[Role of local flaps in the treatment of interdigital commissure contractures].

The authors present a review of the most reliable and effective local flaps in the treatment of moderate web contractures. In their opinion, they are very helpful in that pathology's treatment provided that indications and limits are well pointed out. Advantages and disadvantages of each described procedure reported.

[Reconstruction of the first interdigital commissure with distal flaps].

Authors report some examples of distant flaps which they judge excellent in first web reconstruction following severe posttraumatic skin retraction. The utilization of these procedures is suggested in those cases, particularly if complicated by skin loss, which need fairly large amount of skin with thin subcutaneous tissue like the one of the three described flaps.

Low back pain in athletes: pathogenetic mechanisms and therapy.

A different and more in-depth approach is needed when treating an athlete suffering from low back pain than is the case when the same pathology affects a sedentary subject. In fact, pain symptoms may frequently arise only when there is functional overloading, while pain is absent or not disabling in normal daily activity. Treatment is also different for the athlete: complete functional recovery must take place in as short a period of time as possible, and it will often have to be finalized in relation to sports-related commitments. A group of 60 professional athletes were evaluated in order to examine the incidence and the features of lumbar pain. Attention was particularly paid to a functional evaluation of the spine by means of an accurate clinical examination and the use of the Metrecom system. The data obtained confirm the importance of changes in posture and in vertebral dynamics when the pain symptom occurs.

[The sensitivity of ultradistal mineral measurement of the radius in the early assessment of bone loss].

This study was carried out to assess the sensitivity of single photon bone absorptiometry (SPBA) as an early screening tool for osteoporosis and select the optimum site for measurement. We measured bone mineral density (BMD) at the distal tenth of both radius and ulna (D-BMD) and at the ultradistal radius, a site distal to the point at which the radius and ulna are 8 mm apart (UD-BMD). Our study group consisted of 107 healthy women aged 25-60 years (mean +/- SD: 45.8 +/- 10.0), subdivided according to age into 3 groups: Group 1: 25-40 years, Group 2: 41-50 years, Group 3: 51-60 years. When comparison was made to Group 1, D-BMD was found significantly reduced only in Group 3 (p < 0.0001), while UD-BMD was found reduced in both Groups 2 (p < 0.02) and 3 (p < 0.0001). The percent reduction of BMD in Groups 2 and 3 with respect to Group 1 was significantly greater at the ultradistal site than at the distal tenth of the radius and ulna (p < 0.0001 versus D-BMD). Despite limits inherent in the cross-sectional study design, our results suggest that forearm SPBA indicates post-menopausal bone loss irrespective of scanning site, but detects pre-menopausal loss sensitively only at the ultradistal site. Ultradistal measurement would therefore seem suitable for early screening for osteoporosis as an alternative or in addition to the generally used more proximal site.

Reinnervated free gracilis muscle transplantation in the treatment of Volkmann's syndrome of the forearm.

Between 1989 and 1994 free gracilis muscle reinnervation was used to treat three patients severely affected with Volkmann's syndrome of the forearm. All three patients were males and they had supracondyloid fracture of the humerus, treated nonsurgically an average of 7.6 years previously. Mean age at the time of surgery was 19.3 years. The flap was transplanted in flexor function of the fingers in two of the patients, in extensor function of the wrist in one. Electromyography and successive clinical monitoring revealed an increase in contractile strength up to one year after surgery. In the first two cases final hold strength exceeded by more than 50% that of the contralateral limb, in the third case excursion of the wrist which could not be quantified, but which was useful for elementary activities was recovered. When myotendinous units for transplantation are not available, free gracilis muscle reinnervation constitutes a valid surgical solution in cases of severe Volkmann's syndrome of the forearm.

[Laryngo-tracheal pathology in newborns and children].

In the newborn and in the children are frequent features like congenital laryngeal stridor, croup, weak or absent cry, cyanotic attacks, apneic attacks, and sometimes are due to disease like laryngomalacia, tracheomalacia, laryngeal web, post-intubation lesions and stenosis, tracheo-esophageal fistula. The Author present an experience of four years in the Vicenza's Hospital with rigid telescopes and flexible fiberoptic instruments in the endoscopic valuation and treatment of this pathology. Moreover present a new technics for the preoperative incannulation of the tracheoesophageal fistula using the flexible fiberscope.
