Prognostic value of nucleolar organiser regions (AgNORs) in laryngeal cancer.

The argyrophilic nucleolar organizer regions (AgNORs) of 154 biopsy specimens of laryngeal squamous cell carcinoma were examined. The silver colloid staining technique was used. For the control, normal laryngeal epithelium obtained from 25 patients with benign lesions of the larynx was examined. There was significant difference in mean AgNORs count between control group and cancer patients. No significant correlation was found between AgNORs count and patients' age and sex, tumour site, T stage and lymph node metastasis. There was a significant correlation between AgNORs number per cell and tumour histological grading. The mean number of AgNORs per nucleus was 3.85 +/- 2.09 in patients with poor and 2.47 +/- 0.71 in patients with satisfactory clinical outcome after treatment. Significant correlation between the mean AgNORs number and survival in univariate and multivariate analysis was documented. Among clinical features the lymph node status and the T stage were important prognostic factors. We conclude that AgNORs may be a potential prognostic marker in laryngeal cancer.

Bronchopulmonary carcinoid tumors--a significant diagnostic problem.

The problems with classification and diagnosis of bronchopulmonary carcinoid and other neuroendocrine tumors are described in this paper. Single neuroendocrine cells and so-called neuroepithelial bodies found in normal bronchial epithelium are currently believed to constitute pulmonary components of an extensive neuroendocrine system. In view these opinions, it has appeared a need for a new, standardized nomenclature. Hence presently suggested classification of neuroendocrine carcinomas taking into account their histological structure, immunohistochemical as well as prognostic features starting from carcinoid tumors to small cell anaplastic carcinomas-includes three types of neoplasms.

[Hematologic changes in stomach cancer].

Haematologic disturbances in 13 cases of gastric cancer are described. All the patients had anemia of different origin. Increased leukocytosis was observed in half of the cases, leukemic reaction in one third. Hemolysis was present in 50% of cases. Thrombocytopenia coexisted most frequently with disseminated intravascular coagulation in 4 patients. Bone metastases were visualised as osteolytic foci with radiological methods or increased capture of isotopic marker in the bones under scintigraphic examination. Under the microscope neoplastic metastases were found in bone marrow smears of 5 patients. All patients displayed symptoms of gastric ulcer disease acute or chronic phase. In some cases only repeated gastroscopic examination and mucosa biopsy was the only way to confirm cancer. In other cases the diagnosis was made after the histopathologic examination of the resected stomach, in still others by a section.

[Early cellular and humoral response after vascular prosthesis implantation in the arterial system].

The aim of the study was to determine the level of leukocytes activation after the implantation of vascular graft. As an experimental model we used 20 dogs. The aorto-iliac unilateral graft (dacron 6 mm) was implanted to all animals. The adhesion and migration of the leukocytes was estimated postoperatively by scintigraphy using Tc99 labeled leukocytes. The increased migration and adhesion was found in both proximal and distal anastomoses till the 30th postoperative day and it was more intensive in the distal anastomosis.

[Emperipolesis in megakaryocytes in patients with thrombocytosis in the course of myeloproliferative disorders].

The incorporation of other marrow cells into megakaryocytes, termed emperipolesis, has been studied in paraffin biopsy sections from 17 untreated patients with myeloproliferative disorders (MPDs). The group consisted of 12 females and 5 males, aged from 34 to 72 years (mean 51.3). Patients with essential thrombocythemia (ET)--9, chronic granulocytic leukemia (CGL)--4, polycythemia vera (PV)--3, and myelofibrosis (MF)--1 were included into the study. Clusters of large polyploid megakaryocytes were observed in anatomic relation to the marrow sinusoidal system. Emperipolesis has been scored as being present or absent per 100 megakaryocytes/slide. Cells found within megakaryocytes were mostly erythroblasts and mature granulocytes. The number of incorporated cells varied from 1 to 7 per one megakaryocyte. Considering the 17 patients with MPDs, emperipolesis was observed in a vast majority of those with ET(8/9) and PV(2/3), in some with CGL(1/4), but not in MF. The mechanism of megakaryocytic emperipolesis remains unclear. Adhesion molecules on megakaryocytes and incorporated cells may possible mediate the cell-to-cell interactions important for emperipolesis.

Lipid abnormalities in rats given small doses of lead.

Previous human and experimental studies have demonstrated that lead exposure may modify the metabolism of lipids. Several studies have indicated that exposure to lead produces an increase in lipid peroxidation and inhibits blood superoxide dismutase activity. Recently, lipid peroxides have been shown to impair tissue membranes and to be a risk factor for vascular diseases. The aim of the present investigation was to evaluate the impact of subclinical lead poisoning on rat lipids in the context of atherosclerosis. The degree of poisoning was analogous to that in populations exposed to lead in a contaminated environment. Experiments were performed on male Buffalo rats with body weights of 150-200 g. The experimental animals received lead acetate intragastrically in doses of 35 mg lead/kg body wt. (Pb/kg) once weekly or 70 mg Pb/kg twice weekly for 7 weeks. Control rats were fed in the same manner with sodium acetate equimolar to the acetate in the lead acetate solution. One day after the feeding was over, venous blood samples, under ether anesthesia, were collected. The animals were killed by exsanguination and the liver was excised for determination of the metal (lead, copper, and zinc) content. A segment of the abdominal aorta was excised for histological examination. In venous blood the following were estimated: triglycerides, total cholesterol, high-density lipoprotein (HDL)-cholesterol fraction, serum lipid peroxides, and blood superoxide dismutase activity. Metal content (lead, copper, and zinc) in blood and liver was determined by means of atomic absorption spectrophotometry. In rats poisoned with small doses of lead, decreases in the plasma cholesterol level and the HDL-cholesterol fraction were observed.(ABSTRACT TRUNCATED AT 250 WORDS)

[Experimental studies of mechanism and efficiency of sclerosing agents: varicocid and paraffin].

Based on the analyse of publications the authors treated of history and contemporary applications of sclerotherapy. The role, methodics and results of sclerosing therapy among patients with oesophageal varices were also presented. The aim of the study was to examine the mechanism and efficiency of two sclerosing agents which are frequently used in Poland. Examinations were performed on 30 Wistar rats. Their caudal veins were injected with 0.1 ml of Varicocid or paraffin. In the control group the 0.9% solution of NaCl was applicated. Clinical and histologic appraisal were done at 3 weeks postinjection (first or second) of sclerosants. On the base of obtained results the necro-inflammatory properties and considerable higher efficiency of Varicocid were confirmed.

Tracheo- and bronchoesophageal fistulas in Hodgkin's disease.

Among 330 patients with Hodgkin's disease, in five development of fistula between the respiratory and alimentary tract was observed; tracheoesophageal in three and bronchoesophageal in two patients. Generalization of the disease was the cause of fistula in three patients; it appeared as a result of necrosis of the neoplastic infiltration during cytostatic treatment or radiotherapy. In the fourth patient the fistula was of iatrogenic origin (radiation induced damage), and in the fifth it was the result of secondary neoplasm (bronchial anaplastic cancer).

The natural course of cryptorchidism in rats and the efficacy of orchidopexy or orchidectomy in its treatment before and after puberty.

Both clinical and experimental evidence suggest that fertility is impaired in unilateral cryptorchidism. To investigate the effect of the undescended testis on the contralateral descended gonad, a new experimental model based on natural cryptorchidism in rats was designed. Seventy male Buffalo rats with an undescended right testis noted at the age of 30 days were used. Fifty healthy animals served as a controls. The natural course of cryptorchidism was investigated at the ages of 30, 90, and 180 days. The effects of orchiopexy and orchiectomy performed in cryptorchid animals before and after puberty were evaluated at the age of 180 days. Both nonoperated and operated animals were mated at the age of 150 days in order to estimate their fertility. The animals were killed at 30, 90, and 180 days of life and the testes were removed. In each excised testis testicular weight and seminiferous tubular diameters were measured and the maturity of the germinal epithelium was determined using the Johnsen testicular biopsy score. The experiment demonstrated reduced testicular weight and seminiferous tubular diameters in undescended testis already at 30 days and arrest of spermatogenesis at the spermatocytes stage at 90 and 180 days. There was no significant difference between contralateral descended testes and controls at the age of 30 and 90 days, but at 180 days the degenerative changes were identical with those in the cryptorchid testes. Cryptorchid rats were completely infertile. Both orchiopexy and orchiectomy prevented the damage to the contralateral testis. A significant improvement in size and spermatogenesis was recorded in most cases of the surgically descended testes.(ABSTRACT TRUNCATED AT 250 WORDS)

[Activity of gamma-glutamyltranspeptidase in clear cell carcinomas of the human kidney and renal cortex].

In 30 operated patients (23 men and 7 women) we determined the activity of GGTP in the tissue of renal clear-cell carcinomas and in their renal cortex. It was found that the enzyme activity was constantly lower in the tumours than in the spared kidney however with a correlation between the enhancement of GGTP activity in the renal cortex and clear-cell carcinomas originating in the tissue of the cortex. In contrast we did not observe a relation between the degree of morphological differentiation of the neoplasms and GGTP activity. The authors discuss the problem in the light of literature data.
