Antibacterial activity of norfloxacin.

Norfloxacin, a new quinoline derivative, was studied in vitro, and determinations of agar dilution minimal inhibitory concentrations (MICs) and broth dilution MICs and MBCs were made. Nalidixic acid and cinoxacin were used as comparative agents. Norfloxacin was found to be extremely active against all strains tested of Escherichia coli, Klebsiella spp., Proteus mirabilis, indole-positive Proteus spp. Serratia spp., Citrobacter spp., and Enterobacter spp., with MICs normally below 1 microgram/ml. It also was found to be highly active against Pseudomonas aeruginosa, Staphylococcus saprophyticus, and enterococci, which are all resistant to nalidixic acid and cinoxacin. The MICs for norfloxacin obtained by broth dilution were slightly higher than those obtained by agar dilution, whereas the reverse was true for nalidixic acid and cinoxacin. The MBCs of norfloxacin were only slightly higher than the MICs, even at high inocula. The in vitro activity of norfloxacin was not dependent on the inoculum size, whereas both the MICs and the MBCs of nalidixic acid increased markedly for many of the strains tested when the inoculum was increased in broth dilution from 10(3) to 10(6) colony-forming units per ml. Norfloxacin seems to be a promising antibacterial agent for the treatment of urinary tract infections, especially those caused by Pseudomonas spp. and other species today requiring the use of injectable antibiotics.

Immunosuppression and the additive effect of plasma exchange in treatment of rapidly progressive glomerulonephritis.

Attempts were made to evaluate the separate effect on kidney function of immunosuppressive treatment (IS) and plasma exchange (PE) in 27 patients with rapidly progressive glomerulonephritis (RPGN). Twenty-four of the patients were treated with PE. Initial IS was supplemented with PE within 6-12 days in 5 patients, and after at least 14 days in 13. Because of suspected septicemia, 2 patients were first treated with PE, and IS was not initiated until the possibility of septicemia had been excluded. In 4 severely ill patients wih rapid clinical deterioration, both treatments were started simultaneously. Twenty patients improved during one or both treatments, 4 with IS alone, 2 with IS and doubtfully with PE, 3 with IS and probably also with PE, 5 both with IS and PE and one with PE alone. In 5 patients the individual effects of IS and PE could not be evaluated. In another 2 patients the combined treatment seemed to influence the course favourably. In the remaining 7 patients the effect of the treatment was doubtful or nil. Two further patients with Goodpasture's syndrome were treated. They were admitted late, and both kinds of treatment were instituted simultaneously. One of them died in respiratory insufficiency, the other remained oliguric while the pulmonary changes faded. Thus, PE added a positive effect to IS in several patients with RPGN. The treatment had few and mostly mild side-effects.

Candida albicans pneumonia.

The increased incidence of deep fungus infections, especially of the lungs, should be kept in mind. Although no characteristic pulmonary abnormality is found in candidiasis, the appearance is usually different from that seen in virus pneumonia, bronchopneumonia, or tuberculosis. The diagnosis is more difficult than in these conditions but the clinical and radiologic findings, together with cultures and serologic tests, often justify a provisional diagnosis which is sufficiently quick and reliable for the initiation of adequate treatment.

Diagnosis of herpes simplex encephalitis. A comparison between electroencephalography and computed tomography findings.

In a series of six cases of encephalitis caused by herpes simplex virus, type 1, the diagnostic and prognostic value of the electroencephalogram (EEG) was investigated. Special interest was focused on the time relationship between the appearance of abnormal EEG findings, the initial clinical symptoms and the changes in cranial computed tomography (CT). The characteristic periodic EEG pattern can be demonstrated within 2 days of disease, before typical structural changes appear in CT. A unilateral periodic pattern may be associated with a good prognosis, whereas all patients with bilateral changes died in spite of specific antiviral therapy.

Screening of Rh-antibodies in Rh-negative female infants with Rh-positive mothers.

The sera of 96 consecutive Rh-negative female infants born to Rh-positive mothers were examined at birth, and sera from 88 of these infants were examined for the presence of Rh-antibodies at the ages of about three and eight months. A two-stage papain test and an AutoAnalyzer method were used for antibody screening and identification. Weak anti-D antibodies were found by the papain and AutoAnalyzer techniques in two cord sera, In neither case could the antibodies be demonstrated in samples taken on later occasions. Weak anti-D antibodies were found by the AutoAnalyzer technique but not by the manual methods in the sera of two other infants at the age of eight months. These antibodies could still be demonstrated by the same technique in samples taken about one month later. Though far from conclusively, the results support the "grandmother theory", but because of the low incidence of sensitization and uncertain nature of the anti-D antibodies demonstrable only by the AutoAnalyzer technique, anti-D prophylaxis is not recommended for newborn Rh-negative female infants with Rh-positive mothers.

Mecillinam and ampicillin separately or combined in gram-negative septicemia.

Of 20 patients with gram-negative septicemia treated with mecillinam alone or in combination with ampicillin, successful therapeutic results were obtained in 16. In 11 patients treated with ampicillin alone, three failures responded successfully to a combination of mecillinam and ampicillin. Mecillinam MIC values of isolated Enterobacteriaceae were 0.05-0.4 micrograms/ml. In patients receiving 5 mg/kg mecillinam intravenously every six hours, the mean 0.5 hour concentration was 11.0 micrograms/ml and in those given 10 mg/kg 23.3 migcrograms/ml. No serious side effects were recorded. One patient on mecillinam developed an exanthema, as did three patients on combined therapy.

Isoelectric focusing of the phosphoprotein of rat-incisor dentin in ampholine and acid pH gradients. Evidence for carrier ampholyte-protein complexes.

Rat-incisor phosphoprotein (RIP) has been subjected to isoelectric focusing at 4 degrees in (a) an Ampholine pH gradient of 2.5-4 and (b) an acid pH gradient created by electrolysis of a system of acids and acidic ampholytes and covering the pH range 0.5-3.5. In the Ampholine gradient, the RIP unexpectedly formed several adjacent and strongly opalescent bands in the pH range 2.5-3.1. These bands, which migrated slowly toward the anode on prolonged focusing, are interpreted as being the result of an interaction between the amino groups of the Ampholine and the numerous phosphate groups of the protein. In the acid pH gradient, the RIP focused into one narrow zone corresponding to an isoelectric pH of 1.1 at 4 degrees. This value is consistent with the amino-acid composition and the phosphate content of the protein,

Enterotoxin-producing bacteria stools from Swedish United Nations soldiers in Cyprus.

A study was designed to establish the role of heat-labile enterotoxin producing Escherichia coli and other intestinal pathogens in diarrhoeal disease in Swedish soldiers of the United Nations' Force in Cyprus. Before leaving Sweden for Cyprus, no enteropathogens were isolated from the soldiers. The study was performed from October 1975 through January 1976 and in August 1976. Enterotoxigenic strains of E. coli, Citrobacter freundii, Enterobacter cloacae and Klebsiella pneumoniae were the intestinal pathogens most frequently isolated from the cases of diarrhoea. Such strains were found in 14 of 79 diarrhoeal cases (18%) but also in 6 of 66 (9%) of next-bed-fellow controls and in 7 of 139 (5%) of healthy Cypriotic civilians. Few other pathogens were identified in the soldiers, but 9 of 139 (6%) of the Cypriots were carriers of cysts of Giardia lamblia. The results indicate that enterotoxigenic bacteria were probably the most important cause of diarrhoea of those agents identified among the Swedish UN soldiers on Cyprus. Several soldiers were colonized with these strains without contracting diarrhoea.

Netilmicin treatment of serious infections in patients with renal insufficiency.

Out of 24 patients with suspected moderate to severe infections and treated with the semisynthetic aminoglycoside netilmicin, 11 patients had decreased renal function. Non-bacterial aetiology was present in five cases. There was good netilmicin effect in 10 cases, indeterminate effect in 6 cases (due to simultaneously given other antibiotics) and failure in 3 cases. Pharmacokinetic parameters were studied. Curves are given for serum concentration patterns relative to creatinine clearance, and serum half-lives are calculated in renal dysfunction. In this series of very ill patients including impaired renal function, metilmicin proved to be an efficient antibiotic with low toxicity. Only one transient minor vestibular abnormality in an 80-year-old man with concomitant diuretic medication was registered.

Selective pressure of tetracyclines on the faecal flora. A comparison between tetracycline and doxycycline.

The aerobic faecal flora was studied in two groups of healthy adult persons, each group consisting of 18 individuals, before a course of either doxycycline or tetracycline HCI, at the cessation of therapy and 5 weeks afterwards. The main object of this study was to evaluate the effect of antibacterial treatment on the occurrence of mutagenic resistant and plasmid carried resistant strains in the faecal flora, and in particular development of resistant E. coli. The frequency of resistant strains increased after treatment with both of the drugs without significant differences.
