Tissue polypeptide antigen (TPA) in urinary bladder cancer cytology: a follow-up study.

Tissue polypeptide antigen (TPA) has been detected by immunocytochemical assay on urine cytological samples from 28 asymptomatic patients, who previously had a histological diagnosis of papillary transitional cell carcinoma (PTCT) of the urinary bladder (UB), in order to evaluate its role in follow-up controls. TPA staining intensity (SI) in urothelial cells was evaluated to improve the diagnostic accuracy of cytology. Differentiated tumor cells were strongly stained for TPA, heavier than normal urothelial cells. Undifferentiated neoplastic cells were less stained for TPA with a wide range of SI. TPA detection revealed positive cytology in 21 (75%) of the considered cases. The accuracy of our cytological findings compared with both routine examinations and subsequent histopathological diagnosis was of 95.2%. Follow-up urinary cytology limits could be reduced by TPA searching in differentiated tumor cells, deriving from low-grade neoplasms.

[Cytodiagnosis by fine-needle aspiration (FNAB) in thyroid dysplasias and neoplasms observed in 1983-1987].

Cytology using fine needle aspiration biopsy has been widely used in dysplastic and neoplastic thyroid lesions as it is free of complications and very accurate. Personal experience with 2212 thyroid fine needle aspiration biopsies (1564 patients) performed in the period 1983-1987 is reported. Cytologic and histologic diagnoses are compared with respect to 139 patients. Diagnostic accuracy was 85.61%, with 9 false negatives (6.45%) and 11 false positives (7.51%). Sensitivity was higher in papillary thyroid carcinomas and Hürthle cell neoplasias than in follicular neoplasias and nodular goitres (89.66% and 90% versus 79.47% and 87.03% respectively). The false positives observed in follicular neoplasias depend on the limitations of cytologic and histologic analyses. Fine needle aspiration biopsy makes it possible to select candidates for thyroidectomy although in some cases surgical exploration is required for diagnosis.

Fatal hepatic vascular toxicity of DTIC. Is it really a rare event?

A fatal massive hepatic necrosis with widespread thrombotic occlusion of the small hepatic veins developed in two of 68 patients treated with DTIC for advanced melanoma in a randomized study. Thirteen similar reactions, in patients treated with single-agent DTIC, are reported in the literature. Several clinical and pathologic features distinguish this DTIC toxicity from Budd-Chiari syndrome and veno-occlusive disease (both well-known types of possibly drug-related hepatic vascular disease) and make it a distinctive syndrome. We were impressed by the repeated occurrence of this complication in a relatively small set of patients, in contrast with the rarity of the literature reports. We suggest that this dramatic complication could occur more frequently than commonly thought. Therefore, caution should be used with DTIC, particularly in curable patients.

Melanotic neuroectodermal tumour of infancy (MNTI). Immunohistochemical and ultrastructural study of a case.

A case of melanotic neuroectodermal tumour of infancy (MNTI) which occurred in the left lower jaw of a 15-months old child was studied by electron microscopy and immunohistochemical methods to verify the neural crest origin of this tumour. Two cell population were observed: pigment melanocyte-like cells and non-pigmented nerve-like cells. Immunohistochemical analysis with neuron-specific enolase (NSE) and S-100 protein visualized two cell subtypes in the non-pigmented cell population. The combined ultrastructural findings and immunohistochemical data confirm the neural crest origin of MNTI.

Cutaneous hyperpigmentation induced by amiodarone hydrochloride.

Amiodarone (Cordarone) is an iodinated compound widely used in the treatment of cardiac arrhythmias for more than a decade. A patient who developed a persistent blue-gray skin pigmentation of the light-exposed areas following long-term amiodarone hydrochloride administration is reported. The cutaneous signs and the ultrastructural findings are described. This rare iatrogenic hyperpigmentation is peculiarly due to lipofuscin and not melanin deposits. Its pathogenesis may be related to the basic action of the drug on the lysosome and to the extra phototoxic-induced lysosomal damage, which accounts for the specific location of the hyperpigmentation over the light-exposed areas.

Gastric carcinoids of ECL cells. Pathological and clinical analysis of eight cases.

Clinical, histological, histochemical and ultrastructural characteristics of eight cases of carcinoid tumors of the non-antral portion of the stomach are presented. Four cases with multiple polypoid lesions are accompanied by an increased level of gastrin. A normal level of gastrin was present in the other four cases with isolated tumor and a normal component of endocrine cells in the uninvolved mucosa. In the first group with multiple lesions, the histological and histochemical analysis of the endocrine cells revealed a wide range of appearances: a) "simple hyperplasia", b) "nodular hyperplasia", and c) carcinoid tumor. These aspects suggested a different pathogenesis for the carcinoid tumors of the non-antral portion of the stomach with possible therapeutical implications.

Cerebral mycosis: clinico-pathological report of four cases observed in fifteen months.

Cerebral mycosis is a rare condition, difficult to diagnose in life, but is not easily missed at post-mortem examination. We report clinical and pathological findings in four patients with long duration prodromes, seizures and psychiatric symptoms. All of them had been misdiagnosed. Actinomycotic granules in two and Aspergillus filaments in the other patients were ascertained at histological examination. Two of the patients were in their twenties and apparently healthy. We suggest that the possibility of mycotic infection should be considered even in cases where no lowering of the body's defences can be postulated. Cell mediated immunity defects may be assumed as underlying these infections. Therefore we would suggest the importance of immunological tests for type IV immune responsiveness in all intracranial infective processes of undefined etiology.

Squamous cell carcinoma arising in a dermoid cyst of the ovary. Clinical and pathological report of a case.

A case of squamous cell carcinoma arising in a dermoid cyst of the ovary of a 38 year-old woman is reported. Its frequency is nearly 1%; nevertheless it is opinion that the real frequency could be higher if the hystological examination of the cyst would be made more accurately. Clinical and pathological findings of this case are presented in this brief note. The importance to always make an intraoperatively frozen-section examination is emphasized. The patient is alive and well after six months after surgery.

[Prognostic findings in histopathology in bronchopulmonary carcinoma after surgery].

The prognostic value of hystological cell type, nuclear grading and mitotic index has been examined in 48 carcinomas of the lung after radical surgery. Three hystological cell types have been identified: squamous cell carcinoma (48%), large cell anaplastic carcinoma (33%) and adenocarcinoma (19%). The Nuclear Grade, ranging from 0 to 4, has been classified as low (N.G. less than 2) and high (N.G. greater than or equal to 2). The Mitotic Index (M.I.) has been considered as the mean of the number of the mitosis observed in 10 microscopic high power fields. (400 X). The 5 year survival rate is significantly higher in patients having large cell carcinoma or squamous cell carcinoma in relation to N.G. less than 2 and M.I. less than mean.

[Local cellular immune response in bronchopulmonary carcinoma after surgery in relation to long-term prognosis].

The prognostic value of local stromal cell reaction has been examined in 48 carcinomas of the lung (23 squamous cell carcinomas, 16 large cell anaplastic carcinomas, 9 adenocarcinomas) after radical surgery. The intensity of the peri and intratumoral lymph cell (R.L.), plasma cell (P.R.) and macrophage (M.R.) reactions has been classified in low, moderate and high reactions. The 5 year survival rate is significantly higher in patients having large cell carcinoma and squamous cell carcinoma in relation to a high or moderate lymphocytic reaction. There is no correlation between the age of the patient and the stromal cell reaction.

[Influence of lymphostasis on healing processes of intestinal anastomoses. Experimental research].

The Authors studied the healing of the intestinal anastomoses in rats with mesenteric and celiac lymph-vessels coagulated and cut. The experiment was realized utilizing 104 rats, divided in fours groups: after the induction of the lymphostasis, in the first group an end-to-end ileal anastomosis was made by polyglycolic acid suture, 6/0 in diameter. The control groups consisted of rats with ileal anastomoses, with lymphostasis solely and sham operated. The macroscopic observation and the histological findings were studied 7 and 14 days after the anastomoses' performance. Only in the animals with lymphostasis was observed an anastomosal leakage of 19% at 7th day control. In the same period the histologic sections showed a modified anastomosal healing, related to the significant reduction of the granulomatous tissue formation in presence of the lymphostasis.

[Renal lesions in a case of tuberous sclerosis: association of angiomyolipoma, bilateral polycystic kidney and renal clear-cell carcinoma].

Association cases of tuberous sclerosis (ST) and renal displasia-tumors are infrequently seen in literature. These tumors are bilateral renal cystic displasia (RP) rare hamartomas, p.e. angiomyolipomas, and malignant neoplasms, p.e. renal clear cell carcinomas. There therefor as to the frequency of those pathological associations with ST, AML are frequently seen (50-80%). Reports of polycystic renal disease with ST are rare, whereas occasional associations of renal cell carcinomas with ST are founded out. By extensive literature examination of it's evident that the synchronous association of these pathological lesions is exceptional. This report describes one case of AML, RP and renal clear cell carcinoma in a female, 22 years old, with ST; the pathological, clinical and pathogenetic features are discussed.

[A study of platelet abnormalities in obese subjects (author's transl)].

In 81 obese subjects the following studies were performed: --measurement of fat mass and its distribution in the body, --exploration of carbohydrate tolerance and lipid plasma level, --assessment of platelet aggregation and coagulation activity, --investigation of the chemical composition of platelet phospholipids. Platelet hyperactivity was demonstrated in certain patients, as evidenced by the presence of irreversible platelet aggregation with low doses of aggregation agents and by an increase in platelet coagulant activity; the latter phenomenon was not accompanied by a change in the biochemical composition of platelet phospholipids. Results of this work showed that platelet activity was not related to body weight and displayed no correlation or a slightly negative one to fat mass excess. Platelet activity was significantly increased in cases where obesity predominated in the upper body (hyperandroid obesity). The classical association of diabetes and atherosclerosis with hyperandroid obesity did not allow us to distinguish between the relative importance of hyperandroid obesity and diabetes in the observed platelet hyperactivity. Regardless of the causal mechanism involved, the relationship between platelet hyperactivity and upper body fat excess should be kept in mind.

Ultrastructural and histological study of 11 bronchial carcinoids. Evidence for different types.

Seven of eleven bronchial carcinoids investigated showed cells with small granules resembling P cells which have already been described in human fetal and adult lung; two of these P cell tumours showed distinctive paraganglioid features. One tumour showed peculiar ultrastructural findings resembling closely those previously reported by Black (1969) in a so called "pulmonary oncocytoma". Three remaining cases showed large secretory granules resembling those of type 3 cells already described by Hage (1973b) in bronchial carcinoids; one of these tumours produced large amounts of 5-hydroxytryptamine (5HT). It is concluded that, on cytological grounds, at least two types of tumours can be distinguished among bronchial carcinoids, i.e. P cell and type 3 cell tumours. Moreover, two varieties of P cell carcinoids have been recognized, showing either the less frequent and more distinctive paraganglioid structure or the more common trabecular structure.

Pathologic changes of endocrine cells in chronic atrophic gastritis. An ultrastructural study on peroral gastric biopsy specimens.

In the course of an ultrastructural study on peroral gastric biopsy specimens that were obtained from patients with chronic atrophic gastritis, peculiar pathological changes of endocrine cells were observed and correlated with functional and hormonal data on the patients. An increased number of G (gastrin) cells was found in hypergastrinemic patients. These cells could be divided into a "light" (probably hyperfunctioning) and a "dark" (probably exhausted) type. The light type of cell was prominent regardless of the concomitant gastrin blood levels. The G cells found within the fundic region were always localized within the areas of pyloric metaplasia. Focal micronodular proliferation of antral enterochromaffin cells (EC) was often seen. A proliferation of the closed type of endocrine cells of the fundic mucosa was observed only in patients with elevated gastrin concentrations. In the present study, these cells were identified as enterochromaffin-like cells (ECL). No substantial changes were found in the D and D1 cells. The endocrine cells seen in metaplastic intestinal epithelium exhibited ultrastruct characteristics of at least three different types of intestinal endocrine cells (EC, L, and S cells).
