[Potentials of transhepatic catheterization of the portal vein in diagnosing and treating surgical diseases].

Questions of the application of paracentetic transhepatic catheterization of the portal vein performed from the lateral access are dealt with. Indications for its application are discussed. Morphological changes and disturbances of hemodynamics of the portal system in certain diseases of the abdominal cavity are analyzed. Possible application of catheterization of the portal vein with curative purposes in order to stop oesophageal hemorrhages in portal hypertension and for continuous antienzymatic therapy of pancreonecrosis is shown.

[Diagnostic potentials of angiography in mechanical jaundice].

Angiography of the celiac artery and its branches was used in patients with mechanical jaundice of different etiology. This method gave the correct preoperative diagnosis in 85.1% of patients. In cases of tumors of the pancreas head, Fater's papilla, gall bladder, cholelithiasis accompanied by mechanical jaundice characteristic angiographic features or their combinations were revealed.

[Retrograde cholangiopancreatography and endoscopic papillotomy in mechanical jaundice].

The authors believe that in jaundices of a mechanical type the endoscopic retrograde cholangiopancreatography should be considered the first and foremost diagnostic method and fulfilled in every patient with symptoms of mechanical jaundice. They believe that in order to eliminate the full cholestasis and cholemia in a certain contingent of patients it is expedient to perform endoscopic papillotomy which is followed by spontaneous passage of stone into the intestine.

Ampulla of Vater↗

[Incarcerated calculi of the major duodenal papilla - a problem of biliary tract surgery].

Strangulation of the stones of the great duodenal papilla is found in almost every fourth patient with a benigh mechanical jaundice. A radical method for removing the stones from the ampulla of the papilla is the transduodenal papillosphincterotomy. The 1-1,5 cm dissection of the papilla is followed by an external drainage of the hepatocholedochus after Holstead or Carr. The nearest results of such operations with strangulated stones are favourable.

[Papillosphincterotomy in lesions of terminal segment of the common bile duct].

The dissection of the papilla was carried out in 70 patients with pure stenosing papillitis and with papillitis combined with choledocholithiasis and cholangitis. The authors consider subtotal papillosphincterotomy with the length of incision of 1.5 cm to be an expedient means for the dissection of the papilla, which, in their opinion, allows to prevent developing of restenosis. The dissection of the papilla was carried out upon 35 patients with incarcerated stones and upon 8 patients with floating stones. Some practical and helpful recommendations are suggested.
