Microvascular injury distal to unilateral pulmonary artery occlusion.

We explored three questions: 1) does edema fluid accumulate distal to temporary unilateral pulmonary artery occlusion (TUPAO); 2) if so how rapidly does it accumulate; and 3) how is it affected by positive end-expiratory pressure (PEEP)? Using a tracheal divider we measured pulmonary capillary blood flow (Qc), tissue volume (Vt), and diffusing capacity (DLCO) in each lung with a rebreathing method. After control measurements in 12 dogs, the left pulmonary artery was occluded and measurements were repeated at intervals during 4 h of occlusion and 30 min after release of the occlusion. Six of the dogs were ventilated with 10 cmH2O PEEP. Finally the lungs were removed, weighed, and fixed for histology. TUPAO caused a 29% increase in Vt of the left lung without PEEP and a 59% increase with PEEP. After release of the occlusion, Qc and DLCO in the left lung returned to control levels within 30 min in dogs not on PEEP but remained depressed in dogs ventilated with PEEP even though PEEP was removed. At postmortem the left lung weighed more than expected in both groups of dogs but was significantly heavier in those on PEEP. Histology confirmed bronchovascular cuffing with edema and hemorrhage.

Localized form of congenital generalized fibromatosis. A report of 3 cases with myofibroblasts.

Three cases of the localized form of congenital generalized fibromatosis are described in females under one year. The lesions were in the parotid region, biceps muscle and abdominal wall. The infiltrative or nodular pattern of these tumours with the periphery resembling smooth muscle and the central portion resembling a haemangiopericytoma makes them a distinct histological entity. Ultrastructurally, these tumours are composed of myofibroblasts. The differential diagnosis from other soft tissue tumours in infancy and early childhood is discussed. These tumours are benign and recurrence after complete local excision is rare.

An ultrastructural study of the urinary bladder in children correlated with histological, bacteriological, and clinical findings.

Bladder biopsies from six boys without a history of urinary tract infection were taken during hypospadias repair operations and examined by electron microscopy. The ultrastructure of the epithelium, which was presumed to represent the normal, is described in detail and compared with the reported findings in other mammalian species. The appearances are generally similar although in our material the luminal membrane of the superficial cells was thicker than that surrounding the other epithelial cells but not asymmetrical. We observed membrane-coating granules which have not been reported before in bladder epithelium. The appearances of the normal bladder are compared to those seen in 24 children with urinary infection. In the presence of acute infection in large heterogeneous secondary lysosomes which are present in the intermediate and superficial cells of the normal bladder are reduced in number and the amount of rough endoplasmic reticulum is increased.
