Positioning against niche competitors to reduce revenue erosion.

Health care providers around the country face a growing threat from more limited service "niche" providers, often sponsored by specialty physicians. Mark Grube, vice president of Abendshien+Grube Associates, writes that hospitals and health systems may want to consider joint ventures with physicians to counter this trend.

A small insertion in the SSU rDNA of the lichen fungus Arthonia lapidicola is a degenerate group-I intron.

Insertions of less than 100 nt occurring in highly conserved regions of the small subunit ribosomal DNA (SSU rDNA) may represent degenerate forms of the group-I introns observed at the same positions in other organisms. A 63-nt insertion at SSU rDNA position 1512 (relative to the Escherichia coli SSU rDNA) of the lichen-forming fungus Arthonia lapidicola can be folded into a secondary structure with two stem loops and a pairing of the insertion and flanking sequences. The two stem loops may correspond to the P1 and P2, and the insertion-flanking pairing to the P10, of a group-I intron. Considering these small insertions as degenerate introns provides important clues to the evolution and catalytic function of group-I introns. Multiple origins of lichen symbioses in fungi suggested by SSU rDNA phylogeny.

Phylogenetic hypotheses provide a context for examining the evolution of heterotrophic lifestyles. The lichen lifestyle, which is the symbiotic association of fungi with algae, is found in various representatives of Dicaryomycotina, both Ascomycetes and Basidiomycetes. A highly resolved parsimony analysis of small subunit ribosomal DNA (SSU rDNA) sequences suggests at least five independent origins of the lichen habit in disparate groups of Ascomycetes and Basidiomycetes. Because lichen associations arose from parasitic, mycorrhizal, or free-living saprobic fungi, neither mutualism nor parasitism should be construed as endpoints in symbiont evolution.

[Coping with HIV infection and motivation for therapy in multi-drug dependent patients].

The intrapsychological process of working through the five stages of death in terminally ill patients (according to Kübler-Ross) was documented by videotaping semistructured interviews. There were 67 i.v. drug-dependent polytoxic HIV-positive inpatients. An inquiry was also made into their social niveau, the course of their addiction, the patients' legal status, and their previous experience with long-term therapy. Prevalent forms of working through the five stages of death could be established with reasonable reliability, and their influence could be determined on how the patients actually made use of offers of long-term therapy. The most important finding was that HIV-positive i.v. drug-addicted polytoxic patients started with a similar ratio of 1:3 in drug-free long-term therapy compared to HIV-negative i.v. drug-addicted polytoxic inpatients (n = 71). In the group of HIV-positive i.v. drug-addicted inpatients their individual means of psychologically working through their illness, their legal status and their previous experience with drug-free long-term therapy seem to be relevant factors in positive therapy motivation. This should be kept in mind when methadone programs are discussed.

Maintenance of antidepressant effect of sleep deprivation with the help of lithium.

The antidepressant effect of sleep deprivation is often not longer than 1 or 2 days. Therefore we investigated systematically the combination of lithium and sleep deprivation on 26 depressive patients (ICD/DSM 296). We measured the depression scores with the Hamilton Depression Scale and the Brief Depression Rating Scale on 4 days. A comparison between sleep deprivation with and without lithium showed a highly significant difference on the 2nd and 3rd days after sleep deprivation. The positive antidepressant effect remained with those who received lithium.

The potential of aggressiveness in families of schizophrenics in relation to the danger of relapse.

The question of the potential aggressiveness in families of schizophrenics in relation to the danger of relapse was investigated (schizophrenics n = 40, ICD and AMDP selection). The aspect of subjective intrapsychic coping with the aggressive potential in families was operationalized by 12 self-ratings divided into 3 categories: extrapunitive attitude, intrapunitive attitude, experience of divergency. Analysis of variance showed that there was no significant relation between the 3 categories and the relapse quote. The claim following "expressed emotion" research which would have indicated different results was not supported by investigating the patients' subjective experience. Within the framework of our results various rehabilitation strategies and potential shelter functions in the families are discussed.
