[Disorders of trace element metabolism and their correction using artificial nutrition in patients with cancer of the esophagus in the preoperative period].

Findings of research into disorders of the micronutrient exchange in esophageal carcinoma patients are overviewed. The micronutrient level in the plasma, erythrocytes, and urea was estimated by means of emission plasma spectroscopy. Patients with esophageal carcinoma were found to have pronounced changes in the micronutrient exchange on their admission into the clinic. These changes were corrected by means of artificial nutrition, which included preparations with directed action and micronutrients. After the course of therapy a tendency towards normalization of the micronutrient exchange was observed.

[Substantiation of the expediency of performing extensive lymph node dissection in surgical treatment of esophageal cancer].

The performance of extensive lymph node dissection+ in esophageal carcinoma is substantiated. From their own data the authors conclude that squamous cell carcinoma of the esophagus possesses high lymphatic metastatic activity and point to the statistically significant increase of the incidence of involvement of the superior perigastric lymph nodes in localization of the tumor in its infrathoracic part. At the same time, the frequency of affection of the mediastinal lymph collector does not depend on the localization of the tumor. The authors emphasize that extensive subsequent lymph node dissection+ of the mediastinum and upper part of the abdominal cavity should be an obligatory stage of an operative intervention in esophageal carcinoma.

[Determining the level of the tumor marker--neuron-specific enolase in patients with neoplastic and non-neoplastic diseases].

Radioimmunoassay was employed to measure serum levels of neuron-specific enolase (NSE) in 38 lung cancer patients, 20 patients with noncancer pulmonary lesions and 10 healthy donors. An average NSE level (92.8 +/- 18.7 micrograms/l) in small cell carcinoma (10 patients) was significantly higher (p less than 0.01) than in control, nonmalignant pulmonary diseases, squamous cell and adenocarcinoma. Sensitivity and specificity of the tumor marker to small cell carcinoma reached 80 and 95%, respectively. NSE serum test may serve an additional tool in differential diagnosis of small cell carcinoma of the lung.

[Disorders in the sequence of regional metastasis and long-term results of the surgical treatment of complicated lung cancer].

To ascertain the frequency of disturbance of the stage character of regional metastasis in lung carcinoma complicated by disintegration or obstructive pneumonitis, and its influence on certain results of surgical treatment, the authors analysed the data on 175 patients who underwent lung resection. The stage character of regional lymphogenous metastasis was found to be disturbed in 61 (34.8%) patients, in 52 (85.2%) of them the main disease was complicated by obstructive pneumonitis. Five-year survival in the group of patients without disturbance of the stage character of metastasis was 28 +/- 4.8%, which was significantly greater than that in the group of patients with a disturbed stage character (15.5 +/- 5.2%).

[Computerized tomography in the surgery of esophageal cancer].

Preoperative X-ray computerized tomography (CT) was performed in 60 patients with carcinoma of the esophagus at the All-Union Oncology Research Centre, USSR AMS in 1986-1989. The method is evaluated on the basis of comparison with the operative findings in determining the depth of invasion of the esophageal wall and adjoining structures by the tumor, metastatic involvement of the lymph nodes below the diaphragm, and metastases in the liver. Informativeness of X-ray CT proved to be highest in patients in whom the tumor had not spread beyond the esophageal wall. It was established that the method possesses high resolving possibilities in identification of enlarged lymph nodes in the abdominal cavity. Small metastases in the liver, measuring 0.5 to 2 cm in diameter, cannot be revealed by the method in some cases. The data gained by means of X-ray CT allows tactics of the surgical intervention to be determined and the late-term results to be predicted.

[Steroid hormone receptors in lung cancer tissue].

Receptors to estrogens (ER), progesterone (PR), glucocorticoids (GR) and androgens (AR) were assayed in 44 lung cancer patients to evaluate hormonal sensitivity of tumor. In 30 cases, active forms of GR were additionally measured by DNA-cellulose adsorption method. GR appeared to be most frequent (65%). Tumors contained PR and ER in 22% and 16% respectively, but failed to reveal AR. A correlation was established between stage and GR positivity, with 83 +/- 8% of GR-positive tumors observed at stage T1-3N0M0 as compared to 50 +/- 12% at stage T1-3N 1-2M0 (P less than or equal to 0.05). As few as 55% (11 out of 20) of GR-positive tumors revealed receptors capable of activation and interaction with DNA-cellulose. These tumors alone, one third of tumors studied, are likely to respond to glucocorticoid treatment.

[The ultrastructural characteristics of human lung adenocarcinomas as a factor in the prognosis in surgical treatment].

The material of 70 patients with lung adenocarcinomas radically operated at the All-Union Cancer Research Centre is analyzed. The spread of tumour expressed in terms of TNM system (I group--T1-3 N0M0--37 patients, II group--T1-3 N1M0--18 patients, III group--T1-3 N2M0--11 patients, IV group--T1-3 N1M1--4 patients) and its ultrastructure served as criteria. The correlation is found between the life-span, degree and type of the tumour cell ultrastructural differentiation. The most favourable prognosis at comparable tumour spread is found when the tumour cell ultrastructural differentiation corresponded to that of Clara type, pneumocytes of II order and ciliary type. Less favourable ultrastructural features were the domination of undifferentiated cells and the presence of goblet cells and those having the signs of differentiation into the various directions. The lack of correlation of the differentiation at the light microscopic and ultrastructural levels is found in some cases this being reflected in delayed results.

[Garlock's operation in cancer of the esophagus].

Experience in 88 Garlock-type operations carried out in carcinoma of the esophagus allows a certain opinion to be formed on the advantages and shortcomings of this surgical intervention. In esophageal carcinoma with its proximal boundary within the range of the retropericardial segment of the esophagus, a left abdominothoracic approach (Garlock's operation) is adequate in resection of the esophagus in conformity with the oncological principles and allows the formation of the esophagogastric anastomosis at the level of the arch of the aorta without particular technical difficulties. Reduction of the time needed for the operation, determination of the resectability of the esophageal tumor before mobilization of the stomach are obvious advantages of the abdominothoracic approach.

[Surgical and combined treatment of esophageal cancer].

The authors analyse 37-year experience in the treatment of patients with esophageal carcinoma by various methods at the All-Union Cancer Research Center. The immediate and long-term results of radiotherapy and surgical and combined treatment of 1,850 patients are shown. Bearing in mind the considerable extension of the neoplastic process by the time of the operation, the authors substantiate the expediency of one-stage operations with the formation of intrapleural esophagogastric anastomoses. The mortality in the recent years after 218 Lewis' operations and 36 Herlock's operations was 9.0%. After analysing experience in combined treatment, the authors failed to reveal any particular advantages over solely surgical treatment, but in view of the lack of other alternatives for improving the results of management of esophageal carcinoma to-date, they believe it expedient to continue study and improve various combinations of preoperative radio-(chemoradio)therapy and operation in large research centres.
