[What factors influence healing of duodenal ulcer when Helicobacter pylori eradication treatment is used?].

AIM: To study the influence of various factors on duodenal ulcer healing, specially the success or failure of Helicobacter pylori eradication. METHODS: One-hundred and nine patients with duodenal ulcer and H. pylori infection were studied. At endoscopy biopsies were obtained from the gastric antrum and body, and processed by microbiological (Gram stain and culture) and histological methods (haematoxylin-eosin); also, a 13C breath test was performed. A < > triple therapy (bismuth, tetracycline, metronidazole) or omeprazole plus amoxicillin was administered. Endoscopy and breath test were repeated one month after completing therapy. Eradication was defined as the absence of H. pylori by all diagnostic methods. RESULTS: H. pylori eradication was achieved in 60 patients (55%). In the multivariate analysis H. pylori eradication was the only variable which correlated with ulcer healing (regr. coef. = 2.4; OR = 10.6). Additional variables (age, sex, smoking, time of evolution, ulcer size, and type of therapy) were not significantly correlated. Ulcer healing was achieved in 92% of patients after H. pylori eradication, and in 51% of therapy failures (p < 0.001). CONCLUSION: H. pylori eradication accelerates ulcer healing, which represents an additional argument for employing eradicating therapy in patients with duodenal ulcer disease.

[Organizational models for the hematology areas of the district hospitals of Catalonia].

PURPOSE: The haematological assistance in Catalonia is based upon the district hospitals, in the first step, and the stage III hospitals located usually in higher population nuclei, in the second step. The purpose of this work was to analyse the resources of the "primary haematological assistance" network provided by the district hospitals, to evaluate them and to propose a model for their organisation. MATERIAL AND METHODS: An enquiry was carried out to all members of the Grup de Treball d'Hospitals Comarcals de Catalunya (Catalonia's District Hospitals Task Force). The evaluable data included demographic figures of the population assisted, personnel of each haematological area, organising structure, clinical activity, cytomorphology, blood banks, laboratories and continuous formation activities. RESULTS: The enquiry was answered by 15 of the 21 district hospitals (71.4%) with haematologists in Catalonia. The population assisted in those hospitals is 2,100,000 (ranging between 55,000 an 450,000). All centres are integrated in the National Health network. Eleven of the hospitals analysed have only one haematologist (73.3%). If his dedication is 100% of the time, this would represent a doctor for 105,000 people. The time devoted to work is 690 hours a week for all the population, with a mean of 3,043. Four patients assisted per hour. The total number of hospital beds is 3,353 (50-450), with a mean number of 1 haematologist for every 167.6 beds. The number of patients hospitalized due to blood diseases ranges between 3 and 13 per month. Six of the 15 centres are adjunct to the outpatient clinic. Two centres have a blood bank and 7 have developed an autotransfusion programme. All the centres but one perform oral anticoagulant treatment follow-up, the number of patients assisted ranging from 20 to 210 per week. None of the hospitals has a separate Haematology Service; in most of them haematology is structurally and functionally dependent from Laboratories and in some there is a mixed Laboratory/Internal Medicine functional organisation, depending of the Medical Direction. No haematologist is ever on call specifically for his specialty. Continuous formation activities are carried out in 9 of the 15 centres (60%). COMMENT: Several measures are proposed to improve the haematological assistance, acting on different levels: continuous formation, patient flows and circuits, resident doctors training, anticoagulant treatment network, organisation models, credit cards from the Spanish Association of Haematology.

[Atrial rupture secondary to infarction. Echocardiographic diagnosis].

A 70 year old male was admitted to the hospital 3 days after suffering one episode of ischemic chest pain. On admission he complained of non-productive cough, pleuritic pain, cardiomegaly I/IV and sinus tachycardia. Later on, he developed frequent auricular prematurities and flutter. The cardiac enzymes were elevated confirming the diagnosis of myocardial necrosis. One echocardiogram showed an important pericardial effusion. Color Doppler, high flow velocity from the lateral wall of the left atrium towards the apex, above the mitral valve. The transesophageal echocardiogram clearly showed the presence of a pathological communication at this level. One month later, a nodular image compatible with an organized thrombus was observed at the same level, sealing the rupture site.

A new matrix formulation of spectacle magnification using pupil magnification. II and III: High myopia corrected with contact lenses and intraocular lenses.

In this paper we use the matrix method to calculate spectacle magnification (SM) and relative spectacle magnification (RSM) using pupil magnification for myopic eyes corrected with contact lenses (CLs), and intraocular lenses (IOLs). For myopic eyes corrected with contact lenses, the calculation is very similar to the one done for myopic eyes corrected with ophthalmic lenses (OLs) since both OLs and CLs are external correcting systems. For myopic eyes corrected with intraocular lenses, the calculation is different from the ones carried out for myopic eyes with external correcting systems. In this paper we show the differences that exist between the matrix calculations for these two correcting systems, external and internal. The magnifications are calculated for specific myopic eyes corrected with CLs and IOLs. By comparing the results obtained with three correction types, namely, OLs, CLs and IOLs, we will be able to choose the best correction system for each case.

[Celiac disease with occipital calcifications: 2 late cases].

The association of celiac disease, epilepsy and occipital calcifications with initial clinical manifestations of epilepsy during the first two decades of life with an often progressive and variable course and clinical expression of malabsorption has recently been described. Two cases of celiac disease with occipital calcifications and a presentation with neurologic symptoms in adulthood are reported. The first case is that of a 40-year-old male who presented recurrent and alternating pure brachial monoparesis and later acute abdominal pain following which celiac sprue was diagnosed. The second case is that of a 53-year-old woman diagnosed with celiac sprue 20 years before, presenting permanent myoclonus in the lower limbs which were progressive in severity, ataxic march and generalized tonoclonic seizures. Both patients had bilateral occipital calcifications on CT and celiac disease was demonstrated on biopsy. The first case also showed marked signal alteration in the white matter on MRI. Celiac disease with cerebral calcifications presents also in adulthood with atypical clinical manifestations. Suspicion of celiac disease may be confirmed by non-invasive methods such as antigliadin and antiendomysium antibody determination. CT imaging is characteristic.

Determination of epitestosterone and testosterone in urine by high-performance liquid chromatography.

This paper describes an alternative HPLC method for the determination of testosterone and epitestosterone, which is simple, rapid, selective, sensitive and reproducible. Samples were prepared using a previous enzymatic hydrolysis with liquid-liquid extraction. The determination was carried out on a Hypersil BDS-C18 reversed-phase column with gradient elution and UV absorbance detection (240 nm). The limits of quantification (signal-to-noise ratio = 6) were 20 ng/ml for testosterone and 30 ng/ml for epitestosterone.

Interleukin-1 enhances the ability of cultured human umbilical vein endothelial cells to oxidize linoleic acid.

Human umbilical vein endothelial cells (HUVEC) were treated with recombinant interleukin (IL)-1 beta, and the metabolism of exogenous linoleic acid was studied. High performance liquid chromatography, gas chromatography-mass spectrometry, and chiral analysis revealed that HUVEC enzymatically convert linoleic acid mainly into 13-(S)hydroxy-9(Z),11(E)-octadecadienoic (13-HODE) and 9-(R)hydroxy-10(E),12(Z)-octadecadienoic acids, which may isomerize toward all-trans compounds. IL-1 beta increased the formation of all octadecanoids in a time- and dose-dependent manner with similar EC50 (approximately 1 unit/ml). The apparent Km values of linoleic acid were 15.59 +/- 8.39 and 152.9 +/- 84 microM (p < 0.05) in IL-1 beta-treated cells and controls, respectively, indicating a higher substrate affinity in cells stimulated with IL-1 beta. Ratios of S/R enantiomers for the hydroxyoctadecanoids produced by untreated and IL-1 beta-treated cells were similar to those from isolated cyclooxygenases (COXs), whereas isolated 15-lipoxygenase yielded 13-HODE with a strict S configuration. The formation of octadecanoids was inhibited in a dose-dependent manner by several COX inhibitors in both controls and IL-1 beta-treated cells, COX2 selective inhibitors being more effective on IL-1 beta-treated cells than on controls. COX1 and COX2 protein levels increased less than 2-fold and 8-fold, respectively, after IL-1 beta treatment. The specificity of COX inhibitors was proven since they did not inhibit 13-HODE formation by human polymorphonuclear leukocytes. Overall, these results indicate that COXs are responsible for the oxidative metabolism of linoleic acid in HUVEC, and IL-1 beta increases it by inducing the expression of new enzyme, mainly COX2.

Two types of mitochondria are evidenced by protein kinase C immunoreactivity in the Müller cells of the carp retina.

The localization of protein kinase C (PKC) was studied immunocytochemically in the Müller cells of the carp retina. Electron microscope immunocytochemistry (using a monoclonal antibody to the alpha and beta isoenzymes of PKC) showed PKC-immunoreactivity mainly inside some mitochondria, especially along the mitochondrial cristae whereas other mitochondria in the same Müller cells showed no staining. Despite a detailed analysis we did not find any significant morphological difference between labeled and unlabeled mitochondria. These results demonstrate, for the first time, the presence of PKC immunoreactivity inside mitochondria and suggest that individual mitochondria may differ in signal transduction pathway.

Long term effects of ritodrine on blood pressure and heart rate of adolescents exposed during the prenatal stage.

OBJECTIVE: To determine the long term cardiovascular effects in adolescents exposed to ritodrine during prenatal stage. STUDY DESIGN: Twenty-seven ritodrine-exposed subjects and 19 controls, of both sexes, clinically healthy, and born at normal term, were evaluated for systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) each during 48 hours, with a Colin Ambulatory monitor. Data analysis were performed by individual Cosinor method, and non-parametric Mann Whitney's test and Lamotte procedures. RESULTS: Adolescents who were exposed to ritodrine showed losses in the HR rhythm and rising middle pulse values. Although SBP and DBP rhythm remains constant, the increase of maximum and minimum values for SBP as well as a rise of maximal rates of DBP suggest a possible hypertensive effect mediated by beta mimetic exposure. CONCLUSION: This study suggests a hypertensive effect mediated by beta mimetic exposure.

Ultradian rhythms in gross motor activity of adult humans.

During waking h, the existence of ultradian rhythms in gross motor activity has been described in nonprimates, nonhuman primates and newborn humans, but not in adult humans. Some of the previous studies suggested that the appearance of these rhythms could be favored by conditions of isolation and low environmental demands. To confirm the existence of ultradian rhythmicity in the gross motor activity of adult humans and to describe their characteristics, an actimeter was used to record the mobility of 13 adults who remained alone and isolated for 5 h (15:00-20:00) in a monotonous environment with nothing to do. Least squares rhythmometry analysis showed that the gross motor activity of 12 out of 13 subjects had significant rhythms (p < 0.05) within the ultradian band, with periods of between 0.5 and 2.5 h. There were important individual differences between the dominant periods and the same subject might show more than one significant period. These rhythms were stable and they tended to appear immediately the experiment began. The results suggest that a philogenetically old mechanism that organizes gross motor activity in ultradian rhythms exists in adult humans.

Mapping of the linear antigenic determinants from the Leishmania infantum histone H2A recognized by sera from dogs with leishmaniasis.

Antibodies reacting against the H2A histone protein were frequently observed in the sera from dogs naturally infected with the protozoan parasite Leishmania infantum. Using synthetic peptides covering the complete sequence of the protein we have identified the amino terminal region, comprising from amino acids 1 to 20, and the carboxyl terminal region, comprising from amino acids 106 to 132, as conforming the antigenic determinants of the protein. Those regions, exposed in the nucleosome surface, are highly divergent in sequence relative to the mammalian H2A histones. The anti-H2A histone antibodies present in the sera of these dogs specifically recognize the L. infantum H2A histone and they do not react with mammalian histones. The present data indicate that, in spite of the evolutionary conservation of the H2A histone protein among eukaryotic organisms, the humoral response against this protein during natural infection is specifically triggered by the parasite protein antigenic determinants.

New matrix formulation of spectacle magnification using pupil magnification. I. High myopia corrected with ophthalmic lenses.

In this paper we have applied the matrix method to calculating spectacle magnification (SM) and relative spectacle magnification (RSM) for myopic eyes corrected with ophthalmic lenses (OLs). We have been able to obtain very simple expressions of the magnifications and analogous values to the ones obtained using other geometric methods. Through the paraxial 2 x 2 object-image matrix for spherical systems, we can calculate the correcting lens pupil matrix (ML) and the corrected ametropic eye pupil matrix (Mca). These two matrices, that is ML and Mca, directly provide the lateral magnification and the lens-eye system's image focal parameters that enable us to easily calculate SM and RSM. The magnifications have been calculated for the case of myopic eyes corrected with OLs, which shows the validity of this method since its results are similar to the ones obtained using other methods. The analysis of other correcting systems such as contact intraocular lenses will be presented in two associated papers (to be published as parts II and III of this review).
