[Ocular complications of cerebrospinal fluid leakage after skull trauma].

PURPOSE: Analysis of diagnostic and therapeutic ophthalmological problems after severe cerebrocranial lesions with liquorrhea. MATERIAL: 3 children between 5 months and 5 years in whom the ophthalmological symptoms were the first and important signs of the cerebrospinal fluid leakage after skull lesion. These patients were observed and diagnosed at the department of ophthalmology and then operated at the department of neurosurgery. RESULTS: In all children skull trauma has led to dura mater tear and leakage of cerebrospinal fluid with its complications. As a result of the ophthalmological complications, two patients developed worse visual acuity evoked potentials. Blind eyeball of the third patient was enucleated. CONCLUSIONS: The ocular findings are not typical for liquorrhea after cerebrocranial trauma and they must be diagnosed and treated by a team of ophthalmologists and neurosurgeons.

[Pathomorphological evaluation of the extraocular muscles during strabismus].

PURPOSE: An analysis of the histopathological picture of extraocular muscles removed in the operation of strabismus. MATERIAL AND METHODS: Light microscopy analysis included 131 fragments of rectus muscles, with the majority of lateral, collected from patients with concomitant squint of different angles and disease durations. RESULTS: Normotopic cross-striated muscle fibres were observed in 40 (30.6%) studied muscles, while pathological changes appeared in 91 (69.4%) muscles with a prevailance of fibrous atrophy. A correlation was found between the histopathological picture of the examined muscles and the extent of strabismus. CONCLUSIONS: Pathomorphological changes were observed with the majority of the extraocular muscles in the microscopic picture. The observed changes in the cellular structure of the extraocular muscles influenced the extent of the squint angle, affecting the result of surgery.

[Use of contact lenses in children with severe myopia].

PURPOSE: To present own observation on the usefulness of soft contact lenses in treatment of high myopia in children and youth. MATERIAL AND METHODS: 43 children with high myopia were selected from 2320 patients fitted with lenses due to refraction errors. The whole complex of ophthalmological examinations including ultra sound scan of eyeballs and measurement of corneal sensitivity were performed prior to the lens fitting. Visual acuity and stereoscopy of glass and contact lenses were estimated. RESULTS: The contact lens fitting enabled to achieve significant visual acuity improvement in all patients. The improvement was observed in 53.4% patients in comparison with glass correction. Stereoscopy was achieved in 58.1% patients, with glass correction and 81.4% patients with contact lenses. CONCLUSIONS: Soft contact lenses are very useful in high myopia correction in growing-up patients. The basic and indispensable factor of successful contact lens fitting is a good cooperation with parents.

[Periorbital capillary hemangiomas in children--treatment and monitoring of treatment].

PURPOSE: The purpose of the paper is to present the possibility of treatment of periorbital capillary hemangiomas in infancy. MATERIALS AND METHODS: Doppler colour ultrasonography was used as a method of diagnosis and monitoring of the treatment. The material comprised 11 patients aged from 1 month to 3 years. In 6 cases local corticosteroid injections were applied, in 3 cases surgical treatment was performed, out of which 1 additionally received steroid. RESULTS: Results of the treatment were satisfactory in all patients. No serious complications were observed. CONCLUSIONS: Basing on these cases, the authors confirmed the opinion that intralesional corticosteroid injection is the method of choice in the treatment of periorbital infantile hemangiomas. The authors indicate the usefulness of Doppler colour ultrasonography in diagnosis, demonstrating the extent of lesions and monitoring the response to therapy.

[Usefulness of color Doppler ultrasonography in diagnosis of retinal detachment].

PURPOSE: Possibility of visualising the vessels within retinal detachment. MATERIAL AND METHOD: Examinations were performed using "Color Doppler" and "Power Doppler" methods on 46 eyes with linear hyperechogenic lesions within vitreous body found in US B-mode examination. RESULTS: The results show the possibilities of Doppler examination in visualising branches of the central retinal artery within the detached retina. The presence of vessels in the membranous proliferations was not found. CONCLUSION: Color Doppler ultrasonography can be used in localising the position of retina, especially when proliferative lesions and detachment coexist.

[Etiology of congenital cataract--clinical observations].

PURPOSE: To analyse the etiologic factors of the congenital cataract. METHODS: The authors analyzed 49 cases of congenital cataract in patients hospitalized at Polish Mother's Memorial Hospital in Lódź. The diagnostic procedures were based on own multiprofile algorithm which included natural history, complete ophthalmologic examination, metabolic, cytogenetic and serologic investigation. RESULTS: Four groups of patients were established depending on the nature of the etiologic factors: metabolic, familial and/or genetic, environmental and unknown. CONCLUSION: Etiology of more than one half of the analyzed patients was found and specific treatment was possible in some cases.

[Polyamine system in experimental retinopathy of prematurity].

Spermidine (SPD) and spermine (SP) concentrations and the activities of ODC and SAT have been measured in retina and lens of the rats maintained in 60% atmospheric oxygen for the first 14 days life. Hyperoxia, known to cause retardation of vessel growth, was associated with some reduction of the enzyme activities and polyamine levels.

[Creatine kinase in strabismus surgery].

The paper presents results concerning serum creatine kinase (CPK) activity before and at different times following the surgery of extraocular muscles at concomitant strabismus. The operation evokes statistically significant increase of CPK activity which tends to normalize during healing processes.

[Eye neoplasms during the developmental years based on personal observations].

The paper presents the results of the observations of 81 patients among 84 (44 females and 40 males, aged 0 to 18 years) treated in the period 1990-1993 for neoplasms of the visual system. Surgery was performed in 73 of them (89%). Histopathological examinations of tumors define most of them as benign (96% and 4%) as primary malignant.

[Does histamine participate in diabetic ocular complications?].

The survey presents the results of clinical observation of the visual system and the evaluation of the level of histamine in the retina and choroid (together) and in the lens in poststreptozotocinic diabetes in Wistar tribe rats. The eye fundus exhibited vascular changes of various intensity, also opacities of the lenses were detected. In the chorio-retinal pool and in the lenses of diabetic animals the authors found an increase of the level of histamine in comparison with the control group. The investigations suggest that histamine could be the factor initiating the changes in the permeability of the blood vessels.

Histamine metabolism in diabetic rats.

Streptozotocin-induced diabetes in rats has been used to study the effect of insulin deficiency on histamine metabolism. There were significant increases in the amine content in the pancreas and intestine, and a significant drop in intestinal diamine oxidase (DAO) activity. The reduced DAO activity may be of clinical relevance.
