Contribution of flow cytometric immunophenotyping and bone marrow trephine biopsy in the detection of lymphoid bone marrow infiltration in non-Hodgkin's lymphomas.

The bone marrow (BM) is a frequent site of involvement in non-Hodgkińs lymphomas (NHL) and evidence of an infiltrated BM may implicate different therapeutical regimens. Flow cytometric immunophenotyping of bone marrow aspirates now is included in the assessment of patients with NHL and used as an adjunct to morphologic evaluation in the staging of lymphoma. The aim of the study was to compare flow cytometric immunophenotyping of BM and paraffin section staining of BM biopsies in the marrow involvement of NHL. Cytometric immunophenotyping of bone marrow and immunohistochemical paraffin section staining of bone marrow biopsies in 53 B- and T-cell lymphoma patients were performed. We used the following fluorochrom conjugated monoclonal antibodies specific for: CD3, CD4, CD5, CD7, CD8, CD10, CD19, CD20, CD22, CD23, CD79B, FMC7 and Ig kappagamma light chain. Unilateral BM trephine biopsies were obtained in all cases, fixed, decalcified and paraffin-embedded. Morphologic marrow involvement by lymphoma was found in 24 cases; flow immunophenotyping identified 26 cases with NHL: morphology-positive/flow-positive (n=21), morphology positive/flow-negative (n=3), morphology-negative/flow-positive (n=4), and morphology-negative/flow-negative (n=23). The concurrence rate of BM trephine biopsy and flow cytometric immunophenotyping in evaluation of NHL bone marrow infiltration was 88.7%. Immunophenotyping of the bone marrow of NHL patients by flow cytometry is helpful for assessment of bone marrow infiltration, especially in B-cell disorders. Both trephine biopsies and flow cytometry are better than single investigation for detection of infiltration in NHL.

Laminin, Her2/neu and Ki-67 as prognostic factors in non-small cell lung cancer.

PURPOSE: The aim of this study was to determine the values of Ki-67 antigen, oncoprotein Her2/neu and laminin, as prognostic and predictive factors in NSCLC, and on the basis of these markers to create a prognostic model which would make it possible to identify patients with a high risk of disease recurrence. MATERIAL AND METHODS: The material for the study came from 64 patients with NSCLC, who underwent surgery in Dolnoślaskie Centrum Gruźlicy i Chorób Płuc i 1996-2000, and subsequently were given radiation therapy in Dolnoślaskie Centrum Onkologii. RESULTS: Among the markers researched, a high level of (intracellular) laminin in carcinoma cells was found to be an unfavourable prognostic factor. Also, another group of patients with an overexpression of oncoprotein Her2/neu were found to have a poorer prognosis, although the influence of proliferative index Ki-67 on patient survival could not be explained. The prognostic model LAMHER, which was defined on the basis of the intracellular laminin level and expression HER2/neu, enables the identification of a group of patients with a high risk of disease recurrence. In a multidimensional analysis of the "classification tree", it was found that patients with the highest risk of disease recurrence were those with LAMHER = 1 (overexpression Her2/neu and/or a high level of intracellular laminin), and patients with LAMHER = 0 but a Fractionation Dilution Factor higher than 1.57. CONCLUSIONS: The conclusion of this study is that multiple molecular marker testing is necessary to detect an independent prognostic impact on survival and is therefore superior to single marker testing. Based on LAMHER testing, two groups of patients could be defined: a low-risk group (LAMHER = 0) and a high-risk group (LAMHER = 1) for failure of standardized treatment.

Role of metallothionein expression in non-small cell lung carcinomas.

Metallothionein (MT) is a low molecular weight protein, which participates in differentiation and proliferation of normal and tumour cells. In some malignant tumours (mammary, renal, ovarian cancers), its increased expression is thought to represent an unfavourable prognostic factor. Non-small-cellular lung cancers (mainly squamocellular cancer and adenocarcinoma) are characterised by ill-defined prognosis, which poses problems in the selection of effective post-surgical therapy. The present study aimed at demonstration of the prognostic significance of MT expression in cells of non-small cell lung cancers, attempting to correlate the intensity of MT expression with G grade and with the intensity of proliferation-associated antigen, Ki-67 expression. The studies were performed on archival paraffin blocks with samples of 25 cases of non-small cell lung cancers (5 squamous cell cancers, 20 adenocarcinomas). In paraffin sections of the studied tumours, immunocytochemical reactions were performed, using mouse monoclonal anti-MT and anti-Ki-67 antibodies. The expressions of MT and Ki-67 were demonstrated in all the studied tumours. An analysis of correlation between the expression of MT, Ki-67 antigen and G grade demonstrated a strong positive relation between the latter two parameters (r=0.70; p<0.05). Less pronounced positive correlations were disclosed between MT expression and G grade (r=0.44; p<0.05) and between MT expression and the expression of Ki-67 antigen (r=0.41; p<0.05). In addition, in 15 cases of examined tumours, survival analysis was performed, which disclosed a shorter survival in patients with high MT expression. The obtained results confirmed the relationship between MT expression and Ki-67 antigen expression, indicating an involvement of the proteins in processes of tumour cell proliferation. In turn, the shorter survival of patients with high expression of MT pointed to prognostic significance of the protein in non-small cell lung cancers.

[Hematological disorder caused by a giant myoma of the uterus].

OBJECTIVES: Polycythaemia means an increase in the number of red blood cells and usually a corresponding increase in the haematocrit and in the quantity of haemoglobin. Regarding etiology, primary and secondary forms of polycythaemia can be distinguished. The primary form--polycythaemia vera--is caused by autonomous erythroblastic hyperplasia. Secondary polycythaemia--erythrocythosis--may arise as a consequence of erythropoietin (EP) overproduction due to tissue hypoxia. These changes may stimulate the EP synthesis in the kidneys. EP may also be produced in metaplastic cells. STUDY DESIGN: A 59-year-old woman was admitted to the Clinic of Haematology of the Wrocław Medical University with a 10-years' history of significant increase in HB, VPRC and RBC. Physical examination showed an obvious plethora and a large intra-abdominal painless tumour. Laboratory data revealed increases in Hb, VPRC, RBC and EP. A bone marrow aspiration was normal. During surgery, a large leiomyoma of the uterus was found and a total hysterectomy was performed. Pathological examination showed the tumour to be fibroleiomyoma. 4 weeks after surgery Hb, VPRC, RBC and EP, all returned to normal levels. RESULTS: Excluding the effect of the renal compression, it can be concluded that erythropoietin secreted by the the tumour caused the secondary haematological changes.

[The evaluation of the relationship between malignancy grade and cytokeratin-10 accumulation in human skin squamous cell carcinoma].

The aim of the study was analysis of CK-10 expression in human skin squamous cell carcinoma (SCC) basing on immunohistochemical procedure. The study was carried out on 43 samples of skin SCC which were evaluated histopathologically with regard to 3-grade scale (G) of malignancy. In each case immunohistochemical reactions by use of ABC method were carried out in order to detect CK-10 within cancer cells. The expression of CK-10 was evaluated in accordance with arbitrary 3 grade scale: from +++ to +. The obtained results revealed affection of CK-10 expression in cancer cells. In G1 and G2 skin SCC a moderate expression of CK-10 was found and this occurred in cells grouped in nests. In G3 SCC the expression of CK-10 was very low and this was noted in single cells only. The use of immunohistochemical methods in evaluation of CK-10 expression can be a useful tool in routine histopathologic examination of tumors of epithelial origin.

[Abdominal presentation of B-cell non-Hodgkin's lymphoma (B-NHL) - surgical treatment and its results. Report of the Polish Paediatric Leukaemia/Lymphoma Study Group].

The aim of this study was to analyse the effect of LMB-89 protocol and surgical procedure at initial laparotomy on the outcome in children with abdominal B-cell NHL. The initial surgery intervention was: complete resection (20% pts), subtotal resection (20%), partial resection (4%), biopsy (36%). Postoperative complications occurred in 5 children. Complete recovery (CR) was achieved in 92% pts. There were 4% non responder patients. Two patients died before CR evaluation (tumour lysis syndrome; bleeding and multi organ failure after initial surgery). One patient died in CCR from sepsis probably influenced by the previous local operation. 10.8% patients relapsed. The estimate EFS for all patients with AB-NHL is 81%, 85% for stage III and 73% for stage IV. Major surgery in advanced stages is not recommended since it delays chemotherapy and fails to improve overall survival.

Immunohistochemically stained markers (p53, PCNA, bcl-2) in dysplastic lesions of the larynx.

The percentage of malignant transformation of laryngeal dysplastic lesions is difficult to estimate. There is a need for new histological markers which could enable more objective assessment of the premalignant stages of the larynx and help in estimation of the potential of future neoplastic progression. We performed a retrospective study to determine whether immunohistochemical staining for the proliferating cell nuclear antigen (PCNA), tumour suppressor gene protein p53 and antiapoptotic protein bcl-2 may be prognostic factors in laryngeal epithelial lesions. Staining was performed on 57 paraffin-embedded biopsies from patients with clinically detected precancerous stages of the larynx. Histopathologic examination revealed normal epithelium in six cases, mild dysplasia in 20 cases, moderate dysplasia in 18 cases, severe dysplasia in seven cases, CA in situ in four cases, papilloma in one case and CA invasivum in one case. The p53 count in mild and moderate dysplasia was 26.8 and 38.6%, respectively. This difference was statistically significant. There was significant correlation between PCNA and p53 scores. There was also a relationship between the scores of these markers and bcl-2 expression. In ten out of 45 cases of dysplastic lesions the invasive cancer developed in 4 years of follow-up. The correlation between PCNA score and malignant progression of the dysplastic lesions was on the statistical borderline. There was significant relationship between malignant transformation and age of the patients.

Epidermal growth factor receptor (EGFR), proliferating cell nuclear antigen (PCNA) and Ki-67 antigen in laryngeal epithelial lesions.

The epidermal growth factor receptor (EGFR), proliferating cell nuclear antigen (PCNA) and Ki-67 immunostaining in paraffin sections from 154 cases of laryngeal squamous cell carcinoma, 25 specimens of normal and hyperplastic laryngeal epithelium, and 21 preneoplastic lesions was examined. The difference of EGFR expression and PCNA score between laryngeal cancer and dysplastic lesions vs normal and hyperplastic epithelium was significant. There was significant difference in Ki-67 score between malignant vs premalignant lesions and normal and hyperplastic epithelium. There was significant correlation between PCNA score and histopathological grading of the tumour. The highest PCNA expression was detected in stage G3. Our findings have shown that PCNA and Ki-67 staining can be used as a marker of cell proliferative activity in laryngeal epithelial lesions. EGFR, PCNA and Ki-67 expression correlate with severity of laryngeal lesions. PCNA immunostaining can aid in estimating the histological grade of malignant lesions.

C-erbB-2 immunostaining in laryngeal cancer.

Tumour progression is strongly associated with a series of specific genetic changes in protooncogenes and tumour suppressor genes. One of the potential factors involved in tumorogenesis of squamous cell carcinomas is protooncogene c-erbB-2 (also known as neu or HER2). The authors analysed the expression of c-erbB-2 oncoprotein in 154 cases of laryngeal squamous cell carcinomas and its relationship to the clinical outcome of the patients. The difference in c-erbB-2 oncoprotein expression between the control group and cancer patients was on the statistical borderline (p = 0.0470). There was no significant correlation between c-erbB-2 expression and sex and age of the patients. T stage, lymph node status, site and histopathological grading of the tumour and clinical outcome of the patients. Univariate analysis revealed no correlation between c-erbB-2 expression and survival rates. We conclude that immunohistological examination of c-erbB-2 on paraffin section is not a valuable prognostic factor in laryngeal carcinoma.

[Analysis of basement membranes status in skin squamous cell carcinoma (SCC)].

The aim of study was analysis of basal membranes in skin squamous cell carcinoma (SCC) basing on immunoreactivity of basal membrane component--laminin. The study was carried out on 43 samples of skin SCC which were evaluated histopathologically with regard to 3-grade scale (G) of malignancy. In each case immunohistochemical reactions by use of ABC method were carried out in order to detect laminin within basal membranes. The amount of laminin in basal membranes was evaluated semiquantitatively thus enabling to distinguish 3 groups of SCC: with abundant, moderate and small amount of laminin. The obtained results revealed significant differences within basal membranes according to malignancy grade. Most frequent anomalies referred to differences in basal membranes regularity, thickness and quantitative changes. In group of cancers with abundant laminin the number of tumor nests surrounded with mostly well formed basal membranes was found. Cancers with moderate laminin revealed variously formed basal membranes with distinct morphologic alterations and local gaps in continuity. In group of cancers with small laminin solely remnants of basal membranes were found. The results of present study indicated predominance of moderate amount of basal membranes in skin G1-2 SCC and small amount of basal membranes in poorly differentiated G3 SCC. The use of immunohistochemical methods in evaluation of laminin expression can be a useful tool in routine histopathologic examination of tumors of epithelial origin.

[A case of primary thyroid lymphoma with Hashimoto thyroiditis].

A seventy four year old man presented with a mass of the thyroid which was diagnosed as a lymphoma arising from the thyroid gland with Hashimoto thyroiditis. Postoperative course was complicated by the tumor reoccurence presenting as invasion of the larynx with a critical narrowing of the lumen of the larynx. With chemotherapy complete remission has been achieved. This case illustrates common aetiology of the thyroid lymphoma of MALT type and Hashimoto thyroiditis.

Proliferating cell nuclear antigen in laryngeal cancer.

The expression of proliferating cell nuclear antigen in paraffin sections from 154 cases of laryngeal squamous cell carcinoma were examined. There was significant difference in PCNA expression between the control group and cancer patients (p < 0.001). The mean score of PCNA was higher in patients with poor prognosis than in patients with satisfactory outcome after treatment (p < 0.05). There was no significant correlation between the PCNA count and the patient's age and sex, T and N stage and site of the tumour. Univariate analysis revealed that the PCNA score correlated with the patients' survival rates. In multivariate analysis the prognostic value of PCNA was on the statistical borderline (p = 0.049). In our study clinical features like N and T status had a more important influence on survival rate. Nevertheless it appears that the immunohistological examination of PCNA in paraffin section could be a complementary prognostic tool for laryngeal carcinoma. PCNA expression may also be a valuable tool for differentiating malignant from benign laryngeal epithelium.

Ki-67 immunostaining and prognosis in laryngeal cancer.

The expression of the antigen defined by the Ki-67 antibody in paraffin sections from 154 biopsies of laryngeal squamous cell carcinomas was examined. There was a significant difference in Ki-67 expression between the control group and the patients with cancer (P < 0.001). There was no significant difference in the Ki-67 score between the patients with recurrence and the patients with a satisfactory outcome after treatment. There was no significant correlation between the Ki-67 score and the patient's age and sex, T and N stage and site of the tumour. The survival time of patients with a Ki-67 score > 30 was shorter than patients with a Ki-67 score < or = 30 but the difference did not reach statistical significance (P = 0.055). Multivariate analysis indicated that the only important prognostic factor was the existence of lymph node metastases. We could not confirm the value of Ki-67 as a prognostic factor in laryngeal cancer. Ki-67 score may assist in differentiating malignant from benign laryngeal epithelium.

[The role of local surgical and radiological control in the treatment of soft tissue sarcoma sensitive to chemotherapy in children. Report of Polish Pediatric Solid Tumors Treatment Group].

In this paper the role local surgical and radiological control in the treatment of soft tissue sarcomas in children was analyzed. All children were treated according to CWS-91 and SIOP-IV protocols. Eighty three children with RMS A + E, EES/PNET, SS, UDS were included in the analysis. The primary surgery consisted of R0 (5%), R1 (18%) or R2 (16%) resection. In majority of cases (61%) primary surgical intervention was limited to diagnostic biopsy. Conventional or hyperfractionated radiotherapy was performed in 42.8%, 73.8% and 75% of children with disease stage II, III and IV, respectively. Delayed surgery was performed in 20 out of 53 (37.7%) children with stage III of the disease. In 5 patients without primary focus (urinary bladder in 3 and prostate in 2 cases) removed, progression of the disease occurred. In 5 children (stage IV) with progression of the disease no secondary surgery was performed. In 4 of them the primary tumor exceeded 10 cm in diameter. No delayed surgery was performed in 69% of relapsed children with stage III of the disease. Planned radiation therapy was not performed in 15.9% of cases. Primary local surgical control of primary tumor is of great importance for remission duration. In children who underwent delayed surgery the estimated EFS was of 0.7, in comparison with 0.5 EFS of those without secondary surgical treatment.

[Comparison of peritoneal patch and vascular prosthesis monitored with isotopes and parameters of humoral and cell mediated response].

The aim of this experimental study was to assess the reaction of peritoneal patch and vascular graft after implantation in arterial system. We compare complement haemolytic activation CH-50%, serum elastase and scintigraphy using Tc99 labeled leukocytes. There were significant differences in parameters of humoral and cell-mediated response between implantation of peritoneal patch and vascular graft. Twelve months later histological examination was made to determine healing processes of implanted materials. The results of this study are encouraging and confirm benefits of peritoneal patch in vascular surgery creating a new possibilities of angioplasty.

Subcutaneous T-cell lymphoma: report of two cases.

We have seen two patients, a 24-year-old man and a 15-year-old girl, with subcutaneous nodules and fever. The histopathological findings in both cases showed a lobular, histiocytic panniculitis with 'bean bag' cells and lymphoid cells identified as immature T-cells. This picture was consistent with subcutaneous T-cell lymphoma associated with cytophagic histiocytic panniculitis. The girl died after 5 months of disease with florid hemophagocytic syndrome; the man was subjected to a course of treatment (CHOP; cyclophosphamide/doxorubicin/vincristine/prednisone) and after 6 months observation his symptoms of nodules and fever disappeared.

[Levels of procollagen type III n-terminal peptide (PIIINP) in serum of patients with myeloproliferative syndrome (mps)].

Serum concentration of aminoterminal peptide of type III procollagen (PIIINP) were determined in 48 patients with myeloproliferative disorders (MPD). 27 with and 21 without marrow fibrosis. Serum concentration of PIIINP in MPD was significantly higher than that in control group, but not in secondary thrombocytosis. The highest serum level of PIIINP was found in the primary osteomielofibrosis. In myeloproliferative disorder a positive correlation between megakaryocyte count, marrow fibrosis and serum level of PIIINP was found. This indicates, that in MPD megakaryocytes stimulates fibroblasts to augment their fibrile formation function. Concentration of PIIINP in the serum seems to be a good, though not specific, marker of marrow fibrosis in myeloproliferative disorders.

[Hematological problems in patients with bronchial cancer].

Four patients with lung cancer and hematological disorders occurring in different stages of the disease are presented. In three out of them cancer metastases were found prior to the discovery of the primary focus. In two patients those metastases were localized in the bone marrow resulting in the hematological picture of the myelodysplastic syndrome and osteomyelofibrosis. In one of them acute lymphoblastic leukemia developed after short-lasting remission involving the regression of bone marrow metastases of the cancer. In one patient lymph node enlargement, constitutional symptoms and laboratory signs of inflammation led to the preliminary diagnosis of Hodgkin disease. In the remaining patient acute leukemia resistant to chemotherapy developed 34 month after the diagnosis of lung cancer. Our observations point to the necessity of the through diagnosis in every case of hematologic abnormalities of unknown origin and the search of a possible underlying malignancy.
