[Responders and non-responders of surfactant therapy of very small premature infants. A clinical, histopathologic and immunohistochemical analysis].

The administration of surfactants is highly effective and yields an immediate clinical response in cases of a lack of surfactant in the premature lung of preterm infants. The administration of surfactants might diminish the need for intensive care and artificial ventilation thereby also reducing or preventing the severe side effects associated with these therapeutic measures. However, the lack of clear clinical features precludes a reliable predictive forecast of the therapeutic outcome. Only the therapeutic follow-up period can disclose possible damage due to the administration of surfactants. Histological preparations of lung specimens from non-responding infants reveal cellular inflammatory infiltrations, destruction of the alveolo-capillary basement membrane and monocytic phagocytosis.

[Multifocal carcinoma of the upper female genital tract: diffuse Müllerian neoplasia].

The simultaneous occurrence of moderately differentiated mesonephroid adenocarcinomas of the endocervix and the ovary and a well differentiated adenocarcinoma of the endometrium in a 46-year-old woman are reported. The problems of classification as multiple primary neoplasma or as metastatic spread are discussed. Histogenesis of multiple primary epithelial malignancies of the female genital tract is explained by the theory of area response of embryologically related organs to an unknown carcinogenic stimulus.

Lichen ruber mucosae with esophageal involvement.

The clinical course of a 50-year-old woman with oral lichen ruber planus (Irp) and prolonged dysphagia is described. The swallowing problems of this patient were related to an inflammatory lesion of the middle third of the esophagus, leading to stenosis. Distal to this area, the esophagus was covered with mucosa of the cardia type, as seen in endobrachyesophagus. Apart from reflux disease, the Irp may have accounted for the stenosis of the mid-esophagus. The mucosal lesions disappeared after administration of etretinate (Tigason). After endoscopic dilatation the patient was able to swallow normally again.

[Malignant melanoma of the anorectum].

Malignant melanoma of the anal canal in stages I, II and III, respectively, was diagnosed in a 58-year-old woman and two men, aged 69 and 73 years. The patients' age was thus above the average reported age of manifestation (60 years) of this tumour. In two patient the tumours had been incorrectly diagnosed because of their proximity to the linea dentata and their similarity to haemorrhoids: operation had been delayed for four and about six months, respectively. Treatment of choice for stages I and II was abdominoperineal amputation of the rectum, while in the patient with stage III a colostomy was performed as a local palliative measure. The patients with stage II and III tumour died after six and 12 months, respectively, a survival time which corresponds to that expected from published series. Radiotherapy did not influence tumour growth in the patient in stage III. The patient in stage I has been free of tumour for six months.

Diamine oxidase as a marker of intestinal integrity in acute appendicitis.

In the operative treatment of appendicitis the so called negative appendectomy is an important issue because of its increased morbidity. From the hypothesis that the intestinal diamine oxidase activity is a suitable marker of mucosal integrity, the distribution pattern of the enzyme in appendices histologically classified as inflamed or not inflamed was studied. Histologically apparent inflammation of the appendix was connected with a significant reduction of diamine oxidase activity. The determination of this enzymic activity may be a simple and sensitive test for mucosal inflammation of the appendix even at a very early state. This could reduce the rate of negative appendectomies and influence thereby risk-cost-benefit calculations.

Acute Disease↗

[Granular-cell tumor of the esophagus].

A granular-cell oesophageal tumour was demonstrated in a 47-year-old man. With rare exception this type of tumour is benign. As it grows dysphagia develops. Surgical resection is the treatment of choice. It should be a wide excision, because the non-encapsulated tumour is microscopically more extensive than macroscopically. If too little is excised it may recur. But the high operative risk justifies expectant waiting, perhaps combined with local partial endoscopic removal of the tumour. Only in the rare instance of a small or pedicled tumour is total endoscopic excision possible.

[Animal experiment studies of suture material and suture technic of the stomach].

The aim of these examinations was a comparison of the tensile strength and the histology of single and two-layer sutured gastrotomies applied to rats. Independent of the applied suture material and suture technique all experiments resulted in the same tensile strength of the suture between the 5th and 7th postoperative day as in the control group. Differences of anastomotic dehiscence did not exist. The single-layer can be loaded as well as the two-layer suture. Any difference of the tensile strength of anastomosis depending on the suture material does not exist.

Endoscopic surgery in the rectum.

A new transanal endoscopic operative technique permits microsurgery in the rectal cavity and the placing of surgical sutures. Compared with other procedures this one is non-aggressive, and there were not postoperative complications in twelve cases. A stereoscopic optical system, a new operating rectoscope and special surgical instruments, as well as a modification to an insufflation device are necessary for the endoscopic operation.

Adenocarcinoma↗

Prognostic relevance of ploidy, proliferation, and resistance-predictive tests in ovarian carcinoma.

In a cooperative study specimens of 37 patients with stage III and IV ovarian carcinomas who had been treated with chemotherapy were investigated utilizing flow cytometry and an in vitro short-term test for predicting resistance. Patients with aneuploid tumors had significantly shorter survival rates than did those with diploid tumors. Patients whose tumors showed a low G0/G1 cell proportion or a high proliferation pool (S- and G2/M cell-proportion) seemed to die earlier. There was also a tendency for patients with in vitro resistant tumors to die earlier under chemotherapy than those with sensitive tumors.

[Transanal endoscopic microsurgery].

A new endoscopic surgical method was developed for removal of large sessile adenomas of the rectum and lower sigmoid. Stereoscopic sight and magnifying glasses do allow precise surgery of these tumors. If adenomas are present they can be removed using the technique of mucosa excision; if on the other hand rectum carcinomas have to be removed the complete wall of the rectum in an appropriate extension can be excised. Hemorrhages which occur during preparation always can be stopped by diathermy. The surgical intervention is terminated by a transverse continuous suture of the defect. 33 patients were operated upon in this way from July 1983 to April 1985. Areas with an diameter of up to 10 cm could be removed, even located up to 18 cm in the colon. Suturing of the defect could always be performed without problem. In 1 patient a relapse occurred.
