Influence of acetaminophen and trichloroethylene on liver cytochrome P450-dependent monooxygenase system.

The aim of the study was to evaluate the effect of acetaminophen (APAP) and/or trichloroethylene (TRI) on the liver cytochrome P450-dependent monooxygenase system, CYP2E1 and CYP1A2 (two important P450 isoforms), and liver glutathione (GSH) content in rats. Rats were given three different doses of APAP (250, 500 and 1000 mg/kg b...) and then the above-mentioned parameters were measured for 48 h. The lowest APAP dose produced small changes in the cytochrome P450 content of liver. At 500 mg/kg APAP increased the cytochrome P450 content to 230% of the control. The inductive effect was seen at 1000 mg/kg dose but at 24 h and later. NADPH-cytochrome P450 reductase activity was the highest after the lowest dose of APAP, while after the highest dose it was equal to the control value. TRI increased both the cytochrome P450 content and the NADPH-cytochrome P450 reductase activity. When TRI was combined with APAP, both these parameters increased in the first hours of observation, but they returned to the control values at 24 h. When APAP was given at 250 mg/kg, GSH levels decreased to 55% of the control at 8 h and returned to the control values at 24 h. The higher doses of APAP decreased GSH levels more than the lowest dose, but after 24 h GSH levels did not differ from those of the control. When TRI was given at 250 mg/kg, the GSH levels decreased to 68% of the control at 2 h and then they increased gradually and tended to exceed the control values at 48 h. The effect of TRI combined with APAP on the level of GSH was virtually the same as that of APAP alone given at 500 mg/kg.

Arterial ketone index in assessing liver function and its detoxicative capability after ischemia-reperfusion injury.

Arterial ketone index (AKBR) which is the ratio of acetoacetic acid to 3-hydroxybutyric acid in the arterial blood, is believed to reflect the mitochondrial reduction potential of hepatocytes and general energy state of the liver. In the presented paper we challenged this hypothesis by analysing the correlation between AKBR and the results of typical liver blood tests (AspAT, AlAT, LDH, CRP) and biotransforming potential of the liver (cytochromes P450, b5 and their corresponding NADPH and NADH reductases) in the model of ischemia-reperfusion injury of rat liver. The results were compared with histochemical analysis of distribution and activity of SDH, LDH and G-6-Pase, the key marker enzymes of the liver. We have shown that, except in the case of acute phase protein (CRP), a decrease in AKBR correlated well with the increase of the level of indicator enzymes in serum. Histochemical analysis also confirmed that AKBR correlates with the degree of damage to hepatocytes during early stage of reperfusion after 60 min of liver ischemia. In the Spearman test, AKBR was significantly correlated with the changes in cytochrome P450 content and its NADPH reductase activity which indicates a high sensitivity of this test. We conclude that the decrease of AKBR value reflects the impairment of basic energy pathways and detoxicative capability of the liver.

[Malignant ameloblastoma of the maxilla as an interdisciplinary problem].

The clinico-pathological assessment of three cases of rare dentogenic tumours of the maxilla was made (including one case described earlier). In two patients primary malignant ameloblastoma was diagnosed. In one patient benign ameloblastoma was treated few times; in this case we have observed the transformation of benign disease to malignant because the occurring lymph node metastases. The authors discussed clinical and morphological characteristics of these tumours, which are under interest of many specialists.

[[A case of primary tracheo-bronchial amyloidosis].

A case of tracheobronchial amyloidosis is described in a 39 year-old man. The diagnosis was established histologically by a bronchoscopic biopsy which revealed a diffuse infiltrative process in the tracheobronchial tree. Due to the symptoms of obstruction of the lower respiratory tract the patient was treated with Prednisone and Melphalan and with partial excision of amyloid deposits by bronchoscopy. Bronchoscopy although carrying a risk of bleeding is the procedure of choice in diagnosis and therapy of such cases.

Ontogenesis of hepatocyte respiration processes in relation to rat liver cytochrome P450-dependent monooxygenase system.

The study aimed to evaluate the effect of age on the activity of the cytochrome P450-dependent monooxygenase system and on cellular respiration processes in Wistar rats aged 0.5, 1, 2, 4, 8, 12, 20, and 28 months. The following parameters were determined: cytochrome P450 content, cytochrome b5 content, NADPH-cytochrome P450 reductase activity, and NADH-cytochrome b5 reductase activity, succinate dehydrogenase (SDH) activity, and lactate dehydrogenase (LDH) activity. In the study, cytochrome P450 content increased in the first month of life, which was accompanied by increases in SDH and LDH activities. In the subsequent months, SDH activity decreased, whereas LDH activity increased to reach the maximum in month eight and then decreased. Cytochrome b5 content showed a decreasing tendency throughout the experiment. NADH-cytochrome b5 reductase activity showed only slight deviations in individual age groups.

Ventriculosaccular lymphoepithelioma of the larynx: case report and literature review.

BACKGROUND: Lymphoepithelioma is a very rare neoplasm of the larynx. The authors have reviewed the available published literature on the subject and found eight studies on lymphoepithelioma. Lymphoepitheliomas derive from the epithelial and lymphatic tissues of the laryngeal ventricule. The rarity of the occurrence of lymphoepithelioma makes it difficult to define its clinical features and general treatment methods. METHODS: The authors present a description of a lymphoepithelioma located in the left laryngeal sacculus, extending into the pre-epiglottic space and infiltrating the epiglottis with an ulceration of its laryngeal surface. A total laryngectomy and a left radical neck dissection were performed. After surgery, radiotherapy treatment was applied to the left cervical and laryngeal areas. Two years after the initial radiotherapy, the patient died during palliative chemotherapy (for metastases). RESULTS: In patients with lymphoepithelioma of the larynx, the following can be observed: (1) bulging of the laryngeal ventricule, with or without ulceration of the laryngeal surface of the epiglottic mucosa; (2) early lymph node and remote matastases. CONCLUSIONS: In the early stages of lymphoepithelioma, radiotherapy is the method of choice. In advanced cases of lymphoepithelioma, radiotherapy supplements surgical treatment.

Epithelial-mesenchymal cell interactions and participation of the neuroendocrine markers in tumor development. Immunohistochemical study.

Existence of the organ stem cell population seems to decide on ability of the tissue to regenerate and, likewise, on carcinogenesis. The source of the organ specific stem cells may be perivascular mesenchyma of thin-walled vascular channels. Our previous study on the breast cancer indicates that the perivascular mesenchyma of thin-walled vessels appears to be source of myoid cells (myofibroblasts) from which cancer cells arise. Similar results have been observed in the cancers of lung, salivary gland and colon, investigated in the current study. The perivascular cells of thin-walled channels are the source of myoid cells with expression of synaptophysin (Syn) and/or chromogranin A (Chg A), and from these cells neoplastic cells could originate. Syn and/or Chg A positive neoplastic cells were particularly well visible in connection with the vascular channels on the peripheries of tumors while other parts of tumors were only weak positive or negative for those neuroendocrine markers. Similarly as in breast cancers, the S100-protein positive dendritic cells with various of distribution were seen, expressing intimate connection with neoplastic cells. The epithelial pearls especially abundant in non-small cell lung carcinomas demonstrated immunohistochemical analogy to Hassall's bodies: they had monocytogenic cell inside and they displayed thymosin alpha 1 (TA1), as well as mucin secretion and minute calcification. Some epithelial cells expressed desmin and Syn. All types of investigated cancers demonstrated TA1. Results of our present study suggest that the perivascular cells have a differentiation defect. Such defect may initiate abnormal stromal environment, commonly observed in neogenesis, however, the presence of thymic growth factors may favor tumor growth.

Information flow between hippocampus and related structures during various types of rat's behavior.

The relationships among the CA1 field of hippocampus, the entorhinal-piriform area, the subiculum and the lateral septum were studied in various behavioral states in the rat. The EEG signals recorded simultaneously from chronically implanted electrodes were analyzed by means of a multichannel autoregressive (AR) model. Power spectra, ordinary, multiple and partial coherences, and directed transfer functions were calculated. The method of analysis which took into account all signals simultaneously, not pair-wise, made it possible to estimate the spectral characteristics and the directions of the EEG flow between structures. The pattern of the EEG activity propagation depended on the type of behavior, difficulty of the task performed by the animal, and the phase of the trial. Our results not only confirmed the existence of connections between analyzed structures, but also showed that these connections may have different strengths during various types of behavior.

Topographic analysis of coherence and propagation of EEG activity during sleep and wakefulness.

Overnight sleep EEG recorded from 21 derivations was studied in 8 healthy subjects. The vector autoregressive model was fitted to all 21 channels simultaneously. Ordinary, multiple and partial coherences and directed transfer functions were estimated for sleep stages and wakefulness. Ordinary coherences give rather trivial information that coherence decreases with distance. Partial coherences revealed specific structure that was well repeatable for the subjects studied. Differences in coherence patterns between sleep stages were found by means of statistical tests. An increase of coherence was found for sleep stages 2, 3 and 4. Directed transfer function made possible the identification of the main centers from which EEG activity is spreading during sleep and wakefulness. During sleep the influence of subcortical structures was manifested by propagation of activity from the fronto-central region. The range of this interaction was highest in sleep stages 3 and 4. An EEG analysis, based on the approach of treating time series as a realization of one process and on the simultaneous (not pair-wise) evaluation of signals offers new possibilities in the investigation of synchronization and functional relations in the brain.

[Diagnostic and therapeutic value of laparoscopy in acute abdomen].

The purpose of this work is to evaluate the degree of applicability of the diagnostic laparoscopy (DL) and therapeutic laparoscopy in the acute abdomen and trauma patients. The material of 109 cases of DL performed since 1983 has been evaluated retrospectively. 22 patients were operated by laparoscopy. The patients were assessed in the following groups: I-suspected acute appendicitis, II-acute abdomen of uncertain ethology, III-abdominal trauma. In the first group 60 patients were diagnosed, 13 of them were operated by laparoscopy, 28 by open access, 15 avoided the unnecessary laparotomy. In the second group 30 patients were diagnosed, 6 of them were operated by laparoscopy, 8 open interventions were noted, 16 avoided the non-therapeutic laparotomy. In the third group 19 patients were diagnosed, 13 underwent laparotomy, 6 avoided the unnecessary intervention. There has been one death, not associated with DL (acute bowel ischemia), morbidity was 1% (one case of bleeding from the mesenterial artery, treated by conversion to laparotomy), one DL was considered as falsely negative, one not sufficient to establish the diagnosis. Basing on our experience in DL as a diagnostic tool we can confirm the accuracy of the method in 99.08% with only 1% of morbidity. 20.1% of patients were operated by laparoscopy, without complications. In conclusions the authors consider the DL allows to establish a prompt and accurate diagnosis in the acute abdomen and trauma without a therapeutic delay and unnecessary hospital observation. The fault of the method and morbidity rate is acceptable. The treatment by the same laparoscopic access offers to these patients another benefit, the more the experience is increasing.

[Complications after esophageal resection due to cancer].

The aim of the study was to evaluate complications searching for ways of decreasing their incidence. 225 patients with the diagnosis of thoracic esophageal cancer had been treated in the Department of General and Transplantation Surgery, Medicala Academy, Szczecin, within the period of 1984-1995, esophageal resection was done in 90 of them and 75 were submitted to the analysis. The majority of patients i.e. 65 of them had esophagogastric anastomosis performed within the thorax while in 10 patients the anastomosis was accomplished in the neck. Complications concerning cardiovascular and respiratory system wound healing, anastomotic leakage and gastric blood supply have been analysed. Cardiovascular and respiratory complications occurred in 24 and 66 per cent of patients with mortality rate of 28.6 and 12 per cent, respectively. Wound healing disturbances were found in 3.7 per cent of patients. Anastomotic fistulas and necrosis of the stomach were observed in 4 and 5.3 per cent of patients with mortality rate of 75 and 100 per cent, respectively. A statistically significant correlation between the incidence of cardiovascular complications and disease stage was found. Only in patients operated on in I and II disease stage can we expect the serious complications to be less frequent.

[Biotransformation ability of the liver in ischemic reperfusion injury].

The aim of the study was to estimate the activity of enzymes connected with xenobiotics biotransformation in early reperfusion period after 90 minutes ischemia of rat liver and to state the correlation of obtained results with values of standard liver tests. Full, global liver ischemia was performed using Isozaka model with temporary portocaval anastomosis. The activity of alanin and aspargine aminotransferase and lactate dehydrogenase in blood serum, and P-450 and b5 cytochromes with related to them reductases in microsomal fractions of liver homogenates were estimated in 24th hour of reperfusion. Considerable decrease of the activity of mixed function oxidase system, not correlating with the results of standard liver tests was observed.

[Liver injury as a result of temporary occlusion of the hepatoduodenal ligament in the rat].

The aim of the study was to state consequence of 90 minutes occlusion of hepatoduodenal ligament and 24 hours reperfusion. Full liver ischemia was performed by a Pringle manoeuvre above temporary, internal portocaval anastomosis. Changes of the liver were analysed at the level of light microscope, and the activity of many enzymes participating in processes of energy production and distribution was measured using histochemical methods. It was stated, that hepatocytes included in zone I of the liver cluster possess the biggest regenerative opportunities.

Effect of chlorfenvinphos on rat liver subjected to ischemia and reperfusion.

The study was performed on rats divided into 9 groups. Groups 1-3 served as controls. In groups 4 and 5 rat livers were subjected to 90-min ischemia followed by 12- or 24-hour reperfusion. In groups 6 and 7 rats were injected with intraperitoneal chlorfenvinphos (2 mg/kg b.w.) and sacrificed after 12 or 24 hours. In groups 8 and 9 rat livers were subjected to 90-min ischemia, 12- or 24-hour reperfusion and then rats were injected with chlorfenvinphos (2 mg/kg b.w.). Liver sections were evaluated morphologically, histochemically (SDH, LDH, G6Pase, glycogen, Mg2+ ATPase and AcP). The microsomal fraction of the liver was evaluated for cytochrome P450 content and NADPH-cytochrome P-450 reductase activity. It has been found that liver ischemia and reperfusion result in extensive necrosis, enzymatic disturbances, particularly in acinar zone 3. Ischemia as well as reperfusion decrease the cytochrome P450 content of hepatocytic microsomes and the activity of NADPH-cytochrome P-450 reductase. Intraperitoneal injection of chlorfenvinphos during ischemia and reperfusion dramatically intensifies damage to the liver, although chlorfenvinphos alone produces only mild nonspecific effects on the morphological and enzymatic structure of the liver.

Influence of cholesterol and protein diet on liver cytochrome P-450-dependent monooxygenase system in rats.

The influence of the cholesterol and protein diet on the mixed-function oxidases system activity and desaturation of fatty acids were examined. The phenobarbital innducibility of these parameters was analyzed too. Investigations were carried out on Wistar adult male rats. The animals were on the cholesterol (0.25%) and protein diet (32%) during 45 days. The cytochrome P-450 and cytochrome b5 contents, the NADPH-cytochrome P-450 reductase and NADH-cytochrome b5 reductase activities, aniline hydroxylase and 4-aminopyrine N-demethylase activities were measured in the hepatic microsomal fraction. In both sexes, the cholesterol diet decreased all the examined parameters with the exception of NADH-cytochrome b5 reductase activity. The protein diet did not change the examined enzyme activities or levels with the exception of induction of NADH-cytochrome b5 reductase and 4-aminopyrine N-demethylase activities. Simultaneous treatment with phenobarbital had heterogenous effect dependent on the type of diet and examined parameter.
