[Primary malignant melanoma of the esophagus].

Two case reports are given of patients suffering from malignant melanoma of the esophagus, which occurs very rarely. In both patients the leading clinical symptom was dysphagia. Diagnosis was confirmed by endoscopy and biopsy, and surgery was done. One patient died within 3 weeks from pneumonia, the second patient is alive 3 months after resection of the tumor and is free of symptoms.

[Titer variations in the rheumatoid factor reaction during the menstrual cycle. A contribution on the problem of the value of serologic data].

Sixteen healthy women showed a high variability of their titres in the antistreptolysin reaction, in the antideoxyribonuclease B reaction and in three different tests for determination of the rheumatoid factor during their cycles of menstruation. Two men of the same age had a similar behavoir. Therefore serologic results may be difficult to interpret.

Cytochemical investigations of phagocytes in thyroid gland cysts.

Certain similarities in the cytoplasmic structure could indicate a histogenetic affinity between thyrocytes and pigmented macrophages in thyroid glands. On the other hand, the monocytogenic origin of macrophages in other organs in the body contradicts this assumption. We investigated these cells in smears of thyroid aspirates as well as in imprints, frozen sections and paraffin-embedded sections of freshly removed thyroid glands, using several cytochemical reactions and the usual panoptic stains. With the sodium-fluoride-inhibited alpha-naphthyl acetate esterase and tartrate-inhibited acid phosphatase reactions, the pigmented macrophages exhibited enzyme cytochemical patterns corresponding to those of blood monocytes or monocytogenic histiocytes but differing definitely from those of thyrocytes. Furthermore, we observed macrophages that exhibited a clearly monocytoid nuclear form and chromatin pattern in the interstitial space of the thyroid gland. These findings practically exclude a derivation of thyroidal pigmented macrophages from follicular epithelial cells and positively underline their derivation from blood monocytes.

[Results of combined surgical and radiological treatment of bladder cancer with particular reference to local tumor recurrence].

The combined surgico-radiological treatment of the carcinoma of the urinary bladder has clearly improved the results of treatment. The local number of recidivations after this therapy is, however, still very high and in the own number of patients is about 50%. Most recidivations appear briefly (1 to 2 years) after the end of the radiological treatment and are spread above all multilocularly in the bladder. Thus an additional more radical surgical therapy is indicated. In the planning of the primary therapy, therefore, the scarcely reduced treatment under conservation of the bladder should be taken into consideration, since the recidivations decisively determine the further prognosis of the patient.

[Glucose determination in the hemolysate, manual analysis].

A method for the manual determination of glucose in hemolysate is described. Both hexokinase/G6P-DH (e.g. Glucoquant) and glucose dehydrogenase can be used as reagents. The two methods correlate satisfactorily with an approved method. Correlation with an automated method: Gluc-DH: r = 0,98323; hexokinase/G6P-DH: r = 0,94172.

[Ultrasonographic detection and clinical importance of placental migration (author's transl)].

Pathological insertions were diagnosed in 66 of 4, 804 ultrasonic placentograms, beginning with the 13th week of pregnancy. Placental migration, that is displacement towards the fundus of the uterus of the caudal adge of the placenta, was followed up by consecutive checks. Pathological placenta locations were recorded in 4.1 per cent of all cases, up to the 20th week of pregnancy, was found to emanate from migration in the majority of all cases. The process of placental migration usually takes place up to the 30th week of pregnancy and is completed by the 35th week. If pathological insertions were diagnosed after the 20th week f pregnancy, migration was found to take place with much lower incidence. Methodical problems relating to ultrasonographic representation are discussed together with clinical consequences.

[Role of ultrasonography in attention to multiple pregnancy (author's transl)].

Ultrasonic examination, 482 in totality, were applied to 199 cases of multiple pregnancy, between the twelfth and 41st weeks of gestation. Gestation periods were successfully extended by improved early diagnosis and well-timed prophylaxis of premature births which had all been enabled by the availability of ultrasonography. The period under review was from 1977 to 1979, and gestational periods of 37 weeks and more were reached, within these three years, by 76 per cent of all women with twin pregnancy. Reference is made to issues relating to the use of ultrasonography on multiple pregnancies both prior to the 20th week and in a highly advanced stage. -- Great importance to diagnosis and monitoring of multiple pregnancies with differentiated growth is being assumed by ultrasonic cephalometry and thoracometry. Intensive treatment is, in any case, required by twin pregnancies in which biparietal diameters differ by 5 mm or more.

[Use of the dura in the surgical treatment of bladder cancer].

It is reported on 9 patients in whom a dura graft was performed on account of a carcinoma of the urinary bladder. Apart from one patient who before underwent radiotherapy the dura healed. As to complications three times formations of calculi and twice rejection of larger parts of the dura were found. Indications to dura graft as well as its advantages and disadvantages are discussed and it is referred to immunological problems which are still to be clarified.

[Use of infrared technic in veterinary medicine. (Review)].

More effectiveness of veterinary examination of the living animal is an important demand of socialist practice which applies equally to conditions of stepwise transition to industrialised production processes and to conventional production. The physiological and physico-technical prerequisites for possible use of infrared techniques in veterinary attention and production monitoring are discussed on the basis of available literature, with particular reference being made also to human medicine.

[The maternal and perinatal mortality and morbitity in attendance to Caesarean section in the period from 1956--1976 at the UFK-Jena (author's transl)].

In the period from 1956--76 1 468 caesarean sections were accomplished at the UFK-Jena. 44 043 children were delivery during the 21-year period. The number of caesarean sections rose from 1,08% to 1956 to 10,8% in 1976. The fetal indication in 1956 was zero, on the other side it amounts to 34,3% in 1976. First place of the maternal indication in the 21-year period took in the cephalo pelvine disproportion. In the same time the maternal mortality in attendance of caesarean section was 12,3%. Considering the period between 1969--76, the maternal mortality fell to 2%. Maternal morbidity diminished from 1965 and obtained 23% in the period between 1974/1976. The feverish childbed took in the first place of the maternal morbidity (30%). The perinatal mortality fall from 28,57% in the period between 1956/1958 to 3,71% in the period between 1974/1976. The perinatal morbidity we could'nt state certainly. In 8,3% we found perinatal morbidity by acidosis. 5,5% of children, delivered by caesarean section needed medical care. 16,4% of all children were either small for gestational age or immature. The problem of caesarean section as an important method for termination of labor is discussed.

[The relationship between haptoglobin type and serum agglutination titers against group G streptococci with the T4-antigen in pregnant women and newborn infants].

The relationship between haptoglobin type and titer of agglutination against streptococci carrying the T4-antigen was examined in 100 pairs of sera (mother/child). In healthy pregnant women high titers were predominant in individuals of haptoglobin type 2--2. Lower titers were found in sera of women with diseases during pregnancy. In 4% of the newborns only the haptoglobin type could be recognized. In comparison of maternal and neonate titers of agglutination pointed out the antibody-like activity of haptoglobins.

[Value of the electrophoresis mobility test in the differential diagnosis of obscure kidney findings].

The differential diagnosis of unclear carcinoma-suspicious renal findings often finishes with the test-exposition and nephrectomy. Also when all modern diagnostic possibilities are used not always a clear establishment of the diagnosis may be achieved. The cell electrophoresis mobility test after Field and Caspary may contribute to the differential-diagnostic clarification of the findings, when adequate antigens are used. For this purpose 10 own cases are discussed and these results are compared with suitable comparative groups. Summarizingly by the present study can be shown that the immunologic diagnostics of tumours in the field of unclear renal tumours may be a very good help in the diagnostics. When adequate antigens are used in the cell electrophoresis mobility test a coordination concerning the histospecificity can be achieved.
