[The clinical evaluation of UF-021, a new prostaglandin related compound, in low tension glaucoma patients].

We administered UF-021 isopropyl unoprostone (Rescula) ophthalmic solution, a new prostaglandin-related compound developed as an anti-glaucoma agent, to 13 low tension glaucoma patients for 24 weeks, and clinically evaluated the effect of the intraocular pressure (IOP) reduction, visual field change, and side effects of the agent. Intraocular pressure was reduced in 11 out of 12 patients who were studied for statistical analysis. In these 12 patients, significant IOP reduction was recognized in the mean IOP for 24 weeks after the administration of UF-021. No side effects were detected in any of the 13 patients. From these results, UF-021 could be used as a new anti-glaucoma agent for low tension glaucoma, and also safer than many other anti-glaucoma agents.

Studies on antimutagenic effect of milk cultured with lactic acid bacteria on the Trp-P2-induced mutagenicity to TA98 strain of Salmonella typhimurium.

The inhibitory effects of cultured milk using 76 strains of lactic acid bacteria isolated from milk products were investigated on the mutagenicity of 3-amino-1-methyl-5H-pyrido[4,3-b]indole (Trp-P2), a tryptophan pyrolysate for Salmonella typhimurium TA98. Each cultured milk sample displayed its characteristic antimutagenic effect against the mutagenicity of Trp-P2. The milk cultured with Lactobacillus acidophilus LA106 (LA2) showed the highest inhibition of 82.1% among the strains used. Milk samples cultured with Lactococcus lactis subsp. lactis, Lll103 (10-3) and Lll102 (KM) also exhibited higher inhibition percentages.

Antimutagenicity of milk cultured with lactic acid bacteria against N-methyl-N'-nitro-N-nitrosoguanidine.

The antimutagenic effect of cultured milk using 71 strains of lactic acid bacteria belonging to the genus Lactobacillus, Streptococcus, Lactococcus, and Bifidobacterium on the mutagenicity of N-methyl-N'-nitro-N-nitrosoguanidine was investigated in vitro using Salmonella typhimurium TA 100 as an indicator bacterium. Each cultured milk sample displayed its characteristic antimutagenic effect on the mutagenicity of N-methyl-N'-nitro-N-nitrosoguanidine. The milk cultured with Lactobacillus acidophilus LA 106 (LA2) showed the highest inhibition of 77% against the mutagenicity of N-methyl-N'-nitro-N-nitrosoguanidine among the strains tested. Changes in the antimutagenic effect of the milk cultured by Lb. acidophilus LA 106 (LA2) during incubation were also examined using N-methyl-N'-nitro-N-nitrosoguanidine as a mutagen.

Induction of Fc epsilon RII/CD23 on phytohemagglutinin-activated human peripheral blood T lymphocytes. I. Enhancement by IL-2 and IL-4.

Despite evidence for the expression of low affinity Fc receptor for IgE (Fc epsilon RII)/CD23 in T cell lines and pathologic T cells, Fc epsilon RII/CD23 in normal human T cells is still unclear. We studied the expression of Fc epsilon RII/CD23 on T cells in short-term culture of normal human PBMC stimulated with 15 micrograms/ml PHA. PHA stimulation also resulted in the release of soluble Fc epsilon RII/CD23 (IgE binding factor). Using two-dimensional flow cytometry, more than 10% of the Fc epsilon RII/CD23+ cells were found to co-express CD3 Ag. Both CD4+ and CD8+ T cells expressed Fc epsilon RII/CD23. The induction of Fc epsilon RII/CD23 on PHA-activated T cells was enhanced by IL-2 as well as IL-4. Both IL-2 and IL-4 also augmented PHA-induced production of soluble Fc epsilon RII/CD23. The enhanced expression of Fc epsilon RII/CD23 on T cells by both lymphokines was suppressed by rabbit anti-IL-4 antiserum, suggesting the involvement of an IL-4-dependent process even in the IL-2-dependent Fc epsilon RII/CD23 expression on T cells. The expression of mRNA for Fc epsilon RII/CD23 on PHA and IL-4-stimulated PBMC was examined by Northern blot analysis. Fc epsilon RII/CD23 mRNA was detected in RNA prepared from the T cell fraction depleted of B cells and macrophages (Fc epsilon RII+CD3+ = 6.2%, Fc epsilon RII+CD3- = 0.8%). The expression of the mRNA for Fc epsilon RII/CD23 on CD3+ T cells was also confirmed by in situ hybridization with Fc epsilon RII/CD23 cDNA combined with CD3 rosette formation at the single cell level.

Postural change of intraocular and blood pressures in ocular hypertension and low tension glaucoma.

The effect of body position on the intraocular and blood pressures of normal volunteers and of patients with ocular hypertension and low tension glaucoma was studied. Changing from the sitting to the supine position increased the intraocular pressure by an average of 4.4 (SD 2.0) mm Hg in the control group, 4.0 (SD 2.0) mm Hg in the ocular hypertension group, and 4.1 (SD 1.8 mm Hg) in the low-tension glaucoma group. After 30 minutes in the supine position the intraocular pressure in normal volunteers and patients with low tension glaucoma remained stable. In contrast patients with ocular hypertension showed a further significant increase in intraocular pressure of 1.6 (SD 2.8) mm Hg (p = 0.004). This was accompanied by an equally significant decrease in blood pressure (p less than 0.001). We believe that these are manifestations of different mechanism of intraocular pressure regulation between these groups.

Ultrastructural studies of collagen fibers of the cornea and sclera by a quick-freezing and deep-etching method.

The ultrastructure of collagen fibers of the cornea and sclera of rabbits was studied by a newly developed quick-freezing and deep-etching method. An isopentane-propane mixture (-193 degrees C) cooled in liquid nitrogen was used for quick-freezing of the tissues. The frozen tissues were fractured in nitrogen, deeply etched and shadowed with platinum and carbon. Corneal collagen fibers were observed to be longitudinally arranged and separated by moderately wide interfibrillar spaces. Interconnecting filaments were three-dimensionally radiating between the collagen fibers, which were 40.6 +/- 5.0 nm in diameter and 42.0 +/- 2.8 nm in periodicity. In contrast, scleral collagen fibers were compactly organized, with few interconnecting filaments. Five kinds of striations were clearly observed on the collagen fibers, which were 217.3 +/- 50 nm in diameter and 67.5 +/- 1.4 nm in major periodicity. These striations could hardly be observed in the fractured scleral collagen fibers. The present study reveals the ultrastructural differences between corneal and scleral collagen fibers through the use of a quick-freezing and deep-etching method.

Site specific covalent alkylation of DNA by antitumor antibiotics, duocarmycin A and kapurimycin A3.

DNA alkylation by two different antitumor antibiotics, duocarmycin A and kapurimycin A3 was investigated by using of synthetic oligonucleotides. It was found that duocarmycin A reacted with N3 of adenine6 in d(CGTATACG)2 to provide a stable adduct. Kapurimycin A3 was found to react with d(CGCG)2 at N7 of guanine4 to yield a covalent adduct. Under heating conditions both adducts decompose to give an abasic-containing oligomer and a drug-base adduct.

Adult T-cell leukemia/lymphoma of the tongue.

A case of posterior tongue lymphoma associated with adult T-cell leukemia (ATL) that occurred as a lesion in the lingual dorsal portion is reported in a 64-year-old woman. Initially, a diagnosis of Hodgkin's lymphoma was considered as no findings associated with ATL except lymphadenopathy and serum anti-ATLA antibodies were present. Combined radiotherapy and chemotherapy were administered with favorable results; however, 4 months later, Pneumocystis carinii pneumonia developed, and 2 months later, generalized lymphadenopathy and hypercalcemia evolved. At this time, a diagnosis of ATL was made. The patient died of renal dysfunction 6 months after the initial presentation. In suspected cases of ATL and malignant diseases of T-cell lineage, namely, malignant lymphoma and mycosis fungoides, the presence of HTLV-1 infection should be confirmed by testing for anti-ATLA antibodies.

Decreased serum levels of IgE and IgE-binding factors in individuals infected with HTLV-I.

The cell surface expression of low-affinity Fc epsilon receptor (Fc epsilon RII), which is identical to CD23, and the serum levels of IgE binding factors (IgE BFs), a soluble form of Fc epsilon RII, and IgE were investigated in HTLV-I-infected individuals, and the results were compared with those for HTLV-I-negative controls. The occurrence of Fc epsilon RII+ mononuclear cells did not differ among the blood samples from the HTLV-I-infected groups; however, the levels of serum IgE BFs were significantly decreased (P less than 0.001) in patients with adult T cell leukaemia (ATL), compared with HTLV-I-negative controls. The serum IgE levels were also significantly decreased in these groups, including cases of ATL (P less than 0.001) and HTLV-I-associated myelopathy (P less than 0.05), and HTLV-I carriers (P less than 0.05), as compared with HTLV-I-negative controls. Since cases of strongyloidiasis or filariasis are frequently found among HTLV-I-infected individuals in Japan, these results may explain in part their defective defence mechanisms for parasites. The possible involvement of IgE BFs in the development of immune deficiency state in HTLV-I infection is also discussed.

Differential regulation of the low affinity Fc receptor for IgE (Fc epsilon R2/CD23) and the IL-2 receptor (Tac/p55) on eosinophilic leukemia cell line (EoL-1 and EoL-3).

Two types of activation Ag, low affinity FcR for IgE (Fc epsilon R2)/CD23 and IL-2R (Tac/p55), were expressed and differently regulated on human eosinophilic leukemia cell lines (EoL-1 and EoL-3). Because the binding of IgE on EoL-3 cells was completely inhibited by H107 (anti-Fc epsilon R2/CD23 mAb) but not by irrelevant mAb, essentially all the low affinity Fc epsilon R2 on EoL-3 seemed to be the Fc epsilon R2/CD23 molecules. Both IL-4 and IFN-gamma enhanced the surface expression of Fc epsilon R2, whereas IL-1, IL-2, and IL-5 showed no effects, as determined by surface staining with anti-Fc epsilon R2 antibody (H107). In contrast to Fc epsilon R2 up-regulation, IL-4 and IFN-gamma showed a differential effect on the regulation of IL-2R (Tac/p55). Whereas IFN-gamma up-regulated the receptor expression of IL-2R/Tac, IL-4 did not. The result suggests that these lymphokines are involved in the different aspects of the activation pathway of the eosinophils. The possible role of Fc epsilon R2 and IL-2R on the function of eosinophils in allergic reaction is discussed.

Low affinity IgE receptors: regulation and functional roles in cell activation.

The low affinity IgE receptors (Fc epsilon RII/CD23) homologous to animal lectins have the unique property of cleaving-off the extracytoplasmic portion as the soluble form (IgE binding factor; IgE-BF). Molecular analysis using Fc epsilon RII/CD23 cDNA proved that Fc epsilon RII is not unique to B lymphocytes but is expressed on a variety of cell lineages including T lymphocytes, macrophages and eosinophils. In these cell types, IL-4 is a general inducer of this molecule while IFN-gamma down-regulates B cell Fc epsilon RII/CD23 and up-regulates Fc epsilon RII/CD23 on macrophage and eosinophil cell lines. As predicted by the expression of Fc epsilon RII/CD23 in some HTLV-1(+) T cell lines, Fc epsilon RII/CD23 proved to be induced on normal peripheral T lymphocytes by IL-4 or IL-2 in the presence of additional permissive signals. As indicated by IL-2-dependent Fc epsilon RII/CD23 induction, there is an interesting bilateral co-regulation between Fc epsilon RII/CD23 and the 55 kDa chain of the IL-2 receptor complex with Tac antigen (IL-2R/p55(Tac]. Triggering of Fc epsilon RII/CD23 resulted in the enhanced expression of IL-2R/p55(Tac), whereas IL-2 enhanced the expression of Fc epsilon RII/CD23 in some systems. It is suggested that the triggering of cell surface Fc epsilon RII/CD23 by natural ligands is effectively buffered by soluble Fc epsilon RII/CD23 (IgE-BF).

[Clinical, electrophysiological and histological analysis of adult onset chronic sensory neuropathy].

We reported clinical, electrophysiological and pathological findings in 10 patients with acquired, slowly progressive sensory neuropathy. Group I patients which had delayed sensory nerve conduction velocities (SNCV) or no response of SCV in electrophysiological studies were observed to have marked deep sensory disturbances. On the other hand, Group II who had abnormalities of both SNCV and motor nerve conduction velocities electrophysiologically has tendency to show superficial sensory disturbances more dominantly than that of deep sensation. Histological findings of biopsied sural nerve in Group I were mainly composed of diminution of large myelinated fibers, in Group II the diminution of large myelinated fibers with decreased unmyelinated fibers were observed. However, there were no significant difference between Group I and Group II in the quantitative analysis of myelinated fibers. There were no definite tendency under the basic disease or background factors of these 10 patients. Only 3 of these 10 patients had immunological abnormalities such as RA factors or antinuclear antibodies.

Regulation of Fc epsilon R2/CD23 gene expression by cytokines and specific ligands (IgE and anti-Fc epsilon R2 monoclonal antibody). Variable regulation depending on the cell types.

The regulation of human low affinity FcR for IgE (Fc epsilon R2/CD23) and the soluble Fc epsilon R2 [IgE binding factor (BF)] of monocyte (U937), T (ED), and B (JIJOYE) cell lines was examined by anti-Fc epsilon R2 mAb (H107, Mab176) and the cDNA probe for Fc epsilon R2. The effect of IL-4 and IFN-gamma on Fc epsilon R2 regulation was variable among these three cell lines. IL-4 and IFN-gamma enhanced the Fc epsilon R2 gene expression and the production of Fc epsilon R2 and IgE-BF on U937, whereas IL-4 and IFN-gamma had no significant effect on the Fc epsilon R2 expression on ED. On JIJOYE, IL-4 enhanced the Fc epsilon R2 and IgE-BF production on both protein and mRNA levels. In U937 and JIJOYE cells, there was a marked increase of Fc epsilon R2 mRNA after combined stimulation with IFN-gamma and IL-4. However, in JIJOYE cells, there was a dissociation between the surface expression of Fc epsilon R2 and Fc epsilon R2 mRNA treated with IFN-gamma plus IL-4. In these cells. IFN-gamma even down-regulated the IL-4-induced expression of surface Fc epsilon R2. Stimulation of JIJOYE cells with both IFN-gamma and IL-4 resulted in the increase of the IgE-BF in the supernatant, suggesting that IFN-gamma enhanced the release of IgE-BF from Fc epsilon R2. The results indicated that Fc epsilon R2 and IgE-BF expression is regulated by IFN-gamma at least on two different levels: on transcriptional levels and the levels of cleavage of the surface Fc epsilon R2 to release soluble Fc epsilon R2 (IgE-BF). Ligands binding to the Fc epsilon R2 such as IgE and anti-Fc epsilon R2 mAb enhanced the surface expression of Fc epsilon R2 on these Fc epsilon R2(+) cell lines. This was mainly due to the surface accumulation of the receptors on JIJOYE and U937. However, the stimulation of ED by H107 and anti-Fc epsilon R2 mAb significantly enhanced the mRNA expression, indicating that Fc epsilon R2 synthesis may also be up-regulated by the specific ligands in some cell types.

Electron microscopic observation on the epithelial-connective tissue junction of pemphigus vulgaris. A case report.

An electron microscopic observation on the erosive buccal mucosa of a buccal mucosa of a patient with typical pemphigus vulgaris was carried out. The ultrastructural features of the basal cells and the basal lamina in the advanced oral lesion varied from section to section of the biopsy specimen. The basal lamina showed various morphologic alterations indicating the presence of both destructive and regenerative processes in different sections. The filamentous bodies were also observed both in the basal cell layer and in the upper lamina propria.
