Is tomography of intervertebral disc calcification useful in children?

In the past ten years, we have found cervical intervertebral disc calcifications in three children on plain films of the cervical spine made because of cervical pain. In each case, we required further radiological investigations, antero-posterior and lateral linear tomography for two children and an axial computed tomography for one child. In each case, tomography revealed no supplementary useful information. On retrospect, we think that these examinations caused unnecessary irradiation and that they should only be carried out in the rare circumstances when disc calcification is associated with neurological symptoms.

Low-dose CT and inflammatory disease of the paranasal sinuses.

Computed tomography (CT) is the gold standard for exact delineation of inflammatory sinus disease, especially before endoscopic surgical treatment, and in cases of postoperative recurrences. In routine CT studies, the radiation dose to the patient is not negligible. Therefore, the authors evaluated prospectively the CT scans of 44 patients with inflammatory disease of the paranasal sinuses, to define the imaging ability of low-dose CT (i.e. 60 mA-3 s, 30 mA-3 s, and 30 mA-2 s), comparatively with the standard mAs settings (130 mA-3 s). In all cases, the exact extent of the disease was correctly assessed on each of the low-dose settings, with no false negative study. The increasing graininess of low mAs sections did not induce errors of interpretation, despite a less pleasant appearance to the eyes. In cases of extensive sinus disease, the thickness and integrity of the ethmoid septa were sometimes more difficult to evaluate on low-dose CT sections. The authors recommend the use of low mAs settings in the evaluation of inflammatory disease of the sinuses, complemented, if necessary, in cases of extensive abnormalities, by one or two sections obtained with standard mAs settings, focused on questionably abnormal bone septa.

Evidence for a colocalization of oxytocin mRNA and galanin in magnocellular hypothalamic neurons: a study combining in situ hybridization and immunohistochemistry.

The possible colocalization of oxytocin (OT) and galanin (GAL) was studied by combining, on the same cryostat sections, in situ hybridization (ISH) for OT mRNA with a tritiated oligonucleotide probe and immunohistochemistry (ICC) of GAL. Many cells were either labelled by ISH (OT mRNA containing cells), or by ICC (GAL containing cells). Moreover, some magnocellular neurons in the supraoptic and paraventricular nuclei were labelled for both OT mRNA and GAL. These results demonstrate that some magnocellular neurons of the rat hypothalamus contain both GAL and OT. This approach is suitable for studying the intracellular distribution of OT gene expression and mature GAL under different physiological or experimental conditions.

Further evidence for the presence of "small eaters" and "large eaters" among women.

One hundred thirty-four women, aged 36 +/- 4 (mean +/- SD) y, BMI 20 +/- 3 kg/m2, perceiving themselves as having either a low or high energy intake (EI), participated in a study to determine variations in EI. Information on EI and activity level was obtained from repeated 7-d records. The 40 subjects with the lowest EI (in kcal/kg body wt) were categorized as small eaters (SEs); the 40 subjects with the highest EI were considered to be large eaters (LEs). The absolute (in kcal) and relative (in kcal/kg body wt) EIs of the SE and LE groups were 1488 +/- 312 and 27 +/- 4 for the SE group, respectively and 2393 +/- 509 and 47 +/- 6 for the LE group, respectively. There was no significant difference in activity level or fat-free mass (FFM) between the groups. However, LEs weighed significantly less (51 vs 55 kg) and were leaner (22% body fat vs 33%) than were SEs. Individuals with similar FFM and activity level can vary significantly in EI needs.

Hypothalamic galanin-immunoreactive neurons projecting to the posterior lobe of the rat pituitary: a combined retrograde tracing and immunohistochemical study.

UNLABELLED: To identify the galanin-immunoreactive neurons projecting to the posterior lobe of the pituitary in the rat hypothalamus, a retrograde tracer (complex of wheat germ agglutinin-enzymatically inactive horseradish peroxidase-colloidal gold) was injected into the posterior lobe of the pituitary. Sections of the hypothalamus were treated with a combination of silver enhancement of retrogradely transported tracer and immunohistochemistry of galanin. Of the total number of hypothalamic cells doubly labeled with retrograde tracing and galanin-immunostaining, 56-60% were found in the supraoptic nucleus, 18-23% in the retrochiasmatic nucleus, 8-10% in the lateral magnocellular portion of the paraventricular nucleus. The ratio of (number of doubly labeled cells/number of galanin-immunoreactive cells) in each of the above regions was similar to the ratio of (number of retrogradely labeled cells/number of Nissl-stained cells) in the supraoptic nucleus. Of all retrogradely labeled cells in the hypothalamus, 51-56% also contained galaninlike immunoreactivity. IN CONCLUSION: (1) galanin-immunoreactive fibers in the posterior lobe of the pituitary originate mainly in the supraoptic nucleus, retrochiasmatic nucleus, and lateral magnocellular portion of the paraventricular nucleus, (2) most of galanin-immunoreactive cells in these regions project to the posterior lobe of the pituitary, and (3) about half the neurons constituting the hypothalamo-neurohypophyseal system contain galaninlike immunoreactivity.

[Possibilities for assessing unilateral functional disorders of the upper urinary tract in infants with urethral valves].

Upper urinary tract function was evaluated in 15 infants in whom urethral valves were diagnosed. One baby died at 16 days of age and was not completely investigated. In 4, the valves did not have untoward effect on the bladder and the upper urinary tract. In 10 others, function was evaluated using the Hodson index, creatinine, PAH, inuline clearance and renal I123-Hippuran scintigraphy with measurement of the accumulation indices. The various examination procedures were of value in providing precise evaluation of the renal performances at diagnosis as well as in prognosis and follow-up of the function of each kidney.

Comparison of four methods for typing low-passage herpes simplex virus isolates.

Clinical isolates of herpes simplex virus (HSV) were identified as HSV type 1 or type 2 by sensitivity to (E)-5-(2-bromovinyl)-2'-deoxyuridine (BVDU), by differential replication in chick embryo cells versus guinea pig embryo cells, by restriction endonuclease analysis, and by a direct fluorescent antibody technique using monoclonal antibodies. More than 550 isolates were typed by two or three of the systems with complete agreements as to virus type between systems for each isolate. In appropriately equipped laboratories, any of the above typing systems can be used with complete confidence. However, the BVDU sensitivity assay, particularly when used in a continuous cell line as described, can be economically utilized in any virology laboratory.

[Computerized retrospective study of 141 children operated on for vesico-ureteral reflux].

Follow-up study of 141 cases of vesico-ureteric reflux (less than 10 years old) treated by Cohen's method. Computerized study, 5 years after treatment, with analysis of the influence of different parameters (type of VUR, age, sex, urinary infection or not, space of time between diagnosis and treatment) on the renal parenchymal radiographic measurements ( Hodson 's method). The youngest is the child when he is treated, the best is the functional result.

Cutaneous Endosalpingiosis.

Endosalpingiosis is the aberrant growth of Fallopian tube epithelium outside of its normal location. This phenomenon has long been recognized by gynecologists, since it most often occurs in the pelvic cavity after salpingectomy. We describe a 30-year-old woman with cutaneous columnar ciliated cysts of the umbilicus and adjacent skin that developed shortly after salpingectomy. Evidence is presented that these cysts are cutaneous endosalpingiosis.
