Further observations on the inhibition of tumor growth by C. parvum with cyclophosphamide. VII. Effect of treatment prior to primary tumor removal on the growth of distant tumor.

The present investigations were directed toward determining whether primary tumor manipulation prior to its removal is advantageous for the control of metastases and survival. Studies were carried out to ascertain whether 1) there is justification for delaying surgical removal of a primary tumor to permit preoperative administration of cyclophosphamide (CY) and/or C. parvum (CP) and 2) there is an advantage to administering the immunotherapy directly into a primary tumor. After operation, in all investigations, systemic CP and CY was used. Despite the putative similarity of animals, tumors and treatment regimens there was marked variation in response of tumors to therapy. No benefit was derived from administering preoperative immunotherapy alone. When operation was delayed to employ systemic immuno-chemotherapy, a slight improvement in the control of distant tumor was noted. The employment of preoperative intratumor immunotherapy led to a greater prolongation of survival and more inhibition of distant tumor growth than did immediate primary tumor removal or the use of preoperative systemic immunotherapy. The results suggest that there may be an advantage to delaying removal of a primary tumor so that it may be employed in therapeutic strategies directed toward control of metastatic disease.

Urinary matrix calculi consisting of microfibrillar protein in patients on maintenance hemodialysis.

In seven patients on maintenance hemodialysis, de novo recurrent renal stone formation was observed. In all patients, the underlying disease was glomerulonephritis, with or without the nephrotic syndrome. All patients had considerable persistent proteinuria. The stones consisted predominantly of protein, as revealed by amino acid analysis, and had a negligible carbohydrate and lipid content. Only in some specimens, X-ray diffraction and scanning electron microscopy revealed the presence of small amounts of whewellit (calcium oxalate monohydrate) and/or uric acid. In semithin sections, the stones had a laminated texture and exhibited structural anisotropy under polarized light. With transmission electron microscopy, they were found to consist of peculair microfibrils. The proteinaceous material differed from fibrin or Tamm-Horsfall-protein, as indicated by ultrastructure, carbohydrate analysis, and amino acid analysis. Symptomatic de novo matrix stone formation constitutes another complication of dialyzed patients which has not been reported so far.

[Scanning electron microscopy of different materials after urine contact in vitro (author's transl)].

The nature of the surface of the used material is determining the complication rate in medium and long term application of nephrostomy and bladder catheters as well as ureteral splints. Tubings made of 8 different materials were irrigated for one to three weeks by means of an urolithic simulator, the pH-values from 5.8 to 7.0. Scanning electron Microscope showed good values with respect to silicon elastomer, teflon and ethylenacrylicacid copolymer. Considering technical and economical factors it seems to be proven that silicon elastomer and polyurethane are the most appropriate materials for splints and catheters, whereas rubber and PVC are less suitable for long time application.

[Bone mineral estimation (Hydroxyl apatite equivalent) by direct measurement of photo absorption in the upper extremity (phalanges, radius and ulna) (author's transl)].

The x-ray source of the apparatus developed by the authors for bone mineral estimations consists of a small anode tube with a silver anode. The value of the procedure has been tested by in vitro and in vivo experiments. The in vitro measurements were carried out on a bone reference system consisting of a model bone (Reiss) as well as on a macerated tibia embedded in a resin (Heuck). The results were compared with the values obtained by Rassow. It has been shown that the artificial resin is of only limited value as a connective tissue equivalent. The method of mineral estimation using phalanges is described and its reproducibility was tested on 17 parameters. The absorption coefficient of the spongiosa proved to be particularly critical. Finally, measurements, using the forearm, by Cameron's method are discussed briefly.

[On the effect of methane-bis--phosphonate in vivo (author's transl)].

After s.c. injections of methane-bis-phosphonate into rabbits, calcified incrustations developed around the injection sites. These incrustations were less radiolucent than the surrounding tissues. They appeared to have a good crystalline apatite structure in X-ray diffraction. The findings agree with our previous in vitro studies but disagree with the findings in the literature that ethane-1-hydroxy-1,1-diphosphonate (EHDP) and dichloromethyle-nediphosphonate (Cl2MDP) inhibit crystal growth of apatite already formed.

Correlation of antitumor chemoimmunotherapy with serum inhibition of tumor cell destruction.

The administration of cyclophosphamide and Corynebacterium parvum in combination results in tumor growth inhibition greater than that resulting from the use of either agent alone. The precise mechanism(s) by which this chemoimmunotherapy combination results in a synergistic inhibiting effect is not known. The possibility was entertained that the tumor effect might be related to a greater decrease in serum-mediated interference with cellular cytotoxicity, i.e., "blocking" activity, by both agents in combination rather than by either alone. The present findings fail to support such an explanation. C. parvum by itself failed to decrease serum inhibition and in conjunction with cyclophosphamide resulted in an effect that was no greater than that produced by cyclophosphamide alone.

Comparative effects of Corynebacterium parvum, Brucella abortus extract, Bacillus Calmette-Guérin, glucan, levamisole, and tilorone with or without cyclophosphamide on tumor growth, macrophage production, and macrophage cytotoxicity in a murine mammary tumor model.

In this laboratory, it has been repeatedly demonstrated (using a murine mammary tumor model) that the combination of cyclophosphamide (CY) and Corynebacterium parvum (CP) is more effective than either agent alone in the control of tumor growth. This paper presents information obtained in our model comparing findings on the effects of CP with a Brucella abortus extract (Bru-Pel; BP) and glucan (GL) on tumor growth. In addition, the influence of those agents as well as bacillus Calmette-Guérin, tilorone, and levamisole on bone marrow macrophage colony production and cytotoxicity is presented. None of the nonspecific stimulating agents (NSSAs) inhibited tumor growth when administered systemically without CY, confirming our previous contention that such immunotherapy alone is likely to be an ineffectual form of treatment. Whereas tumor regression was observed following intratumor CP, neither GL nor BP had such an effect. When used with CY, neither BP nor GL administered ip or intratumorally inhibited tumor growth as effectively as did CP and CY. Inhibition of the growth of a distant tumor as well as the treated tumor occurred following intratumor CP and CY but not following intratumor BP and CY. All of the microbial NSSAs increased macrophage colony production to varying degrees in both normal and tumor-bearing mice. In the latter mice, CP had the most prolonged effect. Levamisole and tilorone failed to increase colony production in normal mice while in tumor-bearing mice the effect was inversely proportional to the amount of agent administered. To some extent, the stimulation of colony production by the NSSAs paralled the degree of tumor inhibition observed when those agents were combined with CY. The cytotoxicity of cultured macrophages could not be related to tumor growth inhibition.

[Radiological meneral equivalent determinations in bone. The problems of build-up factor (author's transl)].

The apparatus for measuring mineral equivalents described by Gebhardt, Heinen and Zwicker in 1973 was subjected to theoretical and experimental investigations; as a result, changes were introduced which significantly improved the basis for exact absorption measurments on phalanges. 1.The CuK alpha radiation (8.5 kV) produced the Hohannson monochromator was replaced by higher energy AgK alpha radiation (22.2 kV). The total spectrum was improved by using a palladium filter and an impulse height discriminator. 2. A slit collimator was used giving a beam area of 3 mm X 0.5 mm with improved paralleism ofthe beam and an incident angle alpha of 0.1 degrees compared with a previous angle of alpha approximately 1 degrees. 3. Correction of attenuation N = No. e-ux by a correction factor B to allow for "build-up factor" in the required area of measurement is no longer necessary. Measurements at four of hydroxyle apatite (Reiss) provide the required mineral absorption. 4.AgK alpha radiation permits measurement of thicker bones such as radius and calcaneus and provides adequate impulses for short periods of measurement which is useful in practice.

[Grouping of urinary stones and exactness of analyses (author's transl)].

The composition of 1,327 urinary stones by X-ray diffraction is statistically analyzed, and the exact analyses are discussed. The representative test portion of the urinary stone is important to the result. A comparison of different results of urinary stones with that of other authors is only possible when we know the exactness of the method.

Psychotropic drugs on general medical and surgical wards of a teaching hospital.

We examined the prescribing habits for psychotropic drugs of internists, surgeons, and gynecologists on their inpatient wards in a teaching hospital. Data were gathered from patients' charts and pharmacy records. In a six-week period, 9% of all admissions received such a drug. The male:female ratio and black:white ratios were studied; the maximum incidence of receiving these medications was in the 50- to 60-year age group. Minor tranquilizers were prescribed most frequently, followed, respectively, by major tranquilizers, barbiturates, and antidepressants. Less than half the available drugs were used, but drugs of differernt groups were often used interchangeably in an unsystematic fashion, and there was little evidence as to how effective a drug had been. It also seemed that depression was often overlooked or insufficiently treated.
