One-week-old chicks were given diets containing 0 or 2% LA with or without 0.4% aspirin, until 4 weeks of age. Growth was severely depressed by dietary aspirin when chicks were given the LA-free diet. The liver was enlarged by both the aspirin and LA deficiency. The aspirin treatment induced a significant increase of 18:0 and arachidonic acid (20: 4n-6) and a decrease of 18: 1n-9 in the liver. In chicks fed LA-free diets, the ratio of 20:3n-9/20: 4n-6, which was used as an indicator of LA deficiency, was suppressed by aspirin treatment. In conclusion, the present results suggest that aspirin toxicity is altered by dietary LA concentrations.

Direct association of occludin with ZO-1 and its possible involvement in the localization of occludin at tight junctions.

Occludin is an integral membrane protein localizing at tight junctions (TJ) with four transmembrane domains and a long COOH-terminal cytoplasmic domain (domain E) consisting of 255 amino acids. Immunofluorescence and laser scan microscopy revealed that chick full-length occludin introduced into human and bovine epithelial cells was correctly delivered to and incorporated into preexisting TJ. Further transfection studies with various deletion mutants showed that the domain E, especially its COOH-terminal approximately 150 amino acids (domain E358/504), was necessary for the localization of occludin at TJ. Secondly, domain E was expressed in Escherichia coli as a fusion protein with glutathione-S-transferase, and this fusion protein was shown to be specifically bound to a complex of ZO-1 (220 kD) and ZO-2 (160 kD) among various membrane peripheral proteins. In vitro binding analyses using glutathione-S-transferase fusion proteins of various deletion mutants of domain E narrowed down the sequence necessary for the ZO-1/ZO-2 association into the domain E358/504. Furthermore, this region directly associated with the recombinant ZO-1 produced in E. coli. We concluded that occludin itself can localize at TJ and directly associate with ZO-1. The coincidence of the sequence necessary for the ZO-1 association with that for the TJ localization suggests that the association with underlying cytoskeletons through ZO-1 is required for occludin to be localized at TJ.

Hepatic calcification in an anicteric patient with biliary atresia after hepatic portoenterostomy.

Hepatic calcification was found in an anicteric infant with biliary atresia after hepatic portoenterostomy. When she was 2 years of age, ultrasonography detected an echogenic structure with acoustic shadowing. Computerized tomography located it at the proximal portion of Segment 6 in the right lobe. Although both techniques failed to exhibit dilatation of the peripheral bile ducts, we believe intrahepatic cholelithiasis provides the most likely diagnosis. Although seven cases with biliary atresia were reported to have this complication, all of them were icteric when the diagnosis was confirmed. The patient should be carefully followed up since the succeeding stones may cause jaundice by obstructing the main duct.

Dynamics of cerebral metabolism in patients with chronic subdural hematoma evaluated with phosphorous 31 MR spectroscopy before and after surgery.

PURPOSE: To determine whether the depression of cerebral bioenergetic metabolism caused by chronic subdural hematomas can account for neurologic dysfunction and whether the degree of metabolic depression may be useful for clinical assessment and therapy. METHODS: Sixteen patients who had chronic subdural hematomas with hemiparesis and/or mental disturbances underwent phosphorous 31 MR spectroscopy before and 10 to 14 days after surgery. Phosphorous 31 MR spectroscopy was also performed on 5 patients who had chronic subdural hematomas with only slight headaches who were treated by conservative therapy and on 10 healthy volunteers. RESULTS: The peroperative phosphocreatine-to-inorganic phosphate ratio (2.10 +/- 0.36) improved to normal values (2.69 +/- 0.44) after evacuation of hematomas. This improvement was accompanied by complete disappearance of hemiparesis and/or mental disturbance. Brain tissue pH also improved from 7.07 +/- 0.11 to 7.205 +/- 0.13 after surgery. On the other hand, patients who had chronic subdural hematomas with only slight headaches had the same phosphocreatine-to-inorganic phosphate ratio and brain intracellular pH as healthy volunteers. CONCLUSION: The phosphocreatine-to-inorganic phosphate ratio may be useful for determining when to operate on patients with chronic subdural hematomas and to assess the efficacy of treatment.

Influence of dietary sorbose on diabetes in nonobese diabetic mice.

The effect of dietary sorbose on diabetes after the incidence of the syndrome in the nonobese diabetic mouse was investigated in the animals from 8 to 14 weeks of age. When sucrose (200 g/kg diet) in a control diet was replaced by sorbose, the body weight and the blood glucose concentration were significantly reduced, but the serum insulin concentration was unchanged. The urinary glucose concentration was the same for both sucrose and sorbose diets. It is suggested that after the incidence of diabetes, dietary sorbose could not improve urinary excretion of glucose, even though sorbose could reduce the blood glucose concentration.

[Aging changes of cerebral high intensity areas on T2-weighted MRI--a study in medical checkup patients].

To determine normal aging process of the brain, cerebral T2-weighted MRI was evaluated in 400 healthy individuals who visited our hospital for routine medical checkups. The results are summarized as follows. 1) High intensity areas (measuring 2 mm x 2 mm or more) were observed most frequently in the cerebral white matter including the corona radiata (CR). The incidence of these areas increased linearly with age, and approached about 50% in the patients of the seventh decade. 2) High intensity areas in the basal ganglia (BG) were the second commonest. High intensity areas were rarely seen in the thalamus (TH) and pontine base (PO). 3) High intensity areas were confined to the CR (66.9%) in the majority of the cases, while there were few cases having these areas restricted to the BG, TH and PO; that is, most of them were accompanied by additional high intensity areas in the CR. 4) Periventricular hyperintensity areas increased in size with age, and were prominent in those who had multiple or large high intensity areas in the CR, or those who had signal hyperintensity in the BG, TH or PO.

Cavernous lymphangioma of the thorax: MRI findings.

Cavernous lymphangiomas of the thorax are extremely rare. We encountered an 11-year-old boy with such a disorder. Magnetic resonance imaging (MRI) showed numerous small rounded and tubular structures within the mediastinal mass; we believe these represent dilated lymphatic channels.

Altered growth of human colon cancer cell lines disrupted at activated Ki-ras.

Point mutations that activate the Ki-ras proto-oncogene are presented in about 50 percent of human colorectal tumors. To study the functional significance of these mutations, the activated Ki-ras genes in two human colon carcinoma cell lines, DLD-1 and HCT 116, were disrupted by homologous recombination. Compared with parental cells, cells disrupted at the activated Ki-ras gene were morphologically altered, lost the capacity for anchorage-independent growth, grew more slowly both in vitro and in nude mice, and showed reduced expression of c-myc. Thus, the activated Ki-ras gene plays a key role in colorectal tumorigenesis through altered cell differentiation and cell growth.

p-Chlorophenylalanine damages pancreatic acinar cell of the chicken.

1. Birds were injected intraperitoneally with DL-p-chlorophenylalanine (p-CP), which is an inhibitor of phenylalanine hydroxylase, or saline one day before the experiment. The weight and cell size of pancreas increased by the p-CP treatment. 2. The application of acetylcholine (5.50 x 10(-9) to 5.50 x 10(-5) M) caused a dose dependent increase in amylase release from the superfused segment of pancreas, although the response significantly decreased in the pancreatic segment treated with p-CP, compared with the control.
