Secondary syphilis. Clinical morphology and histopathology.

Twenty-four biopsy specimens of skin were taken from 23 patients with several types of lesions of secondary syphilis. In macular and maculopapular lesions, the histological changes were not specific. No or few plasma cells were observed in specimens at this stage. In early papular lesions, the histological appearance was more diagnostic. A lymphohistiocytic bandlike infiltrate was present in the upper part of the dermis and extended around blood vessels of the deep plexus. In nearly two-thirds of the cases, plasma cells were present in early papular lesions. The epidermis, too, was frequently involved in these lesions of the disease. In late papular lesions, the pathologic features were distinctive. The infiltrate was dense, diffuse, and, in some instances, nearly completely plasmocytic. In nummular syphilides, many neutrophils were present in a hyperplastic epidermis, whereas in corymbiform lesions, the epidermis was not involved at all.

[Aspects of general anesthesia in cerebral angiography].

The need for continuous control of the parameters that may influence cerebral homeostasis has made it advisable to use general anaesthesia for cerebral angiography. A combination of propanidide and neuroleptanalgesia, with controlled ventilation, was employed in a group of 66 patients with various cerebral diseases. The stability of narcosis and the absence of significant changes in the cardiocirculatory and respiratory parameters prevented alterations in cerebral flow and intracranial pressure during the examination. Prompt reawakening enabled the patient's neurological state at the end of the examination to be immediately evaluated. In addition, the angiograms were of better quality.

GAp junctions in skin tumors of molluscum contagiosum.

Thin section, tracer, and freeze-fracture experiments demonstrate that the infection caused by molluscum contagiosum virus implies alterations of cellular contacts in the epidermis, never before observed in cutaneous viroses. These changes are basically represented by a distinctive variation in particle packing of gap junctions, a greater proliferation of these junctions and the presence of a high amount of intracytoplasmic gap junctions. Skin tumors caused by molluscum contagiosum virus appear to be a convenient model to study the morphological and topological variations as well as the turnover of gap junctions in the epidermis.

Internal organization of plasma membranes during the acantholytic process. A freeze-fracture study.

The internal organization of plasma membranes was studied in seven subjects with pemphigus vulgaris and in one subject with familial pemphigus using the freeze-fracture technique. The results obtained seem to suggest that the acantholytic process is produced in a similar fashion whatever the pathogenic agent and that desmosomes represent the "target" membrane specializations in the acantholytic phenomena. The reduction and eventual disappearance of desmosomal particle aggregations is seemingly accompanied by an increase in gap junctions.

Plasma membranes in psoriatic cells. A freeze-fracture study.

A freeze-fracture study of affected and unaffected psoriatic skin has demonstrated the presence of marked modification of the plasma membrane in the psoriatic lesion. In the lower layers of the epidermis, an increase of membrane associated particles was observed in many keratinocytes, possibly representing the morphological intramembranous equivalent of changes in the outer cell membrane demonstrated with cytochemical techniques. Furthermore, in the malphighian layer, numerous gap junctions have been found, which may be interpreted as a phenomenon compensating the uncontrolled proliferation, and may represent a point of differentiation between cell proliferation in psoriasis and neoplasia. This technique confirmed the poor tendency to adhesion of keratinocytes in extrajunctional areas, which had already been shown by other morphological techniques.

Drug-induced changes in the plasma membrane of Microsporum canis. A preliminary freeze--fracture study.

Commercial solutions of clotrimazole, tolnaftate and tolcyclate were tested for 12 h in cultures of Microsporum canis. They induced modifications in particle distribution, a blebbing of particle-free areas and the appearance of lamellar figures on the plasma membrane of fungus cells. The first two changes in the plasma membrane might be ascribed to a modification of the spectrin-like meshwork, while the third could be the result of a crystalline organization of membrane lipids.

Metalloproteinase-7 contributes to joint destruction in Staphylococcus aureus induced arthritis.

Septic arthritis induced by Staphylococcus aureus causes a rapid destruction of joint cartilage and periarticular bone. The mechanisms behind this phenomenon are not fully understood. Earlier studies have shown that cytokines and metalloproteinases are of importance in bone metabolism. Matrix metalloproteinase-7 (MMP-7) has pleiotropic function including facilitating migration of both macrophages and neutrophils. The aim of this study has been to investigate the significance of MMP-7 expression in septic arthritis. MMP-7 deficient mice and congeneic controls were intravenously inoculated with an arthritogenic dose of S. aureus LS-1. This study shows that MMP-7 deficient mice exposed to S. aureus developed significantly less severe arthritis both clinically and histologically. Despite this finding, bacterial growth in the deficient animals was significantly increased. In vitro responses to staphylococcal antigens and superantigens did not differ between MMP-7(+/+) and MMP-7(-/-) mice with respect to cytokine production and if anything increased the production of certain chemokines. In addition MMP-7(-/-) mice exhibited decreased numbers of peripheral blood mononuclear cells before and one day after bacterial inoculation, but increased numbers of peripheral granulocytes on day 1. In conclusion, MMP-7 contributes to the development of a destructive course of septic arthritis despite decreased bacterial load. In addition, expression of MMP-7 is of importance for the distribution of peripheral leukocytes.

Ultrastructural findings in incontinentia pigmenti.

From their ultrastructural study of incontinenti pigmenti, the authors confirm that in this condition there is no pigmentary anomaly, but that pigmentation is the last step of a still unknown process. At birth those who are affected display an intracellular edema which leads to the formation of a vesocobulla. In all 3 stages, the epidermis shows dyskeratotic cells and a transient pigmentary discharge.

Routine immunofluorescence and light microscopy processing with a single renal biopsy specimen: 18 years' experience in a single centre.

To obtain a correct histological renal diagnosis, two adequate cortical biopsy cylinders are routinely taken for light microscopy and immunohistology in nephrology units. We describe our simple method for light microscopy (LM) study using the same frozen renal material used for immunofluorescence (IF). Of over 2,000 biopsies processed with this method, only three showed thawing artifacts and all occurred when the room temperature was above 30 degrees C. Our 18 years' experience and the large number of biopsies indicate that this method offers an interesting new way to do LM and IF with one biopsy core and could help stimulate etiopathogenic and diagnostic studies in nephrology. The other renal specimen can be used for histological investigation, applying recent molecular biology techniques (PCR, RT-PCR, in situ hybridization).

Indications and results of surgical treatment of cervical disc disease by anterior and posterior approach.

The authors report their experience with indications (130 cases) for the treatment of cervical disc disorders, which has recently witnessed an increase in cervicobrachial pain in disc-spinal cord conflict syndromes. After a first period during which the use of hemilanimoarthrectomy prevailed, there has been an evident increase in the number of cases treated by discectomy by Cloward fusion and an anterior approach. Thus, the results obtained in local symptoms have improved; more precisely, the problem of neck pain and cervical mobility have been resolved, while positive results in peripheral symptoms have remained the same.
